Inferior vena caval thrombosis is an unusual complication of permanent pacemaker implantation. The clinical presentation due to thrombosis depends on the site of thrombus. We have described here a rare case of pacemaker lead associated thrombosis of inferior vena cava, its diagnostic work up and briefly reviewed the existing literature of this uncommon complication.  相似文献

Abstract

Blood lead levels in children in the USA have dropped dramatically since lead in food, air and drinking water was reduced. In inner cities and older residential areas, increased lead exposure may still be a problem because of dilapidated housing with high lead paint levels. In these areas, at mining sites and around smelters lead levels in soil may be very high. A review of many studies indicates that lead in soil or mine tailings does not make a meaningful contribution to lead absorption by children. The contribution of lead in soil to overall exposure, if any, lies within the variation of the analytical method for blood lead measurements. The results of exposure studies in the pediatric population reviewed in this article do not support exposure predictions for children under 6 years of age based on the US EPA Integrated Exposure Uptake Biokinetic Model (using default parameters or using results obtained with in vitro digestion models). They also do not support predictions based on the percent of solubility of lead in soil (accessibility studies).  相似文献

Abstract

Sorption of lead(II) or cadmium(II) ions on humic acid particles (at pH 3.5) yields surface complexes which are sparingly soluble in the pH 3.55 region. Interaction of these species with acetic acid or dilute nitric acid released ASV labile metal species (mainly hydrated metal ion). When the pH was adjusted between 4 and 7, increasing amounts of the metal humate species (and humic acid substrate) dissolved and low levels of ASV labile species were detected. Overnight interaction with an excess of acetate ions (0.01 M) led to higher lability values (attributed to acetate/humate ligand exchange). The percentage of labile metal ion detected in acetate solutions varied with the amount of metal ion initially sorbed (range 100 to 500 mmol g?1). With the lower loadings, the ASV labile levels peaked in the pH 6–7 region (at around 12% of total sorbed cadmium, and 4% of total lead). In alkaline solutions, the percentage of labile Cd fell to about half of the peak value, but with Pb, lability values increased at pH > 8, due possibly to the formation of hydroxy complex species. The ASV lability of the Cd and Pb humates, and their 24 hour lability values (determined using a transfer to cation exchanger technique) has been compared with the behaviour of Cu and Zn humates (using similar conditions).  相似文献

Abstract

The relationship between cardiac rate variation, resting sinus rhythm heart rate in beats per minute, and mental state is reviewed. A small series of 12 psychiatric patients in whom these variables were studied both before and after appropriate psychiatric treatment is reported. Comparison with the periodicity of cardiac rate variation in a normal group of subjects showed that the setting of the biological clock governing cardiac rate variation in psychiatric patients is abnormal, and in these cases running at a slower frequency than that of mental health. Appropriate psychiatric therapy re‐sets this clock in patients responding to treatment, but fails to do so in those patients who remain unimproved. Resting mean sinus rhythm heart rate in beats per minute does not show this relationship.  相似文献

目的:炎症反应在动脉粥样斑块变化的病理过程中发挥着重要的作用。本研究探讨CXCR2基因+1235 C/T单核苷酸多态与中国汉族人群急性冠脉综合征发病的相关关系。方法:本研究采用聚合酶链反应-限制性片段长度多态性方法对675例急性冠脉综合征的患者和636例对照组进行检测,分析CXCR2基因+1235 C/T单核苷酸多态的基因型和等位基因频率的分布情况,同时收集济南军区总医院心内科经冠脉造影证实为阳性的急性冠脉综合征患者360例及对照者360例,对上述关联分析的结果进行复制实验的印证。结果:CXCR2基因+1235 C/T单核苷酸多态三种基因型(CC型,CT型和TT型)在急性冠脉综合征组分布频率分别为39.3%,45.3%和15.1%,在对照组分别为41.7%,47.2%和11.1%,CXCR2基因+1235 C/T基因型和等位基因频率对照组和急性冠脉综合征组之间存在统计学差异(P〈0.05)。Logistic回归校正性别、年龄、体重指数、吸烟、高血压、高脂血症、糖尿病等冠心病的易患因素后,CXCR2基因+1235 C/T多态与急性冠脉综合征的发病存在相关关系(P〈0.05)。结论:CXCR2基因+1235 C/T多态与急性冠脉综合征发病存在相关关系,CXCR2基因+1235 C/T多态可能是中国汉族人群急性冠脉综合征发病的独立危险因子。  相似文献

目的:探讨心电图ST-T的动态改变对临床诊断冠心病的实用价值。方法:选取2013年10月~2015年10月在我院诊断治疗的有ST-T段改变的患者,观察并记录所有患者的心电图状态,并重点监测ST-T改变的动态情况,并回归性分析及对比检测结果。结果:心电图ST-T改变有动态变化率的冠心病确诊率为67.9%,明显高于无动态变化组32.3%的确诊率。两组确诊率具有统计学意义(P0.05)。结论:心电图有动态改变的ST-T段改变能提高患者冠心病的临床确诊率,值得进一步推广应用。  相似文献

