重组人骨保护素对聚乙烯颗粒刺激的人工关节置换术患者外周血IL-6和TNF-α表达的影响

摘    要:目的:探讨重组人骨保护素(rhOPG)对聚乙烯颗粒刺激的人工关节置换术患者外周血IL一6和TNF一0l表达的影响,分析rhOPG防止关节置换术后假体无菌性松动的应用价值。方法:收集2009年1月-2011年3月我院收治的人工关节置换术患者的外周血清60份,采用梯度离心法分离出单个核细胞,并将其随机分成5组,每组12份。对照组:单个核细胞,模型组:单个核细胞+1X109/mL聚乙烯颗粒,低剂量组:单个核细胞+1×109/mLL聚乙烯颗粒+10ng/mLrhOPG,中剂量组:单个核细胞+1X109/mL聚乙烯颗粒+100ng/mLrhOPG,高剂量组:单个核细胞+l×109/mL聚乙烯颗粒+1000ng/mLrhOPG。通过酶联免疫酶联免疫法(ELISA)'~4定各组细胞上清液中白介素-6(IL-6)和肿瘤坏死因子(TNF-α)的含量,并进行组间比较。结果i(1)模型组、低剂量组、中剂量组、高剂量组的IL-6、TNF-α.含量均显著高于对照组(P〈0.05)。(2)低剂量组、中剂量组、高剂量纽的IL-6、TNF—α浓度均较模型组低(P〈0.os)。(3)随着rhOPG浓度的增加,IL-6和TNF.仪含量逐渐降低,组间比较差异有统计学意义(P〈0.05)。结论irhOPG可抑制聚乙烯颗粒刺激的人工关节置换术患者外周血1L-6和TNF—α.的表达,可有助于防治人工关节置换术后的无菌性关节松动。关键词:人工关节置换术;重组人骨保护素;假体松动;白细胞介素-6;肿瘤坏死N子

Abstract:Objective: To discover the effect of recombinant human osteoprotegerin (rhOPG) on polyethylene particles-induced peripheral serum IL-6 and TNF-o~ expressions of patients after artificial joint replacement, and to explore the feasibility of rhOPG in preventing prosthetic aseptic loosening after joint replacement. Methods: 60 portions of peripheral serum of patients with artificial joint replacement in our hospital from Jan. 2009 to Mar. 2011 were collected and isolated to single karyocyte by gradient centrifugation, then randomly divided into five groups, 12 portions in each groups. In control group: single karyocyte; in model group: single karyocyte + 1 x 109/mE polyethylene particles; in low dose group: single karyocyte + 1 x 109/mE polyethylene particles +10 ng/rnL rhOPG; in medium dose group: single karyocyte + 1 x 109/mE polyethylene particles + 100 ng/mL rhOPG; in high dose group: single karyocyte + 1 ~ 109/mL polyethylene particles + 1000 ng/mL rhOPG. Determine interleukin -6 (IL-6), tumor necrosis factor (TNF-alpha) content changes of monocyte secreted in each cell supernatant fluid by using enzyme-linked immune enzyme-linked immunoassay (ELISA). Results: (1) The concentrations of 1L-6 and TNF-alpha were higher in the model group, low dose group, medium dose group and the high dose group than in the control group (P〈0.05). (2) The concentrations of IL-6 and TNF-alpha were lower in the low dose group, medium dose group and high dose group than in the model group (P〈0.05). (3) The concentration of IL-6 and TNF-alpha reduced gradually as rhOPG concentration increased (P〈0.05). Conclusion: rhOPG could inhibit the polyethylene particles-induced peripheral serum of IL-6 and TNF-alpha expressions to achieve the purpose of prevention and treatment on aseptic loosening after artificial joint replacement.
