目的:探讨二甲双胍联合西格列汀对2型糖尿病患者氧化应激、胰岛素抵抗的影响。方法:收集我院就诊或住院治疗的80例2型糖尿病患者,随机分为实验组和对照组,每组40例。两组患者入院后均给予相应的治疗措施,对照组患者给予二甲双胍250 mg/次,2次/d;实验组患者在对照组的基础上给予西格列汀100 mg/次,1次/d,治疗均连续8周。治疗结束后对患者血清丙二醛(MDA)、8异前列腺素F2α(8-iso-PGF2α)、空腹血糖(FBG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)以及患者临床治疗效果进行检测并比较。结果:与治疗前相比,治疗后两组患者MDA、8-iso-PGF2α、FBG、FINS以及HOMA-IR水平均下降(P0.05);与对照组相比,实验组患者MDA、8-iso-PGF2α、FBG、FINS以及HOMA-IR水平较低(P0.05),临床治疗总有效率较高(P0.05)。结论:二甲双胍联合西格列汀能够降低2型糖尿病患者血糖水平,降低MDA、8-iso-PGF2α水平,减轻氧化应激反应,降低胰岛素抵抗,临床疗效较好。  相似文献

目的:探讨非酒精性脂肪性肝病(NAFLD)患者血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、脂多糖(LPS)的表达及其与胰岛素抵抗(IR)的关系。方法:选取2015年1月到2017年10月在我院接受治疗的NAFLD患者100例作为观察组,另选取同期在我院体检的健康志愿者50例作为对照组,比较两组受试者的空腹血糖(FBG)和空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)以及TNF-α、IL-6、LPS水平,采用Pearson相关系数分析TNF-α、IL-6、LPS与FBG、FINS、HOMA-IR之间的相关性。结果:观察组的FBG、FINS水平及HOMA-IR均明显高于对照组,存在统计学差异(P0.05)。观察组的TNF-α、IL-6、LPS水平均明显高于对照组,存在统计学差异(P0.05)。经Pearson相关分析显示,TNF-α、IL-6、LPS与FBG无相关性(P0.05),与FINS、HOMA-IR均呈正相关(P0.05)。结论:NAFLD患者存在明显的IR现象,且其血清中的TNF-α、IL-6、LPS水平明显上升,同时TNF-α、IL-6、LPS与IR呈正相关,并与NAFLD的发生、发展密切相关。  相似文献

摘要 目的:分析血清成纤维细胞生长因子-21(FGF-21)与2型糖尿病(T2DM)合并非酒精性脂肪性肝病(NAFLD)患者常见指标及NAFLD纤维化评分(NFS)的相关性,进一步探讨达格列净对T2DM合并NAFLD患者血清FGF-21水平的影响。方法:选取2022年1月至2022年6月徐州医科大学附属徐州市立医院收治的80例T2DM合并NAFLD患者为研究对象(T2DM合并NAFLD组),选择同期80例T2DM不合并NAFLD患者为T2DM组。收集腰围(WC)、身高、体重数据,计算体重指数(BMI)。测定空腹血糖(FPG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-c),低密度脂蛋白胆固醇(LDL-c)、肌酐(Cr)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST )、白蛋白(Alb)、血小板计数(PLT)等指标,计算胰岛素抵抗指数(HOMA-IR)、NFS。采用酶联免疫吸附(ELISA)法测定FGF-21水平。比较T2DM组和T2DM合并NAFLD组各项指标的差异,探讨血清FGF-21水平与T2DM合并NAFLD患者其他指标的相关性,Logistic回归分析T2DM合并NAFLD的影响因素,受试者工作特征曲线(ROC)分析各影响因素对T2DM合并NAFLD的诊断价值。将80例T2DM合并NAFLD患者按随机数字表法随机分为二甲双胍组和达格列净组各40例,治疗前后观测各项指标变化,并密切监测不良反应。结果:T2DM合并NAFLD组患者WC、BMI、FINS、HbA1c、TG、AST、ALT、HOMA-IR、NFS及FGF-21均高于 T2DM组(P<0.05)。相关性分析显示,FGF-21水平与T2DM合并NAFLD组患者WC、BMI、HbA1c、TG、HOMA-IR、NFS均存在正相关(P<0.05)。Logistic回归分析显示,BMI、HbA1c、FGF-21、HOMA-IR为影响T2DM患者合并NAFLD的危险因素。ROC曲线分析显示,BMI、HbA1c、FGF-21、HOMA-IR对T2DM合并NAFLD均具有一定预测价值,其中以FGF-21的预测效能最佳。治疗后,达格列净组TG、AST、ALT、NFS、FGF-21水平较二甲双胍组降低更为明显(P<0.05)。结论:血清FGF-21水平为T2DM合并NAFLD的危险因素,参与了T2DM合并NAFLD发病及进展,且对T2DM合并NAFLD有较好的预测效能。相较于二甲双胍,达格列净可明显降低T2DM合并NAFLD患者血清FGF-21水平并改善NFS,具有一定程度的肝脏保护作用。  相似文献

