目的:评价用B超测量足月胎儿各生理参数来评估胎儿宫内生长的准确性与真实性。方法:414例住院分娩的足月头位单胎孕妇在分娩前4天内超声测量胎儿的双顶径(BPD),枕额径(OFD),枕额周径(HC),腹围(AC),腹横径(ATD),股骨长度(FL)每个参数测量2次,取平均值。出生后24小时内测量新生儿各相应生理参数。用SAS软件进行统计学分析,结果 表明胎儿及新生儿各生理参数均呈正态分布。超声测量的BPD,OFD,HC,AC,ATD,FL与实际测量的结果均有一定相关性。计算二组数据的相关系数,分别为0.710,0.370,0.587,0.698,0.635,0.826。其中BPD,AC,FL相关性较好。结论 临床可选用双顶径,腹围,股骨长等参数来评价胎儿宫内生长发育及推算胎儿体重。  相似文献

摘要 目的:探究超声多参数在预测妊娠期糖尿病巨大儿分娩方式中的应用价值。方法:选择2016年2月至2020年9月于中国人民解放军联勤保障部队第九○一医院(我院)分娩的156例妊娠期糖尿病产妇为研究对象,回顾性分析其一般临床指标及多普勒超声检测参数,按照新生儿是否为巨大儿区分为巨大儿组(52例)和非巨大儿组(104例),对比两组胎儿超声参数差异,评估胎儿双顶径(biparietal diameter,BPD)、头围(head circumference,HC)、腹围(abdominal circumference,AC)及股骨长度(femur length,FL)与新生儿体重的相关性,纳入年龄、宫高、孕妇腹围、孕妇体重、BPD、HC、AC、FL等指标评估巨大儿单因素影响因素,最后多因素Logistic回归分析探究巨大儿独立危险因素。结果:(1)比较显示巨大儿组胎儿BPD、HC、AC和HL均大于非巨大儿组(P<0.05);(2)Spearman相关性分析显示BPD、HC、AC和HL均与新生儿体重呈正相关(P<0.05);(3)分析显示产妇宫高、产妇腹围、孕期增重、BPD、HC、AC和HL均为巨大儿单因素影响因素(P<0.05);(4)Logistic多因素回归分析显示孕妇宫高、BPD、HC、AC为巨大儿独立危险因素。结论:孕期使用超声多参数对妊娠期糖尿病产妇巨大儿发生情况具有较好的预测价值,可协助医师了解胎儿相关指标,并据此选择合适的分娩方式,值得临床推广应用。  相似文献

目的:生长发育评价标准的最佳形式是年龄别百分位数,然而,目前我国缺乏胎儿的孕周别体格发育指标百分位数评价标准,影响了胎儿宫内生长发育水平的准确评价,本研究拟建立陕西省正常单胎妊娠孕妇胎儿体格发育指标百分位数参考标准。方法:以陕西省西安、汉中、延安5所大型综合医院超声科或妇产科超声室为数据采集点,选择2010-01-01到2010-12-31间进行常规孕期检查的正常单胎妊娠胎儿的超声测量数据,随机抽取一次测量结果,共收集到6832个单胎正常妊娠胎儿的超声测量数据。采用三次样条法对陕西省孕16-41周正常单胎妊娠胎儿的双顶径、腹围、股骨长百分位数进行拟合。结果:三次样条拟合胎儿的双顶径、腹围、股骨长百分位数,各百分位数的决定系数均在0.95以上,均方差均在各指标重复测量的容许误差内,拟合结果满意。计算出了胎儿的孕周别双顶径、腹围、股骨长的P3、P10、P25、P50、P75、P90、P97百分位数。结论:三次样条兼顾了曲线拟合中的拟合优度和光滑度,能更好的实现曲线拟合的目的。所建立的孕周别胎儿体格发育指标百分位数符合胎儿宫内生长发育规律,为胎儿宫内生长发育的准确评价提供了科学、准确的评价工具,可用于产科临床和研究工作中。  相似文献

