BACKGROUND:Prior research has consistently shown that the heaviest users account for a disproportionate share of health care costs. As such, predicting high-cost users may be a precondition for cost containment. We evaluated the ability of a new health risk predictive modelling tool, which was developed by the Canadian Institute for Health Information (CIHI), to identify future high-cost cases.METHODS:We ran the CIHI model using administrative health care data for Ontario (fiscal years 2014/15 and 2015/16) to predict the risk, for each individual in the study population, of being a high-cost user 1 year in the future. We also estimated actual costs for the prediction period. We evaluated model performance for selected percentiles of cost based on the discrimination and calibration of the model.RESULTS:A total of 11 684 427 individuals were included in the analysis. Overall, 10% of this population had annual costs exceeding $3050 per person in fiscal year 2016/17, accounting for 71.6% of total expenditures; 5% had costs above $6374 (58.2% of total expenditures); and 1% exceeded $22 995 (30.5% of total expenditures). Model performance increased with higher cost thresholds. The c-statistic was 0.78 (reasonable), 0.81 (strong) and 0.86 (very strong) at the 10%, 5% and 1% cost thresholds, respectively.INTERPRETATION:The CIHI Population Grouping Methodology was designed to predict the average user of health care services, yet performed adequately for predicting high-cost users. Although we recommend the development of a purpose-designed tool to improve model performance, the existing CIHI Population Grouping Methodology may be used — as is or in concert with additional information — for many applications requiring prediction of future high-cost users.

A substantial literature across health systems shows that the highest users of services account for disproportionate shares of the public costs of health care. It has recently been reported that more than three-quarters of individual health care costs in Ontario were incurred by just 10% of the population.1 Similarly, an Ontario Ministry of Health and Long-Term Care (MOHLTC) analysis of inpatient and home care costs found that the top 5% of patients were responsible for 61% of spending in those domains.2 Consistent findings have been reported for Manitoba, Alberta and British Columbia.37Some of the highest-cost cases may be explained by rare, unpredictable events, but others arise in the presence of multiple chronic conditions. Research from the United States has suggested that spending on chronic conditions accounts for the majority of health care expenditures.8 Predicting high-cost users may help us to understand and better manage public spending on health care.Cognizant of this need, the Ontario MOHLTC developed a predictive model for high-cost users based on sociodemographic, utilization and clinical diagnostic characteristics.9 Although the model performed well, it relied on a coarse categorization of 20 diagnostic variables consisting of broadly defined chapters of the International Statistical Classification of Diseases and Related Health Problems, 10th Revision (ICD-10) and a small number of chronic conditions, which limited its utility for explaining predictions. Moreover, this model is not available for use outside the MOHLTC. As such, there is a need for a predictive model that can be applied more widely by researchers and other stakeholders with an interest in health policy and spending in Canada.The Canadian Institute for Health Information (CIHI) has recently released a new population-based case mix product, the Population Grouping Methodology, which uses diagnoses obtained from patient health care encounters in multiple settings to summarize the universe of diagnosis codes into a clinically meaningful set of 226 health conditions. The grouping and modelling methodologies are described in more detail in Appendix 1 (available at www.cmaj.ca/lookup/suppl/doi:10.1503/cmaj.191297/-/DC1) and in previous reports.10,11 The CIHI grouping methodology was not designed to predict high-cost cases, and previous work has already shown that the model performs better for low- and moderate-cost users than for highest-cost users (i.e., those with annual costs exceeding $25 000).11 We evaluated the suitability of CIHI’s model for predicting future high-cost users in Ontario by examining the predicted costs for individuals who exceeded the top 10%, 5%, and 1% thresholds of actual cost.  相似文献

