经鼻蝶窦入路垂体腺瘤显微切除术的疗效及安全性观察

目的:探讨经鼻蝶窦入路垂体腺瘤显微切除术的疗效及安全性。方法:回顾性分析2004年12月至2015年9月于我科进行经鼻蝶窦入路垂体腺瘤切除及经颅入路垂体腺瘤切除术患者的临床资料,随机分为观察组及对照组,比较两组患者的手术情况、疗效及并发症发生情况。结果:观察组患者在手术时间、术中出血量、引流量、下地时间、术后住院时间及全切率等方面均明显优于对照组,差异具有统计学意义(P0.05)。观察组治愈率为63.1%,对照组为39.7%,观察组明显高于对照组(P0.05)。观察组术后并发症发生率为15.4%,对照组为39.7%,观察组显著低于对照组(P0.05)。结论:经鼻蝶窦入路垂体腺瘤显微切除术具有良好的临床疗效,且创伤小,安全性高,值得临床推广应用。

Observation on the Efficacy and Safety of Microscope Assisted NasalTranssphenoidal Pituitary Adenoma Resection

Objective:To explore the efficacy and safety of microscope assisted nasal transsphenoidal pituitary adenoma resection.Methods:Clinical data of pituitary adenoma patients accepted in our department from Dec.2004 to Nov.2015 were retrospected analysisand divided into observation group and control group according the different surgery. The observation group was given nasal transsphenoidalpituitary adenoma resection, and the control group was given transcranial pituitary adenoma resection. Then the surgery situation,efficacy and safety of two groups were observed and compared.Results:The surgery time, blood loss, fluid volume, ambulation time,hospital stay and total removal rate of observation group were much better than control group, which had statistically significance (P<0.05). The cure rate of observation group (63.1%) was obviously higher than the control group (39.7) with statistically difference (P<0.05).The complication rate of observation group (15.4%) was significantly lower than the control group (39.7%) with statistically difference(P<0.05).Conclusion:Microscope assisted nasal transsphenoidal pituitary adenoma resection has good clinical efficacy with little traumaand higher safety, which is worthy clinical application.
