研究长沙参与体检的事业单位宫颈光滑女性人乳头瘤病毒(Human papillomavirus,HPV)感染率和分型。选择2016年1~12月在长沙市妇幼保健院体检中心进行体检的9 001例事业单位女性(4 325例宫颈光滑女性及4 676例宫颈外观异常女性),采用基因扩增和PCR+膜杂交技术进行HPV分型检测。HPV总感染率13.11%,宫颈光滑组HPV感染率13.83%,宫颈异常组HPV感染率12.45%,两组HPV感染率差异无统计学意义(P0.05),均以单一、高危型感染为主,两组患者单一感染率、高危HPV感染率均无统计学差异(P0.05)。正常组高危感染者中前三位的亚型依次为HPV52(占25.20%)、HPV58(占16.42%)、HPV53(占13.33%);异常组高危感染者中前三位的亚型依次为HPV52(占22.17%)、HPV58(占18.95%)、HPV16(占12.01%)。各年龄组中,感染率最高的为56~60岁组(正常组19.46%、异常组18.36%),感染率最低为20~25岁组(正常组10.84%、异常组5.88%)。目前无明确建议对宫颈光滑女性进行常规性HPV筛查,但本研究发现宫颈光滑女性亦有较高的HPV感染率,建议定期进行HPV筛查,对阳性者实行早期干预,从而降低宫颈癌发病率。  相似文献

宫颈癌是一种严重危害女性健康的恶性肿瘤,其发病率较高,位居女性恶性肿瘤的第二位,仅次于乳腺癌。自从1977年德国学者ZurHausen等从宫颈癌标本中发现了人乳头瘤病毒(Human papillomavirus HPV)DNA,并推测HPV感染与宫颈癌发生有关后,许多学者对HPV与宫颈癌的相关性进行了大量的研究,并证实HPV感染是宫颈癌发病的必需因素。目前,对于宫颈HPV感染检测有多种手段,其中聚合酶链反应(PCR)和捕获杂交技术在实验室中应用较广泛。在宫颈癌筛查中联合应用HPV检测和细胞学,不仅可以提高敏感性,而且还可以减少随诊频率,从而大大降低了宫颈癌的发生。  相似文献

目的:探讨高危型人乳头瘤病毒(HPV)感染与女性生殖道常见病原菌以及宫颈病变的关系。方法:选取2017年1月至2018年6月于成都市妇女儿童中心医院进行宫颈癌筛查的732例妇女为研究对象,所有受试者均行HPV检测、生殖道病原菌检测,判定宫颈病变程度,统计高危型HPV感染及亚型分布特征,分析高危型HPV感染与女性生殖道常见病原菌和宫颈病变的关系。结果:732例妇女HPV感染率为44.95%,高危型HPV占85.11%,HPV-16在高危型HPV中占比最高。生殖道常见病原菌中感染率最高的是沙眼衣原体,感染率为18.99%,存在女性生殖道常见病原菌感染者高危型HPV的检出率高于未感染者(P0.05),而低危型HPV检出率在女性生殖道常见病原菌感染者和未感染者无统计学差异(P0.05)。高危型HPV检出率随着宫颈病变程度加重而升高(P0.05)。结论:高危型HPV感染与女性常见生殖道病原菌感染和宫颈病变程度有关,高危型HPV感染率越高,发生宫颈癌的危险性越大。  相似文献

本研究旨在探讨人乳头瘤病毒(human papillomavirus,HPV)16感染与宫颈病变的关系,为宫颈癌防治提供科学依据。通过核酸杂交法进行HPV感染分型,纳入1 057例HPV阳性且行组织切片病理学检查的患者,对各级别宫颈病变中HPV16构成比、不同年龄组HPV16阳性患者中宫颈上皮内瘤样病变(cervical intraepithelial neoplasia,CIN)Ⅱ及以上病变的患病率,以及HPV16单一与多重感染患者中CINⅡ及以上病变的患病率进行分析。结果显示,在1 057例HPV阳性患者中,352例感染HPV16,CINⅢ中HPV16构成比最高,各级别病变中HPV16构成比差异有统计学意义。随着病变级别增加,HPV16构成比有增高趋势(P0.05)。不同年龄组HPV16阳性患者中CINⅡ及以上病变的患病率差异有统计学意义(P0.05),且随年龄增加而升高(P0.05)。HPV16单一、双重与三重以上感染患者中,CINⅡ及以上病变的患病率差异有统计学意义(P0.05),且随着感染型别种类增加,患病率降低(P0.05)。本研究显示,HPV16与高级别宫颈病变有较明显的相关性,老年HPV16阳性患者检出宫颈癌的概率更高。因此,应高度重视HPV16持续性感染,做到及时诊断与治疗,以减少宫颈高级别病变和宫颈癌的发生。  相似文献

