Background

The intra-articular implantation of mesenchymal stromal cells (MSCs) as a treatment for knee osteoarthritis (OA) is an emerging new therapy. In this study, patients with knee OA received intra-articular implantations of autologous bone marrow–derived MSCs. We sought to assess the safety and efficacy of this implantation.

Materials and Methods

This was a phase 1/2 single-center, triple-blind, randomized controlled trial (RCT) with a placebo control. The subjects consisted of patients with knee OA randomly assigned to either an intra-articular implantation of MSCs (40?×?106 cells) or 5 mL normal saline (placebo). Patients were followed up for 6 months after the implantations. The pain level and function improvements for patient-reported outcomes were assessed based on a visual analog scale (VAS), Western Ontario and McMaster Universities Arthritis Index (WOMAC) and its subscales, walking distance, painless walking distance, standing time and knee flexion compared with the placebo group at 3 and 6 months following the implantations.

Results

Overall, 43 patients (Kellgren-Lawrence grades 2, 3 and 4) were assigned to either the MSCs (n?=?19) or placebo (n?=?24) group. Patients who received MSCs experienced significantly greater improvements in WOMAC total score, WOMAC pain and physical function subscales and painless walking distance compared with patients who received placebo. There were no major adverse events attributed to the MSC therapy.

Conclusion

This randomized, triple-blind, placebo-controlled RCT demonstrated the safety and efficacy of a single intra-articular implantation of 40?×?106 autologous MSCs in patients with knee OA. Intra-articular implantation of MSCs provided significant and clinically relevant pain relief over 6 months versus placebo and could be considered a promising novel treatment for knee OA. We propose that further investigations should be conducted over an extended assessment period and with a larger cohort.  相似文献

Familial Mediterranean fever (FMF) is a hereditary autoinflammatory disorder caused by mutations in the MEFV gene. The disease is especially common among Armenian, Turkish, Jewish and Middle East Arab populations. To identify the frequency and the spectrum of common MEFV mutations in different Iranian populations, we investigated a cohort of 208 unselected asymptomatic individuals and 743 FMF patients. Nine hundred and fifty-one samples were analysed for the presence of 12 MEFV mutations by PCR and reverse-hybridization (FMF StripAssay, ViennaLab, Vienna, Austria). Confirmatory dideoxy sequencing of all MEFV gene exons was performed for 39 patients. Fifty-seven (27.4%) healthy individual carried mutant MEFV alleles. Three hundred and ninety-one (52.6%) FMF patients were found positive for either one (172/743; 23.1%), two or three MEFV mutations. Using dideoxy sequencing, three novel variants, A66P, R202W and H300Q, could be identified. Our analysis revealed an allele frequency and carrier rate of 15.6 and 27.4%, respectively, among healthy Iranians. Still moderate compared to neighbouring Armenia, but higher than in Turkey or Iraq, these data suggest that FMF is remarkably common among Iranian populations. E148Q was most frequent in the group of healthy individuals, whereas M694V was the most common mutation among FMF patients, thereby corroborating previous studies on MEFV mutational spectra in the Middle East. Accordingly, MEFV mutations are frequent in healthy Iranian individuals across different ethnic groups. Based on this finding, the awareness for FMF and the implementation of augmented carrier screening programmes considering the multiethnic nature of the Iranian population should be promoted.  相似文献

Background

Avascular necrosis (AVN) of femoral head is a progressive bone disease due to ischemia of femoral head; patients experience pain and they can not do normal activity. There is not an effective way to treat the cause of this disease. In recent studies, treatment of this disease using pluripotent stem cell–derived mesenchyme is safe and effective, but this method needs more investigation. In this study, the safety and efficacy of CD133+ cells were evaluated as a novel method of stem cell therapy to treat AVN.

Methods

In this prospective quasi-experimental study, the participants were selected among patients with AVN who were referred to the Royan Cell Therapy Center. Autologous bone marrow–derived CD133+ cells were injected into the necrotic site of the femoral head during core decompression (CD). The Visual Analogue Scale (VAS), Harris Hip Score (HHS), Western Ontario and McMaster Universities Arthritis Index (WOMAC) and walking distance (WD) were measured before and 2, 6 and 12 months after CD.

