After its first introduction in 2002, transcatheter aortic valve implantation (TAVI) has continuously gained more foothold for the treatment of severe aortic stenosis and is nowadays a viable treatment option for inoperable patients or patients at high risk for conventional surgical aortic valve replacement. Although ideally carried out in a so-called hybrid room, incorporating both the strict hygiene and advanced life support possibilities of the operating theatre and the imaging and percutaneous arsenal of the catheterisation suite, in most centres TAVI is at present performed in the catheterisation laboratory. This may raise concern about an increased risk of infection, since there the criteria that are applied regarding disinfection and sterilisation are not as stringent as those of the operating theatre. Therefore, we retrospectively assessed the number of infective complications in patients undergoing TAVI in the catheterisation lab of our institution. Eleven out of 73 patients developed a postprocedural infection, one of which could be attributed to the procedure itself, being superinfection of a surgical groin cut-down. Our conclusion is that percutaneous aortic valve implantation in a catheterisation laboratory is not associated with an increased risk of infective complications.  相似文献

In infectious disease as well as in cancer, the ultimate outcome of the curative response, mediated by the body itself or through drug treatment, is either successful eradication or a resurgence of the disease (“flare-up” or “relapse”), depending on random fluctuations that dominate the dynamics of the system when the number of diseased cells has become very low. The presence of a low-numbers bottle-neck in the dynamics, which is unavoidable if eradication is to take place at all, renders at least one phase of the dynamics essentially stochastic. However, the eradicating agents (e.g. immune cells, drug molecules) generally remain at high numbers during the critical bottle-neck phase, sufficiently so to warrant a deterministic treatment. This leads us to consider a hybrid stochastic-deterministic approach where the infected cells are treated stochastically whereas the eradicating agents are treated deterministically. Exploiting the fact that the number of eradicating agents typically decreases monotonically during the resolution phase of the response, we derive a set of coupled first-order differential equations that describe the probability of ultimate eradication as a function of the system's state, and we consider a number of biomedical applications.  相似文献

Kingella kingae, producing the cytotoxic RTX protein, is a causative agent of serious infections in humans such as bacteremia, endocarditis and osteoarticular infection, especially in young children. Recently, Kingella negevensis, a related species, has been isolated from the oral cavity of healthy children. In this study, we report the isolation of K. negevensis strain eburonensis, initially misidentified as K. kingae with MALDI-TOF MS, from a vaginal specimen of a patient suffering of vaginosis. The genome sequencing and analysis of this strain together with comparative genomics of the Kingella genus revealed that K. negevensis possesses a full homolog of the rtx operon of K. kingae involved in the synthesis of the RTX toxin. We report that a K. kingae specific diagnostic PCR, based on the rtxA gene, was positive when tested on K. negevensis strain eburonensis DNA. This cross-amplification, and risk of misidentification, was confirmed by in silico analysis of the target gene sequence. To overcome this major diagnostic issue we developed a duplex real-time PCR to detect and distinguish K. kingae and K. negevensis. In addition to this, the identification of K. negevensis raises a clinical issue in term of pathogenic potential given the production of a RTX hemolysin.  相似文献

In sectors like healthcare and hospitality, it has been realized that fabrics play a pivotal role in transfer of nosocomial infections. However, there is a major gap in drawing correlation between different fibre types and their interaction with microorganisms. Such information is important to formulate guidelines for textile materials for use in these sectors. In the current study, the adherence of four important bacteria, Staphylococcus aureus, Acinetobacter calcoaceticus, Escherichia coli, and Pseudomonas aeruginosa was studied on six different fibre types namely polyester, wool, polypropylene, viscose, silk and cotton. Among these fibres, viscose showed maximum adherence while silk fibres showed the least attachment of bacterial strains. Bacterial adhesion was correlated with the surface characteristics (surface charge, hydrophobicity etc.) of bacteria, and nanoroughness of fibres. Adhesion of these bacteria was tested on five hydrocarbons of different hydrophobicities. E. coli, the weakest biofilm producer, and with the highest surface energy and lowest hydrophobicity amongst the bacteria compared in the study, had the lowest load on all fibres. Scanning electron microscopy revealed non-uniform binding of gram-negative and gram-positive bacteria. Nanoroughness of fibres favored bacterial adhesion. Epizootics caused by Beauveria bassiana and Nomuraea rileyi have been observed on boll worms and Spodoptera litura in south Indian fields during winter since the last 15 years. During the N. rileyi-induced natural epizootics, some boll worms were found surviving without infection. Whether they represent pathogen-resistant genotypes was investigated. Two insect populations, collected 3 months prior to and during the epizootic were established. Their sensitivity to both the fungi was compared in laboratory bioassays. No significant difference in sensitivity was observed between the two populations. It was concluded that the boll worm population surviving the epizootic was not genotypically resistant.  相似文献

Fallopia japonica, commonly known as Japanese knotweed, is an increasingly serious invasive alien weed in the UK and large parts of mainland Europe, as well as in North America. There is an urgent need to include classical biological control (CBC) into any integrated pest management strategy. The leaf-spot pathogen Mycosphaerella polygoni-cuspidati, a coevolved natural enemy of F. japonica present throughout its native Japanese range, is considered to have high potential as a CBC agent. In this study, the disease development of M. polygoni-cuspidati in the field and the optimum infection parameters under controlled conditions were investigated to elucidate the pathogen’s potential biocontrol efficacy against Japanese knotweed. Field observation in Japan showed that M. polygoni-cuspidati caused severe damage to its host plant. When sentinel knotweed plants from the UK were placed amongst naturally-infected field populations of F. japonica, disease incidence and severity were highest in July when monthly precipitation was also highest. In greenhouse inoculation tests, F. japonica was shown to be most susceptible at the young leaf stage (7–12 days after opening). Disease severity was highest after an initial dew period of 42–48 h, and severe defoliation followed inoculation at a temperature range of 15–25 °C. The optimum post-inoculation temperature after dew treatment for disease severity was 20–25 °C. In field inoculation tests, high disease incidence and severity indicate that the pathogen has the potential to control the plant effectively in the field. Humidity and temperature were shown to be the main factors influencing disease expression and lesion development of M. polygoni-cuspidati in a field situation. These results provide valuable information for any future use of M. polygoni-cuspidati as a CBC agent for management of Japanese knotweed in the UK.  相似文献

Infectious diseases comprise some of the leading causes of death and disability worldwide. Interactions between pathogen and host proteins underlie the process of infection. Improved understanding of pathogen-host molecular interactions will increase our knowledge of the mechanisms involved in infection, and allow novel therapeutic solutions to be devised. Complete genome sequences for a number of pathogenic microorganisms, as well as the human host, has led to the revelation of their protein-protein interaction (PPI) networks. In this post-genomic era, pathogen-host interactions (PHIs) operating during infection can also be mapped. Detailed systematic analyses of PPI and PHI data together are required for a complete understanding of pathogenesis of infections. Here we review the striking results recently obtained during the construction and investigation of these networks. Emphasis is placed on studies producing large-scale interaction data by high-throughput experimental techniques.  相似文献
