血循毒蛇咬伤激素用量研究

目的研究血循毒蛇咬伤患者激素使用最佳剂量。方法收集我院2007~2014年血循毒蛇咬伤住院患者339例,随机给予5~40mg不同剂量地塞米松治疗,比较其需要使用激素的时间、住院时间及其并发症发生率。结果 10、15mg剂量组需要使用激素时间分别为2.29、2.30天,相对于5、20、25、30、40mg剂量组(分别为5.30、3.22、3.50、3.34、3.25天)具有统计学意义(P0.05)。其中5mg剂量组的住院时间为7.58天,相对于10、15、20、25、30、40 mg剂量组(分别为4.01、3.94、3.90、3.88、3.91、4.30天)具有统计学意义(P0.05);10、15mg剂量组的并发症发生率分别为4.48%、4.00%,相对于5、20、25、30、40mg剂量组(分别为14.70%、10.42%、14.29%、17.50%、21.21%)具有统计学意义(P0.05)。结论地塞米松10、15mg剂量为血循毒蛇咬伤患者激素使用最佳用量。

Research of the amount of hormones in snake bitten of blood ci rculation poison

Objective To study the bulky snakebite patients the optimal dose steroid use . Methods From 2007 to 2014 in our hospital bulky 339 cases of hospitalized patients snakebite 5~40 mg were randomized to receive dif‐ferent doses of dexamethasone ,compared to use of hormones ,hospitalization time and complication rate . Results 10 mg and 15 mg dose of hormones needed time 2.29 ,2.30 days ,respectively ,relative to 5 mg ,20 mg ,25 mg ,30 mg and 40 mg dose groups were 5.30 ,3.22 ,3.50 ,3.34 ,3.25 days ,the difference was statistically significant （P〈0.05） .Duration of hospitalization 5 mg dose of 7.58 days ,compared to 10 mg ,15 mg ,20 mg ,25 mg ,30 mg and 40 mg dose groups were 4.01 ,3.94 ,3.90 ,3.88 ,3.91 ,4.30 days ,the difference was statistically significant （P〈0.05）;10 mg and 15 mg dose groups complication rate was 4.48% ,respectively ,4.00% ,relative to 5 mg ,20 mg , 25 mg ,30 mg and 40 mg dose groups were 14.70% ,10.42% ,14.29% ,17.50% ,21.21% ,and the difference statis‐tically significant （P〈 0.05） . Conclusion Dexamethasone 10 mg ,15 mg dose of steroid use in patients with bulky snakebite optimal dosage .
