目的:探讨经利用网片的前盆底重建术治疗重度盆腔器官脱垂的临床效果。方法:选取2013年1月-2015年3月我院妇科收治的以前中盆腔联合缺陷为主的盆底障碍性疾病患者39例(POP-Q分度III-IV度)。实施经阴道Avaulta前盆腔重建术,24例切除子宫,9例保留子宫,其中6例因中盆腔重度脱垂联合行阴道残端骶棘韧带固定术,2例合并尿失禁同时行经尿道无张力悬吊术。术后随访12-25月,根据盆腔脏器脱垂评分(POP-Q)测量及PFIQ-7评分进行客观评价及主观感受评价进行中短期的临床效果分析。结果:39例患者手术过程顺利,术后无严重并发症,POP-Q评分均较术前显著降低,客观疗效理想。术后阴道前壁及子宫、阴道穹窿脱垂等症状显著改善;到目前为止无一例复发。结论:经阴道前盆腔重建术治疗前盆腔重度盆腔脱垂为主以及合并尿失禁等复杂情况的并发症少,治愈率高,复发率低,临床效果好。  相似文献

目的探讨子宫脱垂(uterine prolapse,UP)患者阴道前壁中Fibulin-3 mRNA和蛋白的表达及其与子宫脱垂临床因素的关系。方法应用免疫组织化学S-P法及RT-PCR法对50例子宫脱垂患者的阴道前壁组织中Fibulin-3蛋白及mRNA水平表达进行测定,并与对照组20例(宫颈上皮内瘤变或妇科良性肿瘤行经阴道子宫全切术的非脱垂患者)组织对比,结合子宫脱垂的临床参数进行分析。结果免疫组化结果显示子宫脱垂组织中Fibulin-3蛋白表达的阳性率为52.0%(26/50),明显低于对照组85.0%(17/20),III+Ⅳ度子宫脱垂患者阴道前壁Fibulin-3蛋白阳性表达率明显低于I+II度子宫脱垂组(P=0.009),伴有压力性尿失禁(SUI)的子宫脱垂患者Fibulin-3蛋白表达较无SUI患者明显下降(P=0.004)。RT-PCR结果显示子宫脱垂患者阴道前壁组织中Fibulin-3 mRNA的相对表达量较对照组明显降低,且其含量与子宫脱垂的严重程度及是否伴有SUI相关,差异具有统计学意义。Fibulin-3蛋白和mRNA表达与患者阴道分娩次数无关(P值均〉0.05)。结论Fibulin-3蛋白和mRNA在子宫脱垂患者阴道前壁组织中存在低表达,并与子宫脱垂的严重程度以及是否伴有SUI密切相关,Fibulin-3可能在子宫脱垂的发生发展中起重要的作用。  相似文献

目的探讨子宫脱垂(Uterine prolapse,UP)患者子宫主韧带和阴道前壁中神经丝蛋白(Neurofilament protein,NFP)组织的表达及其与子宫脱垂临床因素的关系。方法应用免疫组织化学S-P法对50例子宫脱垂患者的子宫主韧带和阴道前壁组织中神经丝蛋白表达进行测定,并与10例对照组组织对比,结合子宫脱垂的临床参数进行分析。结果免疫组化结果显示子宫脱垂患者阴道壁、主韧带组织中神经丝蛋白表达的阳性率为28.0%、32.0%,明显低于对照70.0%、80.0%,Ⅱ度子宫脱垂患者阴道壁、主韧带组织中神经丝蛋白表达阳性表达率明显低于Ⅲ+Ⅳ度子宫脱垂组(45.45%vs14.29%、50.0%vs17.86%)。结论子宫脱垂患者子宫主韧带和阴道前壁组织中神经丝蛋白表达下降,并与脱垂程度相关,提示神经丝蛋白可能在子宫脱垂的发生发展中起重要的作用。  相似文献