目的:探讨胃大部切除后不同吻合术式对胃癌合并2型糖尿病患者血糖的影响.方法:选择66例在本院行手术治疗的胃癌合并2型糖尿病的患者为研究对象,随机分为毕Ⅰ式组26例,毕Ⅱ式组24例,Roux-en-y组16例.检测和比较三组患者术前和术后随访3月时患者的OGTT空腹及负荷后血糖值、糖化血红蛋白等水平.结果:术前三组OGTT空腹血糖、2h血糖、糖化血红蛋白水平、空腹胰岛素水平、稳态模型评估胰岛素抵抗(HOMA-IR)的比较均无统计学差异(P均>0.05).术后3月,与毕Ⅰ式组比较,毕Ⅱ式组与Roux-en-y组未达2型糖尿病诊断标准的患者百分率显著升高(X2=7.866,P=0.020),OGTT空腹血糖(F=6.472,P=0.005)水平、OGTT 2 h血糖水平(F=3.431,P=0.041)、糖化血红蛋白水平(F=5.456,P=0.009)、空腹胰岛素水平(F=6.120,P=0.008)、HOMA-IR(F=8.091,P<0.001)均显著降低.毕Ⅱ式组与Roux-en-y组之间以上指标比较均无统计学差异(P>0.05).结论:胃癌合并2型糖尿病的患者在消化道重建术后部分可得缓解,采用毕Ⅱ式或Roux-en-y吻合术可能更适宜胃癌合并2型糖尿病的患者.  相似文献

目的:研究2型糖尿病伴脂肪肝患者血浆成纤维细胞生长因子21(FGF21)水平与肥胖、脂代谢及胰岛素抵抗的相关性,为临床诊疗提供依据。方法:选取2013年5月到2015年11月我院收治的2型糖尿病伴脂肪肝患者100例为研究组,另选取同期单纯脂肪肝患者100例为脂肪肝组,健康体检者100例为对照组,比较各组入选次日清晨FGF21、谷丙转氨酶(ALT)、谷草转氨酶(AST)、胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、游离脂肪酸(FFA)、体重指数(BMI)、腰臀比(WHR)、空腹胰岛素(FINS)、空腹血糖(FBG)以及胰岛素抵抗指数(HOMA-IR)。结果:研究组TG、TC、AST、ALT、LDL-C、FFA、FBG、BMI、WHR、FINS、HOMA-IR以及FGF21均显著高于对照组,HDL-C显著低于对照组,比较差异具有统计学意义(P0.05);研究组FFA、TG、FINS、FBG、HOMA-IR以及FGF21显著高于脂肪肝组,BMI和WHR显著低于脂肪肝组,比较差异具有统计学意义(P0.05);相关性分析显示:FGF21与TG、FFA、BMI以及HOMA-IR呈正相关关系(P0.05)。结论:2型糖尿病合并脂肪肝患者FGF21水平会显著升高,且与脂肪代谢、肥胖以及胰岛素抵抗有关。  相似文献

目的:探讨脂联素在肝源性糖尿病患者胰岛素抵抗中的作用.方法:检测肝源性糖尿病(A组)、肝硬化(B组)、2型糖尿病(C组)患者及正常人(D组)血清脂联素(ADPN)、空腹血糖(FPG)、空腹胰岛素(FINS)水平,计算其胰岛素抵抗指数(HOMA-IR)、β细胞功能指数(FBCI)、胰岛素敏感性指数(ISI),分析ADPN与胰岛素抵抗的相关性.结果:A组ADPN水平低于B组、D组(P<0.05);FPG水平低于C组(P<0.05);FINS,FBCI水平高于C组及D组(P<0.05);ISI低于D组(P<0.05);HOMA-IR显著高于B组及D组(P<0.05).ADPN与 FPG、FINS、HOMA-IR呈负相关;与ISI呈正相关.结论:肝源性糖尿病患者ADPN水平下降,其与胰岛素抵杭呈负相关,提示脂联素可能在肝源性糖尿病患者胰岛素抵抗中起着重要作用.  相似文献

