肺癌超声造影参数与新生血管的相关性分析

摘    要:目的:探究肺癌超声造影参数与新生血管的相关性。方法:选择2014年1月至2018年1月在我院接受治疗的149例肺癌患者,对其实施超声造影检查、常规病理检查,通过CD34免疫组织染色计算其血管密度。按照病理检查结果将149例患者区分为腺癌组(87例)和鳞癌组(62例),使用时间强度曲线分析计算腺癌和鳞癌患者峰值强度及增强强度指数,对比两组患者新生血管密度,并计算超声造影参数强度与新生血管密度之间的相关性,最后将患者按照肿瘤大小进行分组,对比新生血管密度。结果:(1)腺癌患者新生血管密度、造影峰值强度、强度指数均高于鳞癌患者(P<0.05);(2)腺癌超声造影峰值强度及强度指数与其新生血管密度呈正相关(r=0.635,r=0.881,P<0.05),鳞癌超声造影峰值强度及强度指数与其新生血管密度呈正相关(r=0.519,r=0.669,P<0.05);(3)随着肿瘤直径的增大,新生血管密度呈现增长趋势(P<0.05)。结论:肺癌超声造影能够较准确的反映肺癌患者新生血管密度情况,同时新生血管密度与肺癌瘤体大小具有相关性,可使用肺癌超声造影对肺癌患者血流灌注情况进行判别。

Abstract:Objective:To investigate the correlation between ultrasound contrast parameters and neovascularization in the lung cancer.Methods:149 cases of patients with lung cancer who were treated in our hospital from January 2014 to January 2018 were enrolled in the study.Contrast-enhanced ultrasonography and routine pathological examination were performed.The microvascular density was calculated by CD34 immunohistochemical staining.149 patients were divided into the adenocarcinoma group(87 cases)and the squamous cell carcinoma group(62 cases).Time intensity curve analysis was used to calculate the peak intensity and intensity index of patients.The correlation between intensity of ultrasound contrast parameters and the density of neovascularization were calculated,and finally group the patients according to the tumor size and compare the neovascularization density.Results:The neovascular density,angiographic peak intensity and intensity index of patients with adenocarcinoma were higher than those of squamous cell carcinoma(P<0.05).The peak intensity and intensity index of adenocarcinoma were positively correlated with neovascular density(r=0.635,r=0.881,P<0.05),the peak intensity and intensity index of squamous cell carcinoma were positively correlated with the density of neovascularization(r=0.519,r=0.669,P<0.05).with the increase of tumor diameter,Neovascular density showed an increasing trend(P<0.05).Conclusion:Contrast-enhanced ultrasonography of lung cancer could accurately reflect the neovascular density of lung cancer patients.At the same time,the density of new blood vessels was related to the size of lung cancer.The blood perfusion of lung cancer patients could be discriminated by contrast-enhanced ultrasound.