目的对300例细菌性阴道病(Bacterial Vaginosis,BV)患者随机使用甲硝唑(100例)、乳杆菌活菌制剂定君生(100例)及甲硝唑联合定君生(100例),观察乳杆菌在阴道内定植情况及用药前后阴道内微生物多样性的变化,为临床治疗细菌性阴道病提供实验依据。方法分别提取300例BV患者用药前和用药后的阴道微生物总基因组DNA,以德氏乳杆菌16SrDNA特异性引物进行PCR,检测患者用药后德氏乳杆菌在阴道内的定植情况。以细菌16SrRNAV3区通用引物进行PCR,扩增产物进行变性梯度凝胶电泳(DGGE),对所得指纹图谱进行分析,研究用药对微生态变化的影响状况。结果甲硝唑治疗组用药前后均没有德氏乳杆菌检出;定君生治疗组用药后德氏乳杆菌检出率为54%;甲硝唑联合定君生治疗组用药后德氏乳杆菌检出率为76%。甲硝唑治疗组用药后微生物多样性显著降低,微生物群结构失衡;定君生治疗组用药后微生物多样性有所降低,微生物群结构无显著差异;甲硝唑联合定君生治疗组用药后微生物多样性降低,但较前两组有所增加,微生物群结构差异无统计学意义。结论甲硝唑显著降低BV患者阴道微生物多样性,引起微生态失衡;定君生对BV患者阴道微生态的影响明显小于甲硝唑;定君生的使用有利于甲硝唑f预后阴道微生物多样性的恢复和微生态平衡的重建。  相似文献

目的:探讨应用保妇康栓联合小剂量雌激素治疗老年阴道炎患者的临床疗效及其对血清雌激素水平和生活质量的影响。方法:选择我院于2017年1月-2019年3月收治的老年阴道炎患者96例,按随机数字表法将其分为研究组(n=48)与对照组(n=48)。对照组应用高锰酸钾进行冲洗消毒加已烯雌酚片治疗,研究组在此基础上应用保妇康栓治疗。观察两组治疗效果和治疗前后孕酮(P)、雌二醇(E_2)、黄体生成激素(LH)及卵泡刺激素(FSH)变化以及SF-36生活质量量表评分变化。结果:研究组阴道分泌物异常、外阴瘙痒或灼热、阴道黏膜萎缩症状消失时间短于对照组,总有效率高于对照组(P0.05)。治疗后两组患者血清雌激素水平与SF-36量表各子项评分均较治疗前有改善,且研究组P、LH及FSH水平低于对照组(P0.05),SF-36量表中的总体健康、社会功能、生理功能、躯体疼痛、精神健康、活力评分高于对照组(P0.05)。结论:在小剂量雌激素基础上联合保妇康栓治疗可有效提升老年阴道炎的临床有效率,改善患者的雌激素水平,快速缓解阴道分泌物异常、外阴瘙痒或灼热、阴道黏膜萎缩症状的同时还可提高患者的生活质量,疗效令人满意,具有较好的临床价值。  相似文献

摘要 目的:探讨血清miR-21表达水平与子宫脱垂和阴道壁脱垂的相关性。方法:选取我院2017年12月到2020年12月共收治的60例子宫脱垂和阴道壁脱垂的患者作为观察组,另取60例来我院体检的健康女性作为对照组,分析两组患者胶原蛋白Ⅰ、Ⅲ和miR-21表达水平,并分析血清miR-21表达水平与子宫脱垂和阴道壁脱垂的相关性。对所有患者经盆底重建术和相关治疗之后,将术后1年子宫和阴道壁基本复位未见脱垂,且POP-Q分度<Ⅱ度的40例患者分为预后良好组,将术后1年内再次发生子宫脱垂和阴道壁脱垂,且POP-Q分度≥Ⅱ度的20例患者分为预后不良组,对比两组患者的临床情况,并分期血清miR-21表达水平对于子宫脱垂和阴道壁脱垂的预后预测价值。结果:两组患者胶原蛋白Ⅰ、Ⅲ和血清miR-21表达水平对比差异显著,观察组患者胶原蛋白Ⅰ、Ⅲ水平低于对照组,观察组患者血清miR-21水平高于对照组(P<0.05);Spearman相关分析结果显示:胶原蛋白Ⅰ、Ⅲ水平与子宫脱垂和阴道壁脱垂呈负相关,血清miR-21与子宫脱垂和阴道壁脱垂呈正相关(P<0.05);预后良好组与预后不良组患者孕次、脱垂部位、胶原蛋白Ⅰ、Ⅲ情况对比无差异(P>0.05),预后良好组与预后不良组患者年龄、BIM、产次、miR-21情况对比有差异(P<0.05);logistic回归分析结果得出,只有血清miR-21水平和BMI是子宫脱垂和阴道壁脱垂的预后独立因素(P<0.05)。结论:胶原蛋白Ⅰ、Ⅲ和血清miR-21表达水平与子宫脱垂和阴道壁脱垂疾病具有明显相关性,因此可以考虑应用三种指标对患者进行诊断和治疗参考。而仅有血清miR-21水平和BIM水平是子宫脱垂和阴道壁脱垂的预后独立因素,可以用两者的水平来判断患者的预后情况,及时采取措施提升治疗方法。  相似文献

