目的:通过甲状腺病变术前超声声像图与术后病理间的对照,分析结节单发或多发在鉴别伴钙化甲状腺病变良恶性中的意义。方法:回顾性分析我院2010年1月至2013年12月收治的218例甲状腺病变患者的临床资料,将患者术前超声声像图与术后病理结果进行对比。结果:术前超声探及甲状腺单发结节伴钙化其术后病理恶性比例显著高于多发结节伴钙化,差异具有统计学意义(P0.05);多发结节伴钙化其术后病理恶性比例显著高于多发结节不伴钙化,差异具有统计学意义(P0.05)。结论:术前超声探及甲状腺单发结节伴钙化其术后病理恶性比例较高,应积极手术治疗,多发结节伴钙化因其术后病理恶性病理较低,不宜作为手术治疗甲状腺病变的绝对指征。  相似文献

目的:探讨超声引导下甲状腺穿刺活检(US-FNAB)对甲状腺良恶性结节的诊断价值。方法:本研究选取了60例已做过甲状腺结节手术的患者,入院后先行常规超声(US)检查后行超声引导下甲状腺穿刺活检(US-FNAB)通过观察并记录良恶性结节诊断结果:,诊断结果:符合率及诊断结果:符合率与结节大小的关系,评价US-FNAB对甲状腺良恶性结节的诊断价值。结果:常规超声诊断出恶性结节4例,良性结节33例;超声引导下细针穿刺活检诊断出恶性结节8例,良性结节40例。US组中,15例误诊,8例漏诊中,诊断符合率为61.7%。US-FNAB组中,4例误诊,漏诊2例,诊断符合率80.0%。经统计学分析,US-FNAB组诊断符合率明显高于US组(P0.05),US组与US-FNAB组在各结节直径的患者分布上相比差异无统计学意义(P0.05)。两组组内各结节直径的患者分布上相比差异也无统计学意义(P0.05)。US-FNAB组中,随着结节直径增大,诊断符合率略有降低。对直径10mm的甲状腺结节诊断率达100%。结论:US-FNBA对甲状腺良恶性结节具有较高的诊断准确性,诊断准确性受结节大小影响较小,对直径较小的甲状腺结节检出率较高。  相似文献

目的:探讨术前血清促甲状腺激素(TSH)水平与甲状腺结节良恶性的关系。方法:回顾性分析了1499例甲状腺结节手术切除患者术前血清TSH、甲状腺B超,手术记录、术后病理诊断报告。根据术后病理报告判定甲状腺结节良恶性,分析术前血清TSH水平在甲状腺良恶性结节中的不同分布。结果:分化型甲状腺癌(DTC)患者术前血清TSH水平明显高于甲状腺良性结节组(2.179±2.017vs1.259±0.884μIU/mL),P<0.001;在DTC患者中,有淋巴结转移较无淋巴结转移、TNM分期III、IV期较I、II期以及肿瘤直径≥1cm较<1cm的患者术前血清TSH明显升高(均P<0.001)。结论:术前血清TSH水平是预测甲状腺结节良恶性的重要指标。  相似文献

目的:探讨弹性成像评分法及弹性应变率比值法在甲状腺结节良恶性诊断中最佳诊断界点.方法:对78例共122个甲状腺结节术前行弹性成像检查并获得应变率比值(SR),根据病理结果分为良性组90个结节和恶性组32个结节.结果:(1)弹性成像评分法:78例122个结节术前超声弹性评分为1-3分有90个结节,与病理诊断符合74个结节弹性评分为4分和5分有32个结节,与病理诊断符合24个结节,对应的诊断敏感度为75%,特异度为82%,准确率为80%;(2)弹性应变率比值:良性组结节90个,SR平均值为2.34+1.02:恶性组结节32个,SR平均值为4.15+1.09;(3)通过ROC曲线确定SR最佳诊断点为3.30,对应的应变率比值法诊断敏感度为81%,特异度为90%,准确率为87%,ROC曲线下面积(AUC)为0.831.结论:弹性应变率比值法可用于甲状腺结节良恶性的判断,为甲状腺弹性成像检查提供了一项新的诊断指标.  相似文献

