目的了解本地区凝固酶阴性葡萄球菌(CNS)致新生儿败血症的病原学及耐药性状况,用以指导临床治疗。方法回顾性分析64株CNS致新生儿败血症血培养的病原学及耐药性。结果64株CNS中以表皮葡萄球菌和溶血葡萄球菌为主;耐甲氧西林凝固酶阴性葡萄球菌(MRCNS)39株,分离率为60.9%,β-内酰胺酶检出率达100%,MRCNS对青霉素、红霉素、苯唑西林耐药率最高,对利福平、呋西地酸、替考拉宁、喹奴普汀/达福普汀等耐药率均较低;甲氧西林敏感凝固酶阴性葡萄球菌(MSCNS)25株,分离率为39.1%,13-内酰胺酶检出率为44%,MSCNS对大多数抗生素敏感;64株CNS中未检测到万古霉素耐药株。结论新生儿败血症中CNS以表皮葡萄球菌和溶血葡萄球菌为主,MRCNS检出率高且呈多重耐药,应重视对其监测与控制。  相似文献

目的:分析凝固酶阴性葡萄球菌(CNS)的分布及耐药性,为临床合理应用抗菌药物提供理论依据.方法:对我院2005年1月-2007年12月临床分离的CNS采用琼脂纸片扩散法(K-B法)检测药敏.结果:在426株CNS中共分离13种菌,其中表皮葡萄球菌、溶血葡萄球菌、人葡萄球菌分别为218株、103株、46株,各占51.17%、24.18%和10.80%;CNS对氟霉素、阿米卡星的耐药率小于25%,对万古霉素和替考拉宁的耐药率为0%;对青霉素、笨唑西林、红霉素、头孢西丁、哌拉西林、庆大霉素和环丙沙星的耐药率大于50%.从CNS中检出的耐甲氧西林凝固酶阴性葡萄球菌(MRCNS)和甲氧西林敏感的凝固酶阴性葡萄球菌(MSCNS)分别为350株和76株,分别占82.16%和17.84%;MRCNS对多种抗菌药物耐药,与MSCNS相比差异有显著性统计学意义(p<0.01).结论:需要加强量CNS病原学耐药性监测,了解其分布特点,及时向临床反馈公布,为临床医师合理应用抗菌药物提供依据.  相似文献

本文对临床患者标本中分离的124株凝固酶阴性葡萄球菌(CNS)进行了胞外粘液物(ESS)测定,另由正常人体表分离的20株CNS为对照组,结果发现,临床标本分离出的124株CNS中ESS阴性102株(82.3%),正常人体表分离出的20株CNS中ESS阳性的8株(40.0%),两者有显著性差异(P<0.01),提示ESS阳性的CNS致感染率高(93.3%),可能是由于胆汁标本取自胆管引流管中有关。124株CNS的药敏试验:ESS阳性株对庆大霉素敏感率为14.7%,而ESS阴性株为68.23%,两者有显著性差异(P<0.01)。  相似文献

目的了解重症监护病房(ICU)凝固酶阴性葡萄球菌(CNS)临床分布及耐药情况,以期指导临床合理使用抗菌药物。方法采用VITEK-2细菌鉴定及药敏分析系统,对2009年10月至2010年9月重症监护病房的患者各类标本分离的凝固酶阴性葡萄球菌进行鉴定和药敏试验。结果共检出CNS 189株,以表皮葡萄球菌和溶血葡萄球菌为主,占71.42%。耐甲氧西林凝固酶阴性葡萄球菌(MRCNS)122株,分离率为61.5%,β-内酰胺酶检出率达100%,MRCNS对青霉素、红霉素、苯唑西林耐药率最高,分别达100%、93.44%、91.80%,对复方新诺明、喹诺酮类药的耐药率次之,对利福平、呋西地酸、替考拉宁、喹奴普汀/达福普汀等耐药率均较低;甲氧西林敏感凝固酶阴性葡萄球菌(MSCNS)67株,分离率为38.5%,β-内酰胺酶检出率为61.1%,MSCNS对大多数抗生素敏感;189株CNS中未检测到万古霉素耐药株。结论 ICU分离的CNS以表皮葡萄球菌和溶血葡萄球菌为主,MRCNS检出率高且呈多重耐药,万古霉素、呋西地酸、替考拉宁、喹奴普汀/达福普汀是治疗MRCNS感染的首选药物。  相似文献

