大型公立医院资源整合与运行发展的实践与思考

无锡市第一人民医院（儿童医院）和无锡市第五人民医院2007年12月开始实施资源整合战略，资源整合前，2家医院围绕筹建、整合和运行3大工作任务，科学统筹,稳步推进，确保了新医院的启用开诊。资源整合后，医院稳步实施三级目标责任制，做活激励机制，做实约束机制，并对重点工作实施项目化、规范化和精益化管理，通过2年运行，医院综合服务能力明显提升，医院资产实现增值保值，形成了一整套医院内部管理的工作经验和举措，促进了医院良性运行和健康发展。     

医院品牌内涵建设医院精神的贯彻与实施

医院精神建设是医院品牌内涵建设的主要内容,是一个医院赖以生存和发展的关键,是在长期的历史发展中形成的文化传统,为全体医护工作者的道德规范和价值取向。我院坚持以病人为中心,以医疗质量和医疗安全管理为核心,推行以"德法为本、效能优先、严谨规范、求实创新"的管理理念,全体工作人员均在管理理念的督促下奋力工作。医院将管理理念深入到日常医院建设中,从文化建设、医院管理、医疗工作、科研学术、党群工作、廉政建设等方面进行全面改善,为提升医院的品牌内涵、缓和医患关系起到了重要作用。     

试论循证医院管理的实现途径及影响因素

?????? 循证医院管理是当前在循证医学背景下医院管理中的一种新思维、新理念。循证医学实践的五个步骤决定了循证医院管理的五个实现途径,然而在目前国内环境下,循证医院管理存在一些固有的约束或不足。因此,循证医院管理如需获得期望的效果,必须在实现途径上做好安排,同时密切关注重要的影响因素。     

当前医院管理中几个热点问题的辨识

随着市场经济的发展和医药卫生体制改革的不断深入,医院发展面临着新的挑战。围绕医院的社会责任与自身利益、高新技术与基础质量、过度扩张与适度规模、"以病为本"与"以人为本"以及服务意识与服务艺术五个热点问题进行了辩证的思考,力求正确认识现代医院发展过程中存在的诸多问题,寻求有效的解决途径,以促进公立医院可持续发展。     

“以病人为中心”的实践之路与案例分享

从当前医疗服务存在的问题入手，阐述了践行“以病人为中心”服务理念的现实意义，并结合医院管理实践，以案例分享的形式，从理念创新、流程设计、诊疗技术、服务举措、人文关怀等五个方面，总结了医院践行“以病人为中心”服务理念的经验做法。最后，从以制度固化“以病人为中心”服务举措、以创新紧跟“以病人为中心”时代特点、以文化打造“以病人为中心”精神内核三个方面，探讨了建立“以病人为中心”长效机制的实现之策。     

疾病诊断相关组在医院绩效评价中的应用探讨

目前,尽管国内外关于医院绩效评价的研究较多,但普遍存在评价结果缺乏可比性的不足。其主要原因是不同医院收治的疾病类型不同、治疗难度和风险差别较大。基于DRGs的医院绩效评价方式为研究打开了思路。本文在明确当前公立医院绩效评价内容的基础上,指出了基于DRGs开展医院绩效评价的两个条件,即统一优质的数据和完善的信息系统,阐述了评价的三个维度,即医疗服务产出、效率和质量,并就上述三个维度内的相关评价指标的计算方法进行了说明。本文最后给出了基于DRGs开展医院绩效评价的五点现实意义:一是有助于优化医院内部成本结构;二是有助于规范科室临床路径工作;三是创新科室或医生的绩效考核方式;四是形成政府与医院间的长效监管机制;五是有助于探索DRGs付费模式在全国的推广。     

借鉴Z计分法模型监测医院财务风险的实践

随着卫生事业的改革和发展，医院的经营环境不断变化，财务风险对医院的生存和发展起到至关重要的作用。医院的管理者必须充分了解医院面临的财务风险，通过借鉴Z计分法模型建立财务风险预警体系，防范和化解医院发展中的各种财务风险，使医院走向合理、可持续的发展轨道。     

传染病医院临床实验室的特点及管理对策

目的:结合传染病医院临床实验室自身的性质和特点探讨其有效的管理对策.方法:结合本院的实际情况,从生物安全、质量控制、人员设备、制度以及信息化管理五个方面采取一系列的管理对策,完善传染病医院临床实验室的管理工作.结果:加强并完善了我院临床实验室的管理工作,保证了临床检验质量,成效显著.结论:抓好以上五个环节的管理,是提高医疗质量的重要保障,有助于实现临床实验室管理的规范化、科学化、信息化.     

医院客户关系管理与经营服务平台的构建

在当前面对医疗市场激烈的竞争现状，医院如何立于不败之地，如何使医院建设保持一个良性发展势态，是医院管理者面临的严峻课题。各级医院应主动适应市场变化，大力推行市场营销，促进医院的建设与发展。讨论医院怎样运用现代营销理论来构建面向市场、符合医院行业特点的医院营销体系。     

主辅分离下大型公立医院创新发展运行模式探索

创新作为现代医院发展的支点，是医院科学运营的动力。企业医院主辅分离后，通过观念创新、技术创新、服务创新和整体发展规模的精心规划，在决策方向、经营运作、核心能力、整体规模上的重点把握和持续创新，提升了医院科学发展力、市场适应力和持续创新力，实现了医院的跨跃式发展。     

Investigation of the mechanism of temperature sensitivity of the electroreceptors of ampullae of lorenzini

In experiments on Black Sea skates (Raja clavata), the potential of the receptor epithelium of the ampullae of Lorenzini and spike activity of single nerve fibers connected to them were investigated during electrical and temperature stimulation. Usually the potential within the canal was between 0 and –2 mV, and the input resistance of the ampulla 250–400 k. Heating of the region of the receptor epithelium was accompanied by a negative wave of potential, an increase in input resistance, and inhibition of spike activity. With worsening of the animal's condition the transepithelial potential became positive (up to +10 mV) but the input resistance of the ampulla during stimulation with a positive current was nonlinear in some cases: a regenerative spike of positive polarity appeared in the channel. During heating, the spike response was sometimes reversed in sign. It is suggested that fluctuations of the transepithelial potential and spike responses to temperature stimulation reflect changes in the potential difference on the basal membrane of the receptor cells, which is described by a relationship of the Nernst's or Goldman's equation type.I. Determination of stoichiometry of radioiodination of proteins

A sensitive method for the detection of small quantities of hydrophobic antioxidant free radical scavengers such as butylatedhydroxytoluene (BHT) and butylatedhydroxyanisole (BHA) in aqueous samples is described. The procedure involves extraction of the hydrophobic free radical scavenger into an organic solvent phase, followed by the subsequent reaction of an aliquot of this extract with the stable cation radical tris(p-bromophenyl)amminium hexachloroantimonate (TBACA). In experiments with BHT and BHA, the loss of TBACA absorbance at 730 nm was found to be linearly proportional to the amount of antioxidant added, with quantities of BHT as small as 200 pmol being easily detectable. In aqueous suspensions of dimyristoylphosphatidylcholine vesicles, assays of the aqueous BHT concentration showed that BHT partitioned strongly into the membrane phase, achieving very high BHT/phospholipid ratios. For a given concentration of BHT, partitioning into the membrane phase was greater in large, multilamellar liposomes than in either small, single-walled vesicles or in purified rat brain synaptic vesicle membranes. Direct assay of BHT and BHA in phospholipid membranes, however, was complicated by a nonspecific interaction between TBACA and the phospholipid.
