BackgroundThe Gambia initiated a control programme for schistosomiasis in 2015. In light of this, recent and comprehensive data on schistosomiasis is required to effectively guide the control programme. This study aimed to evaluate the prevalence and associated risk factors of schistosomiasis among primary school children in The Gambia.MethodsWe utilised data from a previous study conducted in 2015 in 4 regions of The Gambia: North Bank Region (NBR), Lower River Region (LRR), Central River Region (CRR) and Upper River Region (URR). In the parent study, ten schools were selected randomly from each region. Urine and stool samples collected from 25 boys and 25 girls (7–14 years) in each school were examined for urinary schistosomiasis (Schistosoma haematobium infection) and intestinal schistosomiasis (Schistosoma mansoni infection) using urine filtration, dipstick and Kato-Katz methods.Principal findingsUrinary schistosomiasis had an overall prevalence of 10.2% while intestinal schistosomiasis had a prevalence of 0.3% among the sampled school children. Prevalence of urinary schistosomiasis was significantly different among regions (χ 2 = 279.958, df = 3, p < 0.001), with CRR (27.6%) being the most endemic region, followed by URR (12.0%), then LRR (0.6%), and NBR (0.0%). Prevalence of intestinal schistosomiasis was also significantly variable among regions, with 4 of the 5 positive cases detected in CRR and 1 case in URR. Every school sampled in CRR had at least one student infected with S. haematobium, 50% of schools in URR had S. haematobium infection, and just one school in LRR had S. haematobium infection. While S. haematobium infection was significantly higher in boys (χ 2 = 4.440, df = 1, p = 0.035), no significant difference in infection rate was observed among age groups (χ 2 = 0.882, df = 2, p = 0.643). Two of the 5 students infected with S. mansoni were boys and 3 were girls. Four of these 5 students were in the 10–12 years age group and 1 was in the 7–9 years age group. Macrohaematuria and microhaematuria were found to be statistically associated with presence of S. haematobium eggs in urine. Being a male was a risk factor of S. haematobium infection. Bathing, playing and swimming in water bodies were found to pose less risk for S. haematobium infection, indicating that the true water contact behaviour of children was possibly underrepresented.ConclusionThe findings of this study provide invaluable information on the prevalence of schistosomiasis in The Gambia. This was useful for the schistosomiasis control efforts of the country, as it guided mass drug administration campaigns in eligible districts in the study area. More studies on S. mansoni and its intermediate snail hosts are required to establish its true status in The Gambia. As children sometimes tend to provide responses that potentially please the research or their teacher, data collection frameworks and approaches that ensure true responses in studies involving children should be devised and used.  相似文献

Background

Introduction of pneumococcal vaccines in Nigeria is a priority as part of the Accelerated Vaccine Introduction Initiative (AVI) of the Global Alliance for Vaccines and Immunisation (GAVI). However, country data on the burden of pneumococcal disease (IPD) is limited and coverage by available conjugate vaccines is unknown. This study was carried out to describe the pre vaccination epidemiology and population biology of pneumococcal carriage in Nigeria.

Methods

This was a cross sectional survey. Nasopharyngeal swabs (NPS) were obtained from a population sample in 14 contiguous peri-urban Nigerian communities. Data on demographic characteristics and risk factor for carriage were obtained from all study participants. Pneumococci isolated from NPS were characterised by serotyping, antimicrobial susceptibility and Multi Locus Sequencing Typing (MLST).

Results

The prevalence of pneumococcal carriage was 52.5%. Carriage was higher in children compared to adults (67.4% vs. 26%), highest (≈90%) in infants aged <9 months and reduced significantly with increasing age (P<0.001). Serotypes 19F (18.6%) and 6A (14.4%) were most predominant. Potential vaccine coverage was 43.8%, 45.0% and 62% for PCV-7, PCV-10 and PCV-13 respectively. There were 16 novel alleles, 72 different sequence types (STs) from the isolates and 3 Sequence Types (280, 310 and 5543) were associated with isolates of more than one serotype indicative of serotype switching. Antimicrobial resistance was high for cotrimoxazole (93%) and tetracycline (84%), a third of isolates had intermediate resistance to penicillin. Young age was the only risk factor significantly associated with carriage.

