This article presents the biochemical intra-skeletal-muscle basis of exercise intensity domains: moderate (M), heavy (H), very heavy (VH) and severe (S). Threshold origins are mediated by a ‘Pi double-threshold’ mechanism of muscle fatigue, which assumes (1) additional ATP usage, underlying muscle V̇O2 and metabolite slow components, is initiated when inorganic phosphate (Pi) exceeds a critical value (Picrit); (2) exercise is terminated because of fatigue, when Pi reaches a peak value (Pipeak); and (3) the Pi increase and additional ATP usage increase mutually stimulate each other forming a positive feedback. M/H and H/VH borders are defined by Pi on-kinetics in relation to Picrit and Pipeak. The values of the ATP usage activity, proportional to power output (PO), for the M/H, H/VH and VH/S borders are lowest in untrained muscle and highest in well-trained muscle. The metabolic range between the M/H and H/VH border (or ‘H space’) decreases with muscle training, while the difference between the H/VH and VH/S border (or ‘VH space’) is only weakly dependent on training status. The absolute magnitude of the muscle V̇O2 slow-component, absent in M exercise, rises gradually with PO to a maximal value in H exercise, and then decreases with PO in VH and S exercise. Simulations of untrained, physically active and well-trained muscle demonstrate that the muscle M/H border need not be identical to the whole-body M/H border determined from pulmonary V̇O2 on-kinetics and blood lactate, while suggesting that the biochemical origins of the H/VH border reside within skeletal muscle and correspond to whole-body critical power.  相似文献

摘要 目的:分析小剂量糖皮质激素联合持续性血液净化治疗儿童严重脓毒症的效果及安全性。方法:选择自2021年1月至2023年1月接诊的102例儿童严重脓毒症患儿作为研究对象,随机分为对照组和观察组,各51例;对照组予以经典治疗方案,观察组在对照组的基础上,予以小剂量糖皮质激素联合持续性血液净化治疗;记录两组治疗后各项信息,比较两组治疗前后外周血乳酸、中心静脉血氧饱和度(ScvO2)、血清炎症指标、PCIS评分、APACHE Ⅱ评分,观察主要并发症发生情况。结果:与对照组相比,观察组机械通气、低血压持续及入住ICU等时间较短,7 d内停升压药率较高(P<0.05);两组28 d病死率比较无差异(P>0.05);观察组治疗后乳酸、降钙素原(PCT)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)水平均较对照组低,ScvO2水平较对照组高(P<0.05);观察组治疗后PCIS评分较对照组高,APACHE Ⅱ评分较对照组低(P<0.05);观察组主要并发症发生率低于对照组(P<0.05)。结论:小剂量糖皮质激素联合持续性血液净化治疗有利于儿童严重脓毒症患儿病情转归,减少主要并发症发生,可能与阻断炎症反应有关,值得进一步研究应用。  相似文献