目的了解血流感染中葡萄球菌的菌种分布及其耐药特性,以指导临床合理使用抗菌药物。方法回顾性分析南昌大学第二附属医院2013年1月至2013年8月期间临床各科室送检的血培养标本中葡萄球菌的菌种分布和耐药特性。采用BACT/ALERT 3D全自动血培养仪进行培养,VITEK2-compact全自动细菌分析仪进行菌种鉴定和药敏试验。WHONET 5.6软件分析数据。结果共分离获得176株葡萄球菌,其中金黄色葡萄球菌24株,占13.6%;路邓葡萄球菌1株,占0.6%;凝固酶阴性葡萄球菌(CNS)151株,占85.8%,人葡萄球菌53株、溶血葡萄球菌32株、表皮葡萄球菌23株及头状葡萄球菌15株居前四位。最常见的葡萄球菌分别为人葡萄球菌、溶血葡萄球菌、金黄色葡萄球菌及表皮葡萄球菌。耐甲氧西林金黄色葡萄球菌(MRSA)和耐甲氧西林CNS(MRCNS)检出率分别为29.2%和85.4%,对青霉素耐药率均100.0%,磺胺甲噁唑对MRSA(耐药率仅14.3%)有较强的体外抗菌活性,利福平对MRCNS(耐药率仅18.2%)有较强的体外抗菌活性,均未发现耐万古霉素、利奈唑胺和替加环素的菌株。结论葡萄球菌引起的血流感染菌种分布较广,以人葡萄球菌、溶血葡萄球菌、金黄色葡萄球菌及表皮葡萄球菌为主,人葡萄球菌较金黄色葡萄球菌耐药及多重耐药性严重,但均仍对万古霉素、利奈唑胺、替加环素保持高度敏感。  相似文献

目的了解玉溪市人民医院临床分离凝固酶阴性葡萄球菌(CNS)的菌种构成和耐药特征。方法对该院近6年来临床分离的808株CNS作回顾性分析。结果808株CNS共有16种;表皮葡萄球菌、腐生葡萄球菌和溶血葡萄球菌占CNS的77.2%;甲氧西林耐药(MRCNS)和甲氧西林敏感(MSCNS)各占63.3%和36.7%;β-内酰胺酶阳性检出率为84.5%,且MRCNS显著高于MSCNS(χ^2=50.88,P〈0.01);药敏结果:MRCNS除万古霉素、呋喃妥因和利福平外,青霉素、氨苄西林等12种临床常用抗菌药物的耐药率在50.6%-100%,显示了高度的耐药性和多重耐药。结论CNS在临床标本中的检出范围广、种类多,以表皮葡萄球菌、腐生葡萄球菌和溶血葡萄球菌为主,MRCNS的高度耐药和多重耐药形势严重,加强CNS的细菌学监测非常必要。  相似文献

目的了解本院儿童血培养凝固酶阴性葡萄球菌(CNS)的感染率及其药物敏感情况,为儿科合理使用抗生素提供依据。方法对本院2006年1月至2007年12月间住院及门诊儿童血液培养的结果进行回顾性统计分析。结果在1265例儿童血培养中共检出CNS117株,其中表皮葡萄球菌41株(占35.0%),人葡萄球菌27株(占23.1%),溶血葡萄球菌21株(占18.0%),其他凝固酶阴性葡萄球菌28株(占23.9%);耐甲氧西林凝固酶阴性葡萄球菌(MRCNS)分离率为79.5%;MRCNS药敏结果显示多重耐药。结论CNS已成为儿童血液感染的重要致病菌,MRCNS检出率高且多重耐药,万古霉素、喹宁始霉素-达福普汀、呋西地酸是治疗MRCNS感染的首选药物。  相似文献