Results

Overall, nine patients (six men and three women) were investigated in this study. Their mean age was 26 years old. All of them significantly improved in VAS, HHS, WOMAC and WD scores and they could do more activity without pain. Also, imaging findings demonstrated significant reductions in joint injuries. Significant complications were not seen in patients.

Discussion

This prospective quasi-experimental study demonstrated that, in patients with AVN, a single bone marrow–derived CD133+ cell injection into the necrotic site of the femoral head during CD is safe and effective in providing significant, clinically relevant pain relief and patients could do more activity over 2, 6 and 12 months. This pilot study suggested further clinical trials over an extended assessment period to approve bone marrow–derived CD133+ cell injection to treat AVN.  相似文献

A survey of C4 plants in Europe was performed with 216 species based on information in the literature and new studies. C4 species were found in 10 families: the eudicots Amaranthaceae, Chenopodiaceae, Euphorbiaceae, Molluginaceae, Nyctaginaceae, Polygonaceae, Portulacaceae and Zygophyllaceae and the monocots Cyperaceae and Poaceae. The majority of the C4 species belong to four families, Amaranthaceae (23), Chenopodiaceae (65), Cyperaceae (27) and Poaceae (88). In central and southern Europe, the abundance of native C4 plants varied between 44 and 88% of total C4 plants occurring, the rest being invasive C4 species. The occurrence of total C4 species, C4 monocots and C4 Chenopodiaceae was assessed for five major phyto‐geographical regions of Europe (north‐west, north‐east, central, south‐west, and south‐east). The abundance of C4 plants of total C4 dicots, C4 Chenopodiaceae, total C4 monocots, C4 Poaceae and C4 Cyperaceae was related to the climatic variables of annual mean daily temperature, annual precipitation and DeMartonne's aridity index. The abundance of total C4 plants decreases with increasing temperature and expression of aridity (decreasing aridity index) and is not correlated with precipitation. The abundance of total C4 dicots and C4 Chenopodiaceae is correlated with precipitation and aridity but not temperature, whereas the abundance of total C4 monocots, C4 Poaceae and C4 Cyperaceae is correlated with temperature and aridity but not precipitation. © 2010 The Linnean Society of London, Botanical Journal of the Linnean Society, 2010, 163 , 283–304.  相似文献

Euphorbia (Euphorbiaceae) is one of the largest and most diversified cosmopolitan genera of flowering plants. South‐west Asia is a major centre of diversity and contains c. 65 annual species, 35 of which occur in Iran. In this article, the seed morphology of all Iranian annual species was studied, including E. aulacosperma and E. rhabdothosperma, both new records for Iran. Quantitative and qualitative macro‐ and micromorphological features of seeds and caruncles were investigated using stereomicroscopy and scanning electron microscopy. Caruncle characters (shape, size, colour) and seed characters (shape, size, colour, ornamentation) are often constant and useful in identification and classification. Euphorbia acanthodes Akhani is described as a new gypsophilous species from south-west Iran. The affinity, morphological and carpological features, and habitat of the species are discussed. A list of a further 36 interesting endemic species of south-western Iran and adjacent areas is given with notes on their affinities and distribution. These include Centaurea gudrunensis Boiss. & Hausskn., as a new record from Iran. The specific status of Pteropyrum naufelum Al-Khayat is confirmed and it is suggested that P. noëanum Boiss. ex Meisn. is synonymous with P. aucheri Boiss. The biogeographic importance of the so-called 'Persian foothills', and biodiversity conservation status of the area are discussed, together with distribution maps of 12 species. It is concluded that the majority of the endemic species in the southern and south-western parts of Iran are Irano–Turanian or have their origins in the Irano–Turanian region. Seasonal changes in the fatty acid composition of the total lipid extracted from the whole body of Cydia pomonella L. larvae were determined by gas chromatography. The six most abundant fatty acids in both non-diapause and diapause larvae of codling moth were oleic (35%-39%), palmitic (23%-33%), linoleic (16%-30%), palmitoleic (5%-10%), stearic (1.5%-3.0%) and linolenic acids (1.0%-2.5%). This represents a typical complement of Lepidopteran fatty acids. The fatty acid composition of total lipid of C. pomonella larvae was related to diapause. In similarity to most other reports, the proportion of unsaturated fatty acids increased in diapause initiation state. The total lipid of diapause larvae contained more linoleic acid (25.8% vs. 16.1%) and less palmitic acid (24.7% vs. 33.4%), than that of non-diapause larvae. The weight percentage of linoleic acid (C 18:2) increased from 16% to 26% from early-August through early-September during transition to diapause, while palmitic acid (C16:0) decreased from 33% to 25% at the same time. These changes resulted in an increase in the ratio of unsaturated to saturated fatty acids (UFA/SFA) from 1.72 in non-diapause larvae to 2.63 in diapause larvae.  相似文献