目的:观察生物蛋白胶封堵合并宫颈环扎术治疗胎膜早破,对孕妇、胎儿及新生儿的影响,明确该技术治疗胎膜早破的疗效。方法:我院产科共收治妊娠<30周胎膜早破患者48例,采用生物蛋白胶封堵合并宫颈环扎术治疗胎膜早破,观察治疗后孕妇羊水指数,胎儿双顶径,新生儿出生时体重,及孕妇的副反应。结果:在本次封堵治疗过程中均一次封堵成功,没有不良反应发生,所有孕妇术后阴道流水情况均明显改善,其中有2例孕妇术后1周阴道流水停止。所有孕妇术后羊水漏出量均明显减少,B超检查显示羊水指数均在(6±2)cm,胎儿每周双顶径增长均达到(0.15±0.02)cm,延长孕周最短(5±2)w,10例早产,38例继续妊娠至37 w。新生儿出生时体重为(2000±260)g,无1例发生NRDS。同期没有进行宫颈内封堵仅行药物保胎治疗的10例孕妇,胎龄延长最长达到10 d,最短者24 h内出现早产分娩,早产儿平均体重1400 g,均发生NRDS,有2例放弃治疗,4例在NICU行治疗,最终因各种并发症死亡。48例行封堵治疗孕妇未发生过敏反应,无阴道出血及产后出血和感染等。结论:应用生物蛋白胶结合宫颈环扎术治疗胎膜早破,手术操作简单,可延长了孕周,提高了新生儿成活率,无不良反应,具有广泛的应用前景。  相似文献

目的:探讨产前手机暴露对子代大鼠海马齿状回增殖细胞核抗原(PCNA)和双皮质素(DCX)表达的影响。方法:构建孕鼠手机射频暴露模型,分为对照组、短时暴露组和长时暴露组(n=6),短时和长时暴露组于孕第1-17天分别给予6 h/d和24 h/d的手机通话暴露,观察孕鼠的孕期长短、孕期体重增长和各组的胎儿数、胎儿出生体重。1月龄子代大鼠行焦油紫染色观察海马齿状回细胞形态,免疫组化观察齿状回PCNA和DCX表达,Western blot检测DCX和脑源性神经营养因子(BDNF)表达。结果:各组孕鼠的孕期、妊娠期体重增长和各组的胎儿数、胎儿出生体重无显著差异,长时暴露组子代大鼠的齿状回多形细胞层锥形细胞和DCX阳性细胞出现形态改变。与对照组、短时暴露组比较,长时暴露组子代大鼠齿状回PCNA阳性细胞和DCX、BDNF表达均明显减少(P<0.05)。结论:产前长时手机暴露可能通过改变子代大鼠海马BDNF而影响齿状回的PCNA和DCX表达。  相似文献

目的:研究运动干预对不同糖耐量超重肥胖孕妇孕期体重增加、妊娠晚期血清游离脂肪酸(Free Fatty Acids,FFA)、甘油三酯(Triglyceride,TG)水平及新生儿体重的影响.方法:2008年6月~2009年12月,对来自高危门诊的的肥胖超重孕妇,分别给予单纯饮食干预(饮食组)或运动饮食联合干预(运动加饮食组).妊娠32周采集两组病例空腹血样,测定FFA及TG.排除各组有运动禁忌的孕妇.足月分娩妊娠期糖尿病(Gestational Diabetes Mellitus,GDM)及正常糖耐量(Normal Glucose Tolerant,NGT)孕妇共143例,运动加饮食组(n=73,GDM 21例,NGT 52例).饮食组(n=70,GDM23例,NGT47例)按不同糖耐量分别进行了对比研究.分析二组在孕期体重增加,孕晚期脂代谢水平及新生儿出生体重方面的差异,并就新生儿出生体重与孕期体重增加及孕晚期血清FFA水平的关系做相关性分析.结果:两组孕妇在年龄,孕前体重指数及分娩孕周方面差别无统计学意义.两组GDM在孕期增重(p<0.01)、新生几出生体重体重(p<0.05)、孕晚期血清FFA(p<0.05)、TG水平(p<0.05)等方面差别有显著性.两组NGT在孕期体重增加(p<0.05)及新生儿出生体重方面差别有显著性(p<0.05);饮食组平均血清FFA及TG水平高于运动加饮食组,但未形成显著性差异.饮食组新生儿出生体重与孕期增重(NGT,r=0.468,P<0.01;GDM,r=0.564,p<0.01)及孕32周血清FFA水平(NGT,r=0.431,P<0.05;GDM,r=0.462,p<0.05)相关有显著性.饮食加运动组新生儿出生体重与孕期增重相关无显著性,与血清FFA相关有显著性(NGT,r=0.375,P<0.05;GDM,r=0.405,p<0.05).两组孕妇在巨大儿发生率方面有显著性差异.结论:肥胖超重孕妇新生儿出生体重与孕晚期血清FFA水平有关.饮食结合运动干预有助于肥胖超重GDM孕妇在孕期体重增加,妊娠晚期血清FFA水平,新生儿出生体重,巨大儿发生率等方面取得良好的妊娠结局.  相似文献

