BACKGROUND: Biochemical testing for pheochromocytoma by measurement of fractionated plasma metanephrines is limited by false positive rates of up to 18% in people without known genetic predisposition to the disease. The plasma normetanephrine fraction is responsible for most false positives and plasma normetanephrine increases with age. The objective of this study was to determine if we could improve the specificity of fractionated plasma measurements, by statistically adjusting for age. METHODS: An age-adjusted metanephrine score was derived using logistic regression from 343 subjects (including 33 people with pheochromocytoma) who underwent fractionated plasma metanephrine measurements as part of investigations for suspected pheochromocytoma at Mayo Clinic Rochester (derivation set). The performance of the age-adjusted score was validated in a dataset of 158 subjects (including patients 23 with pheochromocytoma) that underwent measurements of fractionated plasma metanephrines at Mayo Clinic the following year (validation dataset). None of the participants in the validation dataset had known genetic predisposition to pheochromocytoma. RESULTS: The sensitivity of the age-adjusted metanephrine score was the same as that of traditional interpretation of fractionated plasma metanephrine measurements, yielding a sensitivity of 100% (23/23, 95% confidence interval [CI] 85.7%, 100%). However, the false positive rate with traditional interpretation of fractionated plasma metanephrine measurements was 16.3% (22/135, 95% CI, 11.0%, 23.4%) and that of the age-adjusted score was significantly lower at 3.0% (4/135, 95% CI, 1.2%, 7.4%) (p < 0.001 using McNemar's test). CONCLUSION: An adjustment for age in the interpretation of results of fractionated plasma metanephrines may significantly decrease false positives when using this test to exclude sporadic pheochromocytoma. Such improvements in false positive rate may result in savings of expenditures related to confirmatory imaging.  相似文献

Summary Soil and wheat bromide dynamics are studied in methyl bromide-fumigated plots in a Mediterranean climate. Bromide residues range between 5 to 10 ppm in the fumigated soil and they are distributed to a depth of 50–60 cm, where a compacted layer exists, in accordance with soil organic matter and moisture distribution. The total amount of bromide in soil is 5.8 gm−2 up to a depth of 1 m and it remains almost constant during the wheat growth period. The plant bromide concentration decreases throughout the development of spring wheat. The bromide distribution in the different plant organs was identical in the two years studied and it is correlated with the chloride content. Fumigation increases the phosphorus concentration in wheat and does not affect sulphur and chloride. Though the bromide concentration in the fumigated soils is high, the levels in the plants growing in it do not cause scorched areas. This may be due to antagonic Br/Cl and Br/NO3 effects.  相似文献

The aim of the present study is to examine the impact absorption mechanism of the foot for different strike patterns (rearfoot, midfoot and forefoot) using a continuum mechanics approach. A three-dimensional finite element model of the foot was employed to estimate the stress distribution in the foot at the moment of impact during barefoot running. The effects of stress attenuating factors such as the landing angle and the surface stiffness were also analyzed. We characterized rear and forefoot plantar sole behavior in an experimental test, which allowed for refined modeling of plantar pressures for the different strike patterns. Modeling results on the internal stress distributions allow predictions of the susceptibility to injury for particular anatomical structures in the foot.  相似文献

Vascular endothelial dysfunction occurs during the human aging process, and it is considered as a crucial event in the development of many vasculopathies. We investigated the underlying mechanisms of this process, particularly those related with oxidative stress and inflammation, in the vasculature of subjects aged 18–91 years without cardiovascular disease or risk factors. In isolated mesenteric microvessels from these subjects, an age‐dependent impairment of the endothelium‐dependent relaxations to bradykinin was observed. Similar results were observed by plethysmography in the forearm blood flow in response to acetylcholine. In microvessels from subjects aged less than 60 years, most of the bradykinin‐induced relaxation was due to nitric oxide release while the rest was sensitive to cyclooxygenase (COX) blockade. In microvessels from subjects older than 60 years, this COX‐derived vasodilatation was lost but a COX‐derived vasoconstriction occurred. Evidence for age‐related vascular oxidant and inflammatory environment was observed, which could be related to the development of endothelial dysfunction. Indeed, aged microvessels showed superoxide anions (O2?) and peroxynitrite (ONOO?) formation, enhancement of NADPH oxidase and inducible NO synthase expression. Pharmacological interference of COX, thromboxane A2/prostaglandin H2 receptor, O2?, ONOO?, inducible NO synthase, and NADPH oxidase improved the age‐related endothelial dysfunction. In situ vascular nuclear factor‐κB activation was enhanced with age, which correlated with endothelial dysfunction. We conclude that the age‐dependent endothelial dysfunction in human vessels is due to the combined effect of oxidative stress and vascular wall inflammation.  相似文献