Conclusions

Pneumococcal carriage and serotype diversity is highly prevalent in Nigeria especially in infants. Based on the coverage of serotypes in this study, PCV-13 is the obvious choice to reduce disease burden and prevalence of drug resistant pneumococci. However, its use will require careful monitoring. Our findings provide sound baseline data for impact assessment following vaccine introduction in Nigeria.  相似文献

Purpose

Sustainability differentiation has become an important issue for companies throughout the value chain. There is thus a need for detailed and credible analyses, which show the current status and point out where improvements can be done and how. The study describes how a comprehensive product-centric eco-efficiency indicator framework can be used to evaluate, benchmark, and communicate the sustainability of a copper production value chain. The indicator framework, together with the suggested data collection and simulation methods, aims at evaluating the whole system, while still enabling a focus on scopes of different width. The status of the environment, current production technologies, location-specific and process-specific issues all play a role in achieving sustainable development.

Methods

Copper cathode production from copper ore was chosen to exemplify the developed framework. Data sets from a simulation tool were used when available and LCI databases and LCA software were utilized for the remaining steps. The value chain is analyzed and the benchmark for each indicator built according to the new Gaia Refiner indicator framework. This method enables analysis of specific production steps with a higher degree of accuracy.

Results and discussion

The case study shows how some important environmental sustainability issues in copper production can be analyzed and benchmarked within a product group. Benchmark data is collected and used in the analysis for the selected system scope. Data availability is still an issue and the example shows which areas require more information in this context so that products and value chains can be benchmarked in the future on a more consistent basis. The energy mix, chemical use, and land use contribute to potential environmental sustainability risks within the product benchmarking group, while emissions control shows competitive environmental sustainability advantages for the case study.

Conclusions

The methodology is shown to work well in highlighting the sustainability advantages and risks of value chains in copper production with the selected system scope in a visual manner through the Sustainability Indicator “Flower.” The importance of a baseline is clear. The effect of the metal ore grade on the results shows that the scalability of the analysis system is very important. Scaling the system scope up will show the differences in varying value chains and scaling the system scope down will show efficiency differences between more similar value chains, thus visualizing where innovation has the biggest impact.

Purpose

The purpose of this work was to develop an indicator framework for the environmental sustainability benchmarking of products produced by the metallurgical industry. Sustainability differentiation has become an important issue for companies throughout the value chain. Differentiation is sometimes not attainable, due to the use of average data, lack of comparative data, certain issues being overshadowed by others, and a very narrow palette of indicators dominating the current sustainability assessments. There is a need for detailed and credible analyses, which show the current status and point out where improvements can be made. The indicator framework is developed to give a comprehensive picture of eco-efficiency, to provide methods that enable relevant comparisons as well as the tools for communicating the results. In this way, the methodology presented in this study aims to make differentiation easier and thus aid companies in driving the development toward more sustainable solutions.

Methods

The framework is based on the existing indicator framework Gaia Biorefiner, which is primarily intended for bio-based products. In this work, the framework was further developed for application in the metallurgical industry. The indicator framework is built by first looking at the issues, which are critical to the environment and global challenges seen today and which the activities of the metallurgical industry may have an impact on. Based on these issues, suitable indicators are chosen if they exist and built if they do not. The idea is that all indicators in a group form a whole, showing areas of innovation while refraining from aggregating and weighting, which often compromise a comprehensive and objective view. Both qualitative and quantitative indicators are included. The indicators are constructed following the criteria set by the EU and OECD for building indicators. Each indicator further has a benchmark. The rules for building the benchmark are connected to the indicators. Suitable data sources and criteria for the benchmark and the indicators are gathered from literature, publicly available databases, and commercial LCA software. The use of simulation tools for attaining more reliable data is also studied.

Results and discussion

The result is a visual framework consisting of ten indicator groups with one to five indicators each, totaling up to 31 indicators. These are visualized in a sustainability indicator “flower.” The flower can be further opened up to study each indicator and the reasons behind the results. The sustainability benchmark follows a methodology that is based on utilization of baseline data and sustainability criteria or limits. A simulation approach was included in the methodology to address the problem with data scarcity and data reliability. The status of the environment, current production technologies, location-specific issues, and process-specific issues all affect the result, and the aim of finding relevant comparisons that will support sustainability differentiation is answered by a scalable scoping system.

