BackgroundIn Senegal, with the variable routine vaccination coverage, the risk for illness and death from measles still exists as evidenced by the measles epidemic episode in 2009. Since 2002 a laboratory-based surveillance system of measles was established by the Ministry of Health and the Institut Pasteur de Dakar. The present study analysed the data collected over the 10 years inclusive between 2004-2013 in order to define a measles epidemiological profile in Senegal, and we carried out a phylogenetic analysis of measles virus circulating in Senegal over the period 2009-2012.ConclusionImprovements in the measles surveillance in Senegal are required and the introduction of oral fluid and FTA cards as an alternative to transportation of sera should be investigated to improve surveillance. The introduction of a national vaccine database including number of doses of measles-containing vaccine will greatly improve efforts to interrupt and ultimately eliminate measles virus transmission in Senegal.  相似文献

Background

The consequences of maternal HIV infection for fetal growth are controversial. Here, we estimated the frequency of small for gestational age and gender (SGAG) among neonates born to HIV-infected or uninfected mothers and assessed the contribution, if any, of maternal HIV to the risk of SGAG.

Methods

The data used were obtained from the ANRS-Pediacam cohort in Cameroon. Pairs of newborns, one to a HIV-infected mother and the other to an uninfected mother, were identified during the first week of life, and matched on gender and recruitment site from 2007–2010. SGAG was defined in line with international recommendations as a birth weight Z-score adjusted for gestational age at delivery and gender more than two standard deviations below the mean (−2SD). Considering the matched design, logistic regression modeling was adjusted on site and gender to explore the effect of perinatal HIV exposure on SGAG.

Results

Among the 4104 mother-infant pairs originally enrolled, no data on birth weight and/or gestational age were available for 108; also, 259 were twins and were excluded. Of the remaining 3737 mother-infant pairs, the frequency of SGAG was 5.3% (95%CI: 4.6–6.0), and was significantly higher among HIV-infected infants (22.4% vs. 6.3%; p<.001) and lower among HIV-unexposed uninfected infants (3.5% vs. 6.3%; p<.001) than among HIV-exposed uninfected infants. Similarly, SGAG was significantly more frequent among HIV-infected infants (aOR: 4.1; 2.0–8.1) and less frequent among HIV-unexposed uninfected infants (aOR: 0.5; 0.4–0.8) than among HIV-exposed uninfected infants. Primiparity (aOR: 1.9; 1.3–2.7) and the presence of any disease during pregnancy (aOR: 1.4; 1.0–2.0) were identified as other contributors to SGAG.

Conclusion

Maternal HIV infection was independently associated with SGAG for HIV-exposed uninfected infants. This provides further evidence of the need for adapted monitoring of pregnancy in HIV-infected women, especially if they are symptomatic, to minimize additional risk factors for SGAG.  相似文献

Background

Early infant diagnosis (EID) of HIV is a key-point for the implementation of early HAART, associated with lower mortality in HIV-infected infants. We evaluated the EID process of HIV according to national recommendations, in urban areas of Cameroon.

Methods/Findings

The ANRS12140-Pediacam study is a multisite cohort in which infants born to HIV-infected mothers were included before the 8th day of life and followed. Collection of samples for HIV DNA/RNA-PCR was planned at 6 weeks together with routine vaccination. The HIV test result was expected to be available at 10 weeks. A positive or indeterminate test result was confirmed by a second test on a different sample. Systematic HAART was offered to HIV-infected infants identified. The EID process was considered complete if infants were tested and HIV results provided to mothers/family before 7 months of age. During 2007–2009, 1587 mother-infant pairs were included in three referral hospitals; most infants (n = 1423, 89.7%) were tested for HIV, at a median age of 1.5 months (IQR, 1.4–1.6). Among them, 51 (3.6%) were HIV-infected. Overall, 1331 (83.9%) completed the process by returning for the result before 7 months (median age: 2.5 months (IQR, 2.4–3.0)). Incomplete process, that is test not performed, or result of test not provided or provided late to the family, was independently associated with late HIV diagnosis during pregnancy (adjusted odds ratio (aOR) = 1.8, 95%CI: 1.1 to 2.9, p = 0.01), absence of PMTCT prophylaxis (aOR = 2.4, 95%CI: 1.4 to 4.3, p = 0.002), and emergency caesarean section (aOR = 2.5, 95%CI: 1.5 to 4.3, p = 0.001).

