摘要 目的:对比分析贝那鲁肽与利拉鲁肽对2型糖尿病患者长期结局的影响,为二甲双胍和磺脲类药物治疗后血糖不能达标的患者应用胰高糖素样肽-1(GLP-1) 受体激动剂提供临床依据。方法:选取2018年3月~2019年3月在我院就诊的使用二甲双胍或者二甲双胍联合磺脲类药物血糖未达标的2型糖尿病患者150例作为研究对象,随机分为观察组和对照组,每组各75例。观察组使用贝那鲁肽注射液治疗,对照组使用利拉鲁肽注射液治疗,所有患者均连续治疗24周。比较两组患者治疗前后空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)、收缩期血管峰值血流( PSV)、血管内膜中层厚度(IMT) 、肌酐(Scr)和尿素氮(BUN)水平,两年内的不良事件发生率、心血管相关死亡率和全因死亡率。结果:两组患者之间治疗前、后FPG、2hPG、HbA1c、PSV、IMT、Scr和BUN水平无统计学差异(P>0.05), 观察组的不良事件总发生率(8.00%)较对照组(20.00%)明显降低(P<0.05),两组患者心血管相关死亡率和全因死亡率相比,差异无统计学意义(P>0.05)。结论:在使用二甲双胍或者二甲双胍联合磺脲类药物血糖未达标的2型糖尿病患者中,与利拉鲁肽相比,贝那鲁肽在降低血糖、改善下肢血管功能、肾功能和死亡率方面无明显差异,但不良事件发生率更低。  相似文献

In this study we show that the natural abundance of the nitrogen isotope 15, δ15N, of plants in heath tundra and at the tundra-forest ecocline is closely correlated with the presence and type of mycorrhizal association in the plant roots. A total of 56 vascular plant species, 7 moss species, 2 lichens and 6 species of fungi from four heath and forest tundra sites in Greenland, Siberia and Sweden were analysed for δ15N and N concentration. Roots of vascular plants were examined for mycorrhizal colonization, and the soil organic matter was analysed for δ15N, N concentration and soil inorganic, dissolved organic and microbial N. No arbuscular mycorrhizal (AM) colonizations were found although potential host plants were present in all sites. The dominant species were either ectomycorrhizal (ECM) or ericoid mycorrhizal (ERI). The δ15N of ECM or ERI plants was 3.5–7.7‰ lower than that of non-mycorrhizal (NON) species in three of the four sites. This corresponds to the results in our earlier study of mycorrhiza and plant δ15N which was limited to one heath and one fellfield in N Sweden. Hence, our data suggest that the δ15N pattern: NON/AM plants > ECM plants ≥ ERI plants is a general phenomenon in ecosystems with nutrient-deficient organogenic soils. In the fourth site, a␣birch forest with a lush herb/shrub understorey, the differences between functional groups were considerably smaller, and only the ERI species differed (by 1.1‰) from the NON species. Plants of all functional groups from this site had nearly twice the leaf N concentration as that found in the same species at the other three sites. It is likely that low inorganic N availability is a prerequisite for strong δ15N separation among functional groups. Both ECM roots and fruitbodies were 15N enriched compared to leaves which suggests that the difference in δ15N between plants with different kinds of mycorrhiza could be due to isotopic fractionation at the␣fungal-plant interface. However, differences in δ15N between soil N forms absorbed by the plants could also contribute to the wide differences in plant δ15N found in most heath and forest tundra ecosystems. We hypothesize that during microbial immobilization of soil ammonium the microbial N pool could become 15N-depleted and the remaining, plant-available soil ammonium 15N-enriched. The latter could be a main source of N for NON/AM plants which usually have high δ15N. In contrast, amino acids and other soil organic N compounds presumably are 15N-depleted, similar to plant litter, and ECM and ERI plants with high uptake of these N forms hence have low leaf δ15N. Further indications come from the δ15N of mosses and lichens which was similar to that of ECM plants. Tundra cryptogams (and ECM and ERI plants) have previously been shown to have higher uptake of amino acid than ammonium N; their low δ15N might therefore reflect the δ15N of free amino acids in the soil. The concentration of dissolved organic N was 3–16 times higher than that of inorganic N in the sites. Organic nitrogen could be an important N source for ECM and, in particular, ERI plants in heath and forest tundra ecosystems with low release rate of inorganic N from the soil organic matter. Abstract. The characteristics of microhabitats of established Pinus sylvestris and Betula seedlings were studied in a small windthrow gap in a mature P. sylvestris-dominated forest in the Petkeljärvi National Park in eastern Finland. Seedlings were strongly clustered in disturbed microhabitats, particularly uprooting pits and mounds, formed by tree falls. They covered 3% of the 0.3.ha study area consisting of the gap and some of the forest edge. Although Betula occurred only as scattered individuals in the dominant canopy layer of the forest, it accounted for 30% of the seedlings found in the study area. Betula regeneration was almost completely restricted to pits and mounds, where 91% of the seedlings were found. Uprooting spots were also the most important regeneration microhabitats for Pinus, where 60% of the seedlings grew, even though the seedlings were found in other substrates as well, particularly on sufficiently decomposed coarse wood. Undisturbed field- and bottom-layer vegetation had effectively hindered tree seedling establishment, which emphasises the role of soil disturbance for regeneration. While the establishment of seedlings was found to be clearly determined by the availability of favourable regeneration microhabitats, the early growth of seedlings was affected by a complex interaction of environmental variables, including the type of microhabitat, radiation environment and interferences caused by competing seedlings and adjacent trees. In the most important regeneration microhabitats, i.e. in uprooting pits and on mounds, the distributions of the local elevations of Pinus and Betula seedlings were different. Pinus seedlings occurred closer to ground level, i.e. on the fringes of pits and lower on mounds, while Betula seedlings grew deeper in pits and higher on mounds. The position of the Betula seedlings indicate that they may have a competitive advantage over Pinus seedlings in the dense seedling groups occurring in uprooting spots. We suggest that this initial difference in Pinus and Betula establishment may affect the subsequent within-gap tree species succession and can, in part, explain the general occurrence of Betula in conifer-dominated boreal forests.  相似文献

