目的:探讨双极人工股骨头置换在高龄不稳定股骨粗隆间骨折中的疗效。方法:125例患者随机分为两组,观察组采用双极人工股骨头置换术治疗,对照组采用动力髋螺钉内固定术,比较两Harris疗效和手术时间、失血量、下地时间、再手术率和并发症等。结果:观察组手术时间大于对照组,失血量和下地时间小于对照组,差异均具有统计学意义(P〈0.05);观察组优良率82.54%,对照组优良率66.13%,两组优良率差异有统计学意义(Riditz=11.574,P=0.000);对照组共发生并发症15例(24.19%),再手术率6.45%,观察组出现并发症3例(4.76%),无再手术患者,两组并发症发生率和再手术率有统计学意义(Fisher P〈0.05)。结论:对老年不稳定股骨粗隆间骨折行双极人工股骨头置换治疗疗效显著,值得临床推广应用。  相似文献

目的:探讨髋关节置换手术治疗高龄人群不稳定型股骨粗隆间骨折的稳定性及愈后生活质量。方法:选取我院骨科收治的110例不稳定型股骨粗隆间骨折患者作为研究对象,随机分为两组,观察组55例采用人工全髋关节置换术,对照组55例采用DHS内固定治疗,观察两组患者的手术时间、手术出血量、平均住院时间、临床疗效、术后并发症及愈后生存质量。结果:观察组手术时间及手术出血量大于对照组,但患者平均卧床时间及显著短于对照组(P0.05)。观察组手术总有效率为87.27%,对照组手术总有效率为70.90%,观察组手术效果明显优于对照组(P0.05)。观察组术后并发症发生率为7.27%,显著低于对照组(25.45%)(P0.05)。观察组患者生活质量满意度为80.00%,明显高于对照组(61.82%)(P0.05)。结论:髋关节置换手术能减少术中出血量、患者下地活动时间早,局部组织修复速度快,对于体质较差、年龄大的老年患者,该术式是延长预期寿命的最佳手术方式。  相似文献

目的:比较分析不同治疗方式对老年股骨粗隆间骨折的治疗效果。方法:回顾性分析152例老年股骨粗隆间骨折患者的临床资料,保守治疗组24例;手术治疗组128例,其中人工股骨头置换23例、外固定架23例、髓内固定28例(包括股骨近端髓内钉(proximal femoral nail,PFN)、Gamma钉等)、动力髋螺钉(dynamic hip screw,DHS)26例、股骨近端锁定钢板(the locking compress plate,LCP)28例。对各种治疗方式的相关指标进行比较分析。结果:手术治疗组患者卧床时间和并发症发生率均显著优于保守治疗组(t=8.045,7.583,8.673,8.023,8.672,X2=9.124,4.809,6.603,4.276,14.444,P0.05);各手术组之间比较:各组卧床时间无显著差异(F=0.241,P0.05),外固定架组和LCP组手术时间、切口长度、出血量及术后引流量均显著优于其余各组(P0.05);保守组术后Harries评分低于手术组。结论:手术治疗老年股骨粗隆间骨折可获得满意疗效,且并发症较少;外固定架创伤较小,LCP和人工股骨头置换术后疗效较好,其中LCP可作为优先考虑方案。  相似文献

目的探讨PFNA与解剖型锁定钢板治疗老年股骨粗隆间骨折的临床效果。方法将我院收治的120例老年股骨粗隆间骨折手术患者分为两组,对照组使用解剖型锁定钢板内固定方式治疗,观察组使用PFNA内固定方式治疗,观察比较两组患者的手术时间、术中出血量、切口长度、骨折愈合时间、术后下床活动时间等以及髋关节功能Harris评分情况。结果观察组的手术时间、术中出血量、切口长度、骨折愈合时间、术后下床活动时间均低于对照组,组间比较差异有统计学意义(P0.05);髋关节功能Harris评分比较,观察组明显高于对照组,差异有统计学意义(P0.05)。结论老年股骨粗隆间骨折患者行PFNA内固定方式治疗的效果明显优于解剖型锁定钢板内固定,具有创伤小、负重时间短、固定牢靠等优点,可作为老年股骨粗隆间骨折治疗的首选方式。  相似文献

