腮腺腺淋巴瘤的MRI诊断（附12例分析）

目的：探讨腮腺腺淋巴瘤的MR表现特征。方法：回顾性分析12例经病理证实的腺淋巴瘤患者MR及临床资料,男性10例,女性2例,平均年龄59.1岁。观察病灶的数目、部位、形态、边界、大小以及MR信号特点等。结果：12例中,单发9例,多发3例,共17个病灶。其中12个病灶位于腮腺浅叶后下部,13个病灶为纵向生长椭圆形。T1WI上16个病灶为等或稍高于肌肉信号,1个含有高信号。STIR序列中等稍高信号者7个,10个含有大小不等的明显高信号。增强扫描轻到中度强化者15个,明显强化2个。9个病例颈部可见中等大小淋巴结。结论：50岁以上吸烟男性,发现腮腺浅叶后下方边界清楚、纵向生长椭圆占位,STIR序列信号欠均匀,高度提示腺淋巴瘤。

The MRI Dignosis of Parotid Adenolymphomas

Objective： To investigate the MR imaging appearance of the parotid gland. Methods： The MRI and clinical data of 12 cases （10 men and 2 women, mean age 59.1 years） of pathologically confirmed adenolymphomas were retrospectively analyzed. The number, location, shape, boundary, size, and features of MR signal intensity of the tumors were reviewed. Results： 9 of the 12 cases were solitary and 3 were multiple, added up to 17 tumors. 12 of the tumors were located in the posterior and inferior portion of superficial lobe of the parotid, 13 tumors were longitudinal oval-shaped. On the T1W images, 16 tumors were isointensity or slightly hyperintensity com- pared with muscle, 1 tumor cotained high signal intensity. On the STIR images, 7 tumors were moderate or slightly hyperintensity, 10 tumors cotained significant high signal intensity of various sizes. 15 tumors were slightly to moderate enhanced and 2 tumors were obviously enhanced after Gadolinium agent administration. Moderate sized cervical lymphnodes were observed in 9 cases. Conclusions： In male smoker over 50 years old, when longitudinal oval-shaped lesion with well defined margin and heterogeneous signal intensity on the STIR images is found in the posterior and inferior portion of superficial lobe of the parotid ,the diagnosis of adenolymphoma should be highly suggested.
