以胜任力为导向的住院医师规范化培训考核体系构建

住院医师规范化培训作为毕业后医学教育的重要组成部分,对于住院医师良好的职业道德、扎实的医学理论知识和临床技能的培养起着不可或缺的作用。如何保障培训质量、提高培训效果是工作中亟待解决的问题。文章结合安徽医科大学第二附属医院住院医师规范化培训工作实践,探索构建住院医师规范化培训考核体系,为提高住院医师规范化培训质量提供有力抓手。     

肝胆外科住院医师规范化培训教学探讨

住院医师培训是医学教育中重要环节之一,是提高临床医疗水平的关键。因此,如何探索科学、合理的住院医师培养方案,已成为临床医学教育探索的重要课题。本文对肝胆外科住院医师规范化培训的实践进行了总结分析,希望对提高肝胆外科住院医师规范化培训效果起到一定的促进作用。     

建立住院医师规范化培训管理体系的探索

住院医师规范化培训是医学生向医生转变的必修科目,也是住院医师向高层次医师迈进的必由之路,对于提高住院医师临 床技能及完善医疗水平极为重要。目前,我国住院医师规范化培训存在很多弊端,相关管理体系缺乏统一、完善的评价标准。因 此,建立一个完善的住院医师规范化培训管理体系是我们应该重视的问题。本文结合医院临床教学实习所积累的经验,并学习借 鉴发达国家及地区对住院医师规范化培训管理制度的优点与长处,探索建立住院医师规范化培训管理体系的新方法,旨在促进 住院医师规范化培训工作的持续发展。     

住院医师规范化培训临床教学查房的实践与体会

在国家住院医师规范化培训持续深入推进的背景下,以医院加强住院医师规范化培训临床教学查房为基础,分析了住院医师规范化培训临床教学查房的内涵及特点,提出了组织临床教学查房的实施方案,并就如何实现住院医师规范化培训临床与教学有机融合提出了意见建议。     

住院医师规范化培训管理现状调查与分析

目的 调查分析医院开展住院医师规范化培训现状。方法 选取2011—2013年进入医院住院医师规范化培训基地的住院医师,采用现场发放调查问卷方法进行无记名调查分析。将调查内容分为报名招录、轮转培训、各类考核、组织管理和其他板块。结果 96.7%认为医院招录过程合理或基本合理,92.6%认为培训计划制定具有一定的合理性,90.1%住院医师能够按照规定完成培训内容,各类考核内容基本能够反映住院医师的培训水平。在众多培训期间存在困难中,依次为收入偏低、个人婚恋生育计划难以实施以及工作和学习压力过大。结论 住院医师基本认同目前医院的培训管理制度和方法,逐步提高住院医师薪酬待遇,有效疏导培训压力和加强师资队伍建设是提高住院医师培训积极性和培训质量的保证。     

我院医学影像科（放射科）住院医师规范化培训模式的构建及经验初探

住院医师培训是国、内外公认的医师成长必由之路。2010年上海市实施新形势下的住院医师规范化培训,医学影像科作为独立的学科进行住院医师规范化培训,如何在新的体制下构建医学影像科住院医师规范化培训模式面临新的挑战。本文针对医学影像科的特点,从我院医学影像科住院基地的概况、生源构成及培训的初步经验进行了探讨。     

我院住院医师规范化培训模式实践创新探索

临床住院医师规范化培训是我国加快年轻医师成长成才的极为重要又不可缺少的阶段,是我国提高医疗卫生服务质量的重要保障。哈尔滨医科大学附属第一医院自1993年实施住院医师规范化培训工作,建立了以住院医师为中心,以培训职业胜任力为核心的全方位内容、多渠道方法、全过程评价的管理模式：推进管理创新,完善体系建设;学习先进理念,创新带教模式;完善质量评估,实施全程质控;加强基地建设,改善住培条件;促进协同交流,提升示范作用。     

儿科临床路径管理引导住院医师规范化培训的探讨

?????? 目的 研究应用儿科临床路径管理的方法对住院医师规范化培训管理的价值。方法 采用对照研究的方法,用SPSS10.0软件计算χ²,确定统计学意义。结果 新方法的应用儿科理论成绩明显提高,住院医师定专业成功率提高,住院医师被患者投诉明显减少。结论 儿科临床路径管理应用于住院医师规范化培训管理,管理流程化,充分调动了培训医师的积极性和创新性,提高了学习效率和效果。     

对编写上海市医学影像科(放射科)住院医师规范化培训教材的思考与探讨

始于2010年的上海市住院医师规范化培训模式是我国新的医师培养模式,经历3年的实践及完善,从制度及培训基地建设与管理、培训模式及教学经验等方面均取得了可喜的成绩。鉴于放射科住院医师在一定程度上的特殊性,本文以我院放射科基地为例,从基地医师的教育及知识背景、后续的继续教育及职业规划等方面对编写上海市医学影像科(放射科)住院医师规范化培训教材的必要性及可行性进行了分析与探讨,然后对编写此教材的定位、知识体系、参编人员构成、教材编写中的注意事项等提出了具体的看法并进行了初步探讨,供各基地教学实施过程中借鉴,共同推动上海市医学影像科(放射科)住院医师规范化工作,提高医学影像学(放射科)规范化培训医师的整体水平。