花马湖位于北纬30°15′—30°18′、东经114°58′—115°1′,面积约27.5km~2。湖中分布82种2变种水生维管束植物,隶属于34科66属,其中资源植物众多。花马湖水生高等植物大型属少,小型属较多,单型属相对突出。区系地理成分以温带成分占优势,计34种,占全湖种类的40.48%。起源古老的成分也较多。通过与有关湖泊水生植物区系的比较,及区系成分的分析,说明花马湖水生植物区系具有从暖温带至中亚热带过渡的特点。  相似文献

摘要 目的:探讨老年2型糖尿病(T2DM)患者血清脂肪因子血管生成素样蛋白4(ANGPTL4)、促代谢因子(Betatrophin)、腹腔脂肪型丝氨酸蛋白酶抑制剂(Vaspin)水平与血糖、血脂、下肢血管病变(LVD)的关系。方法:选取我院2018年8月~2019年8月收治的老年T2DM患者108例,根据患者是否合并LVD,分成LVD组(n=38)和无LVD组(n=70),比较两组临床资料、血清ANGPTL4、Betatrophin、Vaspin水平、血糖指标[空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)]、血脂指标[总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]。经Pearson线性相关分析患者血清ANGPTL4、Betatrophin、Vaspin水平与血糖、血脂指标相关性。经Logistic多因素回归模型分析患者LVD发生的影响因素。结果:LVD组舒张压、收缩压均高于无LVD组(P<0.05)。LVD组血清Betatrophin水平及TC、TG、LDL-C、FPG、2hPG、HbA1c高于无LVD组,血清ANGPTL4、Vaspin水平及HDL-C低于无LVD组(P<0.05)。Pearson线性相关分析显示,血清ANGPTL4、Vaspin与TC、TG、LDL-C、FPG、2hPG、HbA1c呈负相关,与HDL-C呈正相关(P<0.05)。血清Betatrophin与TC、TG、LDL-C、FPG、2hPG、HbA1c呈正相关,与HDL-C呈负相关(P<0.05)。Logistic多因素回归模型分析结果显示,血清ANGPTL4、Betatrophin、Vaspin以及舒张压、TC、TG、HDL-C、LDL-C、FPG、2hPG、HbA1c是患者LVD发生的影响因素(P<0.05)。结论:老年T2DM合并LVD患者的血清ANGPTL4、Vaspin水平明显下降,而血清Betatrophin水平升高,且三者与血糖、血脂指标均存在相关性,并且是患者发生LVD的影响因素,临床可考虑通过检测血清ANGPTL4、Betatrophin、Vaspin水平,辅助评估LVD的发生风险。  相似文献

