The oceans and coastal seas provide mankind with many benefits including food for around a third of the global population, the air that we breathe and our climate system which enables habitation of much of the planet. However, the converse is that generation of natural events (such as hurricanes, severe storms and tsunamis) can have devastating impacts on coastal populations, while pollution of the seas by pathogens and toxic waste can cause illness and death in humans and animals. Harmful effects from biogenic toxins produced by algal blooms (HABs) and from the pathogens associated with microbial pollution are also a health hazard in seafood and from direct contact with water. The overall global burden of human disease caused by sewage pollution of coastal waters has been estimated at 4 million lost person-years annually. Finally, the impacts of all of these issues will be exacerbated by climate change. A holistic systems approach is needed. It must consider whole ecosystems, and their sustainability, such as integrated coastal zone management, is necessary to address the highly interconnected scientific challenges of increased human population pressure, pollution and over-exploitation of food (and other) resources as drivers of adverse ecological, social and economic impacts. There is also an urgent and critical requirement for effective and integrated public health solutions to be developed through the formulation of politically and environmentally meaningful policies. The research community required to address “Oceans & Human Health” in Europe is currently very fragmented, and recognition by policy makers of some of the problems, outlined in the list of challenges above, is limited. Nevertheless, relevant key policy issues for governments worldwide include the reduction of the burden of disease (including the early detection of emerging pathogens and other threats) and improving the quality of the global environment. Failure to effectively address these issues will impact adversely on efforts to alleviate poverty, sustain the availability of environmental goods and services and improve health and social and economic stability; and thus, will impinge on many policy decisions, both nationally and internationally. Knowledge exchange (KE) will be a key element of any ensuing research. KE will facilitate the integration of biological, medical, epidemiological, social and economic disciplines, as well as the emergence of synergies between seemingly unconnected areas of science and socio-economic issues, and will help to leverage knowledge transfer across the European Union (EU) and beyond. An integrated interdisciplinary systems approach is an effective way to bring together the appropriate groups of scientists, social scientists, economists, industry and other stakeholders with the policy formulators in order to address the complexities of interfacial problems in the area of environment and human health. The Marine Board of the European Science Foundation Working Group on “Oceans and Human Health” has been charged with developing a position paper on this topic with a view to identifying the scientific, social and economic challenges and making recommendations to the EU on policy-relevant research and development activities in this arena. This paper includes the background to health-related issues linked to the coastal environment and highlights the main arguments for an ecosystem-based whole systems approach.  相似文献

Introduction

Airway surface dehydration, caused by an imbalance between secretion and absorption of ions and fluid across the epithelium and/or increased epithelial mucin secretion, impairs mucociliary clearance. Recent evidence suggests that this mechanism may be implicated in chronic obstructive pulmonary disease (COPD). However, the role of airway surface dehydration in the pathogenesis of cigarette smoke (CS)-induced COPD remains unknown.

Objective

We aimed to investigate in vivo the effect of airway surface dehydration on several CS-induced hallmarks of COPD in mice with airway-specific overexpression of the β-subunit of the epithelial Na+ channel (βENaC).

Methods

βENaC-Tg mice and wild-type (WT) littermates were exposed to air or CS for 4 or 8 weeks. Pathological hallmarks of COPD, including goblet cell metaplasia, mucin expression, pulmonary inflammation, lymphoid follicles, emphysema and airway wall remodelling were determined and lung function was measured.

Results

Airway surface dehydration in βENaC-Tg mice aggravated CS-induced airway inflammation, mucin expression and destruction of alveolar walls and accelerated the formation of pulmonary lymphoid follicles. Moreover, lung function measurements demonstrated an increased compliance and total lung capacity and a lower resistance and hysteresis in βENaC-Tg mice, compared to WT mice. CS exposure further altered lung function measurements.

Conclusions

We conclude that airway surface dehydration is a risk factor that aggravates CS-induced hallmarks of COPD.  相似文献

Introduction

There is a relative lack of information about the death rate and morbidity of non-cystic fibrosis bronchiectasis and most studies are limited due to referral bias. We wanted to assess death rate and morbidity in those patients at our hospital.

Methods

Adult patients seen at our department between June 2006 and November 2009 were recruited if the key string "bronchiect-" was mentioned in electronic clinical records and if chest CT imaging was available. Clinical records of all patients with confirmed radiologic diagnosis of bronchiectasis were reviewed and clinical characteristics were analyzed.

Results

539 patients with a radiographic diagnosis of non-cystic fibrosis bronchiectasis were identified in a retrospective cross-sectional analysis giving a prevalence of 2.6% in our hospital population. A wide range of etiologies was found with idiopathic bronchiectasis in 26%. In the 41 months interval, 57 patients (10.6%) died. We found a median exacerbation rate of 1.94 per year. Bacterial colonization status was associated with more deaths, exacerbation rate, symptoms and reduced pulmonary function. Pulmonary hypertension was found in 48% of our patients.

