Recent studies have improved our understanding of the physiological function of Notch signaling pathway and now there is compelling evidence demonstrating that Notch is a key regulator of embryonic development and tissue homeostasis. Although further extensive studies are necessary to illustrate the molecular mechanisms, new insights into the role of Notch signaling in pancreas development and diabetes have been achieved. Importantly, the ability to regulate Notch signaling intensity both positively and negatively may have therapeutic relevance for diabetes. Thus, this paper reviews the current knowledge of the roles of Notch signaling in the pancreatic endocrine cell system. 相似文献

Three principally different sites of action have been reported for proinsulin C-peptide, at surface-mediated, intracellular, and extracellular locations. Following up on the latter, we now find that (i) mass spectrometric analyses reveal the presence of the C-peptide monomer in apparent equilibrium with a low-yield set of oligomers in weakly acidic or basic aqueous solutions, even at low peptide concentrations (sub-μM). It further shows not only C-peptide to interact with insulin oligomers (known before), but also the other way around. (ii) Polyacrylamide gel electrophoresis of C-peptide shows detectable oligomers upon Western blotting. Formation of thioflavin T positive material was also detected. (iii) Cleavage patterns of analogues are compatible with C-peptide as a substrate of insulin degrading enzyme. Combined, the results demonstrate three links with insulin properties, in a manner reminiscent of amyloidogenic peptides and their chaperons in other systems. If so, peripheral C-peptide/insulin interactions, absolute amounts of both peptides and their ratios may be relevant to consider in diabetic and associated diseases. 相似文献

目的:比较一次法龈下刮治和根面平整术(FM-SRP)和常规的四分法龈下刮治和根面平整术(Q-SRP)治疗2型糖尿病伴慢性牙周炎的临床疗效,并观察慢性牙周炎治疗对2型糖尿病患者血糖控制的影响。方法:将48例2型糖尿病伴慢性牙周炎患者随机分为2组:FM-SRP组在l天内完成全口所有象限的刮治和根面平整,Q-SRP组每周进行1个象限的刮治,连续4周完成全口治疗。于治疗前、治疗3个月和6个月时,检测菌斑指数(PLI)、牙周探诊深度(PD)、临床附着水平(CAL)、探诊出血(BOP)及空腹血糖(FPG)与糖化血红蛋白(HbAlc)的变化。结果:有3例患者被剔除。与治疗前相比,2种治疗方式在3个月和6个月时的PLI、PD、CAL和BOP均有显著改善(P<0.05),FPG和HbAlc无显著性改变(P>0.05)。但各项指标的变化在2组间均无显著性差异(P>0.05)。结论:本组资料,FM-SRP和Q-SRP两种方法治疗2型糖尿病伴慢性牙周炎均可达到相同的临床效果,但对2型糖尿病患者血糖控制无明显影响。 相似文献

目的:探讨交感神经阻滞对有糖尿病、心腔扩张的顽固心衰患者心脏收缩功能和心腔大小的影响。方法:经胸3~胸4棘突间隙穿刺至硬膜外腔,留置硬膜外导管,0.5%利多卡因3~5ml每2h推注一次,持续4w,适当辅以常规治疗。测定并比较TEB治疗前及后4w射血分数(EF)、左室内径(LVED)和左房内径(LAED)等指标的变化。结果:治疗后左室射血分数(EF)由治疗前的(31.53±9.29)%升为(44.75±8.32)%(P0.05);左室内径(LVED)由治疗前的(70.59±6.25)mm缩减到(63.59±7.05)mm(P0.05);左房内径(LAED)由治疗前的(46.16±7.19)mm缩减到(39.05±7.23)mm(P0.05)。结论:心区交感神经阻滞结合常规用药对有糖尿病、心腔扩张的顽固心衰患者的心脏功能有明显改善作用,并使其扩大的心脏明显缩小。 相似文献

