Aim

The present study evaluated the increment of cardiac risk (CR) and absorbed dose in radiotherapy of the internal mammary chain (IMC), in particular with photon portals of 4 6?MV, and cobalt therapy (Co60); and, electron portals of 8, 12 and 16?MeV applied in the left breast, considering the adoption of a combined photon (16?Gy) and electron (30?Gy) protocols.

Materials and methods

The modified ICRP-reference female model of 60?kg, 163?cm and 43 years of age, coil RCP-AF, was modelled. The MCNP6/SICODES codes were employed, where the spatial dose distributions and dose-volume histograms were generated. Toxicity limits and a CR model were considered.

Results

CR associated with the 6?MV, 4?MV and Co60 portals increased 41.1; 40.6 and 34.5%, respectively; while, in 8, 12 and 16?MeV portals, they were 5.0, 32.5 and 49.2%, respectively. High anomalous scatter radiation from electron portals was found in the left lung providing an average dose of 3.3–5.0?Gy.

Conclusions

To RCP-AF, the Co60 portal for IMC-RT presented more attractive dose distribution, whose 16?Gy for photon-component produced less CR increase, 5% lower than the other photon portals. Considering electron portals, the smallest CR increase was produced by 8?MeV portal while 12–16?MeV made the risk higher. There is a call for a less hardened energetic spectrum in order to reduce CR; however, holding suitable IMC penetration. A combined Co60/8–12?MeV may bring benefits, reducing CR. The lowest risk was found to 46?Gy electron portals exclusively.  相似文献

A study of the action of very low doses of the delta-endotoxin of Bacillus thuringiensis isra?lensis on the larvae of Aedes aegypti (Diptera) gave evidence for peculiar properties of the dose/effect relations based on the variations of the regression slopes of functional relationships between the pairs cysteine/serine and fatty acids/histidine, by contrast with the pair fatty acids/glucose which exhibited a classical shaped relation. This indicates "crypto-toxicological" processes, not lethal by themselves and without pathological symptoms, but able to initiate "pathogen-chaining" mechanisms leading, at the end, to a lethal secondary syndrome.  相似文献

Serum samples from 125 patients with clinical suspicion of leptospirosis were tested by the microscopic agglutination test (MAT), IgM ELISA and PCR for the diagnosis of the disease. Most patients were adult males and 74.1% (p<0.001) of the patients exposed to water and 77.6% (p<0.001) of those exposed to animals, were respectively considered confirmed or probable cases by MAT. The clinical symptoms mainly observed among the patients considered confirmed or probable cases were fever (95.6%), jaundice and headache (79.4%), myalgia (77.9%), nausea and vomiting (64.7%). About 63% of the confirmed or probable cases were patients that lived in Belo Horizonte, a big city of the Minas Gerais state, Brazil, showing the occurrence of urban leptospirosis. Among the 47 confirmed cases of leptospirosis diagnosed by MAT, 44 (94%) serum samples were positive by IgM ELISA and 17 (36%) were PCR positive. Among the 33 probable cases, 10 (30%) samples showed positive amplification by PCR. By considering MAT as the standard test, the sensitivity and specificity of IgM ELISA was 96.6% and 93.3%, respectively. A relevant finding in our study was the number of positive cases verified by PCR (13-29%) and IgM ELISA (3-7%) among the 45 unconfirmed cases by MAT, demonstrating the value of PCR in the early diagnosis of human leptospirosis.  相似文献

