目的:观察在开放式鼓室成形术中,重建上鼓室对慢性中耳炎治疗的临床疗效。方法:86例(86耳)慢性中耳炎患者随机均分两组:两组均行开放式鼓室成形术,其中实验组应用自体乳突骨粉联合耳后肌骨膜瓣缩窄乳突根治腔,并垫高上鼓室内壁;对照组不行乳突根治腔缩窄术。回顾性观察两组患者在鼓膜状态(内陷及穿孔)、干耳时间、听力提高,头晕头痛、肉芽增生等几个方面的恢复情况。结果:通过对两组病例进行随访和疗效分析,在鼓膜状态、干耳时间、头晕头痛等方面,其临床疗效差异有显著性(P〈0.05);而在术后听力提高及肉芽增生方面无显著性差异(P〉0.05)。结论:在慢性中耳炎开放式鼓室成形术中,自体材料的应用缩短了千耳时间、提高了手术疗效,减少了手术相关的并发症。  相似文献

目的:探讨中耳炎一期鼓室成形术后听力重建效果及影响因素。方法:2008年6月至2019年9月在本院耳鼻咽喉科进行手术治疗的中耳炎患者70例,所有患者都给予乳突切除和鼓室成形术,其中行上鼓室切开+鼓室成形术3例,行完桥式乳突切除+鼓室成形术2例,行完壁式乳突切除+鼓室成形术25例,行开放式乳突切除+鼓室成形术40例;行Ⅰ型鼓室成形术24例,行Ⅱ型鼓室成形术42例,行Ⅲ型鼓室成形术4例;用颞肌筋膜修补鼓膜的29例,用耳屏软骨-软骨膜复合体41例。计算术前和术后气骨导间距(air-bone gap),即500 Hz、1 KHz、2 KHz和4 KHz 4个频率气导均值与骨导均值的差,术后气骨导间距不大于20 dB为有效,归于有效组,大于20 dB为无效,归于无效组。结果:有效例数为42例,无效例数为28例,有效率为60%。将术前和术后气骨导间距进行配对t检验和直线相关分析,两者有显著性差异(P0.05)和显著性相关(P0.05)。将术后听力效果(即有效和无效)作为因变量,以病人性别、病人年龄、病程、随访间隔、中耳炎种类、手术类型、鼓室成形类型和修补材料作为自变量,行Logisitic回归分析,未能发现对术后听力效果有影响的因素(P0.05)。结论:大多数患者受益于一期听力重建,部分患者听力重建效果不佳,其成因有待于进一步研究。  相似文献

摘要 目的:比较耳内镜与显微镜下Ⅰ型鼓室成形术治疗慢性化脓性中耳炎的疗效,并分析术后短期内听力恢复效果的影响因素。方法:选取2019年3月~2022年2月期间我院收治的158例慢性化脓性中耳炎患者,均接受Ⅰ型鼓室成形术治疗,根据手术方式不同分为耳内镜组(81例)和显微镜组(77例),比较两组临床疗效及术后6个月的听力恢复不良发生率。收集相关资料,采用多因素Logistic回归分析术后短期内听力恢复效果的影响因素。结果:两组鼓膜穿孔发生例数组间对比无统计学差异(P>0.05)。耳内镜组的手术时间、住院时间短于显微镜组,术中出血量、医疗费用、干耳时间>1个月例数、耳廓麻木发生例数少于显微镜组(P<0.05)。耳内镜组、显微镜组术后6个月的听力恢复不良发生率组间对比无统计学差异(P>0.05)。单因素分析结果显示,慢性化脓性中耳炎患者术后6个月听力恢复效果与鼓室黏膜、鼓膜张肌腱、咽鼓管情况、是否鼓室硬化、听小骨周围是否肉芽包裹、术前鼓室内是否有脓性分泌物有关(P<0.05)。多因素Logistic回归分析显示,鼓膜张肌腱缺损、咽鼓管不通、鼓室硬化、听小骨周围肉芽包裹、术前鼓室内有脓性分泌物是慢性化脓性中耳炎患者术后短期内听力恢复不良的危险因素(P<0.05)。结论:与显微镜下Ⅰ型鼓室成形术治疗慢性化脓性中耳炎相比,耳内镜下进行手术可缩短手术时间、住院时间,减少术中出血量和住院费用,降低并发症发生率。此外,患者术后短期内听力恢复效果受到鼓膜张肌腱、咽鼓管、鼓室硬化、听小骨周围肉芽包裹、术前鼓室内脓性分泌物等多种因素的影响。  相似文献