目的:研究葡萄球菌感染病原菌的分类、分布特点及对常用抗生素的耐药性。方法:应用MicroScan WalkAway-40全自动微生物分析仪对临床标本中分离的826株葡萄球菌进行鉴定和药敏试验。结果:826株葡萄球菌感染中,其中金黄色葡萄球菌(SA)294株占35.6%,表皮葡萄球菌为主的凝固酶阴性细菌(CNS)532株占65.4%。葡萄球菌分离株主要来源于呼吸道(46%)和泌尿道(23.7%)。MRS的产生率分别为金黄色葡萄球菌77.9%,表皮葡萄球菌80.3%,溶血葡萄球菌74.8%,腐生葡萄球菌82.5%,其他78.5%。MRSA、MRCNS对青霉素G、氨苄西林、苯唑西林几乎全部耐药,对万古霉素、阿米卡星、利福平、头孢哌酮/舒巴坦仍敏感,但发现3例万古霉素低敏株。结论:葡萄球菌感染仍以呼吸道和泌尿道为主,MRS菌株高达70%以上,万古霉素、阿米卡星、利福平、头孢哌酮/舒巴坦仍是治疗的首选药物。  相似文献

目的分析凝固酶阴性葡萄球菌(CNS)的临床分布及耐药情况,指导临床合理用药。方法采用ATB-Expression细菌鉴定系统对厦门市妇幼保健院2009年1月至2009年12月分离的206株CNS进行鉴定,用K-B纸片法检测其对苯唑西林等10种抗菌药物的敏感度。结果206株CNS菌分布于8类临床标本,血液、生殖道标本分离率较高,分别为52.91%、25.24%。耐甲氧西林(MRS)的发生率分别为:模仿葡萄球菌59.09%,溶血葡萄球菌95.83%,表皮葡萄球菌80.00%,人型葡萄球菌85.71%。对青霉素、红霉素耐药率较高,分别为85%和82%以上;对左氧氟沙星、克林霉素耐药性次之;未检出对万古霉素、替考拉宁耐药菌株。结论该院分离的CNS耐药性严重,MRS发生率高达59%以上,对红霉素、克林霉素高度耐药,临床应尽量减少青霉素、红霉素等药物的经验性用药,依据药敏结果选择抗菌药物进行治疗,防止耐药菌株爆发院感流行。  相似文献

目的:研究葡萄球菌的感染及耐药性状况,指导临床合理用药。方法:常规方法及阳光微生物鉴定仪鉴定菌株。药敏试验用革兰阳性需氧球菌药敏检测卡,检测耐甲氧西林金黄色葡萄球菌(MRSA)和耐甲氧西林凝固酶阴性葡萄球菌(MRCNS),金黄色葡萄球菌(SA)和凝固酶阴性葡萄球菌(CNS)对12种抗生素的耐药率。结果:158株葡萄球菌中,MRSA占47.5%(78/158),MRCNS占21.5%(34/158),SA占70.9%(112/158),CNS占29.1%(46/158)。耐甲氧西林葡萄球菌(MRS)对12种抗生素的耐药率均高于甲氧西林敏感葡萄球菌(MSSA)。结论:MRSA的耐药性较高,需加强其抗生素的耐药性检测。  相似文献

(1) The chemical composition of the CNS (separated into cerebrum, cerebellum, brain stem and spinal cord) was determined in sheep during foetal and post-natal development and in adults. (2) The spinal cord differed from the remainder of the CNS in growing more after the period studied (50-day-old foetuses to 5-week-old lambs) than before it. This was largely attributable to lipid accumulation. (3) Chemical growth (accumulation of DNA, protein and lipid) proceeded linearly in spinal cord, logarithmically in cerebrum and cerebellum while in brain stem growth was described by a sigmoid function. (4) Fat-free dry matter, protein, total lipid, cholesterol and phospholipid concentrations increased progressively in all parts of the CNS but DNA concentrations changed little. In the cerebrum alone there was an increase in DNA concentration during maturation suggesting an increased cell population. Cholesterol was present predominantly in the free form but esters were detected in foetal tissues from 70 up to 120 days gestation. (5) Cerebroside, the characteristic lipid of myelin, increased in concentration soon after 85 days of gestation, up to which point very low values were recorded, the rate varying according to the region of the CNS examined. Rates of increase in total regional cerebroside content were used to identify periods of myelination and the results suggest that there are two periods of peak activity, one about 20 days before birth and the other at 10-20 days after birth. (6) The composition of lipids added during the two phases of myelination and during maturation were characteristically different. In the spinal cord, lipid analyses best reflect changes in myelin composition.  相似文献