目的:探讨BRAF~(V600E)基因检测在甲状腺结节中的诊断价值,评估其诊断良恶性甲状腺结节的应用价值。方法:选取2015年1~12月江苏省常州市武进中医医院收治的甲状腺结节患者,入院后测定BRAF~(V600E)基因表型,采用细针穿刺细胞学(FNAC)检测其性质,采用手术组织病理学对患者甲状腺结节进行良恶性鉴别,分析BRAF~(V600E)基因与良恶性结节的相关性,评估其对于甲状腺结节良恶性鉴别的指导价值。结果:共入组甲状腺结节患者92例,其中恶性65例;多因素分析表明BRAF~(V600E)基因突变与恶性甲状腺结节呈相关性(P0.01),OR为44.42(95%CI:5.66~348.44);BRAF~(V600E)基因突变预测其恶性甲状腺结节发生的敏感性和特异性分别为63%(95%CI:0.50~0.74)和96%(95%CI:0.79~0.99)。结论:BRAF~(V600E)突变与甲状腺癌的发生有显著的相关性,其能显著提高良恶性甲状腺结节的检出率。  相似文献

目的:探讨超声诊断甲状腺结节的临床价值。方法:选取我院2013年4月至2014年5月期间收治的100例甲状腺结节患者作为本次的研究对象,对100例患者进行超声检查,术后对其进行病理分析,比较超声检查甲状腺结节良恶性与病理诊断的符合率。结果:超声检查甲状腺结节良恶性符合率为72%,检查甲状腺结节血流分布特点准确率为94%。结论:超声检查结界内的血流信号及其分布特点准确率较高,为甲状腺结节的临床诊断及治疗方案的确定提供了重要的参考依据。  相似文献

目的:探讨甲状腺钙化在诊断和鉴别诊断甲状腺姑节中的临床意义。方法:回顾性分析经超声检查和手术病理证实的77例甲状腺结节患者的临床资料。结果:超声检查与病理对照分析显示:良性20例,结节性甲状腺肿18例,恶性56例,其中乳头状癌50例,乳头状增生1例。良性结节中超声声像图钙化多表现为均匀强回声光斑,光团或弧形光团,后方伴声影。恶性结节中钙化表现为“针尖样”、“砂粒样”、“点状”的微钙化,后方声影可有可无。结论:依据甲状腺结节内钙化的形态大小,可以鉴别甲状腺结节的良恶性。  相似文献

摘要 目的:分析甲状腺影像报告和数据系统(TI-RADS)分类联合血清促甲状腺激素(TSH)、甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TgAb)检测对甲状腺良恶性结节的诊断价值。方法:回顾性分析2019年1月至2021年1月中国人民解放军联勤保障部队解放军第九〇一医院收治的76例甲状腺结节患者的临床资料。术后病理结果显示32例为甲状腺恶性结节(恶性组),44例为良性结节(良性组)。对比两组超声征象以及血清TSH、TPOAb、TgAb的表达情况。甲状腺恶性结节的影响因素通过多因素Logistic回归进行分析,TI-RADS分类联合TSH、TPOAb、TgAb检测对甲状腺良恶性结节的诊断价值通过受试者工作特征(ROC)曲线进行评估。结果:恶性组结节质地为实性、边界模糊/毛刺、微钙化、内部回声为低回声、纵横比>1、TI-RADS分类≥4类占比明显高于良性组(P<0.05),而两组结节直径、结节数目比较无差异(P>0.05)。恶性组TSH、TPOAb、TgAb水平均高于良性组(P<0.05)。多因素Logistic回归分析结果显示,质地为实性、边界模糊/毛刺、微钙化、内部回声为低回声、纵横比>1、TI-RADS分类≥4类、高TSH、高TPOAb、高TgAb水平为甲状腺恶性结节的危险因素(P<0.05)。ROC曲线结果显示,TI-RADS分类、TSH、TPOAb、TgAb联合检测较各指标单独检测的诊断效能更高。结论:TI-RADS分类联合血清TSH、TPOAb、TgAb检测可有效鉴别甲状腺结节性质,且联合检测诊断效能更高。  相似文献