目的:研究和观察治疗胆脂瘤中耳炎患者时使用鼓室成形术联合乳突根治术的治疗效果。方法:收集胆脂瘤中耳炎患者共64例,随机分为观察组与对照组,各32例,对照组患者进行单纯的乳突根治术治疗,观察组患者结合鼓室成形术进行治疗,将两组患者的临床效果进行观察和对比。结果:观察组患者的治疗有效率明显高于对照组,观察组患者的鼓膜愈合成活率显著高于对照组,P均0.05。结论:在胆脂瘤中耳炎患者的治疗过程中使用鼓室成形术联合乳突根治术的治疗方案,能够有效提高治疗的效果,改善患者的听力,值得推广应用。  相似文献

目的:探讨先天性外耳畸形手术治疗的效果。方法:对12例先天性外耳道闭锁进行手术治疗。结果:10例分别接受外耳道成形或(和)鼓膜修补及鼓室成形术,2例术中未找见鼓室。术后1例出现面瘫,2例术后发生中耳炎,1例外耳道重新闭锁。术后半年听力有不同程度提高,术后三年听力提高程度有所下降。结论:治疗效果与手术并发症的发生与否有关,远期听力改善效果较近期差。  相似文献

目的:探讨外耳道胆脂瘤的临床特征和最佳治疗方法,提高临床诊治水平。方法:回顾性分析30例(32耳)外耳道胆脂瘤患者的临床资料。12耳病变局限于外耳道无明显骨质破坏,经耳内窥镜下行胆脂瘤清除病术;10耳病变局限于外耳道,但有较广泛的骨质破坏或有外耳道口狭窄者,经耳内切口显微镜下清除病变并加行外耳道成形术;10耳病变侵及鼓室或乳突,在显微镜下经耳后切口行乳突根治术。有条件者行乳突根治术加鼓室成形术。结果:本组病例所有患者术后病理均为胆脂瘤。术后外耳道口保持宽大,外耳道皮肤恢复正常,不产生上皮堆积。随访1年以上胆脂瘤无复发。结论:耳内镜与显微镜配合应用可彻底清除胆脂瘤,防止胆脂瘤残留和复发,并能提高听力.  相似文献

胆脂瘤型中耳炎由于其存在于中耳腔内的胆脂瘤能够对听骨链及周围骨质产生慢性破坏,引起继发感染,导致乳突炎,甚至脑脓肿的发生。乳突根治术是目前治疗胆脂瘤型中耳炎的主要方法之一。其治疗目的是在切除外耳道后上骨壁后,使鼓室、鼓窦、乳突腔及外耳道间形成向外开放的空腔,同时,能够彻底清除中耳乳突内的病变组织,利于引流,建立一个根除病灶、安全的中耳[1]。但在行乳突根治术后,由于空腔不能很好地形成上皮化,往往会反复出现由空腔处向外流脓现象,严重影响患者生活质量。传统的治疗方法多为采用碘仿纱条填塞,棉球擦拭法。我院在采用碘仿纱条填塞后,行理疗及肾上腺激素联合治疗,取得了一定的治疗效果,现报告如下。  相似文献

摘要 目的:探讨改良乳突根治术联合后上壁重建对上鼓室胆脂瘤型中耳炎患者听力改善及复发率的影响。方法:选取本院2015年5月-2020年10月收治的62例上鼓室胆脂瘤型中耳炎患者作为研究对象,随机将其分为改良组(n=31)和对照组(n=31)。改良组采用改良乳突根治术联合后上壁重建进行治疗,对照组采用乳突根治术进行治疗,对比两组患者手术前后听力情况、术后干耳所需时间等指标。结果:治疗前两组患者的气骨导差、气导听阀对比无明显差异(P>0.05),治疗后均降低,并且改良组低于对照组(P<0.05);改良组患者术后并发症发生率较对照组低(P<0.05);改良组患者的术后2周、4周、8周干耳率较对照组高(P<0.05),术后干耳所需时间低于对照组(P<0.05);改良组治疗总有效率较对照组高(P<0.05)。结论:将改良乳突根治术联合后上壁重建应用于上鼓室胆脂瘤型中耳炎患者当中,可提高患者听力,降低并发症,还可提高患者干耳率,缩短干耳时间,降低复发率,提高临床疗效,本研究值得临床借鉴。  相似文献