目的研究人乳头瘤病毒(HPV)在宫颈炎症、宫颈癌前病变及宫颈癌中的水平及临床意义。方法分别选取宫颈炎症278例、宫颈癌前病变174例、宫颈腺癌82例、宫颈鳞癌183例患者,并以218例宫颈无病变的病例作为对照,所有患者均行薄层液基细胞学(TCT)检查及阴道镜下病理活检,检测各组患者间HPV阳性水平并对统计结果加以分析。结果 218例宫颈无病变病例中高危性HPV阳性率为9.6%(21/218),其余717例宫颈出现病变的患者中高危型HPV筛查结果阳性例数为405例,阳性检出率56.5%,且随着宫颈病变严重程度的加重,高危型HPV检出率逐渐升高(P0.01),但是宫颈腺癌和宫颈鳞癌组的高危型HPV阳性率比较差异无统计学意义(P0.05)。结论 HPV是宫颈癌前病变及宫颈癌的主要致病因素,对宫颈炎症、宫颈癌前病变及宫颈癌患者行HPV检测对于患者早日确诊及早日治疗具有重要意义。  相似文献

人乳头瘤病毒(简称HPV)能引起人皮肤及粘膜的多种良恶性肿瘤,特别多见于女性生殖系统如宫颈癌、尖锐湿疣和假性湿疣。但由于HPV尚不能在体外细胞培养中增殖,因此目前对其研究主要用核酸杂交和PCR方法,检测HPV DNA。本文采用光敏生物素标记HPV16、HPV18 DNA探针,分别对宫颈癌组织进行斑点杂交,检测宫颈癌组织中HPV16、18DNA并以正常宫颈组织做对照。以同样方法用HPV6b DNA探针检测尖锐湿疣及假性湿疣组织中HPV6b DNA。结果如下:18例宫颈癌组织中仅有1例与HPV16 DNA探针杂交阳性(5.7%);10例与HPV18杂交刚性(55.6%);4例为HPV16+18混合杂交阳性(22.2%)。12例尖锐湿疣组织中8例与HPV6b DNA杂交阳性(66.7%);12例假性湿疣组织中有6例与HPV6b探针杂交阳性(50%)。10例正常宫颈组织对照均为阴性。  相似文献

当前,宫颈癌仅次于乳腺癌成为威胁全世界女性健康的第二大恶性肿瘤。宫颈癌的产生和发展与人乳头瘤病毒(HPV)感染密切相关,在宫颈癌患者中几乎100%能检测到HPV感染。接种人乳头瘤病毒疫苗是预防宫颈癌前病变及宫颈癌的主要方法。欧美等发达国家宫颈癌死亡率相对较低,主要是因为他们建立了完善的宫颈癌筛查项目和HPV疫苗接种项目。而发展中国家的宫颈癌死亡率较高,发展中国家缺乏自主研发的HPV疫苗,人们对HPV和宫颈癌认知水平偏低,直接影响HPV疫苗的接受度。本文综述了影响发展中国家接种HPV疫苗的因素,主要包括人们的宫颈癌和HPV感染相关知识水平、文化背景、HPV疫苗安全性与局限性、接种费用等方面。通过加大宣传HPV感染和HPV疫苗的相关知识,由政府和正规医疗机构提供疫苗渠道,同时国家承担全部或部分接种费用,将会大大提高发展中国家人群接种HPV疫苗,有效降低宫颈癌的死亡率。  相似文献