Conclusions

We evaluated a large non-cystic fibrosis bronchiectasis population, and provided new epidemiological data on associations between clinical characteristics and deaths and morbidity in these patients.  相似文献

To evaluate the effects of the construction of a storm surge barrier in the Oosterschelde, long-term patterns (1979–89) in abundance and biomass of the intertidal macrozoobenthos were studied at 14 permanent stations. Additionally, data of a large-scale survey in late summer 1985 and 1989 were analysed. In this paper, patterns in general parameters are discussed.Late summer values of total biomass, total density, species richness, diversity and abundance- and biomass ratio show no overall significant trend during the study period. The changes in the hydrodynamics and the morphology of the Oosterschelde after the completion of the storm surge barrier do not seem to have influenced the normal patterns in benthic populations. The observed patterns are determined by the occurrence of severe versus mild winters, rather than by hydrodynamic changes caused by the construction of the barrier. Low biomasses, high densities (particularly of opportunistic species) and higher stress-values (abundance- and biomass ratio) in 1985(–87) indicate a temporal disturbance by severe winter weather. At the elevated COST-station 27, total biomass decreased sharply in 1985, due to a short-term increase in exposure time, caused by the manipulation of the storm surge barrier.  相似文献

The construction of a storm-surge barrier in the mouth of the Oosterschelde caused important hydrodynamical and morphological changes that could influence the macrobenthic populations. This paper is one in a series of five all dealing with the effects of the storm-surge barrier on macrozoobenthos and analyses the spatial and temporal distribution of macrozoobenthos in the Oosterschelde and its relationship with some environmental parameters, based on two large scale sampling campaigns, one before and one after the completion of the barrier.The sediment of the sampling stations was fine, well sorted sand, with an average mud content of about 2.5 %. Only in the Krabbenkreek the sediment was coarser in 1989. The tidal elevation of the sampling sites decreased significantly in 1989.The density of macrozoobenthos was significantly lower, the biomass higher in 1989. The density was dominated by deposit feeders, the biomass by filter feeders. The difference in biomass between both years was mainly due to a substantial increase of the biomass of filter feeders in 1989. The number of species per station was significantly smaller in 1989 than in 1985. Between 1985 and 1989, frequency of occurrence decreased in 34 versus 13 which increased, density increased in 13 species and decreased in 34 species, biomass increased in 18 species and decreased in 29 species.Based on TWINSPAN several clusters of stations, each with a different faunal composition, were identified. These clusters did not form distinct zones on the tidal flats but were dispersed widely.The relationship between density and biomass of different trophic groups and the mud content of the sediment and the depth was analysed. This relationship sometimes differed clearly between years. The correlation coefficient of a multiple regression between density and biomass of individual species and environmental factors, although significant in most cases, was very low, indicating that only a small proportion of the species variability was explained. The relationship between benthos and environmental factors was further analysed by canonical correlation analysis and multivariate discriminant analysis that gave different results for the 1985 and 1989 data. This is probably due to the broad tolerance of the species to the range of the environmental variables found in our study area. From a TWINSPAN of the density data of 1985 and 1989 together we could conclude that, although the environmental parameters in a group of stations, showing a large faunal similarity in one year, did not change, the faunal composition did. This indicates that faunal changes are not necessarily linked to changes in the measured environmental parameters.In the discussion the different factors affecting macrobenthic populations are situated and it is suggested that the macrobenthic populations are probably more towards the nonequilibrium end of the continuum between nonequilibrium and equilibrium communities as defined by Wiens (1984).Although the impact of the construction of the barrier on the macrobenthic community seems at present to be rather small this does not mean that on the long-term there will be no effect.  相似文献

In order to evaluate the impact of the construction of the storm-surge barrier and secondary dams on macrobenthos of the tidal flats in the Oosterschelde (SW Netherlands), changes in distribution, density and biomass of five common species (Spio martinensis, Hydrobia ulvae, Arenicola marina, Scoloplos armiger and Bathyporeia sp) were analysed.Data on macrobenthos were collected from 1979 to 1989 on five different tidal flats. Changes in sediment texture and hydrodynamic factors during the construction and after the completion of the coastal engineering project were taken into account.Three severe winters in a row caused more disturbance in the population of the main predator of S. armiger than did the hydrodynamical changes. A temporary prolongation of the emersion time (in 1986 and 1987) caused a temporary decrease in juvenile A. marina. But afterwards they still occupy the same nursery grounds. Increased wave action on the edges of the flats probably created new niches for Bathyporeia sp. and Spio martinensis, replacing other benthic species.It is not yet clear what has caused the decline of H. ulvae in many places in the Oosterschelde estuary. Parasitic infestation is one of the possibilities.  相似文献

The Palliacom project, which is intended to be a communication support for persons with severe difficulties to talk and/or to write, focuses mainly on two objectives: the implementation of a writing assistance device based on either an alphabet or a set of icons, using personal or tablet computers; and the compilation of functional knowledge about how users exploit this writing assistance device, obtained through long term observation of utilization strategies. Depending on the user profile, different dynamic writing options are possible, for instance clicking or touching specific images displayed on the first screen of the writing assistance device enables to directly select the most appropriate set of configuration parameters. Otherwise, the written text can be printed, inserted within another text, or read automatically using a speech synthesizer. The system is currently being studied from a usability perspective, analyzing how impaired children express themselves with icons and handicapped adults communicate with alphabets.  相似文献
