医院内尿路感染致病菌变迁及其耐药性监测分析

摘    要:目的调查院内尿路感染致病菌分布及耐药性变化情况.方法对我院1998年1月-2000 年12月医院内尿路感染患者分离的细菌菌株、真菌菌株及细菌耐药性进行回顾性调查.结果 医院内尿路感染仍以G 菌为主(39.9%),其次为真菌(32.9%),G+菌(27.2%);G -菌以大肠埃希菌为主(41.7%),G+菌以D组链球菌为主(50.3%),真菌以白色念珠菌为主(44.7%),与19 98年分离的菌株相比,2000年G 菌所占比例有下降趋势,而真菌由25.7%上升至36.6%(P< 0.05),3年间主要病原菌对常用抗生素的耐药率基本呈上升趋势.结论院内尿路感染致病菌正在发生变迁 ,耐药性严重,临床应重视病原学检查,开展细菌耐药性监测,合理使用抗生素.

Abstract:Objective: To evaluate the pathogens isolated from nosocomial urinary tract infection and their drug resistance.Methods: The bacteria and fungi isolated from nosocomial urinary tract infectious patients were retrospectively analysed during Jan.1998 to Dec.2000.Results: The most prevalent pathogens isolated from nosocomial urinary tract infection were still Gram-negative bacteria(39.9%),then fungi(32.9%)and Gram-positive bacteria(27.2%).In Gram-negative bacteria Escherichia coli was the major one(41.7%)and in Gram-positive bacteria group D Streptococcus(50.3%)was most prominent,Candida albicans(44.7%)was the main type of fungi among all isolated fungi.The proportion in 2000 of Gram-negative bacteria declined comparing with that in 1998,and the percentage of fungi increased from 25.7% (in 1998) to 36.6%(in 2000).The trend of the resistance of main bacteria to most commonly used antibiotics was going up during 1998 to 2000.Conclusions:The kinds of clinical bacteria in nosocomial urinary tract inrection are changing,their drug resistance to antimicrobial agents is serious.It is suggested that there is ungent need for surveillance of bacterial resistance and rational use of antimicrobial agents should be emphasised during clinical therapy.