目的:调查安康地区女性人乳头瘤病毒(HPV)感染的基因型别及年龄分布特征,分析其与宫颈癌的关系,为宫颈癌防治及HPV疫苗研发提供可靠的依据。方法:收集2010年6月-2012年8月间在本院及安康市部分县级医院妇产科就诊的2736名女性的液基细胞学和组织学标本,分为8个年龄组:16-24岁119例、25-29岁230例、30-34岁343例、35-39岁472例、40-44岁574例、45-49岁512例、50-54岁206例、55-86岁280例,进行病理学分类及HPV分型检测,分析不同年龄组及不同类型宫颈组织中的HPV感染率。结果:2736例女性中发生HPV感染720例(26.32%),共检出21种型别,感染率最高的基因型别是HPV16(25.05%),其他常见型别依次为HPV58、HPV52、HPV6、HPV11。单一感染占76.25%,多重感染占23.75%。HPV感染率在16-24岁、35-39岁和55-86岁三个年龄段出现高峰;而高危型HPV的感染率在35-39岁和55-86岁两个年龄段分别出现高峰。HPV的检出率随着宫颈病变的严重程度而增加,其中正常或炎症人群的HPV感染率显著低于宫颈病变及宫颈鳞状细胞癌患者(均P0.05),且意义未明的不典型鳞状细胞(ASCUS)、CIN1-3及宫颈鳞状细胞癌患者的HPV感染率对比结果存在显著差异(P0.05)。CIN1组、CIN2-CIN3组及宫颈鳞状细胞癌组单一感染率逐渐增加(P0.05),且其二重、三重感染率比较差异均有统计学意义(P0.05)。结论:安康地区HPV16型别感染较广,临床需加强对HPV16型单一感染宫颈病变患者的癌症预防工作。  相似文献

目的:研究人乳头状瘤病毒(HPV)感染与宫颈癌患者临床病理特征和Ki-67、细胞增殖抗原(PCNA)的相关性,从而为临床宫颈癌的诊治提供参考依据。方法:选取2016年3月~2018年6月于我院接受手术治疗的宫颈病变患者130例为研究对象。其中宫颈癌患者30例记为宫颈癌组,宫颈上皮内瘤变患者68例记为宫颈上皮内瘤变组,慢性宫颈炎患者32例记为对照组。采用免疫组织化学法检测各组宫颈组织中HPV感染、Ki-67以及PCNA阳性表达情况,并分析HPV与宫颈癌患者临床病理特征的关系及其与Ki-67、PCNA的相关性。结果:宫颈癌组、宫颈上皮内瘤变组患者HPV、Ki-67以及PCNA阳性率均高于对照组,宫颈癌组高于宫颈上皮内瘤变组(均P0.05)。临床分期Ⅲ~Ⅳ期以及淋巴结转移宫颈癌患者HPV感染率均明显高于临床分期Ⅰ~Ⅱ期与无淋巴结转移患者(均P0.05)。经Spearman相关性分析可得:宫颈癌患者HPV感染与Ki-67、PCNA表达均呈正相关关系(均P0.05)。结论:宫颈癌患者存在明显的HPV感染,且HPV感染与宫颈癌患者临床分期、淋巴结转移、Ki-67、PCNA表达存在一定相关性,临床可通过对HPV、Ki-67、PCNA进行联合检测,从而有助于宫颈癌的早期诊断。  相似文献

Human papillomavirus (HPV) infection is the main cause of cervical cancer, the second most common cancer in women worldwide. More than 200 types of HPV have been described, and within this range more than 40 types attack epithelium of genital tract. The types that were most commonly related to the development of cervical cancer are called high-risk types (HR HPV). There are very few studies about HPV prevalence in Croatia and there is an absolute lack of data for Split and Dalmatian County. Therefore, during a 6 month period, we routinely screened 570 women for HPV DNA at the Educational Public Health Institute of Split and Dalmatian County. HR HPV was detected in cervical exfoliated cells, by using Hybrid Capture 2 HPV DNA test. Out of the total number of tested women, 200 (35%) of them were positive to HR HPV Polymerase chain reaction (PCR) based assays were employed for HR HPV genotyping in positive specimens. The following frequency was observed: HPV 16 in 10%, HPV 18 in 6.1%, HPV 31 in 2.6%, HPV 33 in 1.9%, HPV 52 in 1.4%, HPV 59 in 0.7%, HPV 45 in 0.4% specimens, while 11.9% of tested specimens currently remained untyped. It is necessary to expand this study to a larger number of women, in order to better evaluate genital HPV types distribution among women in this region.  相似文献