目的:了解近9年来医院感染葡萄球菌菌种的变迁与近3年来葡萄球菌药状况。方法:1993年1月至2001年12月我院传染病科等13科室住院病人的各种标本采用血琼脂培养,所分离的葡萄球菌采用美国DADE公司生产的MICROSCAN WALKAY-40全自动微生物分析仪鉴定到种及其亚种。药敏试验药物有青霉素、苯唑西林、氨苄西林、哌拉西林、阿莫西林/克拉维酸(阿莫仙)、头孢唑啉、头孢噻肟、头孢哌酮/舒巴坦(舒普深)、庆大霉素、复方新诺明、环丙沙星、氧氟沙星、利福平、万古霉素、红霉素、林可霉素、四环素、伊米配能/西司他丁(泰能)共18种。采用液体稀释法测定每株葡萄球菌对受试药物的最小抑菌浓度(MIC),操作按说明书进行。质控菌ATCC25923。依据新近NCCLS标准判读结果。结果:1993年至1998年分离的葡萄球菌中,金黄色葡萄球菌(金葡萄)占71.43%,凝固酶阴性葡萄球菌(CNS)占28.57%,包括表皮葡萄球菌(表葡菌)、腐生葡萄球菌、里昂葡萄球菌和头状葡萄球菌4种。1999年至2001年分离的424株葡萄球菌中,金葡菌仅占29.01%,CNS增至13种,占70.995,以表葡菌、溶血葡萄球菌、松鼠葡萄球菌为主。近3年来分离的各种葡萄球菌甲氧西林耐药率在73.03%-100%之间,除对舒普深、复方新诺明、利福平和万古霉素较敏感外,对其余抗菌药物的耐药率均超过60%,以金葡菌、溶血葡萄球菌、松鼠葡萄球菌的耐药率为最高。MRS耐药率普遍高于MSS,且均呈多重耐药。5.42%(23/424)菌株万古霉素MIC>16mg/L,除1株为MSCNS外,其余22株均为MRS。结论:3年来医院感染葡萄球菌菌种构成比发生了显著变化,以CNS为主。对抗菌药物呈多重耐药,部分菌株对万古霉素敏感性降低,应予警惕。  相似文献

Extracts containing myelin basic proteins have been prepared from CNS tissue of representatives of the three suborders of RodentiaMyomorpha, Hystricomorpha and Sciuro-morpha. Analyses of the extracts by electrophoresis at low pH showed that one type (L) of myelin basic protein is present in the CNS of all of the rodents examined (rat, mouse, hamster, guinea pig, chinchilla, prairie dog, woodchuck and squirrel). This protein is comparable in molecular size and charge to the CNS myelin basic proteins found in several other mammalian orders. In the CNS of the myomorphs (rat, mouse, hamster) and sciuro-morphs (prairie dog, woodchuck, squirrel) there is an additional type (S) of myelin basic protein of higher cathodic mobility and smaller molecular size. This additional protein is absent from the CNS of the hystricomorphs (guinea pig, chinchilla). These findings indicate that the presence of two myelin basic proteins originally reported in the CNS of the inbred rat is not an anomaly of inbreeding. These data further suggest that the presence of a single L-type CNS myelin basic protein might be a general characteristic of hystricomorphs, while the presence of both L- and S-type CNS myelin basic proteins might be a general characteristic of the myomorphs and sciuromorphs. Analyses by electrophoresis at low pH failed to reveal differences in mobility among either the L-type or the S-type CNS myelin basic proteins of the different species. In contrast, when electrophoresis was carried out cathodically at high pH, species-related differences in mobility were observed among the L- and S-type CNS myelin basic proteins.  相似文献