摘要:目的 研究复合微生态制剂对子宫内膜炎家兔模型的治疗效果。方法 通过临床症状、子宫剖检和病理组织切片观察治疗效果。结果 家兔模型精神状态差,阴道流出粘性分泌物,剖检子宫严重充血、水肿,复合微生态制剂治疗后,症状明显减轻。结论 复合微生态制剂对子宫内膜炎家兔模型有明显的治疗效果,为今后临床治疗奶牛子宫内膜炎提供试验依据。  相似文献

阴道微生态是由阴道的局部解剖结构、周期性的内分泌变化、阴道局部免疫系统和阴道内微生物菌群共同组成的阴道环境和生态系统。多项研究证实阴道微生态失调通过局部炎症因子释放、黏膜免疫应答的改变和局部代谢变化,可引起早产的发生。阴道内小分子物质如糖类、短链脂肪酸和胺类通过代谢路径和代谢产物在阴道微生态失调与早产的发病机制起作用。近年来,阴道微生态的理念得到重视,治疗方法由传统的抗生素治疗转向了综合治疗,目的是恢复正常阴道菌群和阴道上皮黏膜免疫系统。但阴道微生态与宿主之间存在复杂的相互作用,因此仍需要进一步的研究。  相似文献

Two mouse models of pelvic organ prolapse have been generated recently, both of which have null mutations in genes involved in elastic fiber synthesis and assembly (fibulin 5 and lysyl oxidase-like 1). Interestingly, although these mice exhibit elastinopathies early in life, pelvic organ prolapse does not develop until later in life. In this investigation we developed and validated a tool to quantify the severity of pelvic organ prolapse in mice, and we used this tool prospectively to study the role of fibulin 5, aging, and vaginal proteases in the development of pelvic organ prolapse. The results indicate that >90% of Fbln5(-/-) mice develop prolapse by 6 mo of age, even in the absence of vaginal delivery, and that increased vaginal protease activity precedes the development of prolapse.  相似文献

Matrix metalloprotease (MMP) activity is increased in the postpartum vagina of wild-type (WT) animals. This degradative activity is also accompanied by a burst in elastic fiber synthesis and assembly. The mechanisms that precipitate these changes are unclear. The goals of this study were to determine how vaginal distention (such as in parturition) affects elastic fiber homeostasis in the vaginal wall and the potential significance of these changes in the pathogenesis of pelvic organ prolapse. Vaginal distention with a balloon simulating parturition resulted in increased MMP-2 and MMP-9 activity in the vaginal wall of nonpregnant and pregnant animals. This was accompanied by visible fragmented and disrupted elastic fibers in the vaginal wall. In nonpregnant animals, the abundant amounts of tropoelastin and fibulin-5 in the vagina were not increased further by distention. In contrast, in pregnant animals, the suppressed levels of both proteins were increased 3-fold after vaginal distention. Distention performed in fibulin-5-deficient (Fbln5(-/-)) mice with defective elastic fiber synthesis and assembly induced accelerated pelvic organ prolapse, which never recovered. We conclude that, in pregnant mice, vaginal distention results in increased protease activity in the vaginal wall but also increased synthesis of proteins important for elastic fiber assembly. Distention may thereby contribute to the burst of elastic fiber synthesis in the postpartum vagina. The finding that distention results in accelerated pelvic organ prolapse in Fbln5(-/-) animals, but not in WT, indicates that elastic fiber synthesis is crucial for recovery of the vaginal wall from distention-induced increases in vaginal protease activity.  相似文献