The presence of significant amount of heavy metals in rivers and canals due to mixing of untreated industrial effluents is a common phenomenon, especially in developing countries. The agricultural crops are influenced by the presence of various pollutants in the sewage, being applied for irrigation purpose. The effluents containing copper affect the growth and development of crop species, thereby, ought to be mitigated by foliar spray of osmoprotectants, e.g. proline. A pot culture experiment was conducted at the University of Agriculture, Faisalabad-Pakistan during the crop season 2015–2016. The treatments consisted of (a) three wheat varieties (Punjab-96, MH-97, FSD-83), (b) two levels of copper (0, 400?µM) applied through rooting medium, and (c) two levels of proline (0, 80?mM) applied through foliar application. The treatments were arranged in a completely randomized design with four replications. The results showed that application of 400?µM copper caused a reduction in biomass accumulation, chlorophyll (‘a’ and ‘b’) contents, and eventually yield (100-grain weight). There were also significant decreases in gas exchange parameters (stomatal conductance, internal CO2 concentration), photosynthetic rate, water-use-efficiency, and transpiration rate in response to copper stress. Metal toxicity caused the maximum reduction in productivity of PSII, electron transport rate, and photochemical quenching, while higher values of non-photochemical quenching were recorded in the wheat varieties. The activities of antioxidant enzymes (superoxide dismutase, catalase, peroxidase), as well as quantities of proline, protein and calcium contents were accelerated in response to copper stress. The uptake of calcium, magnesium, and potassium constituents by plants was reduced, while assimilation of calcium was increased in plants under copper stress. However, the occurrence of negative effects on these parameters due to copper stress was mitigated by foliar spray of proline at the rate of 80?mM solution. The exogenous application of proline at the rate of 80?mM resulted in the reduction of generation of reactive oxygen species and enhanced accumulation of proline and protein contents in wheat varieties under copper stress environment.  相似文献

Potassium bromate (KBrO3) is widely used as a food additive and is a major water disinfection by-product. It induces multiple organ toxicity in humans and experimental animals and is a probable human carcinogen. The present study reports the protective effect of dietary antioxidant taurine on KBrO3-induced damage to the rat intestine. Animals were randomly divided into four groups: control, KBrO3 alone, taurine alone and taurine+ KBrO3. Administration of KBrO3 alone led to decrease in the activities of intestinal brush border membrane enzymes while those of antioxidant defence and carbohydrate metabolism were also severely altered. There was increase in DNA damage and DNA-protein cross-linking. Treatment with taurine, prior to administration of KBrO3, resulted in significant attenuation in all these parameters but the administration of taurine alone had no effect. Histological studies supported these biochemical results showing extensive intestinal damage in KBrO3-treated animals and greatly reduced tissue injury in the taurine+ KBrO3 group. These results show that taurine ameliorates bromate induced tissue toxicity and oxidative damage by improving the antioxidant defence, tissue integrity and energy metabolism. Taurine can, therefore, be potentially used as a therapeutic/protective agent against toxicity of KBrO3 and related compounds.  相似文献

When platelet rich plasma is exposed to N-ethylmaleimide, a ten fold increase in measurable prostaglandin E synthesis occurs. This effect is almost completely abolished within 2 hours of ingestion of 600 mg of aspirin by human volunteers. Recovery of this platelet function is slow for the first two days, returning sharply to normal over the next six days and plateauing approximately 8 days following initial removal from aspirin. It is suggested from these studies that platelet prostaglandin E production following NEM may be a useful test of platelet function.  相似文献

We examined a Ser-9-Gly polymorphism in the dopamine D3 receptor gene for allelic association with schizophrenia in 133 patients currently treated with clozapine and 109 controls. Allele 1 (Ser-9) was significantly more frequent in the patients (69%) than in the controls (56%) (P = 0.004). The 1-1 genotype was more common (43% vs 30%) and the 2-2 genotype less common (5% vs 18%) in patients than in controls. When the patient group was subdivided on the basis of clinical response to clozapine, using a 20-point improvement in the global assessment scale as cut-off, genotype 1-1 was found to be more frequent among the non-responders (53% vs 36%, P = 0.04). To place our results in the context of previous studies of this polymorphism and schizophrenia, we performed a meta-analysis of all published data including the present sample. The combined analysis shows evidence for a modest association between genotype 1-1 and schizophrenia (odds ratio 1.25, 95% confidence interval 1.05– 1.49, P = 0.01). These results suggest that the Ser-9 allele, or a nearby polymorphism in linkage disequilibrium, results in a small increase in susceptibility to schizophrenia.