摘要 目的:分析颈性眩晕中医证型与经颅超声脑动脉血流检测结果的相关性。方法:选取2021年5月-2023年5月收治颈性眩晕患者作为研究对象,根据不同中医证型分为痰湿中阻组、肝阳上亢组、肝肾阴虚组和气血亏虚组,每组各纳入20例;并另选取健康体检患者30例作为对照组,均给予多普勒超声检查。分析不同中医证型者与对照组者多普勒超声检查特征与脑动脉血流变化[左右椎动脉、基底动脉及大脑中动脉收缩期峰值血流速度(VS)、平均血流速度(Vm)、舒张期峰值血流速度(Vd)及搏动指数(PI)]。结果:痰湿中阻组、肝肾阴虚组和气血亏虚组左右椎动脉、基底动脉及大脑中动脉VS均低于对照组(P<0.05);肝阳上亢组左右椎动脉、基底动脉及大脑中动脉VS均高于对照组(P<0.05);不同中医证型组间VS比较,肝阳上亢组>痰湿中阻组>肝肾阴虚组>气血亏虚组;痰湿中阻组左右椎动脉、基底动脉均高于对照组(P<0.05),大脑中动脉Vm均低于对照组(P<0.05);肝阳上亢组左右椎动脉、基底动脉及大脑中动脉Vm均高于对照组(P<0.05);肝肾阴虚组左右椎动脉、基底动脉及大脑中动脉Vm均低于对照组(P<0.05);气血亏虚组大脑中动脉Vm均低于对照组(P<0.05),左右椎动脉、基底动脉Vm和对照组无显著性差异(P>0.05);不同中医证型组间Vm比较,肝阳上亢组>痰湿中阻组>气血亏虚组>肝肾阴虚组;痰湿中阻组左右椎动脉、基底动脉Vd均低于对照组(P<0.05),大脑中动脉Vd均高于对照组(P<0.05);肝阳上亢组左右椎动脉、基底动脉及大脑中动脉Vd均低于对照组(P<0.05);肝肾阴虚组左右椎动脉及大脑中动脉Vd均低于对照组(P<0.05),基底动脉Vd和对照组无差异(P>0.05);气血亏虚组左右椎动脉Vd均低于对照组(P<0.05),基底动脉Vd和对照组无差异(P>0.05),大脑中动脉Vd均高于对照组(P<0.05);不同中医证型组间Vd比较,气血亏虚组>痰湿中阻组>肝肾阴虚组>肝阳上亢组;痰湿中阻组和气血亏虚组左右椎动脉、基底动脉及大脑中动脉PI均低于对照组(P<0.05);肝阳上亢组和肝肾阴虚组左右椎动脉、基底动脉及大脑中动脉PI均高于对照组(P<0.05);不同中医证型组间PI比较,肝阳上亢组>肝肾阴虚组>痰湿中阻组>气血亏虚组。结论:不同中医证型的眩晕患者会出现不同程度脑动脉血流动力学异常,且不同组间存在差异,通过经颅多普勒超声检查,可以对眩晕中医证型提供参考价值。  相似文献

基于CNKI数据库和Web of Science数据库,利用网页检索功能分析了生态宜居研究的年发文量、基金资助机构及国家地区分布、研究方向及研究机构状况;利用Citespace对2002—2017年生态宜居研究关键词进行共现性和可视化图谱聚类分析。结果表明:近年生态宜居研究逐渐成为人们关注的热点,但该方面研究获得国家及相关部门基金资助的比例仍然较小;当前生态宜居研究主要集中在宏观经济管理与可持续发展学科,研究机构排名中,天津地区相关机构发文量相对较多。就目前研究来看,研究热点集中于新型城镇化、城镇化、指标体系、生态宜居度、生态文明等。在未来的研究发展趋势方面,新型城镇化和生态宜居标准的制定仍然是重要的研究领域,必须在此持续发力,为生态宜居建设提供必要的理论支撑和政策依据。  相似文献

An enzyme-linked immunosorbent assay (ELISA) was developed for the detection of anti-HCV antibody. We assayed for antibodies against either oligopeptide (S29-1) deduced from the nucleocapsid gene or the product of nonstructural region (NS3) synthesized in a recombinant Escherichia coli (S4). To reduce false-positive results induced by non-specific binding of antibodies with a carrier protein and to increase the sensitivity of an immunoassay, non-fused S4 peptide was prepared by the recombinant DNA technique and site-specific proteolysis (by factor Xa). In 71 non-A, non-B hepatitis patients with chronic liver disease, 70 (98.5%) were positive by S29-1/S4 ELISA as well as by a second-generation test (Abbott II). On the other hand, of 40 serum samples from blood donors, in which anti-N14 (core) and C100-3 antibodies were not detected but hepatitis C virus (HCV) RNA was detectable by polymerase chain reaction (PCR), 24 (60%) were positive by S29-1/S4 ELISA, whereas only 18 (45%) were diagnosed by Abbott II. In addition, based on results in a small group of 92 blood donors, detection of anti-S29-1/S4 antibody correlated well with HCV viremia as confirmed by PCR. These results indicated that the preparation of non-fused protein (S4) by recombinant DNA technique and a combination of S29-1 and S4 as immobilized antigens in an ELISA provide a sensitive and specific diagnosis for HCV infection with good correlation with the presence of viral RNA as confirmed by PCR.  相似文献

