肺炎克雷伯菌的分布特点耐药性分析

摘    要:目的分析2010年至2012年房县人民医院患者肺炎克雷伯菌的耐药特点,为临床合理用药提供依据。方法对2010年至2012年该院各类感染患者标本中分离获得的肺炎克雷伯菌,采用纸片扩散法(K—B法)检测抗菌药物的敏感性,产超广谱β-内酰胺酶(ESBLs)检测采用ESBLs表型筛选与确证试验,并用WHONET5.3软件对药敏结果进行统计分析。结果3年分离的肺炎克雷伯菌共计248株,分离出产ESBLs菌株102株,其对亚胺培南、美洛培南无耐药,对头孢哌酮/舒巴坦、哌拉西林/三唑巴坦、阿米卡星和头孢替坦的耐药率较低,对其他抗菌药物的耐药率均较高。结论肺炎克雷伯菌对多种抗菌药物均具有较高的耐药性,临床上治疗该菌感染时应根据药敏与ESBLs检测结果选择抗菌药物。

Abstract:Objective To analyze the Klebsiella pneumoniae resistent characteristics of the patients from 2010 - 2012 in our hospital to provide the basis for the clinical drug using more reasonable. Methods For 2010 -2012 in our hospital were isolated from patients with various types of infections acquired Klebsiella pneumoniae, Kirby-Bauer disk diffusion method (K-B method) antimicrobial susceptibility testing. Producing extended spectrum [3-1actamas- es (ESBLs) ESBLs were detected by phenotypic screening and confirmatory tests. Software with WHONET5.3 sus- ceptibi|ity results were tatistically analyzed. Results 3 years Klebsiella pneumoniae isolated totaling 248, 102 iso- lated strains producing ESBLs, its imipenem, meropenem no drug, Cefoperazone/sulbactam, piperacillin/tazobac- tam, amikacin and cefotetan resistance rates for other antimicrobial agents were high. Conclution KlebsieUa pneu- moniae to multiple antimicrobial agents have high resistance, the clinical treatment of fungus infection should be based on susceptibility test results with ESBLs selected antimicrobial agents.