Background: No accurate estimates of cervical cancer incidence or mortality currently exist in Georgia. Nor are there any data on the population-based prevalence of high-risk (HR) human papillomavirus (HPV) infection, which, in the absence of good-quality screening, is known to correlate with cervical cancer incidence. Methods: We obtained cervical cell specimens from 1309 women aged 18–59 years from the general population of Tbilisi, and also from 91 locally diagnosed invasive cervical cancers (ICC). DNA of 44 HPV types was tested for using a GP5+/6+-based PCR assay. Results: In the general population (of whom 2% reported a previous Pap smear) HPV prevalence was 13.5% (95% CI: 11.6–15.9), being highest in women aged 25–34 years (18.7%) and falling to between 8.6% and 9.5% for all age groups above 34 years. HR HPV prevalence was 8.6% overall, being 6.8% and 38.9% among women with normal and abnormal cytology, respectively. HPV45 (1.6%) was the most common type in women with normal cytology, whereas HPV16 predominated among women with cervical abnormalities (including 7 of 10 histologically confirmed cervical intraepithelial neoplasia 2/3) and among ICC (57.6%). The next most common types in ICC in Georgia were HPV45 and 18 (13.2 and 11.0%, respectively). Conclusions: We report a relatively high burden of HPV infection in Tbilisi, Georgia. Improving cervical cancer prevention, through screening and/or HPV vaccination, is an important public health issue in Georgia, where 70% of ICC are theoretically preventable by HPV16/18 vaccines.  相似文献

Human papilloma viruses (HPV), particularly type 16, have been associated with cervical cancer. It has been noted that the average onset of cervical cancer is occurring in younger women coupled with a higher prevalence of cervical HPV infection. However, the correlation between HPV 16 infection and the early onset of cervical cancer is still unclear. We hypothesize that HPV infection is an indicator of early onset of cervical cancer. To test this hypothesis, cervical smears from 197 women were evaluated by the polymerase chain reaction for HPV 16. These data revealed that the HPV 16-positive women were significantly younger than the HPV 16-negative women. Moreover, the average age of HPV 16-positive women with CIN 3 or invasive cancer was significantly younger compared with the other groups. These data clearly suggest that HPV 16 infection is a significant risk factor for the progression for cervical cancer in a young population of women.  相似文献

Cervical cancer is the second most common cancer in the women worldwide and the most frequent in developing countries, including India. Human papilloma virus (HPV) is the major etiological factor in cervical cancer patients. Host factors are also critical in regulating tumor growth and cytokines that modulate immunologic control may be of particular importance. In the present study, we investigated the correlation between the presence of HPV and type of cytokines expressed in cervical carcinomas and attempted to elucidate the possible reasons for the immune suppression. Cytokines investigated were type-1 cytokine IFN-gamma (shows immunostimulatory function and capable of limiting tumor growth) and type-2 cytokines IL-4, IL-10 and IL-6 (show immunosuppressive function and capable of stimulating tumor growth). Our data demonstrated the presence of HPV sub-types 16 and 18 in 86% and 13.8% of cervical tumor biopsies, respectively. The cervical tumor biopsies showed increased presence for mRNA for IL-10 and IL-1alpha, while none of the biopsies showed expression for IFN-gamma. A correlation was observed between the presence of HPV in cervical tumor biopsies and mRNA for IL-10. Increased percentages of CD4+CD25+ regulatory T cells (Tregs) were observed in circulation in cervical cancer patients, providing evidence for increased immune suppression. IL-10 may play a key role in maintenance of Tregs and explains the immunosuppressive state of cervical cancer patients.  相似文献