With the world-wide increase of patients with renal failure, the development of functional renal replacement therapies have gained significant interest and novel technologies are rapidly evolving. Currently used renal replacement therapies insufficiently remove accumulating waste products, resulting in the uremic syndrome. A more preferred treatment option is kidney transplantation, but the shortage of donor organs and the increasing number of patients waiting for a transplant warrant the development of novel technologies. The bioartificial kidney (BAK) is such promising biotechnological approach to replace essential renal functions together with the active secretion of waste products. The development of the BAK requires a multidisciplinary approach and evolves at the intersection of regenerative medicine and renal replacement therapy. Here we provide a concise review embracing a compact historical overview of bioartificial kidney development and highlighting the current state-of-the-art, including implementation of living-membranes and the relevance of extracellular matrices. We focus further on the choice of relevant renal epithelial cell lines versus the use of stem cells and co-cultures that need to be implemented in a suitable device. Moreover, the future of the BAK in regenerative nephrology is discussed.  相似文献

血中同型半胱氨酸(homocysteine,HCY)浓度的升高已成为动脉粥样硬化发生的一个独立危险因子.为进一步阐明HCY促进血管平滑肌细胞(vascularsmoothmusclecels,VSMCs)增殖,从而引起动脉粥样硬化发生的机理.本实验采用细胞计数、3H-TdR参入、细胞周期分析、Northern杂交方法证明,一定剂量的HCY可促进离体培养的WKY大鼠血管平滑肌细胞增殖,使其DNA合成增加,细胞周期中S期细胞所占比例增加43%,并促进c-myc与c-fos原癌基因mRNA表达增加.提示HCY可能通过促进VSMCs增殖而诱发动脉粥样硬化  相似文献

Resveratrol (RSVL), an edible polyphenolic stilbene, claims a myriad of cardiovascular benefits. However, the molecular underpinnings of such actions are poorly understood. Currently, in sheep coronary arteries (SCA), RSVL markedly (threefold) enhanced cGMP formation (t(1/2): 6.5 min; EC(50): 3 microM). This response was not abrogated by the phosphodiesterase inhibitor (IBMX, 0.5 mM), but was partly sensitive (20-30%) to either removal of the endothelium, treatment with the nitric oxide synthase-inhibitor (L-NMMA, 10 microM), or with the soluble GC (sGC)-inhibitor (ODQ, 10 microM). In membrane preparations from denuded SCA, either RSVL or the pGC agonist atrial natriuretic peptide (ANP, 0.1-1 microM) activated GC in the particulate, but not in the soluble, membrane fraction. By contrast, the nitric oxide donor, sodium nitroprusside (SNP, 1-10 microM), stimulated GC only in the soluble fraction. Further, pretreatment with RSVL partly desensitized the ANP response, but was additive to that of SNP. In arterial tension studies, RSVL relaxed PGF(2alpha)-precontracted denuded rings in a concentration-dependent manner, a response that was markedly enhanced (approximately 18 fold) in the presence of IBMX. Conversely, precontraction with phorbol ester, which also desensitizes pGC, blunted relaxations to RSVL but not to forskolin or SNP. These findings demonstrate that RSVL increases cGMP in coronary arteries, mostly by activation of pGC. This pathway triggers vasorelaxant responses that remain effective in endothelium-disrupted arteries.  相似文献