目的:探讨糖适平对2型糖尿病合并糖尿病肾病4期患者血糖、肌酐清除率及尿蛋白水平的影响。方法:选择我院收治的2型糖尿病合并糖尿病肾病4期患者100例,按照用药情况分为糖适平实验组和胰岛素对照组,每组50例。对照组患者给予预混人胰岛素及阿卡波糖片,实验组患者给予糖适平及阿卡波糖片。比较治疗前后两组患者空腹血糖(FBG)、肌酐清除率(CCr)、血清肌酐(SCr)、血尿素氮(BUN)及24小时尿蛋白定量(24 h Upro)水平的变化情况。结果:与治疗前相比,两组患者治疗后空腹血糖、血清肌酐、血尿素氮及24小时尿蛋白定量、肌酐清除率均明显改善,差异具有统计学意义(P0.05),但实验组与对照组比较差异无统计学意义(P0.05)。结论:糖适平对2型糖尿病合并糖尿病肾病4期患者无药物积蓄问题,用于治疗2型糖尿病合并糖尿病肾病4期安全有效。  相似文献

目的:探讨糖耐量减低(IGT)患者血清瘦素(Leptin)和血浆内皮素-1(ET-1)含量变化及其意义。方法:采用放射免疫法检测65例IGT患者、50名正常健康体检者和50例2型糖尿病患者血清中Leptin和ET-1的含量,同时分别测定体重指数、腰臀比、空腹血糖(FBG)、餐后2小时血糖(2h PBG)及空腹胰岛素(FINS),并用HOMA稳态模型计算胰岛素抵抗指数(HOMA-IR)。结果:糖尿病组、IGT组血清Leptin、ET-1、FBG、2hPBG、FINS、TC含量及HOMA-IR指数显著高于正常对照组(P<0.05),糖尿病组血清Leptin、ET-1、FBG、2hPBG、FINS、TC含量及HOMA-IR指数明显高于IGT组(P<0.05)。IGT组血清Leptin水平与BMI、血清ET-1、FBG、2hPBG、FINS、TC含量及HOMA-IR指数呈显著正相关(P<0.01),IGT组血清ET-1水平与收缩压、舒张压、BMI、血清FBG、2hPBG、FINS含量及HOMA-IR指数呈显著正相关(P<0.01)。结论:瘦素与内皮素-1均可能在IGT发展成2型糖尿病过程中起一定的作用。  相似文献

目的:探讨吡格列酮治疗对2型糖尿病合并代谢综合征患者血糖、胰岛素抵抗和炎症因子的影响。方法:回顾性分析我院2013年8月-2015年2月收治的96例2型糖尿病合并代谢综合征患者病例资料,根据治疗方法的不同分为观察组和对照组,每组48例,两组患者均给予糖尿病常规基础治疗,在此基础上,观察组患者给予吡格列酮治疗,治疗3个月。检测治疗前后血糖指标包括空腹血糖(FPG)、餐后2小时血糖(2h PG)以及糖化血糖蛋白(Hb A1c),胰岛素抵抗指标包括空腹胰岛素(FINS)和胰岛素抵抗指数(HOMA-IR)和炎症因子包括超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)。结果:观察组治疗后FPG、2 h PG、Hb A1c分别为(6.19±2.24)mmol/L、(8.30±2.48)mmol/L和(6.01±1.23)%,均显著低于治疗前和对照组治疗后,差异有统计学意义(P0.05);观察组治疗后FINS和HOMA-IR分别为(6.60±1.92)m IU/L和2.08±1.00,均较治疗前和对照组治疗后显著降低,差异有统计学意义(P0.05);观察组治疗后hs-CRP、IL-6和TNF-α分别为(6.12±1.67)ng/L、(62.65±10.30)ng/L和(83.16±16.55)ng/L,均较治疗前和对照组治疗后显著降低,差异有统计学意义(P0.05)。结论:吡格列酮治疗可以显著降低2型糖尿病合并代谢综合征患者血糖水平,改善胰岛素抵抗,降低炎症反应,值得进一步研究。  相似文献