目的:探讨应用脐动脉血流用于预测子痫前期新生儿和产妇结局的临床价值。方法:选择在我院产科建档分娩的120例孕产妇作为研究对象,根据子痫前期发病情况分为子痫前期组60例与对照组60例,记录和比较两组孕产妇的一般资料、血脂、血糖水平、分娩前脐动脉血流与新生儿体重、胎盘的重量及Apgar评分,并进行相关性与危险因素分析。结果:两组孕产妇的年龄、孕次、产次、流产次数、孕周等对比差异均无统计学意义(P0.05)。子痫前期组的血清HDL-C水平低于对照组(P0.05),血清TC、TG、LDL-C、FBG水平高于对照组(P0.05)。与对照组比较,子痫前期组脐动脉S/D、RI与PI值显著升高(P0.05)。所有孕产妇都顺利完成分娩,孕产妇与新生儿都存活,子痫前期组的新生儿出生体重及Apgar评分和胎盘的重量均显著低于对照组(P0.05)。在子痫前期组中,脐动脉S/D、RI、PI值与新生儿出生体重呈现显著负相关性(P0.05)。多重线性回归分析显示子痫前期孕产妇的脐动脉S/D、RI、PI值为影响新生儿出生体重的独立危险因素(P0.05)。结论:脐动脉血流与子痫前期新生儿出生体重显著相关,脐动脉S/D、RI、PI值为影响新生儿出生体重的独立危险因素,子痫前期脐动脉血流监测可为预测新生儿和产妇结局以及预后提供参考。  相似文献

Background  In order to consider the non-human primate as an adequate model for studying prenatal diagnosis and therapy, comparative data on fetal growth should be available.
Methods  Sixty ultrasound scans were performed in 22 baboons between 14 and 167 days of gestation. Measurements included greatest length, head circumference, biparietal diameter (BPD), transcerebellar diameter, abdominal circumference (AC), femur length (FL), and amniotic fluid index. For all parameters growth curves were established and compared with human curves. In 18 animals, birth weight and placental weight were determined. Different equations described in the literature for estimating the human fetal weight were tested in the baboon.
Results  The fetal and placental growth pattern in the baboon was comparable with humans. The best predictor of fetal weight was the formula presented by Combs: 0.23966 × AC2 × FL + 1.623 × BPD3.
Conclusions  A high similarity between baboon and human growth charts is shown. The best equation for estimating the baboon fetal weight is proposed.  相似文献