目的:观察生物反馈电刺激联合Kegel训练对产后盆底功能障碍性疾病(PFD)患者盆底功能电生理指标和生活质量的影响。方法:研究对象为2018年3月~2020年12月我院收治的80例产后PFD患者。采用双色球随机分组法将患者分为对照组(n=40)和研究组(n=40)。对照组给予Kegel训练,研究组给予生物反馈电刺激联合Kegel训练,两组均治疗8周。对比两组治疗8周后的疗效和尿失禁、盆底器官脱垂程度的改善情况。对比两组治疗前、治疗8周后的盆底功能电生理指标、日常生活质量和性生活质量。结果:治疗8周后,研究组的临床总有效率较对照组高(P<0.05)。治疗8周后,研究组I类肌纤维疲劳度、Ⅱ类肌纤维疲劳度、快肌最大肌电值及阴道动态压力均优于对照组(P<0.05)。研究组治疗8周后尿失禁、盆底器官脱垂程度的改善情况优于对照组(P<0.05)。治疗8周后,研究组盆底功能影响问卷简表(PIFQ-7)评分、盆腔器官脱垂-尿失禁性功能问卷(PISQ-12)评分均低于对照组(P<0.05)。结论:产后PFD患者采用生物反馈电刺激联合Kegel训练治疗疗效明确,可促进尿失禁、盆底器官脱垂程度情况及盆底功能改善,提高患者日常生活质量和性生活质量。  相似文献

目的:研究探讨补中益气汤联合生物反馈电刺激对产后盆底功能障碍患者的临床治疗效果及对其盆底肌肉张力的影响。方法:选择我院就诊的90例产后盆底功能障碍气虚证患者(2017年11月~2018年10月),按照随机数字表法划分为30例/组的A组、B组、C组,A组实施常规盆底肌锻炼,B组采用盆底治疗仪实施生物反馈电刺激治疗,C组实施生物反馈电刺激联合中药补中益气汤治疗,比较三组的疗效、盆底肌力评分、阴道最大收缩压、盆腔器官脱垂、尿失禁、性生活质量评分。结果:(1)组间临床疗效、证候疗效比较,C组的临床总有效率、证候改善总有效率均高于A组、B组(P0.05),而A组与B组之间比较均无统计学差异(P0.05)。(2)治疗后,C组的盆底肌力评分、阴道最大收缩压、性生活质量评分均高于A组、B组(P0.05),A组与B组之间比较均无统计学差异(P0.05)。(3)C组的盆腔器官脱垂、尿失禁等发生率均低于A组、B组(P0.05),而A组与B组比较均无统计学差异(P0.05)。结论:生物反馈电刺激联合补中益气汤治疗可有效改善产后盆底功能障碍患者的盆底肌力和阴道压力,达到良好的临床疗效,有利于预防盆腔器官脱垂、尿失禁的发生,改善其性生活质量。  相似文献

Compromise of elastic fiber integrity in connective tissues of the pelvic floor is most likely acquired through aging, childbirth-associated injury, and genetic susceptibility. Mouse models of pelvic organ prolapse demonstrate systemic deficiencies in proteins that affect elastogenesis. Prolapse, however, does not occur until several months after birth and is thereby acquired with age or after parturition. To determine the impact of compromised levels of fibulin-5 (Fbln5) during adulthood on pelvic organ support after parturition and elastase-induced injury, tissue-specific conditional knockout (cKO) mice were generated in which doxycycline (dox) treatment results in deletion of Fbln5 in cells that utilize the smooth muscle α actin promoter-driven reverse tetracycline transactivator and tetracycline responsive element-Cre recombinase (i.e., Fbln5f/f/SMA++-rtTA/Cre+, cKO). Fbln5 was decreased significantly in the vagina of cKO mice compared with dox-treated wild type or controls (Fbln5f/f/SMA++-rtTA/Cre-/-). In controls, perineal body length (PBL) and bulge increased significantly after delivery but declined to baseline values within 6–8 weeks. Although overt prolapse did not occur in cKO animals, these transient increases in PBL postpartum were amplified and, unlike controls, parturition-induced increases in PBL (and bulge) did not recover to baseline but remained significantly increased for 12 wks. This lack of recovery from parturition was associated with increased MMP-9 and nondetectable levels of Fbln5 in the postpartum vagina. This predisposition to prolapse was accentuated by injection of elastase into the vaginal wall in which overt prolapse occurred in cKO animals, but rarely in controls. Taken together, our model system in which Fbln5 is conditionally knock-downed in stromal cells of the pelvic floor results in animals that undergo normal elastogenesis during development but lose Fbln5 as adults. The results indicate that vaginal fibulin-5 during development is crucial for baseline pelvic organ support and is also important for protection and recovery from parturition- and elastase-induced prolapse.  相似文献

