Based on its proven anabolic effects on bone in osteoporosis patients, recombinant parathyroid hormone (PTH1-34) has been evaluated as a potential therapy for skeletal repair. In animals, the effect of PTH1-34 has been investigated in various skeletal repair models such as fractures, allografting, spinal arthrodesis and distraction osteogenesis. These studies have demonstrated that intermittent PTH1-34 treatment enhances and accelerates the skeletal repair process via a number of mechanisms, which include effects on mesenchymal stem cells, angiogenesis, chondrogenesis, bone formation and resorption. Furthermore, PTH1-34 has been shown to enhance bone repair in challenged animal models of aging, inflammatory arthritis and glucocorticoid-induced bone loss. This pre-clinical success has led to off-label clinical use and a number of case reports documenting PTH1-34 treatment of delayed-unions and non-unions have been published. Although a recently completed phase 2 clinical trial of PTH1-34 treatment of patients with radius fracture has failed to achieve its primary outcome, largely because of effective healing in the placebo group, several secondary outcomes are statistically significant, highlighting important issues concerning the appropriate patient population for PTH1-34 therapy in skeletal repair. Here, we review our current knowledge of the effects of PTH1-34 therapy for bone healing, enumerate several critical unresolved issues (e.g., appropriate dosing regimen and indications) and discuss the long-term potential of this drug as an adjuvant for endogenous tissue engineering.  相似文献

Sulfonamides can be used in the treatment of impetigo with vastly increased safety and with more effectiveness in powder rather than ointment form when combined with urea powder in a ratio of approximately three parts of sulfonamide to one of urea. Of 701 patients treated with such a mixture, 95.6 per cent were cured within a week. The only complication was local dermatitis which occurred in 0.57 per cent of patients. This compares favorably with results obtained with newer and expensive drugs which usually have the disadvantage of being used in a greasy vehicle. The low incidence of sensitivity reaction to the sulfonamide-urea powder is perhaps ascribable in part to the avoidance of a greasy vehicle.  相似文献

Classification of the 2,557,035 diagnoses made in 1,168,139 different patients at the Los Angeles County General Hospital in the period 1918-48 has been compiled according to the new International List of Causes of Diseases and Death. Similar tabulation of the 377,776 diagnoses made in the 112,409 patients who died while in the hospital reflects the fatality rates associated with these diagnoses. Striking differences are shown in the incidence and fatality rates in the various conditions and great changes have occurred in both during this time. Defects and sources of error in these data are recognized, but they offer a wealth of valuable material for further study and interpretation.  相似文献

In tomato the acid phosphatase-1 isozyme (Apase-1) is inherited as a single locus linked to the nematode resistance gene (Mi). The Apase-11 electrophoretic variant has been purified from a tomato cell suspension culture using ion exchange and concanavalin A sepharose affinity chromatography. A cellulose acetate electrophoresis method was used to distinguish Apase-11 rapidly from other Apase isozymes in tomato. The subunit molecular weight of the purified enzyme was estimated to be 31,000 by sodium dodecyl sulfate-polyacrylamide gel electrophoresis. The native size of the enzyme, which is reported to be a dimer, was determined to be approximately 51,000 by high performance liquid chromatography gel filtration. Apase-11 has a lower pH optimum and a distinct substrate specificity as compared to Apases extracted from tomato fruit or from other plant species. The amino acid composition of Apase-11 is similar to that of a potato Apase.  相似文献
