IntroductionBNT162b2 (BioNTech and Pfizer) is a nucleoside-modified mRNA vaccine that provides protection against SARS-CoV-2 infection and is generally well tolerated. However, data about its efficacy, immunogenicity and safety in people of old age or with underlying chronic conditions are scarce.PurposeTo describe BNT162b2 (BioNTech and Pfizer) COVID-19 vaccine immunogenicity, effectiveness and reactogenicity after complete vaccination (two doses), and immunogenicity and reactogenicity after one booster, in elders residing in nursing homes (NH) and healthy NH workers in real-life conditions.MethodsObservational, ambispective, multicenter study. Older adults and health workers were recruited from three nursing homes of a private hospital corporation located in three Spanish cities. The primary vaccination was carried out between January and March 2021. The follow-up was 13 months. Humoral immunity, adverse events, SARS-CoV-2 infections, hospitalizations and deaths were evaluated. Cellular immunity was assessed in a participant subset.ResultsA total of 181 residents (mean age 84.1 years; 89.9% females, Charlson index ≥2: 45%) and 148 members of staff (mean age 45.2 years; 70.2% females) were surveyed (n:329). After primary vaccination of 327 participants, vaccine response in both groups was similar; ≈70% of participants, regardless of the group, had an antibody titer above the cut-off considered currently protective (260 BAU/ml). This proportion increased significantly to ≈ 98% after the booster (p < 0.0001 in both groups). Immunogenicity was largely determined by a prior history of COVID-19 infection. Twenty residents and 3 workers were tested for cellular immunity. There was evidence of cellular immunity after primary vaccination and after booster. During the study, one resident was hospitalized for SARS-CoV-2. No SARS-CoV-2-related deaths were reported and most adverse events were mild.ConclusionsOur results suggest that the BNT162b2 mRNA COVID-19 vaccine is immunogenic, effective and safe in elderly NH residents with underlying chronic conditions.  相似文献

AimSatisfaction with life is a measure of protection in older adults. There lies the importance of providing quality instruments. The aim of the study was to evaluate the invariance of the life satisfaction scale (SWLS) in two samples of older adults in Spain and Peru.MethodThe participants were 857 older adults in Spain (mean age = 68.23 years, SD = 5.93) and 336 older adults in Peru (average age = 72.42, SD = 7.07). All multi-group confirmatory factor analyzes were estimated in Mplus 8.0.ResultsThe results indicate the presence of a strict invariance of the one-dimensional structure of the SWLS in samples of older adults in Spain and Peru, which allows for meaningful comparisons of latent means and covariances. Comparison of latent means showed small differences in the construct between the cultural groups.ConclusionsThe SWLS is a valid instrument for intercultural comparisons between Spanish and Peruvian population. The measurement invariance assessment contributes to a better understanding of life satisfaction in populations from different cultural contexts.  相似文献

Objectives

The study had three objectives: (a) To determine how staff perceives the frequency of different sexual expressions in long-term care facilities for older people; (2) to quantify policies aimed at guaranteeing residents’ sexual rights in such institutions, and (3) to determine factors influencing the presence of these kind of policies.

Methods

A total of 160 Directors of Nursing and 623 technical staff from Spanish long term care residential facilities completed a self-administered questionnaire including ítems to assess the experience and estimated frequency of different sexual expressions, an inventory of policies regarding sexual expression, and the Spanish version of the Person-Centred Care Assessment Tool.

Results

Certain sexual behaviours, such as kisses and hugs, or masturbation, are very frequent in nursing homes. Many participants have reported to staff members on having witnessed inappropriate sexual behaviours, such as exhibitionism (39%) or unwanted touching (27%). Policies in relation to guaranteeing residents’ sexual rights were the most frequently mentioned. In contrast, there were fewer participants who mentioned policies regarding training or availability of helpful materials and resources. Factors such as occupational level (Directors of Nursing vs.technical staff), personal education, the centre commitment to person-centred care, and estimated frequency of sexual behaviour were associated with a higher presence of sexual expression policies.

Conclusion

Although sexual behaviours among residents are quite frequent, there is still room for improvement in policies that support residents’ expression of sexuality. Person-centred care models might help to guarantee sexual rights of older people living in long-term care facilities.  相似文献

Objective

To determine the frequency of successful aging (SA) and its relationship with frailty in an elderly population.

Material and methods

An analytical cross-sectional study of subjects ≥60 years of age seen as outpatients in a general hospital. Successful aging was defined as scores of ≥ 90 in the Barthel index and ≤ 2 in the Pfeiffer test. Frailty was determined using the Fried criteria.

Results

The study included 400 subjects (272 women and 128 men), with a mean age of 71.6 ± 8.2 years. The SA frequency was 40.4%. frail status was statistically higher in non-successful aging subjects than in SA subjects (161.7 versus 7.9%; P<.001). Women were more frequently frail, while being a pensioner/retired and married were associated less frequently with frailty.

