Background
Carpal tunnel release (CTR) is among the most common hand surgeries, although little is known about its pattern. In this study, we aimed to investigate temporal trends, age and gender variation and current practice patterns in CTR surgeries.Methods
We conducted a population-based time series analysis among over 13 million residents of Ontario, who underwent operative management for carpal tunnel syndrome (CTS) from April 1, 1992 to March 31, 2010 using administrative claims data.Results
The primary analysis revealed a fairly stable procedure rate of approximately 10 patients per 10,000 population per year receiving CTRs without any significant, consistent temporal trend (p = 0.94). Secondary analyses revealed different trends in procedure rates according to age. The annual procedure rate among those age >75 years increased from 22 per 10,000 population at the beginning of the study period to over 26 patients per 10,000 population (p<0.01) by the end of the study period. CTR surgical procedures were approximately two-fold more common among females relative to males (64.9% vs. 35.1 respectively; p<0.01). Lastly, CTR procedures are increasingly being conducted in the outpatient setting while procedures in the inpatient setting have been declining steadily – the proportion of procedures performed in the outpatient setting increased from 13% to over 30% by 2010 (p<0.01).Conclusion
Overall, CTR surgical-procedures are conducted at a rate of approximately 10 patients per 10,000 population annually with significant variation with respect to age and gender. CTR surgical procedures in ambulatory-care facilities may soon outpace procedure rates in the in-hospital setting. 相似文献2.

Biological and genetic variability is a prominent feature of human immunodeficiency virus (HIV) strains, especially in tropism, syncytium formation, and replicative capacity. To determine whether there were variable host cell effects on HIV replication in monocytes, three different strains of low-passage-number monocytotropic blood isolates of HIV and the laboratory-adapted strain Ba-L were inoculated into panels of adherent monocytes drawn from 44 different donors, and peak extracellular HIV p24 antigen titers were compared. The clinical HIV strains showed patterns of either moderate or low-level replication in most donor monocytes (20 to 4,000 pg/ml). However, within this range there was marked variation in peak titers in most donors. HIV type 1 Ba-L replicated in all donor monocytes to much higher levels with less variability (30 to 40 ng/ml). Furthermore, replication of 21 clinical blood-derived strains of HIV in blood monocytes and monocyte-derived macrophages (MDM) from pairs of identical twins and age-matched unrelated donors (URD) of the same sex were compared. In all of the seven pairs of identical twins, the kinetics of replication (measured by extracellular HIV p24 antigen) of panels of four clinical HIV type 1 isolates in monocytes were similar within pairs. However, marked and significant differences in kinetics of HIV production occurred within 10 of the 12 unrelated donor pairs (P = 0.0007). The remaining two URD pairs showed similar kinetic patterns, but only one pair had the same HLA-DR genotype. Similar results were observed with monocytes/MDMs obtained from a second bleed of the same donor. Hence, discordant patterns of HIV replication kinetics between URD monocyte pairs contrasted with concordant patterns in identical twin monocytes. These data strongly suggest a host cell genetic effect on productive viral replication in monocytes and MDMs. So far, no consistent genetic linkage of HIV replication pattern with HLA-DR genotype has been observed. 相似文献

Epilepsy is a chronic neurological disease characterized by recurrent seizures. Epilepsy is observed as a well-controlled disease by anti-epileptic agents (AEAs) in about 69%. However, 30%–40% of epileptic patients fail to respond to conventional AEAs leading to an increase in the risk of brain structural injury and mortality. Therefore, adding some FDA-approved drugs that have an anti-seizure activity to the anti-epileptic regimen is logical. The anti-diabetic agent metformin has anti-seizure activity. Nevertheless, the underlying mechanism of the anti-seizure activity of metformin was not entirely clarified. Henceforward, the objective of this review was to exemplify the mechanistic role of metformin in epilepsy. Metformin has anti-seizure activity by triggering adenosine monophosphate-activated protein kinase (AMPK) signalling and inhibiting the mechanistic target of rapamycin (mTOR) pathways which are dysregulated in epilepsy. In addition, metformin improves the expression of brain-derived neurotrophic factor (BDNF) which has a neuroprotective effect. Hence, metformin via induction of BDNF can reduce seizure progression and severity. Consequently, increasing neuronal progranulin by metformin may explain the anti-seizure mechanism of metformin. Also, metformin reduces α-synuclein and increases protein phosphatase 2A (PPA2) with modulation of neuroinflammation. In conclusion, metformin might be an adjuvant with AEAs in the management of refractory epilepsy. Preclinical and clinical studies are warranted in this regard. 相似文献

