能量平台在腹腔镜结直肠癌根治术中的临床研究

摘    要:目的:探讨能量平台在腹腔镜结直肠癌根治术中的应用价值,为指导临床治疗提供参考依据。方法:按照随机数字表法将2012年3月~2014年3月我院收治的结直肠癌患者分为两组,观察组行能量平台腹腔镜结直肠癌根治术,对照组行传统开腹结直肠癌根治术,术后比较两组的手术效果及并发症情况。结果:观察组的手术时间、术中出血量以及术后住院时间均小于对照组,淋巴结清扫数量大于对照组,差异均有统计学意义(P0.05),两组的术后引流量比较,差异无统计学意义(P0.05)。手术后两组的并发症主要有切口感染、吻合口瘘、局部病灶复发、肠梗阻以及腹腔内出血,其中观察组切口感染发生率为1.85%,低于对照组的14.55%,差异有统计学意义(P0.05),两组的其它并发症发生率比较,差异无统计学意义(P0.05)。结论:能量平台辅助腹腔镜结直肠癌根治术能有效地减少术中出血量、手术时间以及术后住院时间,术后感染几率小,因此在临床上有一定的推广应用价值。

Clinical Study on Energy Platformin Laparoscopic Resection of ColorectalCancer
Abstract:Objective:To explore the application value of energy platform in laparoscopic resection of colorectal cancer and toprovide the reference for guiding clinical treatment.Methods:A total of 109 patients with colorectal cancer, who were admitted to XinjiangUygur Autonomous Region People''s Hospital from March 2012 to March 2014, were divided into observation group (n=54) andcontrol group (n=55). The observation group was treated with energy platform combined with laparoscopic resection of colorectal cancerand the control group, with traditional open resection of colorectal cancer. Then the effect and complications of two groups after operationwere compared.Results:The operation time, postoperative bleeding amount and postoperative hospitalization time in observationgroup were less than those in control group, and the number of lymph node dissection of observation group was more than that of controlgroup, the difference had statistical significance (P<0.05); There was no statistically significant difference in the postoperative drainagevolume of two groups (P>0.05). The major postoperative complications in two groups were wound infection, anastomotic fistula, localfocus recurrence, intestinal obstruction and abdominal bleeding, among which the incidence (1.85%) of wound infection in observationgroup was was less than that(14.55%) in control group, the difference was statistically significant(P<0.05), while the other complicationsof two groups had no statistically significant difference (P>0.05).Conclusion:Energy platform combined with laparoscopic resection ofcolorectal cancer can effectively reduce the postoperative bleeding amount, operation time and postoperative hospitalization time, and ithas a lower probability of wound infection, which is worthy of clinical application.
