The unprecedented progress in bio-medical sciences and technology during the last few decades has resulted in great transformations in the concepts of health and disease, health systems and healthcare organisation and practices. Those changes have been accompanied by the emergence of a broad range of ethical dilemmas that confront health professionals more frequently. The classical Hippocratic ethical principles, though still retaining their relevance and validity, have become insufficiently adequate in an increasing range of problems and situations. Healthcare that has been practised for centuries on the basis of a direct doctor-patient relationship has been increasingly transformed into a more complex process integrating the health-team, the patient (healthcare seeker) and the community. Systematic review of the specialised literatures revealed that Healthcare Ethics education has become a basic requirement for any training programme for health professionals, and should cover the different stages of undergraduate, postgraduate and continuing education. Both theoretical foundations and practical skills are required for the appropriate ethical reasoning, ethical attitude and decision-making abilities. There is growing evidence that physicians' professional and moral development is not only determined by the formal curriculum of ethics; rather more, it is determined by the moral environment of the professional practice, the 'hidden curriculum' which deserves serious consideration by medical education.  相似文献

In buffaloes estrus synchronization provides an opportunity for enhanced use of AI; however, changes in hormone secretion during synchronization are poorly understood. The aim of this investigation was to determine if the concentration of progesterone metabolites in feces would correlate with the concentration of progesterone in blood and thus, could be used for noninvasive monitoring of the reproductive status in buffalo cows. Additionally, the influence of a norgestomet-estradiol treatment (CRESTAR-ear implant) was investigated. According to the clinical examination and the progesterone profile in blood samples during the three wks before the treatment, the 17 animals were allotted to 3 groups: 1) CL = presence of corpus luteum throughout the period of 3 wks before the treatment (n = 8); 2) CY = cyclic, corpus luteum present for less than 3 wks (n = 6); and 3) AE = anestrous, with inactive ovaries (n = 3). In the first group, 4 animals started an estrous cycle after implant withdrawal and conceived after natural mating. In the second group one of the cyclic cows showed estrus two d after implant withdrawal, the other 3 had a delayed estrus (12 to 16 d). The two cows which had had inactive ovaries at the beginning but were cyclic before the treatment started, remained cyclic after implant withdrawal but did not become pregnant. The 3 anestrous cows of the third group remained anestrous after the treatment. The progesterone concentration in blood clearly correlated with the concentration of the metabolites in feces. Therefore, this noninvasive method is a valuable tool for determining the luteal status, and such information may be useful for developing estrus synchronization regimens in buffalo cows.  相似文献

In view of its distinct geographical location and relatively small area, Tunisia witnessed the presence of many civilizations and ethnic groups throughout history, thereby questioning the origin of present-day Tunisian population. We investigated HLA class I and class II gene profiles in Tunisians, and compared this profile with those of Mediterranean and Sub-Sahara African populations. A total of 376 unrelated Tunisian individuals of both genders were genotyped for HLA class I (A, B) and class II (DRB1, DQB1), using reverse dot-blot hybridization (PCR-SSO) method. Statistical analysis was performed using Arlequin software. Phylogenetic trees were constructed by DISPAN software, and correspondence analysis was carried out by VISTA software. One hundred fifty-three HLA alleles were identified in the studied sample, which comprised 41, 50, 40 and 22 alleles at HLA-A,-B,-DRB1 and -DQB1 loci, respectively. The most frequent alleles were HLA-A*02:01 (16.76%), HLA-B*44:02/03 (17.82%), HLA-DRB1*07:01 (19.02%), and HLA-DQB1*03:01 (17.95%). Four-locus haplotype analysis identified HLA-A*02:01-B*50:01-DRB1*07:01-DQB1*02:02 (2.2%) as the common haplotype in Tunisians. Compared to other nearby populations, Tunisians appear to be genetically related to Western Mediterranean population, in particular North Africans and Berbers. In conclusion, HLA genotype results indicate that Tunisians are related to present-day North Africans, Berbers and to Iberians, but not to Eastern Arabs (Palestinians, Jordanians and Lebanese). This suggests that the genetic contribution of Arab invasion of 7th-11th century A.D. had little impact of the North African gene pool.  相似文献

One of the fundamental steps in pest sampling is the assessment of the population distribution in the field. Several studies have investigated the distribution and appropriate sampling methods for leaf-cutting ants; however, more reliable methods are still required, such as those that use geostatistics. The objective of this study was to determine the spatial distribution and infestation rate of leaf-cutting ant nests in eucalyptus plantations by using geostatistics. The study was carried out in 2008 in two eucalyptus stands in Paraopeba, Minas Gerais, Brazil. All of the nests in the studied area were located and used for the generation of GIS maps, and the spatial pattern of distribution was determined considering the number and size of nests. Each analysis and map was made using the R statistics program and the geoR package. The nest spatial distribution in a savanna area of Minas Gerais was clustered to a certain extent. The models generated allowed the production of kriging maps of areas infested with leaf-cutting ants, where chemical intervention would be necessary, reducing the control costs, impact on humans, and the environment.  相似文献

Reliable distribution maps are crucial for the management of invasive plant species. An alternative to traditional field surveys is the use of remote sensing data, which allows coverage of large areas. However, most remote sensing studies on invasive plant species focus on mapping large stands of easily detectable study species. In this study, we used hyperspectral remote sensing data in combination with field data to derive a distribution map of an invasive bryophyte species, Campylopus introflexus, on the island of Sylt in Northern Germany. We collected plant cover data on 57 plots to calibrate the model and presence/absence data of C. introflexus on another 150 plots for independent validation. We simultaneously acquired airborne hyperspectral (APEX) images during summer 2014, providing 285 spectral bands. We used a Maxent modelling approach to map the distribution of C. introflexus. Although C. introflexus is a small and inconspicuous species, we were able to map its distribution with an overall accuracy of 75 %. Reducing the sampling effort from 57 to 7 plots, our models performed fairly well until sampling effort dropped below 12 plots. The model predicts that C. introflexus is present in about one quarter of the pixels in our study area. The highest percentage of C. introflexus is predicted in the dune grassland. Our findings suggest that hyperspectral remote sensing data have the potential to provide reliable information about the degree of bryophyte invasion, and thus provide an alternative to traditional field mapping approaches over large areas.  相似文献
