目的摘要:目的对比分析白带常规镜检法、细菌预成酶谱分析法及联合法对5种常见下生殖道感染鉴别诊断的有效率。方法分别采用白带常规镜检法、细菌预成酶谱分析法对来我院就诊的3420例患者的阴道分泌物进行检测。结果采用白带常规镜检检出阴道感染患者1636例,其中BV患者543例、VVC患者820例、TV患者225例、CV患者48例;采用细菌预成酶谱分析法检出阴道感染患者1078例,其中AV患者461例、BV患者617例;采用联合法检出阴道感染患者2171例,其中AV患者461例、BV患者617例、CV患者48例、TV患者225例、VVC患者820例。结论联合法集白带常规镜检法和细菌预成酶谱分析法的优点与一体,能够鉴别诊断5种常见下生殖道感染。  相似文献

目的 评价北京市某医院开发研制的肝病十二分级诊疗系统疾病管理模式的实施效果。 方法 收集肝病十二分级诊疗系统实施前306名和实施后418名酒精性肝硬化患者病历数据,开展实施前后住院天数、住院费用以及治疗效果比较。 结果 实施后术前等待日减少1天,但总住院费用没有变化;常规治疗收入、材料费和药品收入变化有统计学意义;院内感染减少,第一诊断和第二诊断好转比例增加。 结论 实施肝病十二分级诊疗系统后,取得了一定的效果,建议在此基础上开发肝病相关病种的临床路径研究,科学测算肝病的住院天数和医疗费用。  相似文献

目的 分析上海市浦东新区区属综合性医院心力衰竭住院诊疗的过程质量。 方法 根据国家卫生和计划生育委员会对心力衰竭质量控制的要求,对浦东新区7所区属公立综合性医院随机抽取的145份病案进行评价。 结果 病人入院24小时内进行左心室功能首次评价的比例为77.44%,且住院期间都得到较好的健康教育;但部分心衰住院患者未能得到适宜的药物治疗,或使用后未记录。结论 医院加强对员工培训,强化对关键过程环节 (如心力衰竭住院患者的心功能评价、药物治疗与病案记录) 的监控与良好医疗行为的激励,以提高医疗质量水平。  相似文献

土壤生态毒理学是污染土壤生态风险评价以及土壤污染控制的理论依据,土壤环境中有毒物质的生态毒理效应及其分子机制的研究是土壤生态毒理学的核心内容.土壤污染生态毒理诊断集合了土壤污染的全部信息,提供了土壤的整体毒性效应.因此,开展土壤污染生态毒理诊断研究有重要的理论意义和现实意义.本文在参考国内外土壤污染生态毒理诊断研究成果的基础上,介绍了常用土壤污染生态毒理效应诊断方法和指标,并探讨了不同方法的利弊.传统毒理诊断方法往往只能表征胁迫的程度,不能解释损伤和响应发生分子机理.污染土壤毒性评估的生物标记物和土壤污染分子诊断技术,可深入探讨外源胁迫下生物的解毒机制.本文对其进行了较为系统的介绍,并对未来的研究趋势进行展望.  相似文献

The fast diagnosis of Mycoplasma primary atypical pneumonia is impaired by the lack of routinely available culture methods for isolation of Mycoplasma pneumoniae from clinical specimens. Likewise, serological methods commonly used for diagnosis are insensitive and non-specific. In this study, we have established and applied the polymerase chain reaction (PCR) technique to detect M. pneumoniae DNA in clinical samples originating from the respiratory tract. The PCR results were compared with those from culture and serology tests. To standardize the detection of M. pneumoniae by PCR, we first used DNA from culture grown organisms and clinical samples seeded with M. pneumoniae. PCR amplification was performed with M. pneumoniae-specific primers to amplify 144, 153 and 631 bp DNA fragments by using primer pairs MP5-1/MP5-2, P1-178/P1-331 and P1-178/P1-809, respectively. The amplification of the 631 bp DNA fragment was found to be most sensitive for the detection of M. pneumoniae. Using the most sensitive PCR, a total of 47 respiratory specimens from patients suspected of community acquired pneumonia were tested. While none of the specimens were positive for M. pneumoniae in culture, 6 specimens gave positive results by PCR. In 4 out of the 5 PCR positive samples tested serologically, the results were supported by elevated levels of anti-mycoplasma IgG/IgM/IgA. Thus, these results suggest that PCR is the most sensitive method to detect M. pneumoniae in clinical specimens.  相似文献

