目的:分析椎弓根入路行椎体后凸成形术(PKP)治疗骨质疏松性椎体压缩骨折的预后评价及继发危险因素分析。方法:选择2016年2月-2018年2月我院收治的骨质疏松性椎体压缩骨折患者85例纳入本次研究,采用随机数表法分为观察组(n=43)和对照组(n=42)。对照组使用经皮椎体成形术进行治疗,观察组采用PKP进行治疗。比较两组患者手术情况、术后情况、椎体前缘高度丢失率、Cobb角、继发性骨折发生情况及分析骨质疏松性椎体压缩骨折患者术后继发骨折的危险因素。结果:观察组手术时间、透视次数、骨水泥注入量、术中出血量均显著低于对照组,差异显著(P0.05);观察组疼痛缓解时间、下地时间及住院时间均显著低于对照组,差异显著(P0.05);治疗前,两组椎体前缘高度丢失率、Cobb角比较,无显著差异;治疗后,两组患者的椎体高度丢失率明显下降,但两组术后7 d、术后6月两组椎体前缘高度丢失率、Cobb角比较无显著差异;观察组术后12月椎体前缘高度丢失率、Cobb角低于对照组,差异显著(P0.05);所有患者均随访12月,其中22例(25.88%)发生继发性椎体骨折,进行单因素分析,结果发现,两组患者性别、骨折部位、局部矢状面后凸角度、骨水泥量、椎体高度恢复、术后抗骨质疏松治疗差异无统计学意义(P0.05);骨质疏松原因、骨水泥椎间隙渗漏、术后支具佩戴、原发骨折类型与骨质疏松性椎体压缩骨折患者术后发生继发骨折相关(P0.05)。多因素Logistic分析显示,骨质疏松原因、骨水泥椎间隙渗漏、术后支具佩戴、原发骨折类型均是骨质疏松性椎体压缩骨折患者术后发生继发骨折的独立危险因素(P0.05)。结论:在骨质疏松性椎体压缩骨折患者中应用PKP可有效改善手术情况,随着时间的延长,PKP更有利于维持患者椎体高度;骨质疏松原因、骨水泥椎间隙渗漏、术后支具佩戴、原发骨折类型是骨质疏松性椎体压缩骨折患者术后发生继发骨折的危险因素,临床上对于具有危险因素的患者引起重视,并采取干预措施。  相似文献

目的:探讨单侧与双侧穿刺入路经皮椎体后凸成形术(PKP)对骨质疏松性胸腰椎压缩骨折(OVCF)患者手术效果、生活质量以及血清应激因子的影响。方法:回顾性选取2016年7月~2019年7月期间我院接收的行PKP的OVCF患者83例。根据入路方式的不同分为A组(n=41,单侧穿刺入路)和B组(n=42,双侧穿刺入路),对比两组患者围术期指标、影像学指标、视觉模拟评分法(VAS)评分、Oswestry腰椎功能障碍指数(ODI)评分、健康调查生活质量量表(SF-36)评分、血清应激因子以及并发症发生情况。结果:A组手术时间短于B组,骨水泥用量、术中透视次数少于B组(P0.05)。两组术后6个月VAS评分、ODI评分降低,SF-36评分升高(P0.05)。两组术后3d去甲肾上腺素(NE)、肾上腺素(E)、皮质醇(Cor)均升高,但A组低于B组(P0.05)。两组术后6个月椎体前缘高度、后凸Cobb角均升高(P0.05)。两组并发症发生率比较差异无统计学意义(P0.05)。结论:单侧穿刺入路PKP可获得与双侧穿刺入路PKP相当的治疗效果和安全性,同时可缩短手术时间,减少骨水泥用量及术中透视次数,减轻机体应激反应。  相似文献

目的:探讨交锁髓内钉(IIN)与锁定加压钢板(LCP)两种内固定方法对股骨干骨折(FSF)患者膝关节功能及创伤相关指标的影响。方法:回顾性分析2015年1月-2018年3月我院收治的97例FSF患者的临床资料,依照内固定方式的不同分为IIN组(n=54)和LCP组(n=43),比较两组患者的术后骨折愈合情况,围术期指标、创伤相关指标、膝关节功能恢复及并发症情况。结果:术后IIN组骨折愈合优良率为92.59%,高于LCP组的76.74%(P0.05),IIN组患者手术时间、住院时间、术中出血量、术后引流量、骨折愈合时间少于LCP组(P0.05);IIN组浅表感染率、内固定物断裂率、骨折延迟愈合率、不愈合率及血清C反应蛋白(CRP)、去甲肾上腺素(NE)、肾上腺素(E)、超氧化物歧化酶(SOD)水平低于LCP组(P0.05);IIN组患者Fugl-Meyer下肢运动评分、美国膝关节协会(KSS)评分高于LCP组(P0.05)。结论:相较于LCP内固定,IIN内固定对FSF患者的创伤较轻,术后并发症较少,可更好地促进骨折愈合及膝关节功能恢复。  相似文献

