摘    要:为探讨纤维连接蛋白(Fn)、纤维蛋白原(Fg)、补体 (C5)、肌红蛋白(Mb)、肌动蛋白(HHF35)、结蛋白(Dm)六种指标在诊断早期心肌梗死的敏感性,建立大鼠急性心肌缺血模型,应用免疫组织化学方法和图像分析与统计学处理系统, 检测缺血心肌细胞内Mb、HHF35、Dm的缺失面积和Fn、Fg、C5的阳性反应面积.结果Mb 、HHF35、Dm在大鼠急性心肌缺血15 min即可出现缺失,Fg、C5在心肌缺血15min即可出现阳性反应,而Fn直到心肌缺血3 h才出现阳性反应.随缺血时间延长,Dm、HHF35、Mb的缺失面积和Fg、C5、Fn的阳性反应面积逐步增大.表明Dm、HHF35、Mb、Fg、C5是反映早期心肌梗死的灵敏的形态学指标,Fn的敏感性不及前五种指标.

Abstract:To investigate the sensitivity of fibronectin(Fn), fibrinogen(Fg), complement(C 5), myoglobin(Mb), actin(HHF35) and desmin(Dm) for the postmortem diagnosis of early myocardial infarction, the myocardial ischemia model was established in rats. The areas of Mb, HHF35 and Dm depletion,and the areas of Fn, Fg and C 5 positive reaction in the ischemic myocardial cells were studied with immunohistochemistry, image analysis technique and statistical system. The results showed that the depletion of Dm, HHF35 and Mb as well as the positive staining of Fg and C 5 in ischemic myocardial cells appeared at 15 min after myocardial ischemia, but the positive staining of Fn did not occur until 3 hours after the ischemia. With the lapse of time, the areas of Dm, HHF35, Mb depletion and Fg, C 5 Fn positive staining gradually enlarged. It is suggested that Mb, HHF35, Dm, Fg and C 5 are more sensitive markers than Fn for the postmortem diagnosis of myocardial infarction.
