This study was designed to evaluate the potential of flow cytometry to measure biomarkers of airways inflammation in the peripheral blood of two cohorts of workers reporting work related respiratory symptoms, who were exposed to different respiratory hazards. Nine bakers exposed to wheat flour and 10 glass bottle manufacturers exposed to a range of irritant chemicals were selected for study. Phenotypic and inducible cell surface markers were measured by flow cytometry. Results were compared with a control population of 58 volunteers reporting no respiratory problems. The bakers showed a significant increase above control values for cell types associated with inflammation; in particular CD3 CD4 cells p 0.005 and CD4 CD25 cells p 0.01 . In contrast, the workers reporting work related respiratory symptoms who were exposed to a range of irritant chemicals showed a different pattern of cell surface lymphocyte markers, with a significant decrease in the total T cell population p 0.05 . Comparison of results from a subset of smoking controls with the population of bakers who were all heavy smokers confirmed that the increase in CD3 CD4 cells and CD4 CD25 cells could not be ascribed to the effects of smoking alone. We have shown activation of helper T cells in the peripheral blood of bakers reporting work related respiratory symptoms consistent with the changes observed in mild to severe asthmatics. However, workers with similar symptoms who were exposed to irritant chemicals did not show this pattern of phenotypic or inducible cell surface markers, reflecting an absence of airways inflammation in these individuals. Our results suggest that flow cytometry may be of use as an objective test for detecting workers with airways inflammation to allow the identification of workers at risk of developing occupational asthma.  相似文献

Subjects occupationally exposed to potential mutagens/carcinogens represent the most suitable groups for epidemiological studies aimed at assessing the risk for the individual or the offspring. Several cancer risks to humans have been detected by epidemiological studies performed in occupational settings. Cancer epidemiology studies have been able (a) to identify specific occupations or agents associated with the risk; (b) to verify the results of experimental studies; (c) to test the effectiveness of changes in production or preventive measures in decreasing risks. Reproductive epidemiology has suggested a risk of spontaneous abortions or of malformation in the offspring of workers exposed to some chemicals or occupations, but data are often conflicting due to methodological problems. With the aim of early assessment of risk in mind, the epidemiological use of indicators of exposure or of the early effect of exposure to genotoxic agents is increasingly applied to occupational groups. Cytological monitoring of subjects at risk of occupational cancer of lung or bladder is carried out mainly to diagnose precancerous lesions of target tissues. Cytogenetic methods (chromosome aberrations, SCE, micronuclei) in somatic cells provide a means for detecting early effects of occupational exposure to known or potential mutagens/carcinogens in selected groups of individuals, but their significance is widely debated. Monitoring of urinary mutagenicity, as applied in nurses handling cytostatic drugs, is an example of how an indicator of exposure to genotoxins can be used to evaluate the impact of preventive measures. Among the perspectives, biochemical epidemiology seems to be promising in detecting individuals genetically susceptible to cancer.  相似文献

Occupational exposure to anaesthetic gases is one of the major hazards to healthcare personnel. We evaluated the cytogenetic effects of chronic exposure to low concentrations of anaesthetic gases in operating theatres. The study included 21 anesthetists and 21 control subjects who matched in age and gender. Chromosome aberrations (CAs) and sister chromatid exchanges (SCEs) assays were performed. All subjects were also genotyped for glutathione S‐transferase T1 (GSTT1) gene polymorphisms. Significant differences were found between exposed and controls in terms of SCEs frequency (P = 0.001) and replication index value (P = 0.005), but not in terms of CAs (P = 0.201) and aberrant cells (P = 0.227) frequencies. Regression analyses indicated that age and the years of employment did not influence the level of chromosomal damage in both groups. Finally, among anesthetists, GSTT1 null individuals showed a significant higher frequency of SCE with respect to GSTT1‐positive subjects.  相似文献

Studies were carried out on two populations occupationally exposed to ethylene oxide (EtO) using different physical and biological parameters. Blood samples were collected from 9 hospital workers (EI) and 15 factory workers (EII) engaged in sterilization of medical equipment with EtO and from matched controls (CI and CII). Average exposure levels during 4 months (the lifespan of erythrocytes) prior to blood sampling were estimated from levels of N-(2-hydroxyethyl)valine adducts in hemoglobin. They were significantly enhanced in EI and EII and corresponded to a 40-h time-weighted average of 0.025 ppm in EI and 5 ppm in EII. Exposures were usually received in bursts with EtO concentrations in air ranging from 22 to 72 ppm in EI and 14 to 400 ppm in EII. All samples were analyzed for HPRT mutants (MFs), chromosomal aberrations (CAs), micronuclei (MN) and SCEs. MFs were significantly enhanced by 60% in EII but not in EI. These results are the first demonstration of mutation induction in man by ethylene oxide. CAs were significantly enhanced in EI and EII by 130% and 260% respectively. MN were not enhanced in EI but significantly in EII(217%). The mean frequency of SCEs was significantly elevated by 20% in EI and by almost 100% in EII. SCE was the only parameter that allowed distinction between daily and occasionally exposed workers in EII. An interesting finding in exposed workers was the large increase of the percentage of cells with high frequencies of SCE (3–4 times in EI and 17-fold in EII).