Conclusions

A new framework and its concise visualization has been built for assessing the eco-efficiency of products from the metallurgical industry, in a way that aims to answer the needs of the industry. Since there is a baseline, against which each indicator can be benchmarked, a sustainability indicator “flower” can be derived, one of the key innovations of this methodology. This approach goes beyond the usual quantification, as it is also scalable and linked to technology and its fundamental parameters. In part 2, a case study “A case study from the copper industry” tests and illustrates the methodology.

Environmental changes threaten insect pollinators, creating risks for agriculture and ecosystem stability. Despite their importance, we know little about how wild insects respond to environmental pressures. To understand the genomic bases of adaptation in an ecologically important pollinator, we analyzed genomes of Bombus terrestris bumblebees collected across Great Britain. We reveal extensive genetic diversity within this population, and strong signatures of recent adaptation throughout the genome affecting key processes including neurobiology and wing development. We also discover unusual features of the genome, including a region containing 53 genes that lacks genetic diversity in many bee species, and a horizontal gene transfer from a Wolbachia bacteria. Overall, the genetic diversity we observe and how it is distributed throughout the genome and the population should support the resilience of this important pollinator species to ongoing and future selective pressures. Applying our approach to more species should help understand how they can differ in their adaptive potential, and to develop conservation strategies for those most at risk.  相似文献

IntroductionParasitic infections, especially intestinal protozoan parasites (IPPs) remain a significant public health issue in Africa, where many conditions favour the transmission and children are the primary victims. This systematic review and meta-analysis was carried out with the objective of assessing the prevalence of IPPs among school children in Africa.MethodsRelevant studies published between January 2000 and December 2020 were identified by systematic online search on PubMed, Web of Science, Embase and Scopus databases without language restriction. Pooled prevalence was estimated using a random-effects model. Heterogeneity of studies were assessed using Cochrane Q test and I2 test, while publication bias was evaluated using Egger’s test.ResultsOf the 1,645 articles identified through our searches, 46 cross-sectional studies matched our inclusion criteria, reported data from 29,968 school children of Africa. The pooled prevalence of intestinal protozoan parasites amongst African school children was 25.8% (95% CI: 21.2%-30.3%) with E. histolytica/ dispar (13.3%; 95% CI: 10.9%-15.9%) and Giardia spp. (12%; 95% CI: 9.8%-14.3%) were the most predominant pathogenic parasites amongst the study participants. While E. coli was the most common non-pathogenic protozoa (17.1%; 95% CI: 10.9%-23.2%).ConclusionsThis study revealed a relatively high prevalence of IPPs in school children, especially in northern and western Africa. Thus, poverty reduction, improvement of sanitation and hygiene and attention to preventive control measures will be the key to reducing protozoan parasite transmission.  相似文献

Objectives

To determine the duration of protection from hepatitis B vaccine given in infancy and early childhood and asses risk factors for HBV infection and chronic infection.

Methods

In 1984 infant HBV vaccination was started in two Gambian villages. Cross sectional serological surveys have been undertaken every 4 years to determine vaccine efficacy. In the current survey 84.6% of 1508 eligible participants aged 1–28 years were tested. A spouse study was conducted in females (aged 14 years and above) and their male partners.

Results

Vaccine efficacy against chronic infection with hepatitis B virus was 95.1% (95% confidence interval 91.5% to 97.1%), which did not vary significantly between age groups or village. Efficacy against infection was 85.4% (82.7% to 87.7%), falling significantly with age. Concentrations of hepatitis B antibody fell exponentially with age varying according to peak response: 20 years after vaccination only 17.8% (95% CI 10.1–25.6) of persons with a low peak response (10–99 mIU/ml) had detectable HBs antibody compared to 27% (21.9% to 32.2%) of those with a high peak response (>999 mIU/ml). Time since vaccination and a low peak response were the strongest risk factors for HBV infections; males were more susceptible, marriage was not a significant risk for females. Hepatitis B DNA was not detected after infection, which tested soley core antibody positive. An undetectable peak antibody response of <10 mIU/ml and a mother who was hepatitis B e antigen positive were powerful risk factors for chronic infection.

Conclusions

Adolescents and young adults vaccinated in infancy are at increased risk of hepatitis B infection, but not chronic infection. Married women were not at increased risk. There is no compelling evidence for the use of a booster dose of HBV vaccine in The Gambia.  相似文献
