Dissemination of carbapenem resistance via Enterobacteriaceae, particularly among Klebsiella pneumoniae and Escherichia coli, is a major public health concern. Rapid methods for determining antimicrobial susceptibility are important to ensure adequate and appropriate use of antimicrobial agents and to limit the spread of these bacteria. In the current study, we compared the rapidity, sensitivity and specificity of traditional methods and molecular-based Xpert Carba-R PCR assay to identify sixty isolates, (26 E. coli and 34 K. pneumoniae). The specificity of MicroScan was 100% while sensitivity to ertapenem (ERT), imipenem (IMI), and meropenem (MER) was 93%, 68.9%, and 55.17%, respectively. For the modified Hodge test, the specificity was 96.77% and sensitivity was 89.65%. Although some results of phenotypic assays matched with the definite PCR identification, some results were misleading. Out of the 29 positive PCR samples, three samples of K. pneumoniae were negative for the MHT and one E. coli sample was MHT positive but negative for the PCR. Nine samples were positive for the PCR but were determined as carbapenem sensitive by MicroScan. While MicroScan and MHT requires several hours and multi-steps to obtain results, Xpert Carba-R PCR assay takes less than an hour. Therefore, we recommend using Gene xpert Carba-R assay for the optimal carbapenemnase detection with reducing material, manpower and cost. Also it is important to know the type of carbapenemase is present.  相似文献

Soil samples were collected from 7 sites in the up-, mid- and down-reach along and nearby the wastewater irrigation channel, western Shenyang of China. The concentrations of selected pollutants (mineral oil, PAHs - polycycle aromatic hydrocarbons and Cd) were determined by UV spectrometer, HPLC and AAS (atomic adsorption spectrometer) spectrometer, respectively. Toxicity effects of soils were evaluated by seedling emergence test with root length of wheat as the end-point and by earthworms test with the mortality rate and inhibition rates of body weight as endpoints. Results showed accumulation of pollutants for most soils with concentration of 200.2 mg·kg-1-1600 mg·kg-1 for mineral oil, 0.33 mg·kg-1-1.81 mg·kg-1 for Cd and 900.16 mg·kg-1 -2737.91 mg·kg-1 for PAHs. The inhibition rates of root elongation were from -20% up to 40 %, and mortality rates of earthworms ranged from 0%-40% from the exposure period of two weeks to eight weeks by sampling interval of two weeks, the inhibition rates of earthw  相似文献

As rigid endoscopes are re-used after minimal invasive surgery, they degrade over time. To guarantee the optical quality of a rigid endoscope, the ScopeControl has been developed to measure key optical parameters such as, light transmission (LT), color correctness (CC), focus (FC), fiber transmission (FT), viewing angle (VA) and field of view (FV). To evaluate the performance of the ScopeControl, five ScopeControls SV (study version) have been tested in six hospitals in the Netherlands. Aspects such as robustness, precision, usability of the measured data, acceptation criteria and ergonomic aspects have been assessed. The ScopeControl maintains its stability over time and can measure LT, CC and FC with 5% precision, VA and FV with 2% accuracy and FT with 10% precision. The final thresholds above which an endoscope could still be considered to be in good condition, appeared to be: VA at 75%, CC, FC and FV at 70%, LT at 65% and FT at 35% of that of the best endoscope of that type ever found. The ScopeControl SV fits the workflow of the sterilization department as it is easy to use and can easily be cleaned. The time to perform a measurement is 2–3 min (one minute is spent to select the correct endoscope). The ScopeControl PV (launched in November 2013), has been used clinically from April to November 2014. It proves to be more stable, well accepted by personnel from the sterilization department and be of large value of preventing defect endoscopes on the surgery table.  相似文献

One of the more promising processes for the energetic transformation of waste is the anaerobic digestion of the Organic Fraction of Municipal Solid Waste (OFMSW). An experimental campaign was carried out on three different samples of OFMSW from Waste Separation (WS), one as received and two obtained after mechanical treatment (squeezing): OFMSW slurry (liquid fraction) and OFMSW Waste (residual solid fraction). Anaerobic Biogasification Potential (ABP) and anaerobic digestion tests (AD) were carried out, investigating the effects of inoculum and pH. The OFMSW Waste was also examined to evaluate the possibility to dispose of it in a landfill. Results showed that OFMSW slurry must be diluted and inoculated and that pH control in the start up phase is essential, in order to have significant biogas productions. OFMSW as received did not show a significant biogas production, while OFMSW Waste showed suitable characteristics for landfill disposal, except for Dissolved Organic Carbon.  相似文献

