摘    要:目的:研究高渗盐复合液对大面积脑梗塞后颅内压增高、脑水肿治疗作用和机理,从而指导对大面积脑梗塞引起颅内压升高和脑水肿的治疗作用。方法:动态观察生理盐水组、甘露醇组和高渗复合盐组的颅内压变化,了解三组颅内压改变情况及脑细胞凋亡检测。结果:除生理盐水治疗组外,其它两组药物均在30min~1h内起效,且2组动物在用药后的2h内颅内压均显著降低,两组间无明显差别。结论:高渗盐复合液可以降低颅内压,其起效时间和维持作用的时间均与20%甘露醇类似。

Abstract:Objective: To study the therapeutical effects and the mechanisms of hypertonic sodium chloride hydroxyethyl starch-40 injection(HSH) for increased intracranial pressure and encephaledema in middle cerebral artery oclussion model(MACO), and to identify the therapeutical effects of HSH by further observation on apoptosis of infarcted focus as well as ischemic semi-dark band aiming at providing evidences for clinical application of encephaledema after cerebral infarction. Methods: The hypertonic complex in- jection with certain concentration and the same dose (4 mL/kg) is used on the MACO model of SD rat 4 hours after infarction in each group. The group of physiologic saline is set as blank control while the group of mannitol (4 mL/kg) is set as therapeutic control. The in- tracranial pressure (ICP) is continuously monitored. Results: Except the group of physiologic saline, the effects of the drugs start in 30 minutes to 1 hour in the other two groups. During the monitoring of 4 hours, intracranial pressures of last two groups are dramatically de- creased at each point and there is no difference between the two groups. Conclusions: HSH has similar effects of decreasing ICP as man- nitol which generate favorable effects against post-infarction encephaledema. There is no difference in starting time between two factors mentioned above, Although HSH has a shorter sustaining time in decreasing ICP than marmitol, it can protect the nervous tissue to certain extent.
