摘要 目的:探讨肝动脉灌注化疗栓塞(TACE)联合射频消融(RFA)对中晚期肝癌患者生存率、肝功能和T淋巴细胞亚群的影响。方法:选取2014年7月~2018年4月期间我院收治的中晚期肝癌患者126例,根据随机数字表法将患者分为对照组和研究组,各63例。对照组给予TACE治疗,研究组则在对照组的基础上联合RFA治疗,比较两组患者疗效、生存率、肝功能、T淋巴细胞亚群及不良反应情况。结果:研究组治疗后临床总有效率为69.84%(44/63),显著高于对照组患者的50.79%(32/63)(P<0.05)。两组患者治疗后丙氨酸氨基转移酶(ALT)、总胆红素(TBIL)、天门冬氨酸氨基转移酶(AST)均较治疗前降低,且研究组低于对照组(P<0.05)。两组治疗后CD4+、CD4+/CD8+较治疗前降低,但研究组高于对照组(P<0.05);CD8+较治疗前升高,但研究组低于对照组(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。研究组患者1年、2年生存率均高于对照组(P<0.05)。结论:TACE联合RFA治疗中晚期肝癌患者,安全性较好,疗效较好,可有效改善患者肝功能,减轻机体免疫抑制,提高患者生存率。  相似文献

A 63-year-old lady with a high-grade atrioventricular (AV) block and a structurally normal heart underwent permanent pacemaker implantation (dual chamber, Medtronic Ltd) 8 years back. On follow up, she had a recurrence of syncope after 3 years. The device interrogation at that time had revealed ventricular tachycardia (VT) for which she underwent implantable cardioverter defibrillator (ICD, Medtronic Ltd, Egida DR, DF1) upgradation at another center (electrograms not available). Now, she presents with episodes of presyncope after another 5 years. The Echocardiography was unremarkable. The ICD was interrogated & there was a stored ventricular fibrillation (VF) episode. But the electrograms suggested noise over a true VF electrogram noted in both near and far-field. In all probability, the VF was not a true one which might have arisen from some lead noise or from an electromagnetic interference (EMI). Fluoroscopy revealed an evident lead fracture near the superior vena cava (SVC) coil. The stored electrogram (EGM) characteristics also suggested possible lead noise rather than a true VF. She was advised for lead revision. Interestingly, all pacing parameters were normal along with normal impedance despite the evident lead fracture. This happened due to the ICD lead arrangements as there are separate electrodes for the SVC/RV coil and pacing. While the SVC coil was damaged, the pacing electrodes remained unaffected. Since the patient has no episode of true VT/NSVT and the echocardiography was normal, she was managed temporarily by changing the pacemaker to asynchronous (DOO) mode.  相似文献

31 years lady with complete atrioventricular canal defect, large primum atrial septal defect (ASD), inlet ventricular septal defect (VSD) and Eisenmenger syndrome, presented with atrial flutter and complete heart block. She was not suitable for corrective cardiac surgery and not yet indicated for heart-lung transplantation. She was advised single chamber permanent pacemaker and eventually Micra VR transcatheter leadless pacemaker was finalised for her. Transcatheter leadless pacemaker was deployed in her RV septum despite some unforeseen technical problems. This patient had intrahepatic interruption of IVC with Azygous continuation draining into SVC but this altered venovascular course was detected only fluoroscopically midway during the pacemaker implantation procedure and this was not detected in the preprocedural transthoracic echocardiography. This abnormal venous course was clearly demonstrated in the cardiac CT which was performed only after completion of the pacemaker implantation procedure in this patient. The technical challenges encountered mainly were mostly during the manipulation of the 27F delivery catheter of Micra through this altered cardiovascular anatomy via transfemoral approach and also due to the presence of septal defects. Thus, transcatheter leadless permanent pacemaker was implanted successfully through transfemoral access in this complex congenital heart disease with interrupted IVC and azygous continuation. Besides transthoracic echocardiography, it may be better to perform transesophageal echocardiography or even preferably radiological imaging like cardiac CT or MRI prior to transcatheter leadless pacemaker implantation in patients with complex congenital heart disease to understand the cardiovascular anatomy and plan the procedure.  相似文献

Anti-tachycardia pacing (ATP) is frequently used to terminate ventricular tachycardia (VT), however it is not always successful and may accelerate VT requiring defibrillation. REVRAMP is a novel concept of ATP that involves delivering pacing at a faster rate than VT, but instead of abruptly terminating pacing after eight beats, pacing is gradually slowed until VT continues or normal rhythm is restored. In a pilot study we show that REVRAMP can restore normal rhythm, and that if REVRAMP is unsuccessful, VT is not accelerated.  相似文献

Parahisian pacing (PHP) is a useful maneuver during electrophysiology study of supraventricular tachycardia (SVT) especially when the tachycardia is non-sustained. Various responses during PHP can differentiate between the routes of VA conduction (VAC). In a case of WPW syndrome with orthodromic re-entrant tachycardia, we encountered various responses which one must be cognizant about to avoid erroneous conclusions. Along with para-hisian capture and only ventricular capture, simultaneous atrial capture (SAC) and pure His capture were also noted. Moreover, during pure-His capture underlying distal antegrade right bundle branch block (RBBB) was encountered making it an intriguing case.  相似文献

Ablation using non-irrigated catheter can lead to rapid rise in temperature of electrode tissue interface from tissue desiccation, steam and coagulum formation and this may limit power delivery and lesion size. We present a novel technique of external irrigation through the long sheath while using a non-irrigated ablation catheter during AV Node ablation and conduction system pacing. By bringing the long sheath closer to the tip of non-irrigated catheter and injecting cold normal saline allows cooling of electrode tissue interface leading to increase in power delivery resulting in a deeper lesion.  相似文献

The genesis of cardiac resynchronisation therapy (CRT) consists of ‘bedside’ research and ‘bench’ studies that are performed in series with each other. In this field, the bench studies are crucial for understanding the pathophysiology of dyssynchrony and resynchronisation. In a way, CRT started with the insight that abnormal ventricular conduction, as caused by right ventricular pacing, has adverse effects. Out of this research came the ground-breaking insight that ‘simple’ disturbances in impulse conduction, which were initially considered innocent, proved to result in a host of molecular and cellular derangements that lead to a vicious circle of remodelling processes that facilitate the development of heart failure. As a consequence, CRT does not only correct conduction abnormalities, but also improves myocardial properties at many levels. Interestingly, corrections by CRT do not exactly reverse the derangements, induced by dyssynchrony, but also activate novel pathways, a property that may open new avenues for the treatment of heart failure.  相似文献