目的:探析全髋关节和双极人工股骨头置换术治疗老年股骨颈骨折的治疗效果。方法:随机将我院收治的98例老年股骨颈骨折病患分为观察组和对照组,针对两组病患的治疗效果进行周详地记录与分析。结果:两组患者在住院时间、手术时间、术中出血量和引流量等方面无显著性差异,相比之下观察组疗效优于对照组。结论:从远期治疗效果上分析,全髋关节置换术优于双极人工股骨头置换术值得在临床实践中广泛开展。  相似文献

目的:探讨股骨近端防旋髓内钉、SL-R柄人工股骨头置换术两种术式治疗老年不稳定型骨质疏松性股骨粗隆间骨折的应用效果。方法:选取2015年1月-2017年6月间于河北医科大学附属秦皇岛市第一医院行手术治疗的老年不稳定型骨质疏松性股骨粗隆间骨折患者98例,以数表法将患者随机均分为对照组(n=49)和观察组(n=49)。其中对照组采用股骨近端防旋髓内钉,观察组采用SL-R柄人工股骨头置换术。比较两组患者临床各项指标、手术前后髋关节功能评分以及术后并发症发生情况。结果:观察组手术时间、术中失血量均高于对照组,但住院时间、开始负重时间则低于对照组(P0.05)。观察组术后并发症发生率为4.08%(2/49),低于对照组的18.37%(9/49)(P0.05)。术后1个月、3个月时患者髋关节各项评分相比术前均有升高,术后3个月的各项评分高于术后1个月(P0.05);观察组术后1个月、3个月各项评分均高于对照组(P0.05)。结论:在老年不稳定型骨质疏松性股骨粗隆间骨折患者治疗过程当中,采用SL-R柄人工股骨头置换术可减少住院时间,疗效确切,无严重并发症,且可改善髋关节各项功能评分。  相似文献

目的:探讨人工髋关节置换术与功能训练对老年股骨粗隆间骨折患者术后康复的效果。方法:选取2013年5月-2016年5月我院收治的老年股骨粗隆间骨折患者191例,选择2013年5月-2016年5月在我院接受治疗的股骨粗隆间骨折患者191例,年龄均65岁,根据治疗方式不同将所选研究对象分为研究组(109例)和对照组(82例)。研究组采用人工关节置换手术治疗,对照组采用内固定手术治疗,两组同时配合功能训练。观察并比较两组患者的手术时间、术中出血量、骨折愈合时间、住院时间、Harris评分及生活质量评分。结果:研究组患者手术时间、术中出血量、骨折愈合时间及住院时间均少于对照组,差异具有统计学意义(P0.05)。两组患者术前Harris评分比较,差异无统计学意义(P0.05);两组患者术后Harris评分均低于治疗前,且研究组低于对照组,差异均具有统计学意义(P0.05)。两组患者术前生活质量评分比较,差异无统计学意义(P0.05);两组患者术后生活质量评分均高于术前,且研究组高于对照组,差异均具有统计学意义(P0.05)。研究组优良率(94.12%)显著高于对照组(80.00%),差异具有统计学意义(P0.05)。两组并发症比较,差异无统计学意义(P0.05)。结论:人工关节置换术配合功能训练能够改善老年股骨粗隆间骨折患者髋关节功能,提高患者术后恢复及生活质量,值得临床推广应用。  相似文献

目的:探讨全髋关节置换术与人工股骨头置换术治疗老年股骨颈骨折的疗效。方法:选择在我院的82例行手术置换治疗的老年股骨颈骨折患者,随机分为观察组和对照组,每组41例。观察组采用全髋关节置换术,而对照组实施人工股骨头置换术。观察并比较两组患者的手术时间、术中出血量、血压、术后引流及髋关节功能等。结果:观察组手术时间、术中出血量及收缩压均显著高于对照组,差异具有统计学意义(P<0.05);术后引流量及并发症的发生率,两组比较无显著性差异(P>0.05)。观察组患者术后Harris评分为(93.25±4.51),对照组患者Harris评分为(82.76±3.82),观察组显著优于对照组,差异具有统计学意义(P<0.05)。结论:与人工股骨头置换术相比,全髋关节置换术用于治疗老年股骨颈骨折具有创伤小,恢复快的临床效果,患者术后髋关节功能恢复情况良好,值得在临床推广应用。  相似文献