Conclusions

In urban areas of Cameroon, HIV-infected women diagnosed sufficiently early during pregnancy opt to benefit from EID whatever their socio-economic, marital or disclosure status. Reduction of non optimal diagnosis process should focus on women with late HIV diagnosis during pregnancy especially if they did not receive any PMTCT, or if complications occurred at delivery.  相似文献

Fractionation of a MeOH/CH2Cl2 (1/1) extract of the aerial parts of Senecio erechtitoides led to the isolation of six compounds including the hitherto unknown N-phenethylamide derivative named N-(p-hydroxyphenethyl)pentacosanamide (1), and a kauranoid derivative named derivative named ent-7-oxo-16α,17-dihydroxykauran-19-oic acid (2), as well as four known compounds, ent-Kaur-16-en-19-oic acid (3), ent-7β-hydroxykaur-16-en-19-oic acid (4), ent-7-oxokaur-16-en-19-oic acid (5), steppogenin 4′-O-β-d-glucoside (6). Their structures and relative configurations were elucidated on the basis of spectroscopic methods, chemical reactions, and comparison with previously known analogs. All isolates were evaluated for their antimicrobial activity and only diterpenoids were found to possess a potent inhibitor effect against the range of microorganism.  相似文献

Background

Deforestation is one of the most ubiquitous forms of land degradation worldwide. Although remote sensing and aerial photographs can supply valuable information on land/use cover changes, they may not regularly be available for some tropical coasts (e.g., Cameroon estuary) where cloud cover is frequent. With respect to mangroves, researchers are now employing local knowledge as an alternative means of understanding forest disturbances. This paper was primarily aimed at assessing the mangrove forest products usage, along with the local people's perceptions on environmental changes, between Littoral (Cameroon estuary) and Southern (mouth of the Nyong River and Mpalla village) regions of Cameroon.

Methods

The data from both locations were obtained through conducting household interviews and field observations.

Results

In the Cameroon estuary (Littoral region), 69.23% of respondents (mostly elders) could distinguish two to four mangrove plants, whereas the informants (65.45%) in the mouth of the Nyong River and Mpalla village (mostly young people interviewed from the Southern region) are familiar with only one or two commonly found mangroves. Also, more respondents from the Cameroon estuary are depending on mangroves for fuelwood (Rhizophora spp.) and housing (Rhizophora spp., Avicennia germinans (L.) Stearn and Nypa fruticans (Thumb.) Wurmb.) purposes, in contrast to Nyong River mouth and Mpalla village. Although local people perceived wood extraction as a greater disruptive factor, there are several causes for mangrove depletion in the Cameroon estuary. Among others, over-harvesting, clear-felled corridors, sand extraction and housing were found important. Furthermore, a decline in mangrove fauna composition (in terms of fishery products) was recorded in the Littoral as well as Southern regions. However, the causes of such perceived negative changes were not similar in both cases.

Conclusions

Findings of this study highlight the need to improve sustainable management of the mangrove ecosystems through afforestation (in large impacted areas), selective removal of senescent tree stems and branches (in little damage stands), regulating sand extraction and housing activities, and creating awareness and law enforcement.