South-West and Central Asia are major centres of diversity in the genus Heliotropium . On the basis of detailed taxonomic studies and information gathered from the literature, a synopsis of 61 known species of Heliotropium and two species of Arguzia in the area is given. Iran, with 32 species and 14 (sub)endemic species, has the highest diversity. The photosynthetic pathways of 42 taxa were determined using the isotope composition method. Except for H. marifolium Retz s.l. and H. rariflorum Stocks, all remaining species analysed showed δ13C values characteristic for C3 photosynthesis. Evidently, by contrast with the families Chenopodiaceae and Polygonaceae, the Irano-Turanian area is not an authochthonous region of developing C4 species in Heliotropium . The distribution maps of 57 taxa are provided and their biogeographical importance is discussed in order to elucidate the distribution patterns in South-West Asia. In the Irano-Turanian region, the Irano-Anatolian province of Zohary, which extends from central Anatolia to the western Himalaya, is a very large and vaguely defined phytochorion that should be split into smaller units. The consideration of southern Iran and adjacent Pakistan as part of the Sudanian or Saharo-Sindian regions ( sensu either Zohary or Léonard) cannot be accepted, because most endemic species in this area are either typical Irano-Turanian or isolated relicts. Furthermore, it is concluded that the Saharo-Sindian flora is not an autochthonous flora; most species are of transgressive origin from the surrounding phytochoria. White blister rust caused by Albugo candida (Pers.) Kuntze is a common and often devastating disease of oilseed and vegetable brassica crops worldwide. Physiological races of the parasite have been described, including races 2, 7 and 9 from Brassica juncea , B. rapa and B. oleracea , respectively, and race 4 from Capsella bursa-pastoris (the type host). A gene named WRR4 has been characterized recently from polygenic resistance in the wild brassica relative Arabidopsis thaliana (accession Columbia) that confers broad-spectrum white rust resistance ( WRR ) to all four of the above Al. candida races. This gene encodes a TIR-NB-LRR (Toll-like/interleukin-1 receptor-nucleotide binding-leucine-rich repeat) protein which, as with other known functional members in this subclass of intracellular receptor-like proteins, requires the expression of the lipase-like defence regulator, enhanced disease susceptibility 1 ( EDS1 ). Thus, we used RNA interference-mediated suppression of EDS1 in a white rust-resistant breeding line of B. napus (transformed with a construct designed from the A. thaliana EDS1 gene) to determine whether defence signalling via EDS1 is functionally intact in this oilseed brassica. The eds1-suppressed lines were fully susceptible following inoculation with either race 2 or 7 isolates of Al. candida. We then transformed white rust-susceptible cultivars of B. juncea (susceptible to race 2) and B. napus (susceptible to race 7) with the WRR4 gene from A. thaliana . The WRR4-transformed lines were resistant to the corresponding Al. candida race for each host species. The combined data indicate that WRR4 could potentially provide a novel source of white rust resistance in oilseed and vegetable brassica crops.  相似文献
