目的:探讨就诊于急诊室诊断为脓毒症或者脓毒性休克的患者30天内死亡率与在急诊室初次获得的参数的关系,并探讨死亡率的危险因素。方法:选取2014年6月到2017年6月就诊于我院急诊室且诊断为脓毒血症或者脓毒性休克并有完整随访资料大于65岁的老年患者145例,将30天内存活的患者分为A组,将30天内死亡的患者分为B组,比较两组之间检验指标及生命体征的差异。根据脓毒症序贯器官衰竭评估快速评分(qSOFA)将qSOFA2分的定义为a组,qSOFA≥2分的定义为b组,比较两组的死亡率。并采用二项Logistic回归分析探讨30天死亡率的独立危险因素。结果:145例患者中,44.8%(n=65)的患者在30天内死亡,33.1%(n=48)的患者需要无创或者侵入性机械通气。A组与B组之间舒张压(P=0.003)、收缩压(P=0.002)、格拉斯哥昏迷量表评分(GCS)(P0.001)、血尿素氮(P0.001)及qSOFA(P0.001)比较均具有统计学差异。a组死亡率(35.7%)显著低于b组(53.3%)(P=0.033)。qSOFA是30天内死亡率的独立危险因素(OR=2.871,P=0.004)。结论:qSOFA是老年脓毒症及脓毒性休克患者30天内死亡的的独立危险因素。 相似文献

Rosa setigera is unique among known roses because it is truly dioecious, yet the plants and flowers are almost impossible to sex. Subtle differences in the inflorescences have been detected, for example, there are more flowers per inflorescence on male plants than on female plants and petal expansion from the day of opening to the next day is greater in females than in males; in addition, pollen from female plants appears somewhat collapsed and does not germinate. Pollinators ( Apis mellifera , other bees (Apidae), Eristalis tenax and other Syrphidae: Diptera) visit the blossoms mostly in the morning to collect pollen (there is no nectar) and in choice experiments do not discriminate between male flowers and female flowers on landing, but do spend significantly more time on the former. They do discriminate between freshly opened flowers and day-old flowers, and show marked preference for bouquets of five flowers vs . single flowers. The female plants, with smaller inflorescences and lesser interest to pollen foraging insects, seem to encourage them to forage at more flowers than do their male counterparts. This may be biologically significant for effective pollen flow in a dioecious pollenonly plant with pollenivorous pollinators. 相似文献

Previous lines of investigation assuming potential advantage of clonal integration generally have neglected its plasticity in complex heterogeneous environments. Clonal plants adaptively respond to abiotic heterogeneity (patchy resource distribution) and herbivory‐induced heterogeneity (within‐clone heterogeneity in ramet performance), but to date little is known about how resource heterogeneity and simulated herbivory jointly affect the overall performance of clones. Partial damage within a clone caused by herbivory might create herbivory‐induced heterogeneity in a resource‐homogeneous environment, and might also decrease or increase the extent of heterogeneity under resource‐heterogeneous conditions. We conducted a greenhouse experiment in which target‐ramets of Leymus chinensis segments within homogeneous or heterogeneous nutrient treatments were subject to clipping (0% or 75% shoot removal). In homogeneous environments with high (9:9) nutrient availability, ramet biomass of L. chinensis with intact or severed rhizomes is 0.70 or 0.69 g. Conversely, target‐ramet biomass with intact rhizomes is obviously lower than that of the severed target‐ramets in the homogeneous environments with medium (5:5) and low (1:1) nutrient availability. High resource availability and the presence of herbivory can alleviate negative effects of rhizome connection under homogeneous conditions, by providing copious resource or creating herbivory‐induced heterogeneity respectively. Herbivory tolerance of clonal fragments with connected rhizomes was higher than that of fragments with severed rhizomes under heterogeneous conditions. These findings confirmed the unconditional advantage of clonal integration on reproduction under the combined influence of resource heterogeneity and simulated herbivory. Moreover, our results made clear the synergistically interactive effects of resource heterogeneity and simulated herbivory on costs and benefits of clonal integration. DNA methylation is an epigenetic mechanism that has the potential to affect plant phenotypes and that is responsive to environmental and genomic stresses such as hybridization and polyploidization. We explored de novo methylation variation that arises during the formation of triploid asexual dandelions from diploid sexual mother plants using methylation‐sensitive amplified fragment length polymorphism (MS‐AFLP) analysis. In dandelions, triploid apomictic asexuals are produced from diploid sexual mothers that are fertilized by polyploid pollen donors. We asked whether the ploidy level change that accompanies the formation of new asexual lineages triggers methylation changes that contribute to heritable epigenetic variation within novel asexual lineages. Comparison of MS‐AFLP and AFLP fragment inheritance in a diploid × triploid cross revealed de novo methylation variation between triploid F1 individuals. Genetically identical offspring of asexual F1 plants showed modest levels of methylation variation, comparable to background levels as observed among sibs in a long‐established asexual lineage. Thus, the cross between ploidy levels triggered de novo methylation variation between asexual lineages, whereas it did not seem to contribute directly to variation within new asexual lineages. The observed background level of methylation variation suggests that considerable autonomous methylation variation could build up within asexual lineages under natural conditions. 相似文献