宫颈癌Cervical cancer是最常见的妇科恶性肿瘤,是威胁女性健康的第二大恶性肿瘤。宫颈癌疫苗是预防宫颈癌发病的有效途径。2019年以前葛兰素史克(GSK)和默沙东(MSD)垄断了全球宫颈癌疫苗药物市场。虽然国内宫颈癌疫苗起步较晚,但在国内创新政策驱动下,20种疫苗已进入临床阶段,特别是由厦门万泰联合厦门大学研发的馨可宁于2020年4月获批上市,它是我国首个自主研发、全球第三个的宫颈癌疫苗。相比较欧美等发达国家,我国在宫颈癌疫苗推广力度上还有较大差距,我国9~45岁女性的HPV疫苗接种率不足0.05%。面对HPV疫苗接种覆盖率低,提出了优化HPV疫苗审评审批流程、将HPV疫苗纳入国家免疫规划、提高女性对HPV疫苗的认识等对策建议。  相似文献

【背景】人乳头瘤病毒(human papillomavirus,HPV)感染与宫颈癌及癌前病变的发生密切相关,HPV疫苗是预防感染的重要手段,但HPV亚型分布具有明显的区域差异。【目的】了解川西高原地区妇女宫颈HPV感染现状,探讨其与阴道微环境改变之间的关系,为该人群HPV疫苗使用和宫颈癌预防工作提供指导。【方法】选取2017年11月至2019年5月在成都三六三医院体检和就诊的3 816例川西高原地区妇女,进行HPV基因分型、薄层液基细胞学检测(thinprep cytologic test,TCT)和阴道微环境检查,分析HPV感染率和基因型分布,并探讨高危型HPV (high risk HPV,hrHPV)感染与阴道微环境改变之间的关系。【结果】3 816例妇女的HPV总感染率为19.81% (756/3 816),hrHPV感染率为16.48% (629/3 816),HPV52在hrHPV感染中占比最高,达17.20% (135/785),后面依次为HPV58 (92/11.72%)、HPV16 (89/11.34%)、HPV53 (88/11.21%)、HPV18 (55/7.01%);415例宫颈细胞病变中有101例(24.3%)感染了hrHPV,基因分型排名前5位的亚型为HPV52/58/16/18/31,高度细胞病变hrHPV感染前5位是HPV52/58/18/31/16,低度细胞病变hrHPV感染前4位是HPV58/16/56/53/52/68/51 (后4个排名相同)。1 696例阴道微环境改变中,hrHPV感染率为19.16% (325/1 696),比阴道微环境正常者高(P<0.05),其中细菌性阴道炎(bacterial vaginitis,BV)与hrHPV感染有关联(P<0.05),单纯白细胞增高、真菌性阴道炎(vulvovaginal candidiasis,VVC)、滴虫性阴道炎(trichomonal vaginitis,TV)及混合感染与hrHPV感染无关联性(P>0.05)。【结论】预防川西高原地区妇女HPV感染及宫颈癌选择九价疫苗可能更加适宜,HPV53可作为国内HPV疫苗研制对象之一。另外,可把BV患者作为宫颈癌防控重点之一。  相似文献

Background

Knowledge of prevalence rates and distribution of human papillomavirus (HPV) genotypes prior high HPV vaccine coverage is necessary to assess its expected impact on HPV ecology and on cervical lesions and cancers.

Methods

Residual specimens of cervical cytology (N = 6,538) were obtained from 16 sites participating in organised cervical cancer screening pilot programs throughout France, anonymised and tested for HPV DNA using the PapilloCheck® genotyping test. Samples were stratified according to age of women and cytological grades.

Results

The age-standardised prevalence rates of HPV 16 and/or 18 (with or without other high-risk types) was 47.2% (95% Confidence Interval, CI: 42.4–52.1) in high-grade squamous intraepithelial lesions (HSILs), 20.2% in low-grade SIL (95% CI: 16.7–23.7) and 3.9% (95% CI: 2.8–5.1) in normal cytology. Overall HR HPV were detected in 13.7% (95%I CI: 11.7–15.6) of normal cytology. In women below 30 years of age, 64% of HSILs were associated with HPV16 and/or 18. In our study population, HPV16 was the most commonly detected type in all cervical grades with prevalence rates ranking from 3.0% in normal cytology to 50.9% in HSILs. HPV16 was also detected in 54% (27/50) of invasive cervical cancers including 5 adenocarcinomas.

