益血降浊解毒汤联合西药治疗狼疮性肾炎的临床研究

目的:观察益血降浊解毒汤对狼疮性肾炎(lupus nephritis,LN)临床疗效。方法:选择LN患者40例随机分为治疗组与对照组,每组20例,两组均使用激素(强的松)联合环磷酰胺(cyclophosphamide,CTX)疗法。治疗组在对照组基础上加用中药益血降浊解毒汤。两组患者均治疗1年,观察治疗前后两组临床症状、实验室检查(血浆白蛋白、24 h尿蛋白定量、肝功能、肾功能、血糖)、SLE临床活动度(Systemic Lupus Erythematosis Disease Activity Index,SLEDAI)及副作用变化。结果:治疗组治疗总有效率显著高于对照组,副作用发生率显著低于对照组(P0.05)。治疗后,两组的血浆白蛋白升高,24 h尿蛋白定量和血肌酐含量降低,且治疗组的变化优于对照组(P0.05)。治疗后,两组3,6,9,12月SLEDAI评分逐渐下降(P0.05),且治疗组低于对照组(P0.05)。治疗后,两组中医临床症状积分均降低,且治疗组更低(P0.05)。结论:益血降浊解毒汤联合西药治疗狼疮性肾炎具有明显优势,且能降低西药的副作用。

Clinical Study on Treatment of Lupus Nephritis with Yi Xue Jiang Zhuo Jiedu Decoction Combined with Western Medicine

ABSTRACT Objective: To observe the clinical effect of blood, turbidity and detoxification soup decoction on lupus nephritis (LN). Methods: 40 patients with LN were randomly divided into the treatment group and the control group, with 20 patients in each group. Routine hormone therapy (prednisone) combined with cyclophosphamide (CTX) was used in both groups. After one year of treatment, clinical symptoms laboratory tests(serum albumin, 24 h urinary protein quantification, liver function, renal function) and side effects of the two groups were observed. Results: The total effective rate of treatment group was significantly higher than that of control group, and the incidence of side effects was significantly lower than that of control group(P<0.05). After treatment, plasma albumin increased, 24 h urine protein quantification and serum creatinine content decreased in both groups, and The change in the treatment group was better than that in the control group (P<0.05). After treatment, the SLEDAI scores in the two groups decreased gradually in March, June, September, and December (P<0.05), and the treatment group was lower than that in the control group (P<0.05). After treatment, the clinical symptom scores and main symptoms of the two groups decreased, and the treatment group was lower than the control group (P<0.05). Conclusion: Blood, turbidity and detoxification soup combined with western medicine in the treatment of lupus nephritis has obvious advantages and can reduce the side effects of western medicine.