Parasites that are molecular mimics express proteins which resemble host proteins. This resemblance facilitates immune evasion because the immune molecules with the specificity to react with the parasite also cross‐react with the host's own proteins, and these lymphocytes are rare. Given this advantage, why are not most parasites molecular mimics? Here we explore potential factors that can select against molecular mimicry in parasites and thereby limit its occurrence. We consider two hypotheses: (1) molecular mimics are more likely to induce autoimmunity in their hosts, and hosts with autoimmunity generate fewer new infections (the “costly autoimmunity hypothesis”); and (2) molecular mimicry compromises protein functioning, lowering the within‐host replication rate and leading to fewer new infections (the “mimicry trade‐off hypothesis”). Our analysis shows that although both hypotheses may select against molecular mimicry in parasites, unique hallmarks of protein expression identify whether selection is due to the costly autoimmunity hypothesis or the mimicry trade‐off hypothesis. We show that understanding the relevant selective forces is necessary to predict how different medical interventions will affect the proportion of hosts that experience the different infection types, and that if parasite evolution is ignored, interventions aimed at reducing infection‐induced autoimmunity may ultimately fail. 相似文献

1. Plant stress and association with mycorrhizal fungi have been shown to significantly influence plant quality, yet their roles in influencing plant–insect interactions remain unclear. Even less is known about how these factors might interact with or be modified by within‐trophic level interactions. 2. In the present study, the results of a factorial field experiment are reported in which the effects of within‐trophic‐level interactions, plant stress, and mycorrhizae on three herbivores of Baccharis halimifolia were examined. 3. Plant stress was increased by adding salt to the soil, and availability of mycorrhizal fungi was increased by inoculating plant roots. These treatments were applied to plants with either low or high densities of a competitor (Trirhabda baccharidis). 4. For the two leaf miners, Amauromyza maculosa and Liriomyza trifolii, increased soil salinity and high densities of the competitor Trirhabdabaccharidis resulted in significant decreases in density. Neither of these treatments affected the gall maker Neolasioptera lathami. 5. Mycorrhizal fungi increased the densities of all three herbivores, possibly by increasing foliar nitrogen levels. For the two leaf miners, there was also evidence that mycorrhizae ameliorated the negative effects of salt stress. There was also evidence that high levels of competition dampened the positive effects of mycorrhizae on the two leaf miners. 相似文献

Aim In this study we present a molecular phylogenetic and phylogeographical analysis of Peltophryne (Anura: Bufonidae), an endemic genus of Antillean toads, to investigate the spatial and temporal origins of the genus, with particular focus on the eight Cuban species. Location Greater Antilles, with extensive sampling of the Cuban archipelago. Methods We obtained DNA sequence data from two mitochondrial genes, cytochrome c oxidase subunit I (COI) and ribosomal RNA (16S), for 124 toads representing all eight Cuban species, and combined this with published data from Hispaniola (one of three species) and Puerto Rico (one of one species) to establish a molecular phylogeny for Peltophryne. In addition, we explored the phylogeographical structure of widespread Cuban species. For a subset of 42 toads we also obtained DNA sequence data from two nuclear genes, recombination activator‐1 (RAG‐1) and chemokine receptor 4 (CXCR‐4). We combined our molecular data with published DNA sequences from a global sample of bufonid toads to place the spatial and temporal origins of Peltophryne in the Caribbean within a fuller geographical and phylogenetic context. Results All phylogenetic analyses supported the monophyly of West Indian toads. The ancestor of Peltophyrne diverged from its mainland source around the Eocene–Oligocene boundary, with a subsequent radiation across the Caribbean islands taking place during the Miocene. Cuban species are monophyletic with a basal split in the early–middle Miocene that separates extant small‐bodied from large‐bodied species. Extensive mitochondrial DNA (mtDNA) sampling within widespread Cuban species revealed contrasting phylogeographical patterns. Peltophryne taladai and P. empusa showed deeply divergent lineages, whereas no geographical structure was observed in the widespread P. peltocephala. Main conclusions Our timeline for Peltophryne diversification is consistent with a biogeographical model requiring no long‐distance overwater dispersal. Although confidence intervals on divergence time estimates are wide, the stem age of Peltophyrne coincides with the hypothesized GAARlandia landspan or archipelago, which may have connected South America briefly with the Antilles. The ages of Peltophryne for Puerto Rico, Hispaniola and Cuba are consistent with a recently proposed vicariance scenario for the region. Our molecular results support the recognition of all eight species in Cuba, and provide evidence of possible cryptic species. 相似文献