目的:探究TNF-α、NF-KBp65以及PAI-1在妊娠期糖尿病孕妇血清和胎盘组织中的表达变化及其临床意义。方法:选取自47例定期产检并且住院分娩的妊娠期糖尿病孕妇作为观察组,再选取同期分娩的47例正常孕妇作为对照组,比较两组孕妇血清和胎盘组织中TNF-α、NF-KBp65和PAI-1的表达水平,并分析血清中TNF-α、NF-KBp65、PAI-1表达水平的相关性。结果:观察组孕妇血浆的TNF-α和PAI-1表达水平明显较高,差异具有统计学意义(P均0.001)。妊娠期糖尿病患者血浆中PAI-1表达水平和TNF-α表达水平呈正相关关系(r=0.843,P0.001)。观察组孕妇的空腹-OGTT、1h-OGTT、2h-OGTT、空腹胰岛素、空腹血糖以及胰岛素抵抗指数均明显高于对照组(P均0.001)。胰岛素抵抗指数与妊娠期糖尿病患者血浆中TNF-α的表达水平相关性最高(Beta=0.769);口服葡萄糖耐受实验中1-h的血糖水平与PAI的表达水平相关性最高(Beta=0.637)。观察组孕妇的TNF-α、NF-KBp65和PAI-1的阳性率显著高于对照组,差异具有统计学意义(P0.001,=0.039,0.007)。结论:血清和胎盘组织中的TNF-α、NF-KBp65和PAI-1可能参与了妊娠期糖尿病的发生与发展。  相似文献

综述了白蚁螱客的主要种类、共生关系及相关机制的研究进展。白蚁螱客中,已报道的动物种类达170种。在与动物的共生关系中存在偏利共生(宾主共栖和异种共栖)、互利共生和无关共生三种;在与微生物的共生关系中,存在与内生菌(原生动物、细菌、真菌和放线菌)和外生菌(蚁巢伞菌等)间的互利关系。指出了白蚁与螱客研究中存在的问题,给出了解决方案,并提出了今后可能的研究热点或方向,为白蚁的综合利用(如纤维素酶)及今后研究物种间的协同进化提供了基础资料。  相似文献

To elucidate accumulation of minerals in human iliac arteries with aging, the content of minerals was analyzed by inductively coupled plasma atomic emission spectrometry. Bilateral common, internal, and external iliac arteries of 16 men and 8 women, ranging ages from 65 to 93 yr, were examined. It was found that an extremely high accumulation of calcium and phosphorus occurred in the common iliac artery at old age, being higher than that of the internal and external iliac arteries. It should be noted that the accumulation of calcium and phosphorus is the highest in the common iliac artery among the human arteries examined to date. Regarding sexual differences, the content of calcium and phosphorus in the common and internal iliac arteries was higher in women than in men, whereas their content in the external iliac artery was lower in women than in men.  相似文献

New sulfur derivatives of phosphoramidite ligands were synthesized and the impact of the sulfur unit on the spectroscopic properties of their rhodium and iridium complexes was investigated. The new ligands Bn2NPSCH2CH2Sa(P-Sa) (Bn = benzyl, 4), Bn2NPSCHCHSa(CH2)3CaH2(P-Sa)(Ca-Sa) (6) and Bn2NP(4-XC6H4OMe)2 (X = S, 7a; X = O, 7b) were converted to the rhodium and iridium complexes trans-[Rh(CO)Cl(L)2] (L = 4, 6, 7), [RhCl(COD)(L)] (L = 4, 6, 7), [IrCl(COD)(7a)] and [IrCl2Cp∗(6)]. For comparison, some phosphoramidite complexes of these formulations also were synthesized. The new metal complexes were spectroscopically analyzed. For the carbonyl complexes, the νCO IR stretching frequencies were lower than for the corresponding phosphite and phosphoramidite ligands. The 1JPRh coupling constants for the rhodium complexes with the new ligands were also smaller than for the respective phosphoramidite and phosphite complexes. Finally, the 1JPSe coupling constants of the selenides of the new ligands were lower than those of the phosphoramidite ligands but higher than for PPh3. The spectroscopic data reveal that the new thio ligands 4, 6 and 7a are more electron donating than phosphites and phosphoramidites but less electron donating than PPh3.  相似文献

Astrocytes transport the monocarboxylate acetate, but synaptosomes do not. The reason for this is unknown, because both preparations express monocarboxylate transporters (MCT). The transport and metabolism of lactate, another monocarboxylate, was examined in these two preparations, and the results were compared to those for acetate. Lactate transport is more rapid in astrocytes than in synaptosomes, but of lower affinity (Kms of 17 and 4 mM, respectively). Lactate (0.2 mM) is metabolized to CO2 more rapidly in synaptosomes than in astrocytes (rates of 0.37 and 0.07 nmol x mg protein(-1) x min(-1), respectively). The reason for this is unclear, but cellular differences in lactate dehydrogenase isotype expression may be involved. Acetate is metabolized to CO2 more rapidly in astrocytes than in synaptosomes (rates of 0.43 and 0.02 nmol x mg protein(-1) x min(-1), respectively). This is likely due to cellular differences in the expression of monocarboxylate transporter subtypes.  相似文献