Staphylococcal DNA was digested with endonucleases and probed with labelled ribosomal RNA (rRNA) from Escherichia coli. Reproducible restriction patterns containing between seven and 22 bands were obtained for seven different species of staphylococci. These profiles were species-specific with different strains of a particular species sharing an identical or similar restriction pattern. The results reported here indicate that rRNA gene restriction pattern analyses have an application in the taxonomy of staphylococci.  相似文献

Miraglia, Gennaro J. (Bryn Mawr College, Bryn Mawr, Pa.) and L. Joe Berry. Possible source of secondary invading staphylococci in mice exposed to acute cold. J. Bacteriol. 85:345-348. 1963.-In an effort to determine the origin of the staphylococci known to invade the deep tissues (liver, spleen, kidneys, lungs, and heart) of mice exposed continuously to 5 C in individual compartments without bedding, the intestinal tract was freed of these organisms, as judged by absence of growth when fecal suspensions were inoculated into selective media. Substitution of 0.01 n hydrochloric acid for drinking water eliminated staphylococci within a few days, yet the incidence of tissue invasion was unaltered. The coagulase-negative strains normally present in feces and in tissues persisted in tissues even though the intestine was seeded with a coagulase-positive strain by feeding contaminated food. Cultures from the external nares continued unaltered. This suggests that the respiratory tract is a possible origin of the staphylococci found in tissues of the cold-stressed mice.  相似文献

为了观察急性低氧条件下中枢神经系统机能状态的改变,用改进了的方法测定了健康人乘车抵达海拔4700m高原现场前后,和在实验室内吸入低氧混合气体(相当于海拔4500~4700m)前后的“光单纯反应时”。在两种条件下,所得结果基本相同:人体中枢神经系统对急性低氧的反应有明显个体差异,可区别出稳定型、兴奋型和抑制型三类。其中抑制型现场和实验室分别有83%及75%的人出现急性低氧反应,其他两型合并统计,分别有79%及80%的人基本无反应。反应时抑制型的人急性低氧反应的发生率高。反应时测定方法用来评定急性低氧反应出现程度,其结果与症状学评定法相比,在两种条件下,吻合率分别为81%及78%,不吻合率分别为19%及22%,其中假阳性率分别为6%及10%,假阴性率分别为14%及12%。  相似文献

目的了解凝固酶阴性葡萄球菌的耐药性情况。方法分析2008年至2010年沈阳军区总医院临床分离的484株凝固酶阴性葡萄球菌的药敏结果。结果 2008年至2010年凝固酶阴性葡萄球菌的分离率分别为4.7%、3.6%和3.1%。耐甲氧西林凝固酶阴性葡萄球菌(MRSCN)分别为76.6%、78.5%和79.9%。青霉素G、红霉素和苯唑西林的耐药率较高,平均分别为91.2%、87.5%和82.6%。万古霉素、替考拉宁和利奈唑胺的耐药率均为0。结论 2008年至2010年凝固酶阴性葡萄球菌检出率相对稳定,对常用抗菌药物的耐药率相对稳定,这为临床用药选择及加强感染控制提供了有力指导。  相似文献