Dense shrublands constitute highly hazardous fuels in Mediterranean countries. The combination of agricultural land abandonment and fire occurrence in many Mediterranean areas has led to a landscape dominated by shrublands where resprouter species are scarce or absent. Major goals in the management of these areas are to reduce both the fuel loads and their continuity and increase the resilience of the ecosystems by introducing resprouter species. We investigated the performance of one-year-old seedlings of native resprouters, that is, Pistacia lentiscus , Quercus ilex , and Rhamnus alaternus , in a combination of vegetation clearing and mulching with brush chipping in three sites with high amount of fuel load. The planting holes inside the shrublands showed lower radiation and soil surface temperature than those in the cleared areas, whereas soil water content was higher in the mulched holes than in the unmulched ones, especially when water availability was lowest. Seedling survival of Q. ilex and R. alaternus significantly increased within the shrubland; P. lentiscus showed the opposite effect, but its survival was enhanced by the mulching treatment. The three species grew faster in the cleared plots although, unexpectedly, mulching showed a negative effect on Q. ilex relative growth rate in diameter. Our results suggest that the combination of fuel control and reforestation as techniques for shrubland management is an appropriate option, at least in the short term, to redirect vegetation dynamics toward later successional stages, improving ecosystem resilience by the introduction of woody resprouter species.  相似文献

After enrichment of Odontesthes platensis intestinal contents, 53 lactic acid bacteria (LAB) were isolated. From the four isolates that showed inhibitory activity against Lactococcus garvieae 03/8460, strain TW34 was selected because it exerted the strongest inhibition. It also inhibited other Gram-positive bacteria, but not Gram-negative fish pathogens. Phenotypic and 16S rDNA phylogenetic analyses showed that TW34 belongs to Lactococcus lactis. In addition, TW34 showed to be sensitive to different antibiotics. The production of the inhibitory agent against L. garvieae was growth associated, and it was significantly influenced by the incubation temperature. The optimal temperature for the antimicrobial production was as low as 15°C. Both acidification and hydrogen peroxide production were ruled out as the source of inhibition. In contrast, the antimicrobial activity was completely lost by treatment with proteolytic enzymes, which confirmed that the inhibitory substance was a bacteriocin. The bacteriocin was highly thermostable (121°C for 15 min) and active between pH 3 and 11. It remained stable for up to 2 months when stored at 4°C and up to 6 months at −20°C. Our results suggest that the strain L. lactis TW34 could provide an alternative for lactococcosis control and therefore be considered for future challenge experiments with fish.  相似文献

We measured plant and soil carbon (C) storage following canopy-replacing wildfires in woodlands of northeastern Spain that include an understory of shrubs dominated by Quercus coccifera and an overstory of Pinus halepensis trees. Established plant succession models predict rapid shrub recovery in these ecosystems, and we build on this model by contrasting shrub succession with long-term C storage in soils, trees, and the whole ecosystem. We used chronosequence and repeated sampling approaches to detect change over time. Aboveground plant C increased from <100 to ~3,000 g C m−2 over 30 years following fire, which is substantially less than the 5,942 ± 487 g C m−2 (mean ±1 standard error) in unburned sites. As expected, shrubs accumulated C rapidly, but the capacity for C storage in shrubs was <600 g C m−2. Pines were the largest plant C pool in sites >20 years post fire, and accounted for all of the difference in plant C between older burned sites and unburned sites. In contrast, soil C was initially higher in burned sites (~4,500 g C m−2) than in unburned sites (3,264 ± 261 g C m−2) but burned site C declined to unburned levels within 10 years after fire. Combining these results with prior research suggests two states for C storage. When pine regeneration is successful, ~9,200 g C m−2 accumulate in woodlands but when tree regeneration fails (due to microclimatic stress or short fire return intervals), ecosystem C storage of ~4,000 g C m−2 will occur in the resulting shrublands.  相似文献
