股动脉及股静脉血气在烧伤脓毒症患者早期诊断价值

摘    要:目的:探索股动脉及股静脉血气对烧伤脓毒症患者早期诊断价值。方法:选择自2009年7月至2014年12月我院收治的特重烧伤患者80例,根据是否患有脓毒症,将35例脓毒症患者作为观察组,45例非脓毒症患者作为对照组。在所有患者左侧股动脉、股静脉置管,并抽血,进行血气分析。结果:动脉血气分析中观察组患者股动脉血中HCO3-水平明显低于对照组,且差异具有统计学意义(P0.05);静脉血气分析中观察组患者股静脉血中Pv CO2显著高于对照组,Pv O2、Sv O2、HCO3-水平明显低于对照组,且差异均具有统计学意义(P0.05)。观察组患者的△PO2、△PCO2、△SO2、△HCO3-、△p H水平均显著高于对照组,且差异均具有统计学差异(P0.05)。结论:股动脉和股静脉血气分析可反映出机体组织的血流灌注情况及氧的供需状态,对烧伤脓毒症患者早期诊断具有重要价值。

The Early Diagnosis Value of Femoral Artery and Vein Blood Gas in Patientswith Burn Sepsis
Abstract:Objective:To explore the early diagnosis value of femoral artery and vein blood gas in patients with burn sepsis.Methods:According to whether the patients with sepsis, from July 2009 to December 2014, 80 cases of patients with extra heavy burn. Therewere 35 patients with sepsis in observation group, while there were 45 cases of patients without sepsis as control group.The left femoralartery and vein were cathetered in all patients, blooding and analysising the blood gas.Results:The HCO3- level of femoral artery in theobservation group was significantly lower than the control group in arterial blood gas analysis, which the difference was statistically significant(P<0.05); the PvCO2 level in the observation group of patients was significantly higher than the control group, while the levels ofPvO2, SvO2, HCO3-were significantly lower than the control group in the vein blood gas analysis,which the differences were statisticallysignificant (P<0.05). The levels of DPO2, DPCO2, DSO2, DHCO3-,DpH in observation group of patients were significantly higher than thatof control group, which the differences were statistically difference (P<0.05).Conclusion:Femoral artery and venous blood gas analysiscan reflect the blood perfusion and oxygen supply and demand state of body tissues, which has important value for early diagnosis of patientswith burn sepsis.