Background: A comparison of fetal ultrasonographic biometric parameters of the head (head circumference - HC, biparietal diameter - BPD) in breech presented fetuses. Methods: Ultrasound biometry was performed in accordance with the method presented in the reference tables. In all breech presented fetuses, the HC, BPD and FL (femur length) were measured. High-risk and multiple pregnancies were excluded from the study. Results: A total of 111 ultrasonographic biometries were performed between the 31(st) - 38(th) week of gestation. Fetuses in the breech position had a significantly lower BPD compared to HC and FL. The difference between BPD and HC was 16.2 days (95%Cl 14.3-18.1; p = 0.001). Maternal age at delivery was 20 - 36 years (average 28.1; median 28.0). Conclusions: According to our results, fetuses in the breech position have a significantly lower BPD compared to HC or FL. HC and FL parameters correlate with gestational age. In cases of ultrasonographic biometric discrepancy between BPD and FL, the fetal position should be taken into account. Breech-presented fetuses have an elongated head shape and ultrasound biometrics should evaluate its circumference (HC). It is important to responsibly interpret the results so as not to stress the expecting mother with suspicions of fetal pathology.  相似文献

Parturition in spotted hyenas (Crocuta crocuta) is a fascinating event to witness, as females of this species are highly masculinized and give birth through a penis-like clitoris. Furthermore, shortly after birth, a high rate of aggression occurs between littermates that can sometimes end in siblicide. To study these events thoroughly, an accurate estimate of the date of parturition is necessary. To this end, we performed transabdominal ultrasounds every 20-30 days in five captive spotted hyenas of known gestational age, beginning approximately 30 days after mating. We measured the femur length (FL), abdominal circumference (AC), and biparietal diameter (BPD) of eight fetuses from Days 42 to 100 of their 110 days of gestation. FL proved to be the most effective measurement, as it correlated well with gestational age and was easy to obtain consistently. The relationship between estimated gestational age (EGA) and FL is described by the equation: [EGA = 37.3 + (14.0 x FL)]. AC also correlated well with EGA, but was more difficult to measure than FL. Measuring BPD became increasingly difficult as pregnancies advanced beyond 70 days of gestation. Because gestational age is often not known in captive and free-ranging spotted hyenas, measuring fetal FL ultrasonographically is a rapid and reliable way to determine an approximate date of parturition. This technique proved invaluable when used to track and monitor a free-ranging spotted hyena during the days just before and after parturition.  相似文献

Capuchin monkey (Cebus apella) is a new world primate that in recent years has become important in biomedical research. The purpose of this study was to establish and correlate normal fetal growth parameters with gestational age in capuchin monkeys. In seven pregnant animals serial ultrasonic assessment of gestational sac (GS), embryo/fetal greatest length (GL), biparietal diameter (BPD), thorax height (TH), and femur length (FL) were performed. Identification of the GS was possible on day 23+/-2.8 (X +/- SE). The embryo and its heartbeat was detected on day 32.7+/-2.8, the GL being measurable thereafter. By day 45.4+/-1.4 BPD and TH were measurable. FL could only be measured from day 70.6+/-2.1. Predictive regression equations of gestational age (GA) were modeled with data obtained. In addition, preliminary data of fetal heart rate showed a decrease in frequency with advancing gestation.  相似文献

Knowing fetal birth weight could help in detecting abnormalities before parturition. A Caesarean-section or a postnatal management plan could be scheduled. Consequently, the neonatal mortality rate should be decreased. We used ultrasonographic measurements of femur length (FL) or fetal biparietal diameter (BPD), pregnancies, and maternal factors to obtain a model of prediction. For this purpose, linear mixed-effects models were used because of random effects (several fetuses for one queen and a few paired measurements) and fixed effects (litter size, pregnancy rank, weight, wither height, and age of the queen). This study was performed in 24 purebred queens with normal pregnancies and normal body conditions. Queens were scanned in the second half of pregnancy, using a micro-convex probe. They gave birth to 140 healthy kittens whose mean birth weight was 104 g (ranged 65 to 165 g). No correlation between the birth weight and the age of the queen, as a maternal factor alone, was observed. But the birth weight was found to be inversely proportional to the pregnancy rank and the litter size. Moreover, birth weight increased when the weight and wither height of queen increased. BPD and FL increased linearly during pregnancy so a model was used to estimate mean birth weight. Using this model, we found a correlation between mean birth weights and an association of parameters: maternal factors (wither height and age), and litter size.  相似文献
