阳和膏湿热敷治疗乳腺增生症的前瞻性随机对照研究

摘要 目的：观察阳和膏湿热敷治疗乳腺增生症的临床应用价值。方法：根据随机数字表法将湖南中医药大学第一附属医院2020年3月~2021年11月期间接收的97例乳腺增生症患者分为对照组（安慰剂治疗）和研究组（阳和膏湿热敷治疗），例数分别为48例和49例。对比两组疗效、激素指标、乳腺增生相关临床指标、疼痛情况及不良反应发生率。结果：研究组的临床总有效率明显高于对照组（P<0.05）。治疗后，研究组视觉疼痛模拟评分法（VAS）评分低于对照组（P<0.05）。治疗后，研究组乳腺腺体厚径、乳腺导管最粗管径、乳房肿块硬度评分低于对照组（P<0.05）。治疗后，研究组血清雌二醇（E₂）、催乳素（PRL）水平较对照组低，而血清孕酮（P）水平较对照组高（P<0.05）。两组不良反应发生率组间对比无差异（P>0.05）。结论：阳和膏湿热敷治疗乳腺增生症疗效显著，可促进症状改善，缩小乳腺腺体厚径、乳腺导管最粗管径，降低乳房肿块硬度，同时还可调节E₂、P、PRL分泌，安全可靠。

A Prospective Randomized Controlled Study of Yanghe Ointment Wet Hot Compress in the Treatment of Breast Hyperplasia

ABSTRACT Objective: To observe the clinical application value of Yanghe ointment wet hot compress in the treatment of breast hyperplasia. Methods: According to the random number table method, 97 patients with breast hyperplasia who were received in The First Affiliated Hospital of Hunan University of Traditional Chinese Medicine from March 2020 to November 2021 were divided into control group (placebo treatment) and study group (Yanghe ointment wet hot compress treatment), with 48 cases and 49 cases respectively. The curative effects, hormone indexes, clinical indexes related to breast hyperplasia, pain and the incidence of adverse reactions were compared between the two groups. Results: The total clinical effective rate of the study group was significantly higher than that of the control group (P<0.05). After treatment, the Visual pain analogue scale (VAS) score of the study group was lower than that of the control group (P<0.05). After treatment, the thick diameter of breast gland, the thickest diameter of breast duct and the hardness score of breast mass of the study group were lower than those of the control group (P<0.05). After treatment, the levels of serum estradiol (E₂) and prolactin (PRL) of the study group were lower than those of the control group, and the level of serum progesterone (P) was higher than that of the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: Yanghe ointment wet hot compress is effective in the treatment of breast hyperplasia. It can promote the improvement of symptoms, reduce the thick diameter of breast gland and the thickest diameter of breast duct, reduce the hardness of breast mass, and regulate the secretion of E₂, P and PRL, which is safe and reliable.