目的:探讨全髋关节置换术与股骨头置换术治疗对比。方法:回顾性的分析对2007年1月-2008年12月年我院接收的70例60岁以上的老年股骨颈骨折的进行股骨头置换和全髋关节置换两组不同手术方式的临床资料。结果:两组股骨颈骨折治疗的手术时间、住院时间及术后并发症及治疗效果无明显统计学差异,两组术后半年到一年半的Harris评分较术前评分有显著的统计学意义。结论:人工股骨头置换术和髋关节置换是治疗老年股骨颈骨折的有效方法,可根据患者的自身情况、疾病特点等选择不同术式。  相似文献

It has now been over twenty years since a novel herpesviral genome was identified in Kaposi's sarcoma biopsies. Since then, the cumulative research effort by molecular biologists, virologists, clinicians, and epidemiologists alike has led to the extensive characterization of this tumor virus, Kaposi's sarcoma-associated herpesvirus(KSHV; also known as human herpesvirus 8(HHV-8)), and its associated diseases. Here we review the current knowledge of KSHV biology and pathogenesis, with a particular emphasis on new and exciting advances in the field of epigenetics. Curcumin is the yellow pigment of turmeric that interacts irreversibly forming an adduct with thioredoxin reductase (TrxR), an enzyme responsible for redox control of cell and defence against oxidative stress. Docking at both the active sites of TrxR was performed to compare the potency of three naturally occurring curcuminoids, namely curcumin, demethoxy curcumin and bis-demethoxy curcumin. Results show that active sites of TrxR occur at the junction of E and F chains. Volume and area of both cavities is predicted. It has been concluded by distance mapping of the most active conformations that Se atom of catalytic residue SeCYS498, is at a distance of 3.56 from C13 of demethoxy curcumin at the E chain active site, whereas C13 carbon atom forms adduct with Se atom of SeCys 498. We report that at least one methoxy group in curcuminoids is necessary for interation with catalytic residues of thioredoxin. Pharmacophore of both active sites of the TrxR receptor for curcumin and demethoxy curcumin molecules has been drawn and proposed for design and synthesis of most probable potent antiproliferative synthetic drugs.  相似文献

Microbial resistance to antibiotics is an unresolved global concern, which needs urgent and coordinated action. One of the guidelines of the Centers for Disease Control and Preventions (CDC) to combat antibiotic resistance is the development of new antibiotics to treat drug-resistant bacteria. In our effort to find new antibiotics, we report the synthesis and antimicrobial studies of 30 new pyrazole derivatives. These novel molecules have been synthesized by using readily available starting materials and benign reaction conditions. Some of these molecules have shown activity with MIC values as low as 0.78?µg/mL against four bacterial strains; Staphylococcus aureus, methicillin-resistant S. aureus, Bacillus subtilis, and Acinetobacter baumannii. Furthermore, active molecules are non-toxic to mammalian cell line.

The young pistils in the melanthioid tribes, Hewardieae, Petrosavieae and Tricyrteae, are uniformly tricarpellate and syncarpous. They lack raphide idioblasts. All are multiovulate, with bitegmic ovules. The Petrosavieae are marked by the presence of septal glands and incomplete syncarpy. Tepals and stamens adhere to the ovary in the Hewardieae and the Petrosavieae but not in the Tricyrteae. Two vascular bundles occur in the stamens of the Hewartlieae and Tricyrtis latifolia. Ventral bundles in the upper part of the ovary of the Hewardieae are continuous with compound septal bundles and placental bundles in the lower part. Putative ventral bundles occur in the alternate position in the Tricyrteae and putative placental bundles in the opposite. position in the Petrosavieae. The dichtomously branched stigma in each carpel of the Tricyrteae is supplied by a bifurcated dorsal bundle.  相似文献

Cyclin-dependent kinases (CDKs) and Polo-like kinases (PLKs) play key role in the regulation of the cell cycle. The aim of our study was originally the further development of our recently discovered polo-like kinase 1 (PLK1) inhibitors. A series of new 2,4-disubstituted pyrimidine derivatives were synthesized around the original hit, but their PLK1 inhibitory activity was very poor. However the novel compounds showed nanomolar CDK9 inhibitory activity and very good antiproliferative effect on multiple myeloma cell lines (RPMI-8226).  相似文献