The present study describes the size at first sexual maturity, fecundity, length–weight (LWRs) and length–length relationships (LLRs) of the pool barb, Puntius sophore, using data obtained from different geographical locations in Bangladesh. A total of 905 specimens were caught by traditional fishing gear from March 2010 to February 2011. Additionally, a total of 121 females were collected from a commercial catch of the Padma River during June–July 2011 to estimate size at first maturity and to determine fecundity. Total length (TL), fork length (FL) and standard length (SL) were measured with digital slide calipers. Individual body weights (BW) were determined for all specimens, and gonad weights (GW) from 121 females were weighed to an accuracy of 0.001 g. The female gonadosomatic index (GSI) was calculated as [GSI (%) = (GW/BW) × 100]. Female size at first maturity was estimated using GSI and TL as indicators, and estimated as 5.00 cm TL in the Padma River. Specimens larger than 5.00 cm TL were used to determine fecundity. Mean total fecundity was 5300 ± 2700, ranging from 1580 to 16590. A positive exponential correlation was recorded between total fecundity and total length (r2 = 0.421). Relative fecundity ranged from 466 to 4036 (mean 1100 ± 580) in the Padma River. The LWR of pooled data for sexes combined was estimated as BW = 0.0155 TL2.98 as ancova revealed no significant differences in LWRs between rivers (P > 0.05). All LLRs were highly correlated (r2 > 0.983; P < 0.001), and ancova analyses further indicated that LLRs did not differ between rivers (P > 0.05). Increased kynurenine pathway metabolism has been implicated in the aetiology of the AIDS dementia complex (ADC). The rate limiting enzyme for this pathway is indoleamine 2,3-dioxygenase (IDO). We tested the efficacy of different strains of HIV-1 (HIV1-BaL, HIV1-JRFL and HIV1-631) to induce IDO in cultured human monocyte-derived macrophages (MDM). A significant increase in both IDO protein and kynurenine synthesis was observed after 48 h in MDM infected with the brain derived HIV-1 isolates, laboratory adapted (LA) HIV1-JRFL, and primary isolate HIV1-631. In contrast, almost no kynurenine production or IDO protein was evident in MDM infected with the high replicating macrophage tropic LA strain, HIV1-BaL. The induction of IDO and kynurenine synthesis by HIV1-JRFL and HIV1-631 declined to baseline levels by day-8 post-infection. Together, these results indicate that only selected strains of HIV-1 are capable of inducing IDO synthesis and subsequent oxidative tryptophan catabolism in MDM. 相似文献

This study describes the size at first sexual maturity, length–weight relationships (LWR) in relation to size at first sexual maturity, and Fulton’s condition factor (KF) of Eutropiichthys vacha in the Ganges River, northwestern Bangladesh. Sampling was done using traditional fishing gear including cast nets, square lift nets and conical traps during January and April, and July to December 2010. For each individual, total length (TL) was measured to the nearest 0.01 cm, and total weight (BW) was determined to the nearest 0.01 g. The gonadosomatic index (GSI) was calculated by the equation, GSI (%) = (Gonad weight in g/BW) × 100. The size at first sexual maturity of males and females was estimated by the relationship between gonadosomatic index and total length. A total of 583 specimens (289 males; 294 females) ranging from 8.30 to 27.00 cm TL and 3.16 to 159.50 g BW were analyzed. Sizes at first sexual maturity for male and female E. vacha were 13.15 and 14.00 cm TL, respectively. The analysis of covariance (ancova ) revealed significant differences in slope and intercept between early and late phases for males (F = 4.532, P < 0.001) and females (F = 21.984, P < 0.001). The KF was not significantly correlated with TL for males (rs = 0.052; P = 0.378), but was highly correlated for females (rs = ?0.165; P = 0.005). This study establishes a strong base for monitoring changes in length at first sexual maturity attributable to high fishing pressures or other reasons within the Ganges and associated river ecosystems. 相似文献

AIDS patients often contain HIV-1-infected mast cells (MCs)/basophils in their peripheral blood, and in vivo-differentiated MCs/basophils have been isolated from the blood of asthma patients that are HIV-1 susceptible ex vivo due to their surface expression of CD4 and varied chemokine receptors. Because IL-16 is a ligand for CD4 and/or an undefined CD4-associated protein, the ability of this multifunctional cytokine to regulate the development of human MCs/basophils from nongranulated progenitors residing in cord or peripheral blood was evaluated. After 3 wk of culture in the presence of c-kit ligand, IL-16 induced the progenitors residing in the blood of normal individuals to increase their expression of chymase and tryptase about 20-fold. As assessed immunohistochemically, >80% of these tryptase(+) and/or chymase(+) cells expressed CD4. The resulting cells responded to IL-16 in an in vitro chemotaxis assay, and this biologic response could be blocked by anti-IL-16 and anti-CD4 Abs as well as by a competitive peptide inhibitor corresponding to a sequence in the C-terminal domain of IL-16. The additional finding that IL-16 induces calcium mobilization in the HMC-1 cell line indicates that IL-16 acts directly on MCs and their committed progenitors. IL-16-treated MCs/basophils also are less susceptible to infection by an M/R5-tropic strain of HIV-1. Thus, IL-16 regulates MCs/basophils at a number of levels, including their vulnerability to retroviral infection. 相似文献

Tetracycline (TC) is one of the common antibiotics which is widely used in livestock growth promotion. The prevalent application TC may pave way to progression of antibiotic resistant bacteria. The main objective of this study is to determine the effect of Autothermal Thermophilic Aerobic Digestion (ATAD) on the fate of TC residues found in digested poultry litter. For the determination of TC in poultry litter, thin layer chromatography (TLC) and high performance liquid chromatography (HPLC) were done. TLC result revealed that the Rf value of standard TC on TLC plate was 0.97 which correlates with the Rf value of TC at 0, 12, 24 and 36 h of digested poultry litter sample and not at 48, 60 and 72 h. HPLC chromatogram revealed that the limits of detection and the recovery were 5 µg/kg and 96% for standard TC. Linear correlation curves were obtained over the series of 100–500 µg/mL with correlation coefficient of 0.996 and the calibration curve was Y = 0.001X + 0.066. These results confirmed the degradation of TC in ATAD digestion of poultry litter by abiotic processes. 相似文献