Background and Aims: The branch-base xylem structure of the endangered Wollemia nobiliswas anatomically investigated. Wollemia nobilis is probablythe only extant tree species that produces only first-orderbranches and where all branches are cleanly abscised. An investigationwas carried out to see if these unusual features might influencebranch-base xylem structure and water supply to the foliage. Methods: The xylem was sectioned at various distances along the branchbases of 6-year-old saplings. Huber values and relative theoreticalhydraulic conductivities were calculated for various regionsof the branch base. Key Results: The most proximal branch base featured a pronounced xylem constriction.The constriction had only 14–31 % (average 21 %)of the cross-sectional area and 20–42 % (average28 %) of the theoretical hydraulic conductivity of themore distal branch xylem. Wollemia nobilis had extremely lowHuber values for a conifer. Conclusions: The branch-base xylem constriction would appear to facilitatebranch abscission, while the associated Huber values show thatW. nobilis supplies a relatively large leaf area through a relativelysmall diameter ‘pipe’. It is tempting to suggestthat the pronounced decline of W. nobilis in the Tertiary isrelated to its unusual branch-base structure but physiologicalstudies of whole plant conductance are still needed.  相似文献

BACKGROUND AND AIMS: In response to inclination stimuli, gymnosperm trees undergo corrective growth during which compression wood develops on the lower side of the inclined stem. High compressive growth stress is generated in the compression wood region and is an important factor in righting the stem. The aims of the study were to elucidate how the generation of compressive growth stress in the compression wood region is involved in the righting response and thus to determine a righting mechanism for tree saplings. METHODS: Cryptomeria japonica saplings were grown at inclinations of 0 degrees (vertical) to 50 degrees. At each inclination angle, the growth stress on the lower side of the inclined stem was investigated, together with the degree of compression-wood development such as the width of the current growth layer and lignin content, and the upward bending moment. KEY RESULTS: Growth stress, the degree of compression wood development, and the upward moment grew as the stem inclination angle increased from 0 to 30 degrees, but did not rise further at inclinations > 30 degrees. CONCLUSIONS: The results suggest the following righting mechanism for gymnosperm saplings. As the stem inclination is elevated from 0 to 30 degrees, the degree of compression wood development increases to force the sapling back to its original orientation; at inclinations > 30 degrees, the maximum degree of compression wood is formed and additional time is needed for the stem to reorient itself.  相似文献

Idahoa scapigera produces solitary flowers in the axils of rosette leaves without elongation of the shoot axis, a rosette-flowering architecture. Previous work with one of the two I. scapigera LFY paralogs, IscLFY1, showed that this gene caused aerial flowering rosettes in Arabidopsis thaliana. In this paper, we report that after three generations IscLFY1 transgenic lines are phenotypically indistinguishable from wild-type Arabidopsis, indicating that IscLFY1 protein is able to replace normal LFY function. Additionally, we found that ectopic LFY expression late in development can phenocopy aspects of the aerial rosette phenotype, suggesting that shoot compression caused by IscLFY1 could be caused by localized overexpression of a functional IscLFY protein. We also characterized the expression and function of the second I. scapigera LFY paralog, IscLFY2, in A. thaliana. In contrast to IscLFY1, this paralog was expressed in floral meristems and the shoot apical meristem (SAM). In I. scapigera, LFY-specific antibodies detected high protein levels in developing flowers but not in the apex, suggesting trans-regulatory differences between I. scapigera and A. thaliana. Most IscLFY2 transgenic A. thaliana plants were indistinguishable from wild type, but in a minority of lines the SAM was converted to a terminal flower as would be expected from the reporter-expression pattern. Taken together these results show that both I. scapigera paralogs have conserved LFY function, both proteins can rescue lfy and both can modify inflorescence architecture in an A. thaliana background: either by affecting internode elongation (IscLFY1) or by causing homeotic conversion of shoots into flowers (IscLFY2).  相似文献

With larger, higher speed detectors and improved automation, individual CryoEM instruments are capable of producing a prodigious amount of data each day, which must then be stored, processed and archived. While it has become routine to use lossless compression on raw counting-mode movies, the averages which result after correcting these movies no longer compress well. These averages could be considered sufficient for long term archival, yet they are conventionally stored with 32 bits of precision, despite high noise levels. Derived images are similarly stored with excess precision, providing an opportunity to decrease project sizes and improve processing speed. We present a simple argument based on propagation of uncertainty for safe bit truncation of flat-fielded images combined with lossless compression. The same method can be used for most derived images throughout the processing pipeline. We test the proposed strategy on two standard, data-limited CryoEM data sets, demonstrating that these limits are safe for real-world use. We find that 5 bits of precision is sufficient for virtually any raw CryoEM data and that 8–12 bits is sufficient for intermediate averages or final 3-D structures. Additionally, we detail and recommend specific rules for discretization of data as well as a practical compressed data representation that is tuned to the specific needs of CryoEM.  相似文献

Skeletal muscles are embedded in an environment of other muscles, connective tissue, and bones, which may transfer transversal forces to the muscle tissue, thereby compressing it. In a recent study we demonstrated that transversal loading of a muscle with 1.3 N cm−2 reduces maximum isometric force (Fim) and rate of force development by approximately 5% and 25%, respectively. Clavicle injuries were frequently observed in automotive side and frontal crashes. Finite element (FE) models have been developed to understand the injury mechanism, although no clavicle loading response corridors yet exist in the literature to ensure the model response biofidelity. Moreover, the typically developed structural level (e.g., force–deflection) response corridors were shown to be insufficient for verifying the injury prediction capacity of FE model, which usually is based on strain related injury criteria. Therefore, the purpose of this study is to develop both the structural (force vs deflection) and material level (strain vs force) clavicle response corridors for validating FE models for injury risk modeling. 20 Clavicles were loaded to failure under loading conditions representative of side and frontal crashes respectively, half of which in axial compression, and the other half in three point bending. Both structural and material response corridors were developed for each loading condition. FE model that can accurately predict structural response and strain level provides a more useful tool in injury risk modeling and prediction. The corridor development method in this study could also be extended to develop corridors for other components of the human body.  相似文献