Curcuminoids as inhibitors of thioredoxin reductase: A receptor based pharmacophore study with distance mapping of the active site

Curcumin is the yellow pigment of turmeric that interacts irreversibly forming an adduct with thioredoxin reductase (TrxR), an enzyme responsible for redox control of cell and defence against oxidative stress. Docking at both the active sites of TrxR was performed to compare the potency of three naturally occurring curcuminoids, namely curcumin, demethoxy curcumin and bis-demethoxy curcumin. Results show that active sites of TrxR occur at the junction of E and F chains. Volume and area of both cavities is predicted. It has been concluded by distance mapping of the most active conformations that Se atom of catalytic residue SeCYS498, is at a distance of 3.56 from C13 of demethoxy curcumin at the E chain active site, whereas C13 carbon atom forms adduct with Se atom of SeCys 498. We report that at least one methoxy group in curcuminoids is necessary for interation with catalytic residues of thioredoxin. Pharmacophore of both active sites of the TrxR receptor for curcumin and demethoxy curcumin molecules has been drawn and proposed for design and synthesis of most probable potent antiproliferative synthetic drugs.     

A Unique Protease Cleavage Site Predicted in the Spike Protein of the Novel Pneumonia Coronavirus (2019-nCoV) Potentially Related to Viral Transmissibility

正Dear Editor,In December 2019, a novel human coronavirus caused an epidemic of severe pneumonia(Coronavirus Disease 2019,COVID-19) in Wuhan, Hubei, China(Wu et al. 2020; Zhu et al. 2020). So far, this virus has spread to all areas of China and even to other countries. The epidemic has caused 67,102 confirmed infections with 1526 fatal cases     

Comparative morphology of the carpel in the Liliaceae: Hewardieae, Petrosavieae, and Tricyrteae

The young pistils in the melanthioid tribes, Hewardieae, Petrosavieae and Tricyrteae, are uniformly tricarpellate and syncarpous. They lack raphide idioblasts. All are multiovulate, with bitegmic ovules. The Petrosavieae are marked by the presence of septal glands and incomplete syncarpy. Tepals and stamens adhere to the ovary in the Hewardieae and the Petrosavieae but not in the Tricyrteae. Two vascular bundles occur in the stamens of the Hewartlieae and Tricyrtis latifolia. Ventral bundles in the upper part of the ovary of the Hewardieae are continuous with compound septal bundles and placental bundles in the lower part. Putative ventral bundles occur in the alternate position in the Tricyrteae and putative placental bundles in the opposite. position in the Petrosavieae. The dichtomously branched stigma in each carpel of the Tricyrteae is supplied by a bifurcated dorsal bundle.

传染性法氏囊病(Infection bursal disease, IBD)是由鸡传染性法氏囊病毒(Infectious bursal disease virus, IBDV)引起的鸡和火鸡的一种高度接触性传染病,给世界各国的禽养殖业带来了巨大损失.自IBDV发现至今新的变异株不断出现,分子结构的改变导致病毒致病力的改变及宿主对疫苗应答的改变,使得传统的疫苗已不能控制其流行,因此各国学者对其基因组结构和功能进行了广泛深入的研究,并积极研制新型有效的疫苗以达到防治的目的.     

Development of a Novel Reverse Transcription Loop-Mediated Isothermal Amplification Method for Rapid Detection of SARS-CoV-2

In conclusion, the novel visual RT-LAMP assay is a simple, rapid, and sensitive approach for detection of SARS-CoV-2, and it is ready for application in primary care and community hospitals or health care centers, and even patients' own houses in response to the current SARS-CoV-2 epidemic because the assay does not require sophisticated equipment and skilled personnel. Furthermore, it is also ready to be used in fields for screening samples from wild animals and environments to facilitate the identification of potential intermediate hosts that mediate the cross-species transmission of SARS-CoV-2 from bats to humans.     

Perinatal Vertical Transmission of Chikungunya Virus in Ruili,a Town on the Border between China and Myanmar

正Dear Editor,Chikungunya virus (CHIKV), an arbovirus in the family of Togaviridae, genus Alphavirus, is transmitted by the A.aegyptii or A. albopictus mosquito, and causes disease in humans characterized by fever, rash, and arthralgia (Silva and Dermody 2017; Suhrbier 2019). It was first reported in 1953 in Tanzania, and caused only a few outbreaks and sporadic cases in Africa and Asia in last century. However, in the epidemic in 2004, CHIKV acquired mutations that conferred enhanced transmission by the A. albopictus mosquito(Schuffenecker et al. 2006). Since then, it has successively caused outbreaks in Africa, the Indian Ocean, South East Asia, the South America, and Europe (Zeller et al. 2016).