Collagen metabolism is altered in the pelvic organ tissues of women with genital prolapse. The aim of this study was to compare collagen metabolism by measuring matrix metalloproteinase-1 (MMP-1) expression in uterosacral ligament tissues of postmenopausal women with and without genital prolapse. Uterosacral ligament tissues were obtained at the time of abdominal or vaginal surgery from twenty-four patients with pelvic organ prolapse (POP) and 21 women who underwent gynecologic surgery for benign indications. The tissue samples were analyzed by immunohistochemistry. There were no differences in age, BMI and parity between two groups. The patients with genital prolapse demonstrated significantly higher occurences of MMP-1 expression compared to controls. These findings indicate that increased MMP-1 expression in uterosacral ligaments is associated with genital prolapse. Our data are consistent with the theory that increased collagen breakdown may play an important role in the onset and development of pelvic organ prolapse (POP).  相似文献

Objective

There are increasing numbers of reports describing human vaginal tissue composition in women with and without pelvic organ prolapse with conflicting results. The aim of this study was to compare ovine and human posterior vaginal tissue in terms of histological and biochemical tissue composition and to assess passive biomechanical properties of ovine vagina to further characterise this animal model for pelvic organ prolapse research.

Study Design

Vaginal tissue was collected from ovariectomised sheep (n = 6) and from postmenopausal women (n = 7) from the proximal, middle and distal thirds. Tissue histology was analyzed using Masson''s Trichrome staining; total collagen was quantified by hydroxyproline assays, collagen III/I+III ratios by delayed reduction SDS PAGE, glycosaminoglycans by dimethylmethylene blue assay, and elastic tissue associated proteins (ETAP) by amino acid analysis. Young''s modulus, maximum stress/strain, and permanent strain following cyclic loading were determined in ovine vagina.

Results

Both sheep and human vaginal tissue showed comparable tissue composition. Ovine vaginal tissue showed significantly higher total collagen and glycosaminoglycan values (p<0.05) nearest the cervix. No significant differences were found along the length of the human vagina for collagen, GAG or ETAP content. The proximal region was the stiffest (Young''s modulus, p<0.05), strongest (maximum stress, p<0.05) compared to distal region, and most elastic (permanent strain).

Conclusion

Sheep tissue composition and mechanical properties showed regional differences along the postmenopausal vaginal wall not apparent in human vagina, although the absolute content of proteins were similar. Knowledge of this baseline variation in the composition and mechanical properties of the vaginal wall will assist future studies using sheep as a model for vaginal surgery.  相似文献

Objectives: The purpose of this study is to develop a validated 3D finite element model of the pelvic floor system which can offer insights into the mechanics of anterior vaginal wall prolapse and have the ability to assess biomedical device treatment methods. The finite element results should accurately mimic the clinical findings of prolapse due to intra-abdominal pressure (IAP) and soft tissues impairment conditions. Methods: A 3D model of pelvic system was created in Creo Parametric 2.0 based on MRI Images, which included uterus, cervix, vagina, cardinal ligaments, uterosacral ligaments, and a simplified levator plate and rectum. The geometrical model was imported into ANSYS Workbench 14.5. Mechanical properties of soft tissues were based on experimental data of tensile test results from current literature. Studies were conducted for IAP loadings on the vaginal wall and uterus, increasing from lowest to extreme values. Results: Anterior vaginal wall collapse occurred at an IAP value corresponding to maximal valsalva and showed similar collapsed shape as clinical findings. Prolapse conditions exhibited high sensitivity to vaginal wall stiffness, whereas healthy tissues was found to support the vagina against prolapse. Ligament impairment was found to have only a secondary effect on prolapse.  相似文献

Objective

To evaluate the differences between the in-office and intraoperative techniques used to evaluate pelvic organ prolapse.

Materials and Methods

A prospective study included 25 women undergoing vaginal reconstruction surgery including vaginal hysterectomy for pelvic organ prolapse. The outpatient pelvic and site-specific vaginal examination was performed in the lithotomy position with the Valsalva maneuver. Repeated intraoperative examination was performed under general anesthesia with standard mild cervical traction. The Pelvic Organ Prolapse Quantification system (POPQ) was used for both measurements and staging. The values found under the two conditions were compared.

Results

The intraoperative POPQ-measurements values were significantly higher than the outpatient values for apical wall prolapse in 17/25 (68%) women and for anterior wall prolapse in 8/25 (32%) women. There was not a significant difference in the posterior wall where increase in staging was shown in 3/25 (12%) patients.

Conclusions

Clinicians and patients should be alert to the possibility that pelvic organ measurements performed under general anesthesia with mild traction may be different from preoperative evaluation.  相似文献