Schizophrenia (SZ) is a mental disorder, which affects the ability of human thinking, memory, and way of living. Manual screening of SZ patients is tedious, laborious and prone to human errors. Hence, we developed a computer-aided diagnosis (CAD) system to diagnose SZ patients accurately using single-channel electroencephalogram (EEG) signals. The EEG signals are nonlinear and non-stationary. Hence, we have used wavelet-based features to capture the hidden non-stationary nature present in the signal. First, the EEG signals are subjected to the the wavelet decomposition through six iterations, which yields seven sub-bands. The l1 norm is computed for each sub-band. The extracted norm features are disseminated to various classification algorithms. We have obtained the highest accuracy of 99.21% and 97.2% using K-nearest neighbor classifiers with ten-fold and leave-one-subject-out cross-validations. The developed single-channel EEG wavelet-based CAD model can help the clinicians to confirm the outcome of their manual screening and obtain an accurate diagnosis.  相似文献

Sitaxentan sodium (Thelin) is a once daily, orally bioavailable, highly selective endothelin A receptor antagonist. Initially approved for the treatment of pulmonary arterial hypertension, sitaxentan was withdrawn in 2010 following the recognition of a pattern of idiosyncratic liver injury. During development of this drug, a series of nonclinical studies investigated the effects of orally administered sitaxentan on fertility, embryofetal development, and pre‐ and postnatal development in the rat; results of these studies are reported here. In the fertility study, sitaxentan did not affect mating behavior, fertility, sperm morphology, or estrous cycle. Sitaxentan was teratogenic in the embyrofetal development study, which was expected based on its pharmacologic mechanism of action. Teratogenic effects included malformations of the head, mouth, face, and large blood vessels. In the pre‐ and postnatal study, sitaxentan administration was associated with reduced pup survival, large or abnormally shaped livers, and delays in markers of auditory and sexual development. Sitaxentan was detected in plasma of suckling pups receiving milk from females dosed with sitaxentan. These nonclinical study findings were reflected in the sitaxentan product label warnings. Prenatal exposure both to maternal psychiatric illness and psychiatric medication has been linked with adverse child outcomes that affect physiological, emotional and psychiatric development. Studies suggest that epigenetic mechanisms, such as DNA methylation, may facilitate these effects. In this report, we explore the association between maternal psychiatric illness and treatment during pregnancy and neonatal DNA methylation patterns in a prospectively-characterized clinical cohort of 201 dyads. Associations between the percent of umbilical cord blood DNA methylated at 27,578 CpG sites and maternal psychiatric diagnosis, symptoms and antidepressant use were evaluated by fitting a separate linear mixed effects model for each CpG site. There were no significant changes in neonatal DNA methylation attributable to maternal psychiatric diagnosis or depressive symptoms during pregnancy. Exposure to an antidepressant medication was associated with differential methylation of CpG sites in TNFRSF21 and CHRNA2 (false discovery rate < 0.05), but the average difference in methylation for both CpG sites was less than 3% between each group. The results were not specific to type of antidepressant or duration of the exposure. This study suggests that there are no large effects of maternal psychiatric illness, depressive symptoms or prenatal exposure to antidepressants on neonatal DNA methylation. Delineation of the influence of maternal psychiatric illness and pharmacological exposures on the developing fetuses has critical implications for clinical care during pregnancy.  相似文献

Currently, there is no method of accurately and non-invasively diagnosing pregnancy in polar bears. Specific proteins may exhibit altered profiles in the feces of pregnant bears, but predicting appropriate candidate proteins to investigate is speculative at best. The objective of this study was to identify potential pregnancy biomarker proteins based on their increased abundance in the feces of pregnant polar bears compared to pseudopregnant females (controls) using two-dimensional in-gel electrophoresis (2D-DIGE) and mass spectrometry (MS). Three 2D-DIGE gels were performed to evaluate fecal protein profiles from controls (n = 3) and pregnant polar bears (n = 3). There were 2224.67 ± 52.39 (mean ± SEM) spots resolved per gel. Of these, only five proteins were elevated in the pregnant group (P < 0.05), and seven additional spots tended to be higher (0.05 < P < 0.10). All 12 were submitted for MS analysis and the identities of 11 were ascertained with a >99.9% confidence interval. The 11 spots represented seven distinct proteins, five of which were significantly more abundant in the pregnant group: IgGFc-binding protein, filamin-C, carboxypeptidase B, transthyretin, and immunoglobulin heavy chain variable region. To our knowledge, this was the first study that employed 2D-DIGE to identify differentially expressed proteins in fecal samples to characterize a physiological condition other than those related to gastrointestinal disorders. These promising results provided a strong foundation for ensuing efforts to develop a non-invasive pregnancy assay for use in both captive and wild polar bears.  相似文献
