Arthrite rhumatoide traitée par noréthynodrel associée à mestranol: Aspects cliniques et tests de laboratoire

A cause de l''effet favorable de la grossesse sur l''activité de l''arthrite rhumatoïde, on s''est demandé si une pseudo-grossesse, produite par un progestatif de synthèse n''entraînerait pas une rémission du moins partielle de cette maladie.Noréthynodrel associée à mestranol (Enovid), 30 mg./jour, a été administrée à 44 femmes pendant quatre à 24 mois. A cause d''effets secondaires indésirables, 11 patientes furent soustraites de l''investigation. Les résultats s''appuient sur 33 cas. Une rémission apparente complète s''est manifestée chez sept patientes aux stades précoces de la maladie; chez 15, une amélioration objective des signes inflammatoires a été observée; chez quatre, une amélioration subjective seule a été notée; chez sept, il n''y a eu aucune amélioration. Dix-sept femmes sur 36 ont présenté une altération d''un ou plusieurs tests hépatiques. Trois présentèrent un ictère cholestatique. Les 17-OH plasmatiques se sont élevés à trois ou quatre fois la normale.De cette étude il ressort que noréthynodrel associée à mestranol peut produire une atténuation des signes inflammatoires de l''arthrite rhumatoïde. L''effet est palliatif, mitigé et non curatif et ne résulte pas nécessairement de l''état de pseudo-grossesse en soi.     

Active Ion Transport Across Canine Blood Vessel Walls

Experiments giving evidence of active Na and Cl ion fluxes across large canine blood vessel walls (aorta, vena cava) in vitro have been presented. The information has been obtained using ion tracer techniques after Ussing and with diffusion cells of the Hogben type. The available data suggest that the membranes are satisfactorily oxygenated by the bathing solutions saturated with oxygen at atmospheric pressure. Evidence is offered which indicates that active ion transport does occur across the aorta and vena cava in in vitro experiments. Under the conditions of the experiment net Na and Cl flux takes place from intima to adventitia across the aorta, and from adventitia to intima across the vena cava at low measured potential differences. The possible relationships of derangement of active ion transport mechanisms, produced by electric currents and tissue injury potential differences, to intravascular thrombosis are alluded to. It would appear that sodium and chloride fluxes across large blood vessel walls in vitro occur at least in part as the result of metabolic processes and cannot be explained simply on the basis of diffusion across a semipermeable membrane.     

MANAGEMENT OF EARLY PROSTATIC CARCINOMA

It is important to make a diagnosis of prostatic carcinoma as early as possible, because early treatment gives the best results whether radical prostatectomy is done or endocrine therapy used. Open perineal biopsy is the most accurate method of making a diagnosis. Perineal needle biopsy or the newer approach of transrectal needle biopsy is probably about 75 per cent accurate in making a diagnosis.Ten-year survival with conservative therapy, as determined in a review of a series of cases, was 50 per cent—about the same as that following radical prostatectomy; but the patients with prostatectomy are clinically free of malignant disease whereas the former are not. Radical prostatectomy is indicated in a few selected cases.The results from endocrine therapy begun immediately after diagnosis are significantly better than those from delayed treatment. Orchiectomy and estrogens promise a little longer survival than estrogens alone.     

Comparative structure of the gas-vacuoles of blue-green algae

Summary An investigation was made of 5 species of blue-green algae reported to contain gas-vacuoles. All organisms were grown and harvested under standard conditions. Gas-vacuoles were characterised as reddish structures which are destroyed by applying pressure. Using a simple direct preparation technique gascylinders were observed with the transmission electron microscope in gas-vacuolate cells. Gas-vacuoles were present in the strains of Anabaena flos-aquae, Gloeotrichia echinulata and Oscillatoria agardhii studied and absent from Microcystis aeruginosa and Nostoc linckia. The reddish, refractile central area of N. linckia and M. aeruginosa cells was tentatively identified as nucleoplasm. Gas-vacuoles are collections of gas-cylinders 70 m wide, which in A. flos-aquae and G. echinulata are clearly bounded by photosynthetic lamellae and associated with -granules. The presence of bounding photosynthetic lamellae in these species is suggested as a causal factor of the unusual optical properties of their gas-vacuoles. The range of lengths of gas-cylinders in G. echinulata and O. agardhii is from 100 m to 500 m and in A. flos-aquae it is from 100 m to 1300 m. The percentage of cell volume occupied by gas-vacuoles was estimated by direct measurement. In A. flos-aquae and G. echinulata it was 22%. In O. agardhii gas-cylinders were not clearly associated with photosynthetic lamellae and -granules and occupied 39% of cell volume. Gascylinder membranes showed reasonable preservation in KMnO₄ and excellent preservation in OsO₄. The widths of membranes after treatment with these two fixatives was 3 m and 2 m respectively.     

An ultrastructural study of the carotid body of horse and dog

Summary Carotid body tissue from horse and dog has been investigated ultrastructurally. Several cell types are recognized: glomus cells which are regarded as chemoreceptors, sustentacular cells which enclose the glomus cells, and nerve fibers.The glomus cells contain electron dense granules which are interpreted as packages of biogenic monoamines. There are both dark and light glomus cells, the former containing more granules and ribosomes. Invaginations of the plasma membranes as well as free coated vesicles are often seen in the cytoplasm of glomus cells. Nerves within the glomus lobules are generally wrapped by sustentacular cells, but nerve endings are also seen in close contact with the glomus cells. Some endings contain synaptic vesicles as well as a great concentration of mitochondria. The corresponding fibers are thought to be efferent. Another type of contact is interpreted as en passant synapses of afferent fibers.The author wishes to express his gratitude to Professor L.