The anuran middle ear serves to transmit eardrum vibrations to the inner ear. In order to do this efficiently, the eardrum and middle ear must operate as an impedance transformer matching the low impedance of air to the higher impedance of the fluid-filled inner ear. In amniotes, one of the mechanisms used to achieve impedance transformation is to have the middle ear work as a force-amplifying lever system. Here, we present evidence that the grass frog middle ear also implements a lever system. The columellar footplate, which sits in the oval window, is firmly connected to the otic capsule along its ventral edge. Therefore, simple in-out movement of the columella is prevented while a rotational movement around the footplate's ventral edge is possible. The latter movement pattern was confirmed by laser vibrometry measurements of eardrum and footplate vibrations. The results showed that the footplate vibrations were 20–30 dB weaker than those of the eardrum and that the two structures vibrated 180° out of phase (at low frequencies). The lever ratio was approximately 6, i.e. somewhat higher than lever ratios reported for amniotes. Hence, the middle ear lever probably makes a significant contribution to impedance matching in frogs. Accepted: 1 July 1997  相似文献

This paper briefly recapitulates work on the preterrestrialphase of the evolution of hearing which was published in detailelsewhere. Following this, the problem of the origin of theterrestrial middle ear is examined in some detail, and it isdemonstrated that Eusthenopteron, and probably other rhipidistiansas well, possessed an eardrum. This drum was formed by the fusionof the distal surface of a spiracular diverticulum and a ligamentbetween parietal shield and squamosal. It is further demonstratedthat the geometry of this eardrum in relation to the middle-earcavity was adequate to allow the fish to make the transitionfrom aquatic hearing to terrestrial hearing without loss ofsensitivity.  相似文献

Although protein is the third most abundant solid in bile and is important in cholesterol crystal formation, methods for quantitating the concentration of total protein in bile have not been systematically evaluated. To establish a reliable protein assay for bile, we evaluated three protein assays (Lowry's method and the fluorescamine and Coomassie blue methods), and employed amino acid analysis as a reference technique. Large protein-to-protein variations were observed with the fluorescamine and Coomassie blue methods. Although all assays were affected by interfering substances, Lowry's method and the fluorescamine technique (after trichloroacetic acid precipitation and delipidation of bile) and the Coomassie blue method with native bile showed excellent correlations (P less than 0.0001) with those obtained by amino acid analysis. Using these reliable protein assays, we examined gallbladder bile obtained at surgery from subjects with and without gallstones. No differences in the concentrations of total biliary proteins were observed among patients with cholesterol (n = 23) or pigment (n = 7) gallstones and subjects without gallstones (n = 10). Protein values obtained by amino acid analysis also did not differ among groups. As expected, bile from patients with cholesterol gallstones was supersaturated with cholesterol while bile from nongallstone subjects and those with pigment stones was unsaturated. These results indicate that it is not possible to separate patients with and without gallstones on the basis of the total protein concentration of gallbladder bile.  相似文献

The human ear is a complex biomechanical system and is divided into three parts: outer, middle and inner ear. The middle ear is formed by ossicles (malleus, incus and stapes), ligaments, muscles and tendons, which transfers sound vibrations from the eardrum to the inner ear, linking with mastoid and Eustachian tube. In this work, a finite element modelling of the tympano-ossicular system of the middle ear was developed. A dynamic study based on a structural response to harmonic vibrations, for a sound pressure level (SPL) of 110, 120 and 130 dB SPL applied in the eardrum, is presented. The connection between the ossicles is made using a contact formulation. The model includes the different ligaments considering its hyperelastic behaviour. The activation of the muscles is based on the constitutive model proposed by previous work. The harmonic responses of displacement and pressure obtained on the stapes footplate, for a frequency range between 100 Hz and 10 kHz, are obtained simulating the muscle activation. The results are compared considering the passive and active states. The results are discussed and they are in accordance with audiological data published with reference to the effects of the middle ear muscles contraction.  相似文献

目的:观察腺样体手术治疗儿童分泌性中耳炎的临床效果。方法:选择分泌性中耳炎合并腺样体肥大患儿80例(160耳),将其随机分为手术组和对照组,每组40例,手术组患儿给予腺样体消融手术治疗,而对照组患儿进行保守治疗,观察和比较两组治疗1个月后的临床疗效。结果:治疗1个月后,手术组治愈72耳,好转6耳,治疗总有效率为97.5%,而对照组治愈8耳,好转20耳,治疗总有效率为35%,较手术组显著降低(P0.05)。结论:腺样体切除是治疗分泌性中耳炎的有效途径,治疗中应尽可能避免并发症的发生,恢复和保护咽鼓管的生理功能。  相似文献

