OBJECTIVES: To determine whether a two hour training programme for general practice reception staff could improve uptake in patients who had failed to attend for breast screening, and whether women from different ethnic groups benefited equally. DESIGN: Controlled trial, randomised by general practice. SETTING: Inner London borough of Newham. SUBJECTS: 2064 women aged 50-64 years who had failed to attend for breast screening. Women came from 26 of 37 eligible practices, 31% were white, 17% were Indian, 10% Pakistani, 14% black, 6% Bangladeshi, 1% Chinese, 4% were from other ethnic groups, and in 16% the ethnic group was not reported. MAIN OUTCOME MEASURES: Attendance for breast screening in relation to ethnic group in women who had not taken up their original invitation. RESULTS: Attendance in the intervention group was significantly better than in the control group (9% v 4%). The response was best in Indian women--it was 19% in the intervention group and 5% in the control group. CONCLUSIONS: This simple, low cost intervention improved breast screening rates modestly. Improvement was greatest in Indian women--probably because many practice staff shared their cultural and linguistic background. This intervention could be effective as part of a multifaceted strategy to improve uptake in areas with low rates. 相似文献

More than one isoform of bothrojaracin (BJC), a potent and specific thrombin inhibitor isolated from Bothrops jararaca venom, has been found in individual venoms collected from adult snakes. Variations in snake venom composition have previously been associated with factors such as age, sex, geographic origin, season of the year and diet. In order to obtain further information concerning individual patterns of expression of BJC isoforms, we have analyzed five individual Bothrops jararaca snake venoms collected at the same time from adult female snakes from the same geographic region. As expected, crude venoms showed a similar migration pattern on SDS-PAGE. BJC was purified using a procedure which includes an affinity chromatography step (PPACK-thrombin Sepharose). A slight variation in the amount of BJC obtained from individual venom samples was noticed. Inhibition of thrombin-induced platelet aggregation as well as migration pattern on SDS-PAGE (under reducing and non-reducing conditions) and isoelectric focusing varied considerably among BJC samples from the five snakes. The amino-terminal sequences (residues 1–34) of individual BJC samples were compared with the sequence deduced from isolated cDNAs encoding α and β chains of BJC. A high degree of homology was detected, although some residues differed from one sample to other. Altogether, data confirmed the heterogeneity found for BJC purified from individual snakes. Thus, the results indicate that: (1) individual specimens of Bothrops jararaca have different patterns of BJC isoform expression; and (2) it seems that genetic factors, at least in part, determine the variability found in BJC production. 相似文献

MOTIVATION: Complete genomic sequences will become available in the future.
New methods to deal with very large sequences (sizes beyond 100 kb)
efficiently are required. One of the main aims of such work is to increase
our understanding of genome organization and evolution. This requires
studies of the locations of regions of similarity. RESULTS: We present here
a new tool, ASSIRC ('Accelerated Search for SImilarity Regions in
Chromosomes'), for finding regions of similarity in genomic sequences. The
method involves three steps: (i) identification of short exact chains of
fixed size, called 'seeds', common to both sequences, using hashing
functions; (ii) extension of these seeds into putative regions of
similarity by a 'random walk' procedure; (iii) final selection of regions
of similarity by assessing alignments of the putative sequences. We used
simulations to estimate the proportion of regions of similarity not
detected for particular region sizes, base identity proportions and seed
sizes. This approach can be tailored to the user's specifications. We
looked for regions of similarity between two yeast chromosomes (V and IX).
The efficiency of the approach was compared to those of conventional
programs BLAST and FASTA, by assessing CPU time required and the regions of

Following an initial report urging conservative management of severe diabetic foot infections, the authors have managed 45 patients with a minimum 3-year follow-up. By using standard principles for soft-tissue infection, 78 percent of the patients healed minor amputation sites and maintained biped ambulation following the initial foot involvement. Only 22 percent required a major amputation at the time of the initial foot involvement. The 45 patients were followed and 22 (or 49 percent) developed a severe infection involving the contralateral foot within 18 months. Although 15 of the 22 patients developing contralateral infection (or 33 percent of the total series) required some type of amputation on the contralateral foot, the conservative approach allowed 64 percent of the patients with severe infections in both feet to maintain biped ambulation. This included 40 percent of the patients who required amputation of some portion of both feet. 相似文献

To study the direction of jaw movements in the koala from wear facets on the molar teeth by scanning electron microscopy, gold coated epoxy resin replicas from the right maxillary and mandibular tooth quadrants were examined from 12 koala skulls. The progressive development and location of facets, the orientation of striae on them and directional data were recorded and transferred from electron micrographs to superimposable transparencies.
Polished facets with laterally oriented striations developed on the cristids and cristae progressively into dentine, where Greaves' effect indicated that the direction of the chewing stroke was labiolingual. Polished and pitted facets, aligned and striated in the parasagittal plane, occurred on the smooth interactive enamel surfaces of maxillary and mandibular cusps.
Labiolingual transit of the crislids over the cristae, with a slight anteromedial shift, was inferred to be the predominant chewing stroke on the working side with no contralateral balancing contact. A propalinal isognathous movement in which successive cusps made contact was also deduced.

Nucleotide sequence of a full-length complementary DNA clone and amino acid sequence of human phenylalanine hydroxylase 总被引:65,自引:0,他引:65
A full-length human phenylalanine hydroxylase complementary DNA (cDNA) clone was isolated from a human liver cDNA library, and the nucleotide sequence encoding the entire enzyme was determined. The cDNA clone contains an inserted DNA fragment of 2448 base pairs, including 19 base pairs of poly(A) at the 3' end. The first methionine codon occurs at nucleotide position 223, followed by an open reading frame of 1353 base pairs, encoding 451 amino acids. Translation of the nucleotide sequence in the open reading frame predicts the amino acid sequence of human phenylalanine hydroxylase. The human protein shows a 96% amino acid sequence homology with the corresponding rat enzyme. The determination of the complete primary structure for phenylalanine hydroxylase represents the first among mixed-function oxidases. 相似文献