Conclusion

HPV16 was strongly associated with cervical precancer and cancer. The high prevalence rates of HPV16/18 infection among women below 30 years of age with HSILs suggests that the impact of vaccination would be primarily observed among young women.  相似文献

Human papillomavirus (HPV) infection is the most common sexually transmitted infection, especially among young, sexually active individuals. As persistent infection with oncogenic types may lead to cervical cancer, HPV testing is a useful tool to screen for women at risk for subsequent development of cervical cancer. The aim of the study was to determine the prevalence of high-risk HPV (hrHPV) infection in different age groups of cytologically selected women from the Zagreb region, and to evaluate the frequency and results of repeat hrHPV testing. During a one-year study period (November 2005 to November 2006), a total of 3,440 cervical samples from women attending gynecological services of public and private health care systems were received. They were tested for 13 hrHPV genotypes by the polymerase chain reaction based AMPLICOR HPV test (Roche Molecular Systems). The overall prevalence of hrHPV was 34.6%. Most samples were obtained from women aged 21-30 years (44.2%), followed by the 31-40 (27.6%), 41-50 (15.7%), 51-60 (5.3%) and 261 (2.4%) age groups. Out of 3,227 cervical samples obtained from women of known age, 4.9% were obtained from the group of girls younger than 21, in which the highest prevalence of hrHPV (49.4%) was found. A similar prevalence was observed in women aged 21-30 (45.1%). The prevalence gradually decreased with age. During the study period, repeat hrHPV testing was performed in samples from 66 women at different intervals. Out of 28 women that were hrHPV negative on initial testing, only five women turned positive on repeat testing. Out of 38 women that were positive on initial testing, in one-third hrHPV could not be detected on repeat testing. As expected, hrHPV infection was highly prevalent in female adolescents and young women. Further investigation on repeat hrHPV testing is needed to assess virus clearance and rate of newly acquired infection.  相似文献

Background

Human Papillomavirus (HPV) -associated cervical cancer is the second-most common cancer in women worldwide but it is the most frequent gynaecological cancer and cancer associated death in India women. The objective of this study was to assess knowledge about cervical cancer, HPV, HPV vaccine, HPV vaccine acceptance among school and undergraduates students and their parent’s perception about acceptance of HPV vaccine in Northern part of India (Delhi and NCR regions).

Materials and Methods

A qualitative questionnaire based survey among 2500 urban/rural students aged 12–22 years was conducted.

Results

Overall, a low frequency (15%) of HPV and cervical cancer awareness was observed in students and their parents. However, the awareness was much higher in females belonging to urban setup compared to boys with a perception that HPV causes cervical cancer in women only. Additionally, only (13%) participants who were aware of cervical cancer and HPV) were willing to accept HPV vaccination. Apparently, parents of female students were two times more willing to accept HPV vaccination for their ward than male students (p<0.001; OR 95%CI = 2.09 (1.58–2.76).

Conclusion

Cervical cancer and HPV awareness among school, undergraduate students and also to their parents was found to be very low in this part of India. The level of awareness and education appears to be insignificant determinants in rural compared to urban setup. Better health education will be needed to maximize public awareness for cervical cancer prevention.  相似文献

Human papillomavirus type 16 (HPV16) is known to be a major causative agent of cervical cancer. To test the hypothesis that an enhanced Th1 response favors the natural course of cervical intraepithelial neoplasia (CIN), we measured IgG subclasses toward HPV16 L1-capsids because IgG1/IgG2 balance reflects Th2 and Th1 responses, respectively. We examined IgG2/IgG1 ratios in sera from 67 anti-HPV16 L1-positive women; 18 were cytologically normal women, 29 were CIN patients, and 20 were cervical cancer patients. The IgG2 dominance (IgG2/IgG1 ratio >1) was observed in 94, 48, and 5%, respectively (p < 0.001). The regression rate of CIN lesions was significantly different between patients with and without IgG2 dominance: 83.3% (5/6) versus 16.7% (1/6), respectively (p < 0.05). These findings raise the possibility that IgG2 dominance toward HPV16 L1-capsids, i.e., Th1 dominance, may be a useful marker to predict viral clearance or the regression of HPV16-positive CIN.  相似文献