The relative sensitivity of endpoints for detection of EtO exposure in the present investigation was in the following order: HOEtVal adducts > SCEs > chromosomal aberrations > micronuclei > HPRT mutants.  相似文献

Agriculture is one of the most hazardous sectors in terms of fatal and non-fatal accidents. This paper utilises an administrative dataset that recorded farm fatalities in Northern Ireland over a 50 year timeframe (1968–2017) to undertake an age-period analysis of accident related mortality rates by sex, cause of death, season, and day of the week. Public policies aimed to improve farm safety should consider that fatalities due to animals have increased while the incidence of deaths due to vehicles and equipment has substantially decreased over the years although it is still the primary cause of death. With respect to age, elderly still actively involved in farming and children in the spring and at week-ends are most exposed to the risk of a fatal accident. Overall, men die on farms five times more than women.  相似文献

Values of maximal isometric strength of five muscle groups and associated factors including occupational status, life style and health were studied in three groups of men aged from 31 to 35, 51 to 55, and 71 to 75 years. The results indicated significant differences between the age groups in isometric handgrip, elbow flexion, knee extension, trunk extension and trunk flexion strength. In the youngest group, the manual workers tended to have higher strength values in all muscle groups than the lower and higher status white collar workers; whereas among the middle-aged and oldest men the manual workers tended to have the poorest performance. Good self-rated health and the intensity of physical exercise during leisure were positively associated with muscle strength in the youngest and middle-aged groups whereas in the oldest group the most important variable was home gymnastics. The multivariate structural equation models of isometric strength differed somewhat among the age groups and in these models the above-mentioned variables accounted for from 33% to 43% of the variance in isometric strength within the age groups. The results illustrated the most important factors associated with isometric strength in samples of men of different ages and also provided suggestions as to how these determinants might influence comparisons between different age groups in respect of muscle strength.  相似文献

Medical exposure of the general population due to radiological investigations is the foremost source of all artificial ionising radiation. Here, we focus on a particular diagnostic radiological procedure, as only limited data are published with regard to radiation measurements during urethrograpic imaging. Specifically, this work seeks to estimate patient and occupational effective doses during urethrographic procedures at three radiology hospitals. Both staff and patient X-ray exposure levels were calculated in terms of entrance surface air kerma (ESAK), obtained by means of lithium fluoride thermoluminescent dosimeters (TLD-100(LiF:Mg:Cu.P)) for 243 urethrographic examinations. Patient radiation effective doses per procedure were estimated using conversion factors obtained from the use of Public Health England computer software. In units of mGy, the median and range of ESAK per examination were found to be 10.8 (3.6–26.2), 7.0 (0.2–32.3), and 24.3 (9.0–32.0) in Hospitals A, B, and C, respectively. The overall mean and range of staff doses (in µGy) were found to be 310 (4.0–1750) per procedure. With the exception of hospital C, the present evaluations of radiation dose have been found to be similar to those of previously published research. The wide range of patient and staff doses illustrate the need for radiation dose optimisation.  相似文献

This study examined the impact of rest breaks on temporal trends in industrial accident risks in an attempt to replicate earlier findings of a linear increase in risk as a function of elapsed time on task. In two separate studies, the trend in work‐related injuries were studied in relation to the timing of rest breaks. In study one, comparisons were made between on‐ and off‐track workers on weekly rotating three‐shift systems operating in a large engineering company. Records of on‐duty injuries that occurred over 12 months were examined (N=4645 incidents). Study two involved interviewing patients who had suffered work‐related hand injuries in a variety of occupational settings (N=407 patients). Hierarchical log linear analysis was used in both studies. In study one, risk increased from the first to the second half‐hour of continuous work following a break, but then remained relatively constant in subsequent half‐hour periods, although there was a fall in the third half‐hour for on‐track workers. In some of the data, there was also a decrease in risk in the period leading up to the end of a work period. There was a sharp decline in reported injuries toward the very end of a shift, but otherwise the observed trends did not differ between successive periods of continuous work or between morning, afternoon, and night shifts. In study two, risk increased from the first to the second half‐hour of continuous work and then remained relatively constant in the third half‐hour. The contrast between the current and previous findings may be due to the relatively unique work environment of the previous study. It is suggested that the current trends reflect the effects of working in a relatively unconstrained task environment, and that causes other than fatigue may underlie the trends observed in both the previous and current studies.  相似文献

Although commercial production of polychlorinated biphenyls (PCBs) was banned in 1979, PCBs continue to be an environmental and health concern due to their high bioaccumulation and slow degradation rates. In fact, PCBs are still present in our food supply (fish, meat, and dairy products). In laboratory animals, exposure to single PCB congener or to mixtures of different congeners induces a variety of physiological alterations. PCBs cross the placenta and even exposure at low level is harmful for the foetus by leading to neurodevelopment alterations. Serotonin system which regulates many physiological functions from platelet activation to high cerebral processes and neurodevelopment is one of the targets of PCBs toxicity. The effects of PCBs exposure on serotonin system have been investigated although to a lesser extent compared to its effect in other neurotransmitter systems. This review provides a summary of the results concerning the impact of PCBs exposure (in vitro and in vivo) on serotonin system. Further research is needed to correlate specific deficits with PCB-induced changes in the serotonin system.  相似文献