To strengthen the regeneration and management of Cameroon's coastal ecosystems, the biological characteristics of seedlings of Avicennia germinans (L.) Stearn and associated species were analysed from January 1999 to December 1999 in the mangroves under different environmental conditions. Site factors such as temperature, conductivity, salinity and pH were studied in relation to species distribution, and showed significant variation within species. These factors were strongly correlated in the case of A. germinans. The phenology of Avicennia extends throughout the year. Structural parameters of the trees, seedlings and of associated species have been determined. The survey of plant populations shows that A. germinans species represent more than 95% of the total seedlings of the study area, in spite of their variable density. A correlation was found between seedling diameter and height. The intra-specific competition is high and the percentage of seedlings that reach maturity is extremely low (less than1%). Seedlings are over-scattered and weakly aggregated (Green index: GI = 0.023). Floristic diversity is greater and A. germinans zones appear to be the diversification centre of taxa in the mangroves. A Cameroonian patient with antibodies reacting simultaneously to human immunodeficiency virus type 1 (HIV-1) group O- and group M-specific V3-loop peptides was identified. In order to confirm that this patient was coinfected with both viruses, PCRs with O- and M-specific discriminating primers corresponding to different regions of the genome were carried out with both primary lymphocyte DNA and the corresponding viral strains isolated from three consecutive patient samples. The PCR data suggested that this patient is coinfected with a group M virus and a recombinant M/O virus. Indeed, only type M gag sequences could be amplified, while for the env region, both type M and O sequences were amplified, from plasma or from DNA extracted from primary lymphocytes. Sequence analysis of a complete recombinant genome isolated from the second sample (97CA-MP645 virus isolate) revealed two intergroup breakpoints, one in the vpr gene and the second in the long terminal repeat region around the TATA box. Comparison of the type M sequences shared by the group M and the recombinant M/O viruses showed that these sequences were closely related, with only 3% genetic distance, suggesting that the M virus was one of the parental viruses. In this report we describe for the first time a recombination event in vivo between viruses belonging to two different groups, leading to a replicative virus. Recombination between strains with such distant lineages (65% overall homology) may contribute substantially to the emergence of new HIV-1 variants. We documented that this virus replicates well and became predominant in vitro. At this time, group O viruses represent a minority of the strains responsible for the HIV-1 pandemic. If such recombinant intergroup viruses gained better fitness, inducing changes in their biological properties compared to the parental group O virus, the prevalences of group O sequences could increase rapidly. This will have important implications for diagnosis of HIV-1 infections by serological and molecular tests, as well as for antiviral treatment.  相似文献

Global declines in biodiversity, together with widespread degradation of ecosystem services, have led to urgent calls to safeguard. Humans maintain plants around their houses to increase their quality of life. In the present study, 90 and 30 plots of 50 m2 each were studied in urbanised and rural areas in Cameroon. In each plot, vascular plant species were identified and their coefficient of abundance–dominance was determined. Different diversity indices were calculated to compare the flora of urbanised and rural areas as well as the distribution of plants. A total of 119 species belonging to 95 genera and 45 families were sampled. Shannon–Weaver diversity index, Pielou and Sheldon values vary when taking into account not only erected or wooded plants, but also herbs and erected or wooded plants. Plots near houses in urban areas comprised weeds, ornamentals and fruits, whereas in rural areas medicinal or edible plants dominated. Based upon the results, we conclude that the use of plants as food, medicinal resources or for other purposes, might be helpful for biodiversity conservation, ecosystem services, the management of resources and to fight poverty. However, the composition of the flora is likely to change as urbanisation proceeds.  相似文献

From the leaves and stem bark of Campylospermum flavum (Ochnaceae), three compounds, namely 4?-O-methylagathisflavone, flavumchalcone, and flavumindole have been isolated together with 10 known compounds, including three flavonoids, two biflavonoids, two alkaloids, two nitrile glucosides, and glucopyranosyl-β-sistosterol. The structures of these compounds and their relative configurations were established by 1D and 2D NMR experiments. The methanolic crude extracts of leaves and stem bark of C. flavum and compounds displayed a significant cytotoxicity towards Artemia salina larvae.  相似文献