The cGMP-dependent protein kinase type I (PKG I) is an essential regulator of cellular function in blood vessels throughout the body. DT-2, a peptidic inhibitor of PKG, has played a central role in determining the molecular mechanisms of vascular control involving PKG and its signaling partners. Here, we report the development of (d)-amino acid DT-2 derivatives, namely the retro-inverso ri-(d)-DT-2 and the all (d)-amino acid analog, (d)-DT-2. Both peptide analogs were potent PKG Iα inhibitors with Ki values of 5.5 nM (ri-(d)-DT-2) and 0.8 nM ((d)-DT-2) as determined using a hyperbolic mixed-type inhibition model. Also, both analogs were proteolytically stable in vivo, showed elevated selectivity, and displayed enhanced membrane translocation properties. Studies on isolated arteries from the resistance vasculature demonstrated that intraluminally perfused (d)-DT-2 significantly inhibited vasodilation induced by 8-Br-cGMP. Furthermore, in vivo application of (d)-DT-2 established a uniform translocation pattern in the resistance vasculature, with exception of the brain. Thus, (d)-DT-2 caused significant increases in mean arterial blood pressure in unrestrained, awake mice. Further, mesenteric arteries isolated from (d)-DT-2 treated animals showed a markedly reduced dilator response to 8-Br-cGMP in vitro. Our results clearly demonstrate that (d)-DT-2 is a superior inhibitor of PKG Iα and its application in vivo leads to sustained inhibition of PKG in vascular smooth muscle cells. The discovery of (d)-DT-2 may help our understanding of how blood vessels constrict and dilate and may also aid the development of new strategies and therapeutic agents targeted to the prevention and treatment of vascular disorders such as hypertension, stroke and coronary artery disease.  相似文献

Chronopathology of cardiovascular disease is now well documented. Silent myocardial ischaemia involves the same pathophysiological changes as conventional ischaemia. Early morning peaks in angina and myocardial ischaemia call for adequate timing of medication. β-blockers abolish the morning peak, and aspirin reduces morning infarctions. Abstract: A successfully developed enzyme-based lactate microsensor with rapid response time allows the direct and continuous in vivo measurement of lactic acid concentration with high temporal resolution in brain extracellular fluid. The fluctuations coupled to neuronal activity in extracellular lactate concentration were explored in the dentate gyrus of the hippocampus of the rat brain after electrical stimulation of the perforant pathway. Extracellular glucose and oxygen levels were also detected simultaneously by coimplantation of a fast-response glucose sensor and an oxygen electrode, to provide novel information of trafficking of energy substances in real time related to local neuronal activity. The results first give a comprehensive picture of complementary energy supply and use of lactate and glucose in the intact brain tissue. In response to acute neuronal activation, the brain tissue shifts immediately to significant energy supply by lactate. A local temporary fuel "reservoir" is established behind the blood-brain barrier, evidenced by increased extracellular lactate concentration. The pool can be depleted rapidly, up to 28% in 10–12 s, by massive, acute neuronal use after stimulation and can be replenished in ∼20 s. Glutamate-stimulated astrocytic glycolysis and the increase of regional blood flow may regulate the lactate concentration of the pool in different time scales to maintain local energy homeostasis.  相似文献

Pseudopheochromocytoma is a poorly understood, rare cause of severe paroxysmal hypertension that mimics the symptomatology of pheochromocytoma in the absence of biochemical evidence of this tumor. Symptoms such as headache, nausea, sweating, and palpitations during hypertensive episodes have been described. In this paper, we describe previously unreported findings of lateralizing sensorimotor deficits in a patient with pseudopheochromocytoma. These changes presented during a hypertensive episode and were concerning for stroke but were not accompanied by acute radiologic abnormalities. The deficits improved over 1.5 weeks as blood pressure stabilized with beta-blockade. We also review relevant literature on the clinical features, pathophysiology, and management of pseudopheochromocytoma.  相似文献