(1) The assumption that the circulatory system of the insect is instrumental in transporting insecticides to their site of action appeared not to be based on good evidence. On the contrary, experiments specifically designed to test the hypothesis provided ample evidence to refute the idea. Salient points to disprove the haemolymph route of entry can be summed up as follows: (a) A topically applied dose of insecticide does not readily penetrate the insect and the minor fraction that does so is largely retained in the body wall. The small amount that actually passes into the blood is too small to cause symptoms of toxicity if injected into the haemocoel. (b) The amount of insecticide present in the haemolymph (and CNS) does not appear to have any bearing on toxicity - internally introduced insecticides are, in fact, basically very much less toxic than those externally applied. Where injected doses appear to be more toxic than equal amounts topically applied, this is due to the boosting effect of organic solvent carriers. Tests with parabiotically joined insects provide support for the view that haemolymph-borne insecticide is of no consequence. (c) The topographical proximity of the locus of external application with the site of action (the thoracic ganglia) seems to be important irrespective of the general direction of the blood flow, and this should not be so if the circulatory system was instrumental in the transport of insecticide. (d) The introduction into the haemocoel of material such as olive oil, which is an excellent absorbent for insecticides, does not affect the toxicity (speed of action) of externally applied compounds to a significant extent. It should have a pronounced effect if haemolymph-borne insecticide were an essential element in the process of poisoning. (e) As judged by speed of action, blocking of the blood circulation does not hamper the insecticide's movement to the site of action. (2) Only two other feasible routes remain. (a) The insecticide might reach the CNS via peripheral nerves and nerve cord, but the results of histochemical assays of cholinesterase inhibition in the insect's CNS make the idea improbable for organo-phosphates. The nerve route is also incompatible with the observation that a wax barrier blocked the movement in and over the body wall so as to delay the onset of symptoms of toxicity, as such a barrier would not hinder movement into lateral nerves near the locus of application. (6) The only other feasible alternative, i.e. entry by means of lateral transport via the integument of the body wall and tracheae, is supported by autoradiographic and other evidence which showed the insecticide to accumulate in the tracheal system. It is further supported by the fact that inter-tracheal introduction is faster acting than topical treatment, indicating that the tracheal system offers a very effective pathway to the internal organs. (3) Regarding the mechanism of entry, earlier reviews and text books maintain this to be associated exclusively with penetration into and through the integument by a physicochemical process. However, there is good evidence to show that an active process (requiring metabolic activity as the driving force) plays an essential part in lateral movement in the integument - the epidermis, being the only living tissue continuous throughout the general integument, must perform this function. (4) As to the mode of action, the new hypothesis expounded here implies a single mode based on the fact that insecticides cause the extrusion of fluid from the epidermis into the cuticle and beyond, fluid lost from the epidermal cell layer being replaced from haemolymph and internal tissues. The precise mechanism is not clear but could conceivably involve an as yet hypothetical local endocrine system designed to keep the water content in the integument within certain limits. It is suggested that water extrusion affects the integument's permeability to respiratory gases, resulting in a rate of respiration not commensurate with metabolic need, and that the insecticide's arrival in the trachea (tracheoles) of the CNS leads to excitation and knockdown. Death is thought most likely to be due to dehydration of the CNS and subsequent histological degeneration.  相似文献

The sequence of differentiation of major elements of the skeletal, muscular and nervous systems of the head is examined in developmental series of five eutherian (placental) and four metatherian (marsupial) mammals. The analysis identifies the elements that are conserved across the Theria, those that are unique to the Metatheria and to the Eutheria, and those that are variable. It is shown that although there are slight shifts in the sequence of development within the somatic tissues of the head, the primary difference between marsupial and placental mammals involves the timing and rate of differentiation of structures of the central nervous system (CNS) relative to a specific subset of structures of the cranial skeleton and musculature. In eutherians, CNS morphogenesis is well underway before the somatic tissues of the head begin differentiation. In metatherians, CNS development is delayed considerably and certain elements of the skeletal and muscular systems are advanced. It is concluded that the developmental differences between marsupial and placental mammals are best explained by the interaction of several processes including neurogenesis as a potential rate-limiting step, the developmental requirements of somatic elements, and the extremely short period of organogenesis of marsupial mammals. Several other issues, including the way that these data may be applied to determine the primitive therian developmental condition, and the use of comparative developmental data to address basic questions on morphogenetic processes, are discussed.  相似文献

The level of antibiotic-sensitivity of 73 S. aureus strains isolated from children with dysbacteriosis of the large intestine in an outpatient clinic was determined. The isolation rate of polyresistant strains was 44%. Methicillin-resistant S. aureus (MRSA) were isolated from 25 children (34.2%). 60% of MRSA strains could not be typed with the international set of phages. Among the strains capable of being lyzed by the phages the representatives of phage groups 3 and 4 prevailed. All MRSA strains were sensitive to vancomycin, 84-88% of the strains were sensitive to chloroamphenicol, rifampicin, spiramycin and neomycin, 80% of the strains were sensitive to fusidin and phosphomycin. The level of sensitivity of methicillin-sensitive S. aureus strains (MSSA) to different groups of antistaphylococcal antibiotics was higher. 36-64% of MRSA strains and 21-27% of MSSA strains were resistant to the action of curative bacteriophages.