In-depth study of structure-based drug designing can provide vital leads for the development of novel, clinically active molecules. In this present study, twenty six novel pyrazolo[1,5-a]pyrimidine analogues (6a-6z) were designed using molecular docking studies. The designed molecules were synthesized in good yields. Structural elucidation of the synthesized molecules was carried out using IR, MS, 1H NMR and 13C NMR spectroscopy. All the synthesized compounds were evaluated for their in-vitro anti-tubercular activity against H37Rv strain by Alamar Blue assay method. Most of the synthesized compounds displayed potent anti-tubercular activities. Amongst all the tested compounds 6p, 6g, 6n and 6h exhibited promising anti-tubercular activity. Further, these potent compounds were gauged for MDR-TB, XDR-TB and cytotoxic study. None of these compounds exhibited potent cytotoxicity. Stability of protein ligand complex was further evaluated by molecular dynamics simulation for 10 ns. All these results indicate that the synthesized compounds could be potential leads for further development of new potent anti-tubercular agents.  相似文献

The program MolSim designed to calculate the similarity of different molecules quantitatively in a fast and easy way is described. The molecular similarity is estimated for the molecular shape as well as for the electrostatic potentials of the molecules derived from ab initio calculations. A grid-based method is used to determine the steric and electrostatic similarities between a lead compound and the corresponding test set by calculating the Spearman correlation coefficient. The superpositioning of the molecules was accomplished with the SEAL algorithm incorporating a Monte Carlo simulated annealing approach while preserving the conformational flexibility of the calculated structures.The ability of the program was tested on a set of Sandalwood odour compounds, a class of substances that is difficult to analyse with respect to its structure-activity relationship because of the structural diversity of Sandalwood odour compounds, in contrast to their high selectivity and pronounced structural specificity. The application of the program on a small test set of these compounds showed that the program is able to explain the Sandalwood odour activity correctly.Electronic Supplementary Material available.  相似文献

The force fields Tersoff, CHARMM, COMPASS, CVFF and PCFF are compared using molecular calculations and simulations of SWNT thermal properties. The heat capacity results from the force fields vary significantly in the low (room) temperature range. The COMPASS force field best reproduces the phonon frequencies calculated from density functional theory and is consistent with the Raman scattering results. The temperature dependent behavior of SWNT heat capacity is investigated using harmonic and quasi-harmonic dynamics theories. The impact of quasi-harmonic analysis is not significant in the low and intermediate temperature range (below 500 K). Thus, force field comparisons based on the harmonic approximation are valid in that temperature range. Above 500 K, heat capacity results based on the Tersoff force field using a quasi-harmonic approximation are further investigated.  相似文献

This paper aimed at investigating the influence of polymerization temperature on the molecular recognition of molecularly imprinted polymers (MIPs) based on multiple non-covalent interactions. 3-l-Phenylalanylaminopyridine (3-l-PheNHPy) imprinted polymers were prepared using azobisnitriles as either thermal initiators or photoinitiators at various temperatures of 10, 40 and 60 degrees C, respectively. These polymers were subsequently evaluated in the high-performance liquid chromatographic (HPLC) mode for enantioselectivity. An unexpected result shows that polymer prepared at 40 degrees C has the highest enantioselectivity, but not the polymer prepared at lower temperature of 10 degrees C. Further, the effect of elution temperature and sample load on the selectivity of polymers was investigated in detail. In order to get a better understanding of the "exception", the influence of polymerization temperature on the polymerization extent and polymer morphology was studied by FT-IR spectrum test, cross-polarization magic angle spinning (CP-MAS) (13)NMR spectra experiment and pore analysis. Based on these results we attribute this "exception" to that there is a tradeoff between the extent of polymerization and stabilization of the template-functional monomer complexes. And an optimal polymerization temperature can be found for each combination of template and monomer.  相似文献

We present a model for tumour metabolism that incorporates both microenvironmental (extracellular) and oncogenic (intracellular) influences. We explore the effects of the interaction between the hypoxic microenvironment and intracellular signalling on the glycolytic response of tumour tissue, finding that the glycolytic state is dependent on a delicately balanced interplay between the cellular hypoxic response, mediated by hypoxia-inducible factor-1α (HIF-1α), and growth-factor signalling cascades, which are frequently mutated in cancers. Our findings demonstrate the importance of considering both environmental and intracellular regulation when interpreting tumour metabolism for diagnostic or prognostic purposes. To illustrate this, we demonstrate the potential impact of this multi-factorial regulation on the kinetics of radiolabelled glucose analogues, used in positron emission tomography (PET).  相似文献
