Objectives

To search for imaging characteristics distinguishing patients with successful from those with futile microbiological pathogen detection by CT-guided biopsy in suspected spondylodiscitis.

Methods

34 consecutive patients with suspected spondylodiscitis underwent CT-guided biopsy for pathogen detection. MR-images were assessed for inflammatory infiltration of disks, adjacent vertebrae, epidural and paravertebral space. CT-images were reviewed for arrosion of adjacent end plates and reduced disk height. Biopsy samples were sent for microbiological examination in 34/34 patients, and for additional histological analysis in 28/34 patients.

Results

Paravertebral infiltration was present in all 10/10 patients with positive microbiology and occurred in only 12/24 patients with negative microbiology, resulting in a sensitivity of 100% and a specificity of 50% for pathogen detection. Despite its limited sensitivities, epidural infiltration and paravertebral abscesses showed considerably higher specificities of 83.3% and 90.9%, respectively. Paravertebral infiltration was more extensive in patients with positive as compared to negative microbiology (p = 0.002). Even though sensitivities for pathogen detection were also high in case of vertebral and disk infiltration, or end plate arrosion, specificities remained below 10%.

Conclusions

Inflammatory infiltration of the paravertebral space indicated successful pathogen detection by CT-guided biopsy. Specificity was increased by the additional occurrence of epidural infiltration or paravertebral abscesses.  相似文献

The Blue Swallow is an endangered species in southern Africa and is probably the most endangered passerine. It is restricted to escarpments with grasslands above 1 000 m where mists are frequent. It appears to forage on the wing even in thick mist raising the question of feather wettability in relation to its adaptation. Extensive physical and behaviourial adaptations are known to occur in a wide variety of birds to deal with the problem of shedding water continuously. To study the water repellency and resistance to water penetration of Blue Swallow feathers, we have examined the microscopic structure of head, back, throat, breast and abdominal feathers as well as remiges and tail feathers by transmission light microscopy. The width (2R) and separation (2D) of rami and barbules have been measured and were used to calculate the parameter (R + D)/R that serves as an indicator ofwater shedding potential. For the remiges and tail feathers the values of the (R+D)/R range from 5 to 10 which is comparable to values for other terrestrial birds. However, for body feathers the range is from 10 (head) and 35 (abdomen)-higher than previously observed for any other bird including Swifts, Apodidae. Blue Swallow feathers are thus the most effective feather yet discovered at repelling water drops. The water repellency is highest in those feathers that are relatively shielded From the direct impact of small water drops (throat, breast, abdomen, back). By contrast, the flight feathers must possess a relatively large resistance to water penetration to avoid becoming waterlogged and this is coupled to low (R+D)/R values. Values for the barbules lay between 2 and 6—the same as found for other bird families—supporting an earlier conclusion that they have little direct effect in repelling water.  相似文献

The chemolithotrophic Zetaproteobacteria represent a novel class of Proteobacteria which oxidize Fe(II) to Fe(III) and are the dominant bacterial population in iron-rich microbial mats. Zetaproteobacteria were first discovered at Lō''ihi Seamount, located 35 km southeast off the big island of Hawai''i, which is characterized by low-temperature diffuse hydrothermal venting. Novel nondegenerate quantitative PCR (qPCR) assays for genes associated with microbial nitrogen fixation, denitrification, arsenic detoxification, Calvin-Benson-Bassham (CBB), and reductive tricarboxylic acid (rTCA) cycles were developed using selected microbial mat community-derived metagenomes. Nitrogen fixation genes were not detected, but all other functional genes were present. This suggests that arsenic detoxification and denitrification processes are likely cooccurring in addition to two modes of carbon fixation. Two groups of microbial mat community types were identified by terminal restriction fragment length polymorphism (T-RFLP) and were further described based on qPCR data for zetaproteobacterial abundance and carbon fixation mode preference. qPCR variance was associated with mat morphology but not with temperature or sample site. Geochemistry data were significantly associated with sample site and mat morphology. Together, these qPCR assays constitute a functional gene signature for iron microbial mat communities across a broad array of temperatures, mat types, chemistries, and sampling sites at Lō''ihi Seamount.  相似文献

The family Ischyroceridae is analysed herein by cladistic methods based on morphological characters, using both PAUP 4.0b and TNT. The data matrix of 41 characters × 32 terminal taxa was constructed using DELTA. Based on the results, we comment on the phylogenetic relationships of certain genera and their synapomorphic characters, also discussing the phylogenetic position of M yersius gen. nov. , which appeared as the sister group of Bathyphotis. In addition, Pseudischyrocerus crenatipes is removed to Bathyphotis, for which a new diagnosis is provided. A taxonomic study with the Ischyroceridae collected on the continental slope (depth, 700–2000 m) in the Campos Basin (20.5–24°S, 40–41°W) was also performed. Samples were collected in November–December 2002 and July–August 2003 using a box core device. As a result, a new genus and eight new species are described: B onnierella campensis sp. nov. , B onnierella laurensi sp. nov. , M yersius denticaudatus gen. et sp. nov. , N otopoma lowryi sp. nov. , N otopoma teresae sp. nov. , P seudericthonius bousfieldi sp. nov. , P seudericthonius concavus sp. nov. , and P seudischyrocerus caecus sp. nov. The genus Bonnierella is recorded for the first time from Brazilian waters, and the subspecies Bonnierella linearis linearis and Bonnierella linearis californica are elevated to species rank. Keys to the genera of Ischyroceridae used in the cladistic analysis and the world species of Notopoma are given. © 2014 The Linnean Society of London  相似文献

Considering the eventuality of an interaction between the two post-translational modifications, phosphorylation and ADP-ribosylation, we investigated the possibility of phosphorylation of the mRNP polyADPR polymerase by a protein kinase C associated to these particles. We demonstrated that cytoplasmic poly (ADP-ribose) polymerase associated with ribonucleoprotein particles containing silent mRNA is phosphorylated by a specifically activated endogenous protein kinase C which in turn induces an inhibition of the polymerase activity. In the absence of protein kinase C activators the mRNP polyADPR-P is also phosphorylated but without changes of its enzymatic activity.  相似文献
