The influence of anatomical boundaries, age, and sex on the assessment of abdominal visceral fat. Single-slice abdominal computed tomography (CT) scanning has been used extensively for the measurement of abdominal visceral fat (AYF). Optimal anatomical scan location and pixel density ranges have been proposed and are specifically reported to allow for the replication and standardization of AVF measurements. Standardization of the anatomical boundaries for CT measurement of AVF and the influence of age and gender on results obtained with different boundary locations have received much less attention. To determine the influence of three boundary analysis methods (AVF-1, AVF-2, and AVF-3) on the measurement of AVF by CT, 54 older (60 years to 79 years) and 37 younger (20 years to 29 years) healthy men and women were examined. The measurement boundary for AVF-1 was the internal most aspect of the abdominal and oblique muscle walls, and the posterior aspect of the vertebral body. AVF-2 used fat measurements enclosed in a boundary formed by the midpoint of the abdominal and oblique muscle walls, and the most posterior aspect of the spinous process. AVF-3 used fat measurements enclosed in a boundary formed by the external border of the abdominal and oblique muscle walls, and the external border of the erector spinae. Greater AVF measures were obtained with AVF-2 and AVF-3 compared with AVF-1 (p<0.0001). These differences were greater in older compared with younger subjects (p<0.0001) and greater in women compared with men (p<0.02). The significantly greater AVF measurements obtained with AVF-2 and AVF-3 resulted from the inclusion of larger amounts of fat that are not drained by the portal circulation. This included retroperitoneal, intermuscular, and intramuscular lipid droplets, which increase with aging. On the basis of these results, we recommend the AVF-1 anatomical boundaries for the measurement of AVF in clinical investigations, particularly with older subjects. These data demonstrate the importance of precise and reproducible anatomical boundaries for the measurement of AVF, particularly in longitudinal studies. 相似文献

Circumcision is often claimed to be simpler, safer and more cost-effective when performed in the neonatal period as opposed to later in life, with a greater benefit-to-risk ratio. In the first part of this paper, we critically examine the evidence base for these claims, and find that it is not as robust as is commonly assumed. In the second part, we demonstrate that, even if one simply grants these claims for the sake of argument, it still does not follow that neonatal circumcision is ethically permissible absent urgent medical necessity. Based on a careful consideration of the relevant evidence, arguments and counterarguments, we conclude that medically unnecessary penile circumcision—like other medically unnecessary genital procedures, such as ‘cosmetic’ labiaplasty—should not be performed on individuals who are too young (or otherwise unable) to provide meaningful consent to the procedure. 相似文献

Medical imaging using X-rays has been one of the most popular imaging modalities ever since the discovery of X-rays 125 years ago. With unquestionable benefits, concerns about radiation risks have frequently been raised. Computed tomography (CT) and fluoroscopic guided interventional procedures have the potential to impart higher radiation exposure to patients than radiographic examinations. Despite technological advances, there have been instances of increased doses per procedure mainly because of better diagnostic information in images. However, cumulative dose from multiple procedures is creating new concerns as effective doses >100 mSv are not uncommon. There is a need for action at all levels. Manufacturers must produce equipment that can provide a quality diagnostic image at substantially lesser dose and better implementation of optimization strategies by users. There is an urgent need for the industry to develop CT scanners with sub-mSv radiation dose, a goal that has been lingering. It appears that a new monochromatic X-ray source will lead to replacement of X-ray tubes all over the world in coming years and will lead to a drastic reduction in radiation doses. This innovation will impact all X-ray imaging and will help dose reduction. For interventional procedures, the likely employment of robotic systems in practice may drastically reduce radiation exposures to operators- but patient exposure will still remain an issue. 从农业资源与环境、农业生产与经济、农业人口与社会3个评价维度构建了区域农业可持续发展指标体系, 运用主成分分析法、熵值法、模糊Borda法、因子分析法及聚类分析法和GIS技术, 通过组合评价方法定量研究湖南省农业可持续发展区域分异现状。结果表明: 湖南省农业可持续发展在区域上总体呈现出不平衡的发展状态, 农业可持续发展综合水平以长株潭城市群为核心, 由东至西递减; 农业人口与社会可持续发展水平南北低中部高, 东部强西部弱; 农业生产与经济可持续发展水平围绕湘北洞庭湖农业区和湘南农业综合区向外围递减; 农业资源与环境可持续发展水平表现为一横两纵优势带和南北两个落后圈。 相似文献

Abstract Mining activities have introduced various heavy metals and metalloids to surrounding soil environments, causing adverse impacts to the ecological environment system. The extremely high concentration of various heavy metals and metalloids make the Xikuangshan (Hunan, China) an excellent model to assess their ecological risk. In this study, the soil samples from 26 locations of different land use methods in four areas (smelting area, road nearby ore, mining area, and ore tailing area) in Xikuangshan with different levels of various heavy metals and metalloids (Sb, As, Pb, and Cd) were analyzed; in addition, the index of geo-accumulation and the potential ecological risk index were used to evaluate ecological risk. The results showed that the average contents of Sb, As, Pb, and Cd in all soil samples were 4368.222?mg·kg?1, 40.722?mg·kg?1, 248.013?mg·kg?1, and 40.941?mg·kg?1, respectively, implying serious contamination of compound pollution of heavy metals in soil. The concentration of heavy metals in soil among smelting area, road nearby ore, mining area, and ore tailing area showed significant distribution characteristics of region because different mining activities such as smelting, mining, transportation, and stacking caused different pollution intensity. Moreover, the contents of Sb in soil samples decreased successively in residue field, wasteland, forestland, sediment, grassland, and vegetable field, and the contents of Sb in vegetable-field and ecological restoration grassland were relatively low, which indicate that the method of grassland ecological restoration is an effective method to control antimony pollution in soil. The results of ecological risk assessment showed that the antimony mining area was seriously polluted by Sb, As, Pb, and Cd, and had strong ecological risk, and Sb and Cd were the most important pollution factors, which indicated that the pollution of Sb and Cd should be a major concern of relevant departments of environment and health. 相似文献

The microenvironment plays a major role in conferring chemoresistance to cancer cells. In order to better inform clinical response to chemoresistance, preclinical models that recapitulate its hallmark features are needed to enable screening for resistance‐specific therapeutic targets. A novel platform for seeding cancer cells in 3D hydrogels is presented utilizing derivatives of chitosan and alginate that, critically, is amenable to high throughput screening: cell seeding in hydrogels, media changes, dosing of anticancer compounds, and cell viability assays are all automated using a standard and commercially available liquid handling robot. Culture in these hydrogels elicits resistance in ovarian, lung, and prostate cancer cells to treatment by doxorubicin and paclitaxel. In correlation, proteomics analysis of SKOV3 cells cultured in 3D reveals enrichment of proteins associated with extreme drug resistance including HMOX1 and ALDH2. Subsequently, therapeutic antibodies targeted to tumor‐associated antigens upregulated in 3D cultures are shown to have higher efficacy compared to 2D cultures. Collectively, this automated 3D cell culture platform provides a powerful tool with utility in identification of drugs that may overcome chemoresistance. 相似文献