目的:研究冠状动脉严重狭窄稳定型心绞痛(Stable angina pectoris, SPA)患者循环内皮祖细胞(endothelial progenitor cells, EPCs)及基质细胞衍生因子(SDF)-1-alpha与冠状动脉侧支循环(CCC)形成的相关性,以期为治疗冠心病提供新的思路。方法:选择 2012 年8 月到2014 年12月在我院就诊的88 例冠状动脉严重狭窄的稳定型心绞痛患者(CCC 良好40 例、不良48 例),均采集 外周血测定EPC 数量、体外生成血管能力,并用ELISA 法检测其血浆SDF-1alpha 水平,采用直线相关和Pearson 检验分析CCC良好 与不良者各指标间及与CCC 分级的相关性;将所有入选病例随机分为6 组,并分离外周血单个核细胞并分别加入不同的培养 液,培养7 天后体外测定EPCs 数量以及生成血管的能力,并通过ELISA 法检测培养液上清中VEGF 的蛋白水平。结果:CCC 不 良组EPCs 数量、体外生成血管能力及SDF-1-alpha水平均明显低于CCC 良好组(P<0.05)。体外生成血管能力、循环EPCs 数量以及 SDF-1alpha水平均与CCC分级呈现显著的正相关性(r =0.72、0.67、0.79,均P<0.05);循环EPCs 数量、SDF-1alpha水平以及体外生成血 管能力亦均呈现显著正相关性(r =0.78、0.62,均P<0.05)。与PBS、SDF-1alpha+ AMD3100 及SDF-1alpha+ KI8751 干预物质比较,SDF-1琢 能够呈剂量依赖性的明显提高EPCs 数量、增强其体外生成血管的能力及VEGF水平(P<0.05)。结论:冠状动脉严重狭窄稳定型 心绞痛患者循环EPCs及SDF-1琢与CCC 形成有关,VEGF可能参与该过程。  相似文献

目的:基于全基因组关联分析(Genomewideassociationstudy,GWAS)数据与生物信息学方法,识别冠心病潜在致病基因。方法:利用生物信息学方法和GWAS数据,对单核苷酸多态性(SingleNucleotidePolymorphisms,SNP)进行疾病风险打分,依据特定距离阈值内的SNP-SNP互作关系,筛选出疾病相关SNP显著风险模块,识别潜在致病基因。结果:设定阈值20kb,经筛选获得279个SNP显著风险模块,映射到79个基因,文献验证率为71.01%。结论:基于SNP互作识别的潜在致病基因,能更加准确的分析冠心病的发生发展过程。  相似文献

目的:从分子遗传学角度分析血管紧张素转换酶(ACE)基因I/D多态与中国北方汉族人群中冠心病发病的相关关系。方法:本研究收集在沈阳军区总医院心内科住院的行冠脉造影检查的病例为研究对象,冠脉动脉照影检查显示冠状动脉主支狭窄程度大于等于70%的入选为冠心病组,冠状动脉照影检查显示冠状动脉主支狭窄程度小于20%的为对照组,共入选568名冠心病患者以及性别与年龄相匹配的529名对照个体为研究对象,利用测序的方法分析检测血管紧张素转换酶(ACE)基因I/D多态在冠心病组与对照组中的频率分布情况。结果:血管紧张素转换酶(ACE)基因I/D多态基因型频率符合Hardy-Weinberg定律。血管紧张素转换酶(ACE)基因I/D多态(II型,ID型和DD型)在我们入选的568例冠心病组分布频率分别为50.3%,30.3%和19.4%,而在我们入选的524例对照组中的分布频率为57.7%,31.2%和11.1%,研究发现血管紧张素转换酶(ACE)基因I/D多态可能是中国北方汉族人群冠心病发病的独立危险因素(P0.05);利用多元回归分析发现,在调整了冠心病的其他危险因素后,血管紧张素转换酶(ACE)基因I/D多态的变化仍然是中国北方汉族人群冠心病发病一个独立危险因素,可以预测中国北方汉族人群冠心病的发生。结论:在中国北方汉族人群中,血管紧张素转换酶(ACE)基因I/D多态可能是冠心病发病的独立危险因素,在临床上可以早期预判冠心病的发生。  相似文献

Diabetes mellitus (DM) is an important risk factor for adverse outcomes of coronary artery bypass grafting. The bypass grafts harvested from patients with DM tend to go into spasm after their implantation into the coronary circulation. To clarify the contribution of 5-hydroxytriptamine (5-HT) and angiotensin II (AngII) in the bypass graft spasm, we examined the contractile reactivity to 5-HT or AngII of isolated human endothelium-denuded saphenous vein (SV) harvested from DM and non-DM patients. The 5-HT-induced constriction of the SV was significantly augmented in the DM group than in the non-DM group, which is similar to our previous report. AngII-induced constriction of the SV was also significantly augmented in the DM group than the non-DM group. Especially in the non-DM group, the AngII-induced maximal vasoconstriction was markedly lower than the 5-HT-induced one. Meanwhile, the increasing rates of AngII-induced vasoconstriction in the DM group to the non-DM group were significantly greater than those of 5-HT-induced vasoconstriction. These results indicate that 5-HT is a potent inducer of SV graft spasm in both DM and non-DM patients, while AngII is a potent inducer of SV graft spasm only in patients with DM. Furthermore, the protein level of AngII AT1 receptor (AT1R), but not the protein level of 5-HT2A receptor, in the membrane fraction of the SV smooth muscle cells of DM patients was significantly increased as compared with that of the non-DM patients. These results suggest that the mechanism for hyperreactivity to AngII in the SV from DM patients is due to, at least in part, the increase in the amount of AT1R on membrane of the SV smooth muscle cells.  相似文献
