Histoplasmosis, caused by the dimorphic fungus Histoplasma capsulatum, is endemic in many regions of the Americas, Asia and Africa. It has a wide spectrum of clinical manifestations, from asymptomatic infection to severe disseminated disease. A retrospective study was carried out to describe the clinical forms and assess the clinical significance of the laboratory diagnostic tests of patients with histoplasmosis during the period of July 1987 to December 2003 at Instituto de Pesquisa Clínica Evandro Chagas/ FIOCRUZ, RJ, Brazil. Seventy-four patients were included. Forty-nine percent of the cases (n = 36) occurred in HIV positive patients who presented with disseminated disease. The remaining 38 cases were classified in different clinical forms. Histoplasma capsulatum was isolated from 69.5% of the clinical specimens sent to culture. Immunodiffusion and immunoblot were positive in 72.6% and 100% of the performed tests, respectively. Histopathologic findings suggestive of H. capsulatum were found in 63.2% of the performed exams. Serology had a lower proportion of positivity amongst AIDS patients, when compared with HIV negative patients (X2 = 6.65; p lower than 0.008). Statistical differences between AIDS and non-AIDS patients were not observed with culture and histopathology. The specific role of each test varies according to the clinical form. Physicians need to know the value and limitations of the available diagnostic tests, but before that, they have to think about histoplasmosis and consider this clinical entity in their differential diagnosis.  相似文献

Pressed grape pomace obtained from the wine production of Cabernet sauvignon (Vitis vinifera) vintage was dried until 9.8% moisture content, ground and submitted to extraction of soluble components from different extraction techniques. Low pressure extractions were performed with ethanol maceration followed by fractionation with n-hexane, dichloromethane, butanol and ethyl acetate. These solvents were furthermore applied for soxhlet extraction. Supercritical fluid extraction (SFE) was also performed to obtain grape pomace extracts by using pure CO(2) and CO(2) with ethanol as co-solvent in concentrations of 10, 15 and 20%w/w. The operating condition used in high pressure extractions was 150bar and 40 degrees C. The antioxidant activity of the grape pomace extracts was determined considering the free radical scavenging assay using 1,1-Diphenyl-2-picrylhydrazyl (DPPH) and was correlated with the total phenol content determined according to the Folin-Ciocalteu method. The results obtained in DPPH tests indicate the highest antioxidant activity of 96.6+/-0.3%AA, with an IC(50) value of 13+/-1, for the extracts obtained with ethyl acetate in solid-liquid extraction. The highest yield values were achieved in soxhlet extraction with ethanol (13.2%w/w) and with butanol (12.2%w/w), and also by SFE with 15% ethanol (9.2%w/w). The lipophilic composition of grape pomace extracts was evaluated by gas chromatography-mass spectrometry with the identification of components like linoleic acid and ethyl linoleate, with important therapeutic activities.  相似文献

Climatic chamber experiments were carried out with male subjects who were submitted to various rapid temperature changes. All experiments were performed first with the subjects at rest and later at work on a bicycle ergometer. The aims of the study were to obtain quantitative data enabling to determine effects of abrupt thermal load, abrupt work load, and combined load on the topography and the dynamics of temperatures and effectors, and to answer the question whether the effects of combined load may be computed by a linear superposition of pure thermal plus pure work load. Skin temperatures generally respond more directly to abrupt changes of thermal than of work load. This is in contrast to the dynamic behaviour of central temperatures which moreover exhibit the interesting effect of a transient paradoxical response both to the onset of work and of thermal load. Time constants of the dynamics of metabolic heat production are high in response to changes of thermal load as compared to the time constants at the onset and end of work. Generally the time constants of skin temperatures are shorter at rest than at work. Temperature topography changes only to a small extent in exercising subjects. The central temperature increase to combined thermal and work load is not significantly different from the added amount of temperature increases due to pure thermal and to pure work load. This suggests a quasi-linear superposition of both thermal effects and confirms, in accordance with further evidence of this and former studies, the hypothesis that work load does not interfere non-linearly with the regulatory processes.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献