Previous reports suggest that inflammatory bowel diseases may be accompanied by abnormalities in the neural autonomic profile. We tested the hypotheses that 1) an exaggerated sympathetic activity characterizes active ulcerative colitis (UC) and 2) a reduction of sympathetic activity by clonidine would be associated with clinical changes of UC. In 23 patients with UC and 20 controls, muscle sympathetic nerve activity (MSNA), ECG, blood pressure, and respiration were continuously recorded, and plasma catecholamine was evaluated both at rest and during a 75 degrees head-up tilt. Autonomic profile was assessed by MSNA, norepinephrine, epinephrine, spectral markers of low-frequency (LF) cardiac sympathetic (LF(RR); normalized units) and high-frequency (HF) parasympathetic (HF(RR); normalized units) modulation and sympathetic vasomotor control (LF systolic arterial pressure; LF(SAP)), obtained by spectrum analysis of the R-R interval and systolic pressure variability. Among UC patients, 16 agreed to be randomly assigned to 8-wk transdermal clonidine (15 mg/wk, 9 subjects), or placebo (7 patients). An autonomic profile, Disease Activity Index (DAI), and endoscopic pattern were compared before and after clonidine/placebo. At rest, MSNA, heart rate (HR), LF(RR), LF/HF, and LF(SAP) were higher and HF(RR) was lower in patients than in controls. Tilt decreased HF(RR) and increased MSNA and LF(RR) less in patients than in controls. Clonidine decreased HR, MSNA, epinephrine, LF(RR), and increased HF(RR), whereas placebo had no effects. Changes of the autonomic profile after clonidine were associated with reduction of DAI score. An overall increase of sympathetic activity characterized active UC. Normalization of the autonomic profile by clonidine was accompanied by an improvement of the disease.  相似文献

The main aim of this work was to identify zooplankton assemblages by means of statistical testing and associate them with hydrographic properties of the Strait of Magellan and its microbasins. Zooplankton samples were collected by the R/V Cariboo in late austral summer 1991. Nineteen stations were sampled by BIONESS from the surface layer to 900 m depth, along the main longitudinal axis of the Strait of Magellan. There was a marked regional-scale pattern in zooplankton species diversity and richness, related to the hydrographical features of the Strait and its sub-basins. Six groups of samples were identified by cluster analysis in terms of zooplankton structure, and related to sea-water properties of temperature, salinity, and Chl a (Sperman’s correlation coefficient). Plankton assemblages of the Strait of Magellan seem closely linked to the remote sub-Antarctic and adjacent ocean biota, but populations may have independent evolutionary lines and population dynamics.  相似文献

Cerebral aspergillosis is a mostly lethal infection of the central nervous system. Former results identified low cerebral complement levels as one cause for insufficient immune reaction. Therefore we studied cerebral complement expression after fungal invasion and investigated putative mechanisms of Aspergillus spp to cope with the complement-induced selection pressure. Brain tissue derived from patients with cerebral aspergillosis or non-infected individuals was analyzed immunohistochemically for complement synthesis. Correlations between expression levels were determined statistically. Increased complement synthesis, a prerequisite for strengthened antifungal potency, was visible in resident astrocytes, neurons, oligodendrocytes as well as in infiltrating macrophages after fungal infection. Surprisingly, microglia, although regarded as major immune cells, only marginally participated in synthesis of most complement proteins. Several evasion mechanisms were found that help the fungus to establish a cerebral infection even in the presence of complement: Fungal hyphae limit the surface deposition of C3 and thus interfere with complement-dependent phagocytosis. Furthermore, the "sealing off" in brain abscesses not only inhibits fungal spreading but also forms protection shields against complement attack. Complement indeed seems to represent an important selection pressure and evokes the development of fungal evasion mechanisms. Counteractions for these evasion processes might represent interesting therapeutic approaches.  相似文献

This study investigates the morphological and molecular changes that occur in the inner hair cell area of the rat cochlea following aminoglycoside treatment. Rats were injected daily with 500 mg/kg of amikacin between postnatal day 9 (PND9) and PND16. Cochleae were examined at PND16 to PND120 using both scanning and transmission electron microscopy and molecular fluorescent labeling. The inner hair cells showed obvious signs of apoptosis in response to amikacin treatment and most of them were missing by one week after the end of the aminoglycoside exposure period. Concomitantly, the epithelium became scarred as the surrounding supporting cells expanded and filled the space vacated by the missing IHCs. The mid-basolateral region of these modified supporting cells was surrounded by many afferent and efferent terminals. However, these cells expressed neither calbindin nor SNAP25, proteins that are both expressed by IHCs in the normal, untreated organ of Corti in the rat. In addition, these supporting cells remained attached to the basal lamina by a thin cytoplasmic process. The supporting cells surrounding the inner hair cells therefore appear unable to convert directly into inner hair cells following aminoglycoside induced hair-cell loss but may be able to provide trophic support for the remaining afferent and efferent neurites.  相似文献