以人胎盘脐带组织为材料,提取组织总RNA,用netRTPCR方法合成人血管能抑素cDNA基因,将该cDNA克隆进pSP72载体获得重组质粒pSP72C, DNA序列分析结果与预期序列一致。用BamHⅠ和NdeⅠ双酶切,切下pSP72C上的血管能抑素cDNA,插入pET3c载体的相应位点获得重组表达质粒pETC, 转化E. coli BL21(DE3), SDSPAGE分析显示:在IPTG诱导下,血管能抑素基因获得了高效表达,表达量约占菌体总蛋白的 27.9 %,主要以包涵体形式存在。包涵体经过洗涤、裂解、蛋白复性以及Sephadex G75凝胶过滤层析等步骤后,获得了纯度达91.4 %的人血管能抑素。CAM实验证明10 μg纯化蛋白就能显著抑制鸡胚新生血管生成。  相似文献

The ability of partially purified human and guinea-pig haematogenous cell populations, when cultured in vitro, to metabolise arachidonic acid (AA) has been studied. Supernatants from 24 hour cell culture have been subjected to analysis for products of AA metabolism by gas chromatography with electron-capture detection.The cell types studied were human peripheral blood monocytes (both glass adherent and non-adherent), neutrophils, eosinophils and leukemic leucocytes; thoracic duct lymphocytes and lung alveolar macrophages. From the guinea-pig, induced and non-induced macrophage or neutrophil enriched peritoneal exudate populations, lymph node cells, peritoneal eosinophils and peripheral blood platelets were examined. Supernatants were assayed for the presence of PGE2, PGD2, PGF, TXB2 and 6-keto-PGF. In all types studied PGE2 and TXB2 were the major products formed. The identification of PGE2 and TXB2 was confirmed by GC/MS with multiple ion monitoring.The results have been compared with other reports and their possible significance discussed in relation to the proposed role of prostaglandins as mediators and modulators in immunopathology.  相似文献

To examine the hypothesis that foot-strike hemolysis alters vascular volumes and selected hematological properties is trained athletes, we have measured total blood volume (TBV), red cell volume (RCV) and plasma volume (PV) in cyclists (n = 21) and runners (n = 17) and compared them to those of untrained controls (n = 20). TBV (ml x kg(-1)) was calculated as the sum of RCV (ml x kg(-1)) and PV (ml x kg(-1)) obtained using 51Cr and 125I-labelled albumin, respectively. Hematological assessment was carried out using a Coulter counter. Peak aerobic power (VO2peak) was measured during progressive exercise to fatigue using both cycle and treadmill ergometry. RCV was 15% higher (P < 0.05) in male cyclists [35.4 (1.0), mean (SE); n = 12] and runners [35.3 (0.98); n = 9] compared to the controls [30.7 (0.92); n = 12]. Similar differences existed between the female cyclists [28.2 (2.1); n = 9] and runners [28.4 (1.0); n = 8] compared to the untrained controls [24.9 (1.4); n = 8]. For the male athletes, PV was between 19% (cyclists) and 28% (runners) higher (P < 0.05) in the trained athletes compared to the untrained controls. The differences in PV between the female groups were not significant. Although the males had a higher (P < 0.05) TBV, RCV and PV than the females, no differences between cyclists and runners were found for either gender. Mean cell volume was not different between the athletic groups. VO2peak (ml x kg(-1) x min(-1)) was higher (P < 0.05) in both male [68.4 (1.5)] and female [54.8 (2.1)] runners when compared to the untrained males [47.1 (1.0)] and females [40.5 (2.1)]. Although differences existed between the genders in VO2peak for both cyclists and runners, no differences were found between the athletic groups within a gender. Since the vascular volumes were not different between cyclists and runners for either the males or females, foot-strike hemolysis would not appear to have an effect on that parameter. The significant correlations (P < 0.05) found between VO2peak and RCV (r = 0.64 and 0.64) and TBV (r = 0.82 and 0.63) for the males and females, respectively, suggests a role for the vascular system in realizing a high aerobic power.  相似文献