In many birds, the middle ears are connected through an air-filled interaural pathway. Sound transmission through this pathway may improve directional hearing. However, attempts to demonstrate such a mechanism have produced conflicting results. One reason is that some species of birds develop a lower static air pressure in the middle ears when anaesthetized, which reduces eardrum vibrations. In anaesthetized budgerigars with vented interaural air spaces and presumed normal eardrum vibrations, we find that sound propagating through the interaural pathway considerably improves cues to the directional hearing. The directional cues in the received sound combined with amplitude gain and time delay of sound propagating through the interaural pathway quantitatively account for the observed dependence of eardrum vibration on direction of sound incidence. Interaural sound propagation is responsible for most of the frontal gradient of eardrum vibration (i.e. when a sound source is moved from a small contralateral angle to the same ipsilateral angle). Our study confirms that at low frequencies the interaural sound propagation may cause vibrations of the eardrum to differ much in time, thus providing a possible cue for directional hearing. The acoustically effective size of the head of our birds (diameter 28 mm) is much larger than expected from the dimensions of the skull, so apparently the feathers on the head have a considerable acoustical effect.Dedicated to Professor Franz Huber on the occasion of his 80th birthday.  相似文献

目的:探讨糖尿病性下肢缺血性血管病的治疗方法。方法:对24例Ⅰ型糖尿病患者合并下肢动脉缺血性疾病的39条肢体体进行了手术,其中动脉旁路手术31例,占79%。对3例患者进行了下肢截肢处理。结果:接受动脉旁路手术的患者出院时动脉血流均保持通畅。一例51岁男性患者四肢肿胀、变黑,经治疗无效死亡。结论:糖尿病性缺血性血管病可以通过外科手术治疗。如下肢远端动脉旁路移植、腔内血管成型术等。外科治疗的方法正在探讨阶段,研究方法不断进展。外科治疗不仅可以挽救肢体或降低截肢平面,而且可为足部创面的愈合提供较好的营养环境,有利于创面的愈合和提高生活质量。  相似文献

We investigated directionalities of eardrum vibration and auditory nerve response in anesthetized northern leopard frogs (Rana pipiens pipiens). Simultaneous measures of eardrum velocities and firing rates from 282 auditory nerve fibers were obtained in response to free-field sounds from eight directions in the horizontal plane. Sound pressure at the external surface of the ipsilateral eardrum was kept constant for each presentation direction (± 0.5 dB). Significant effects of sound direction on eardrum velocity were shown in 90% of the cases. Maximum or minimum eardrum velocity was observed more often when sounds were presented from the lateral and posterior fields, or from the anterior and contralateral fields, respectively. Firing rates of 38% of the fibers were significantly affected by sound direction and maximum or minimum firing rate was observed more frequently when sounds were delivered from the lateral fields, or from the anterior and contralateral fields, respectively. Directionality patterns of eardrum velocity and nerve firing also vary with sound frequency. Statistically significant correlation between eardrum velocity and nerve fiber firing rate was demonstrated in only 45% of the fibers, suggesting that sound transmission to the inner ear through extratympanic pathways plays a non-trivial role in the genesis of directionality of auditory nerve responses.Abbreviations CF characteristic frequency - SVL snout-vent length - TM tympanic membrane  相似文献

报道须癣毛癣菌引起的人兔共患体癣3例.患者皮损均为环状红斑丘疹,上覆鳞屑,炎症明显,伴瘙痒,均与兔子有密切接触史,且兔子均有明显皮损.取患者及兔子的皮屑真菌镜检及培养,鉴定为须癣毛癣菌,经口服及外用抗真菌药物后治愈.  相似文献

目的:探讨颅底脊索瘤的CT、MRI表现及治疗措施。方法:回顾性分析经病理证实的26例脊索瘤患者的临床资料。肿瘤位于鞍区及鼻咽部者9例,位于中颅窝者5例,位于后颅窝者8例,混合型4例。手术入路:额颞入路-翼点开颅9例,鼻内镜下经蝶入路5例,颞下、颞枕及扩大中颅窝入路8例,4例未手术行放射治疗。结果:肿瘤全切8例,大部及次全切14例,围手术期未见死亡病例。25例患者获得随访;3例于术后1年内复发,其中2例死亡,余者颅神经损伤及肢体症状均有改善。结论:脊索瘤无典型临床表现,现多可通过术前影像学检查得以诊断,少数位于特殊位置者需与垂体瘤,颅咽管瘤,三叉神经鞘瘤等鉴别;肿瘤全切较为困难,根据需要选择相应的手术入路可明显提高手术效果;放疗可作为术后辅助治疗。  相似文献