目的探讨糖尿病下肢溃疡的原因和治疗方法。方法回顾总结我院近年来急诊科临床收治的糖尿病并发下肢溃疡患者12例,全部病例均使用胰岛素控制血糖、抗生素抗感染、静脉滴注活血化瘀的中药、常规局部换药并结合使用具有去腐、生肌、消炎作用的中药膏剂进行治疗,同时对患者进行糖尿病相关知识的教育和自我护理技能培训。结果全部病人均治愈出院,其中截趾术后伤口愈合出院1例,点状邮票植皮后伤口愈合1例,经换药伤口愈合10例。随访期间全部病例溃疡未复发。结论糖尿病下肢溃疡是由血管、神经、感染等多重因素造成的。对糖尿病并发下肢溃疡患者采用多种方法综合治疗可以取得较好的疗效,同时强调重视糖尿病的管理和教育。 相似文献

The present study was designed to explore the antioxidative status and circulating biochemical markers having a potential role in the pathogenesis of ethambutol (EMB) induced toxic optic neuropathy (TON) among diabetic and non-diabetic patients.Fifty patients under complete therapy of EMB for tuberculosis were included in the present study. Inclusion criteria for patients were to receive EMB everyday during treatment, a dose of 25 mg/kg for initial 2 months and 15 mg/kg during the rest of therapy period. We conducted color vision and visual acuity test for all patients.Fifteen out of fifty EMB induced TON patients, were found to be diabetic. Color vision and visual acuity test results were evaluated for diabetic and non-diabetic as well as twenty age matched controls. The results demonstrated a significant pattern of circulating biochemical markers between the studied groups. Data regarding hematological (RBC, p value = 0.02; Hemoglobin, p value = 0.02), hepatic (total bilirubin, p value = 0.01), renal (urea, p value = 0.03; creatinine, p value = 0.007), lipid (total cholesterol, p value = 0.01; total triglycerides, p value = 0.03) and antioxidative (superoxide dismutase, p value = 0.005; glutathione, p value = 0.02; catalase, p value = 0.02) profile showed a highly significant difference among the studied groups specially patients with diabetes. Malondialdehyde (MDA) level had gone significantly up in diabetic TON patients (p value = 0.02), in comparison to other antioxidants and vitamins (Vit). Vit-A, E, B1, B12 and Zinc seem to be playing a major role in the pathogenesis of TON, specially Vit-E and B1 surpassed all the antioxidants as having highly significant inverse relationships with MDA (MDA vs Vit-E, r = −0.676** and MDA vs Vit-B1, r = −0.724** respectively).We conclude that during the ethambutol therapy the decreased levels of Vit-E and Vit-B1 possibly play a role in the development of TON and may be used as therapeutic agents to lessen the deleterious effects of ethambutol. 相似文献

目的:探讨疏肝益阳胶囊联合万艾可对糖尿病男性性功能障碍患者血清生殖激素睾酮(T)、雌二醇(E2)水平和性功能的影响。方法:选择2015年7月到2016年7月我院接诊的94例糖尿病男性性功能障碍患者,通过随机数表法分作观察组及对照组,每组47例。对照组使用万艾可进行治疗,观察组在其基础上加用疏肝益阳胶囊进行联合治疗。比较两组的临床疗效、治疗前后国际勃起功能指数-5(IIEF-5)问卷表测评评分、部分雄激素缺乏(PADAM)问卷评分、生殖激素睾酮(T)、雌二醇(E2)水平的变化。结果:治疗后,观察组的临床总有效率明显高于对照组(P0.05),IIEF-5评分显著高于对照组(P0.05),PADAM评分明显低于对照组(P0.05),血清T激素水平明显高于对照组(P0.05),血清E2激素水平明显低于对照组(P0.05),治疗组不良反应总发生率明显低于对照组(P0.05)。结论:疏肝益阳胶囊联合万艾可治疗糖尿病男性性功能障碍可有效提高临床疗效,改善患者性功能,可提高血清T水平,降低E2水平。 相似文献