Soil-transmitted helminth (STH) infections are common. Indeed, more than 1 billion people are affected, mainly in the developing world where poverty prevails and hygiene behavior, water supply, and sanitation are often deficient1,2. Ascaris lumbricoides, Trichuris trichiura, and the two hookworm species, Ancylostoma duodenale and Necator americanus, are the most prevalent STHs3. The estimated global burden due to hookworm disease, ascariasis, and trichuriasis is 22.1, 10.5, and 6.4 million disability-adjusted life years (DALYs), respectively4. Furthermore, an estimated 30-100 million people are infected with Strongyloides stercoralis, the most neglected STH species of global significance which arguably also causes a considerable public health impact5,6. Multiple-species infections (i.e., different STHs harbored in a single individual) are common, and infections have been linked to lowered productivity and thus economic outlook of developing countries1,3.For the diagnosis of common STHs, the World Health Organization (WHO) recommends the Kato-Katz technique7,8, which is a relatively straightforward method for determining the prevalence and intensity of such infections. It facilitates the detection of parasite eggs that infected subjects pass in their feces.With regard to the diagnosis of S.stercoralis, there is currently no simple and accurate tool available. The Baermann technique is the most widely employed method for its diagnosis. The principle behind the Baermann technique is that active S.stercoralis larvae migrate out of an illuminated fresh fecal sample as the larvae are phototactic9. It requires less sophisticated laboratory materials and is less time consuming than culture and immunological methods5.Morbidities associated with STH infections range from acute but common symptoms, such as abdominal pain, diarrhea, and pruritus, to chronic symptoms, such as anemia, under- and malnutrition, and cognitive impairment10. Since the symptoms are generally unspecific and subtle, they often go unnoticed, are considered a normal condition by affected individuals, or are treated as symptoms of other diseases that might be more common in a given setting. Hence, it is conceivable that the true burden of STH infections is underestimated by assessment tools relying on self-declared signs and symptoms as is usually the case in population-based surveys.In the late 1980s and early 1990s, Stephenson and colleagues highlighted the possibility of STH infections lowering the physical fitness of boys aged 6-12 years11,12. This line of scientific inquiry gained new momentum recently13,14,15. The 20-meter (m) shuttle run test was developed and validated by Léger et al.16 and is used worldwide to measure the aerobic fitness of children17. The test is easy to standardize and can be performed wherever a 20-m long and flat running course and an audio source are available, making its use attractive in resource-constrained settings13. To facilitate and standardize attempts at assessing whether STH infections have an effect on the physical fitness of school-aged children, we present methodologies that diagnose STH infections or measure physical fitness that are simple to execute and yet, provide accurate and reproducible outcomes. This will help to generate new evidence regarding the health impact of STH infections.  相似文献

Toxicity testing for regulatory purposes raises the question of test selection for a particular endpoint. Given the public's concern for animal welfare, test selection is a multi-objective decision problem that requires balancing information outcome, animal welfare loss, and monetary testing costs. This paper demonstrates the applicability of cost-effectiveness analysis as a decision-support tool for test selection in a regulatory context such as, for example, the new European chemicals legislation (REACH). We distinguish different decision-making perspectives, in particular the regulator's and chemical industry's perspectives, and we discuss how cost-effectiveness analysis can be applied to test selection from both perspectives. Furthermore, we show how animal welfare goals can be included in cost-effectiveness analyses, and we provide a three-dimensional extension to the standard cost-effectiveness analysis if animal welfare loss cannot be valued in monetary terms. To illustrate how cost-effectiveness analysis works in different settings, we apply our model to a simple case of selecting short-term tests for mutagenicity. We demonstrate that including sufficiently high values for animal welfare induces cost-effective replacements of animal testing. Furthermore, we show that the regulator and chemical companies face different tradeoffs when selecting tests. This may lead to different choices of tests or testing systems.  相似文献