Conclusions

Successful aging is associated with a lower level of frailty.  相似文献

Introduction

Physical activity and physical inactivity patterns can affect health status. In the elderly people, their study is relevant given the importance that they have on the morbidity and mortality.

Objective

To present preliminary data on activity and inactivity patterns of a sub-sample of older adults from the IMPACT65+ Study.

Material and methods

The sample included the first 84 participants (57% women) over 65 years (age 70.7±4.7). Time spent in activity and inactivity patterns was obtained from an Intelligent Device for Energy Expenditure and Activity monitor over a continuous period of 24 hours. The patterns analysed were: standing, lying down, sitting or reclining, and the transition between them. The physical activity patterns analysed were; walking, step up or step down, running, and jumping.

Results

Time spent in inactivity patterns like reclining, lying down, and sitting was 16.1±1.9 hours (67% day), while the amount of time spent in activity patterns was 2.4±1.9 hours (10% day). Differences were observed between men and women in the amount of hours sitting (9.7±3 men vs. 7.5±2.7 women) and standing (4.5±1.4 men vs. 5.6±2.7 women). These differences were greater in the older participants.

Conclusions

Preliminary results show that older adults spend a great part of day in inactivity patterns like sitting, and that gender is the only factor analysed that affects the time spent in the activity patterns analysed.  相似文献

Introduction

This study presents an analysis on the effects of positive social relationships as determining factors on the health status of the elderly, from an interdisciplinary point of view.

Materials and methods

A survey was carried out on a total of 2,332 subjects in the province of Toledo, Spain, using several scales related to the study objectives. For the multivariable analysis of the data, the linear regression, along with goodness of fit, as well as the tests of Cox, Snell and R-squared of Nagelkerke, were calculated to determine the correlation between the networks and the social contacts with the health.

Results

Good social relationships result in better health, because they allow us to feel good and add life to the years. The size of the social network, and the frequency of contacts, although decreasing with age, become vitally important at this stage of life. All these factors constitute stimuli to improve the perception of health, to provide social support in activities of daily living, as well as for instrumental or disease management.

Conclusions

The main outcome of this study is that positive social relationships can be considered as a determining factor in the health of the elderly, but not all these interactions influence the same extent. The factor of the human relationship in terms of emotional proximity, together with the frequency or quantity of these encounters, are also correlated with health, due to the emotional impact they imply.  相似文献

Introduction

This pre-experimental study analyses the Functional Capacity (FC), Body Mass Index (BMI), Health Related Quality of Life (HRQoL) and Health Capital Stock (HCS) of elderly people participating in a Walking program in Granollers (Barcelona).

Material and methods

One hundred and seventy-three participants were invited to participate. One hundred and one of them were users of the Municipal Program for Elderly People, and 72 received information by the communication media. One hundred and thirty-one (76.3%) of the participants completed the Program.The intervention consisted of a 6-month program, supervised by a specialist instructor once a week and controlled daily by a pedometer.The 6-minute walk test (6MWT) was analysed for the FC, and the EuroQoL for the HRQoL. The preferences derived from the EuroQoL scores and the Life Expectancy were used to calculate the FC. The monetary data were obtained using a fixed value.

Results

An increase in the average number of steps was observed after the program, especially in men. The results also showed a decrease in the BMI. The participants showed a improvement in overall health perception at the end of the program. The HCS scores confirmed the improvement in the HRQoL in men.

Conclusions

The Program was positive for increasing the number of steps and improving physical condition and health. The usefulness of the pedometer as a motivational tool of physical exercise is discussed.  相似文献

Introduction

The purpose of this study is to determine the opinion of professionals working in residential aged care facilities on the regulation of sexuality in these settings.

Materials and methods

Fifty-three professionals from five residential aged care facilities located in the metropolitan area of Barcelona answered several questions regarding the advisability of establishing measures for the regulation of sexuality in RACFs, the elements that could contribute to this, and the aspects that such regulations should consider.

Results

Around 50% of the participants recognized the advisability of having some type of measures for sexuality regulation in residential aged care facilities. According to their responses this should be developed taking into account professional opinions, but also the points of view of the residents and their relatives. The most frequently mentioned regulations were those that ensured that any kind of sexually charged situation occurred in a private environment. The development of strategies are suggested to distinguish those people with dementia that are competent to consent to sexual acts from those who are not.

Conclusions

The opinion of professionals working in RACFs regarding the advisability of establishing measures for sexuality regulation seems to be considerably divided. Thus, whilst around 50% of them recognize their potential usefulness, the other half consider them unnecessary or even counterproductive for the sexual freedom of residents. Associating regulation with prohibition and sexuality with sexual activity was not uncommon among the responses of the participants.  相似文献
